Successful Management of Recurrent Cholecystitis Four Years After Endoscopic Gallbladder Stenting (EGBS) Placement Using Internal and External Fistula Drainage.

Recurrent cholecystitis can occur even after permanent endoscopic gallbladder stenting (EGBS), and optimal management in such situations remains challenging. In the present case, endoscopic transpapillary gallbladder drainage (ETGBD) was successfully achieved by using the indwelling EGBS to facilitate guidewire access to the gallbladder. Endoscopic nasobiliary gallbladder drainage was also placed, enabling combined internal and external drainage. The patient experienced a favorable clinical course following this approach. This case suggests that ETGBD is a feasible therapeutic option for recur

Retained gallstone as a rare cause of recurrent fistula in the scar after laparoscopic cholecystectomy.

INTRODUCTION: Cholecystectomy is one of the most common operations in surgical departments. Complications after gallbladder removal are mainly bleeding, infection including abscess in the gallbladder bed or in the abdominal wall, wound dehiscence, acute pancreatitis or injury of the bile ducts. In the further course, hernias in the scar may appear after both laparoscopic and open cholecystectomy, strictures of the bile ducts and symptoms of the so-called postcholecystectomy syndrome. The presence of residual gallstones is rare, statistically reported in 0.08-0.3%. CASE REPORT: The goal of our

A case report of gallstone ileus caused by biliary-intestinal fistula: Multidetector computed tomography features analysis.

INTRODUCTION AND SIGNIFICANCE: Gallstone ileus is a mechanical intestinal obstruction caused by gallstones or stones from the common bile duct entering the intestinal canal through a biliary-intestinal fistula and becoming impacted. Multidetector computed tomography (MDCT) scanning is the most important auxiliary diagnostic method for this disease. CASE PRESENTATION: We report a rare case of gallstone ileus involving an elderly female patient and analyze the characteristics of the MDCT images: atrophic cholecystitis, gallstones, the formation of a local duodenal-gallbladder fistula, pneumatos

Management of a Large Bowel Obstruction Secondary to a 5cm Gallstone in a Rural Setting: A Case Report.

We present a case of a large bowel obstruction secondary to a large impacted gallstone measuring 5cm in the sigmoid colon. Significant features include a cholecysto-colonic fistula and the presence of a perforation initially missed on imaging. Endoscopic retrieval was attempted, followed by a Hartmann's procedure. This case highlights the significance of operative management in ensuring the safe treatment of colonic gallstone ileus, especially in a rural setting.

Endoscopic Treatment of Bouveret Syndrome with Combined Laser and Mechanical Lithotripsy: A Case Report.

Background: Bouveret syndrome is a complication of cholelithiasis, characterized by the migration of a large gallstone from the gallbladder to a part of the stomach or intestine through a bilio-enteric fistula. This condition results in a rare form of gallstone ileus, presenting signs and symptoms of gastric outlet obstruction. Methods: This case report aims to present a rare instance of Bouveret syndrome in a 64-year-old woman who presented to our emergency department with recurrent epigastric pain and vomiting for over 2 months. After a CT scan, an esophagogastroscopy was performed following

Immediate Reconstruction of Pancretaticogastrostomy Following Hepatopancreatoduodenectomy for Biliary Tact Cancer: A Single-Center Review of 33 Consecutive Patients.

BACKGROUND AND PURPOSE: Although hepatopancreaticoduodenectomy (HPD) is effective for margin-negative curative resection of biliary tract cancer (BTC), it is a highly invasive procedure, and its high complication rate, especially post-operative pancreatic fistula, and hospital mortality are still problems. In this study, we investigated the clinical usefulness of pancreaticogastrostomy (PG) in BTC patients with HPD. PATIENTS AND METHODS: Of the 878 BTC resected in our institution from April 1992 to December 2022, 44 patients who underwent HPD for perihilar cholangiocarcinoma (PHC) or gallblad

Gallstone Ileus in a 25-Year-Old Female With Cyclin-Dependent Kinase-Like 5 Deficiency Disorder: A Case Report.

Gallstone ileus is an uncommon complication of cholelithiasis and the delayed diagnosis may be associated with increased risk of mortality. When gallstones block the cystic duct, they can lead to cholecystitis. If a fistula forms between the inflamed gallbladder and the adjacent intestine, the gallstones may pass into the intestinal tract and cause obstruction in the intestine. We report a case of 25-year-old female with developmental and epileptic encephalopathy who was intraoperatively diagnosed with gallstone ileus during surgery for small bowel obstruction of unknown origin. The patient ha

Double Large Gallstones Ileus: An Unusual Cause of Mechanical Large Bowel Obstruction.

Gallstone ileus (GSI) is an unusual complication of cholelithiasis resulting from the formation of a fistula between the gallbladder and the gastrointestinal tract. When a sufficiently large gallstone passes into the bowel, it can obstruct a narrowed segment. The most common site of GSI is the terminal ileum, typically presenting as small bowel obstruction. However, a rare but potentially serious complication of calculous cholecystitis is gallstone sigmoid ileus, which can cause large bowel obstruction. This report presents the case of a 59-year-old woman who exhibited symptoms of large bowel

Acute gallbladder pathologies beyond uncomplicated cholecystitis.

The gallbladder can be affected by various acute conditions beyond uncomplicated cholecystitis, with the prevalence of gallstones having risen over the past three decades. Management of biliary diseases is complex and necessitates careful case-by-case consideration. Radiologic imaging plays a crucial role in the evaluation, diagnosis, and management planning of biliary pathologies. This review explores acute gallbladder conditions such as simple cholecystitis, gangrenous cholecystitis, emphysematous cholecystitis, hemorrhagic cholecystitis, perforated cholecystitis, gallbladder fistulas, galls

Gallstone Ileus as an Uncommon Etiology of Intestinal Obstruction: A Case Report.

Although gallstones are commonly confined to the biliary system, in rare instances, they may migrate into the intestinal tract and cause obstruction, a scenario most often observed in older adults with a history of gallbladder pathology. We describe the case of a 46-year-old woman who developed small bowel obstruction secondary to a large impacted gallstone, despite having no prior biliary disease. Her clinical presentation included a 15-day history of abdominal pain, bilious vomiting, and diarrhea. Laboratory tests revealed leukocytosis and acute kidney injury. Contrast-enhanced computed tomo

Gallstone ileus 16 years postcholecystectomy?

Gallstone ileus, a rare complication of cholecystitis, occurs when the distal ileum becomes obstructed by gallstone causing, mechanical bowel obstruction. Gallstone ileus with history of cholecystectomy is exceptionally rare, with few cases documented in literature. This case report details the discovery of a large, impacted gallstone 16 years postcholecystectomy. An 87-year-old male presented with progressively worsening right-sided abdominal pain over 2-3 days. Exploratory laparotomy unveiled a 2.0 cm × 2.5 cm gallstone was obstructing the terminal ileum. Patient improved remarkably postoper

Gallstone ileus secondary to cholecystoduodenostomy causing mechanical duodenal obstruction in a cat.

CASE SUMMARY: A 9-year-old neutered male domestic shorthair cat was presented for anorexia and lethargy with a 2-day history of ptyalism and dysorexia. The cat had undergone a cholecystoduodenostomy 8 years before presentation and had suffered from episodes of clinical cholangitis ever since. Abdominal ultrasound revealed duodenal obstruction. Two stones were surgically removed, subjected to spectrophotometric examination and identified as choleliths. Clinical improvement was noted postoperatively, and 12 months after surgery the cat was doing well and had no further digestive problems. RELEV

Cholecystocolonic Fistula Presenting With Gallstone Ileus: A Report of a Rare Surgical Case.

Cholecystocolonic fistula (CCF) is a rare entity resulting from chronic gallstone disease. Here we describe a 60-year-old female who presented with abdominal distension, colicky pain, and constipation for a period of five days. Imaging studies revealed pneumobilia and small bowel obstruction. Exploration via laparotomy revealed a 3.5 cm gallstone lodged in the terminal end of the ileum and a fistulous tract from the gallbladder to the hepatic flexure of the colonic bowel. Surgical intervention consisted of enterolithotomy, cholecystectomy, and colonic resection with primary anastomosis. Fortun

Rescue for an advanced aging patient with synchronous AOSC and gallstone ileus: a case report and literature review.

BACKGROUND: Cholecystolithiasis is the most common disease of the gallbladder. Both acute obstructive suppurative cholangitis (AOSC) and gallstone ileus are critical clinical conditions requiring urgent intervention. However, their synchronous occurrence, particularly in elderly patients, presents a significant therapeutic challenge. In such scenarios, an optimal treatment strategy is essential to ensure patient safety while minimizing procedural risks. CASE PRESENTATION: Herein, we described a 91-year-old women with cholecystolithiasis who was admitted for a day of abdominal pain accompanied

Concurrent Congenital Umbilicobiliary Fistula and Vesicourachal Diverticula in a Dog.

Congenital umbilicobiliary fistula is a rare disease reported in humans and dogs. A 2-month-old, intact, male French Bulldog presented with a greenish-yellow discharge dripping from the umbilicus since birth. Complete blood count and serum biochemistry were within normal limits, but serum alkaline phosphatase activity was mildly elevated. A positive contrast cystogram was performed to rule out a patent urachus and confirmed the presence of a vesicourachal diverticula, a type of urachal anomalies. An abdominal ultrasound and computed tomography fistulogram demonstrated a communication between t
