Cholesterolosis mimicking gallbladder adenomyomatosis on imaging in an asymptomatic patient with concurrent cholelithiasis: a case report.

BACKGROUND: Gallbladder cholesterolosis and adenomyomatosis (GA) share overlapping cross-sectional imaging features on CT enterography and MRCP, creating a significant diagnostic pitfall. Histopathology remains the only definitive means of distinction. CASE PRESENTATION: A 44-year-old asymptomatic female was incidentally found to have gallbladder wall thickening and cholelithiasis on CT enterography. MRI/MRCP demonstrated focal fundal wall thickening with intramural high-signal foci, which was interpreted as fundal-type GA with a coexisting gallstone. Elective laparoscopic cholecystectomy was

Histopathological findings in gallbladders of asymptomatic living liver donors at a liver transplant center.

BACKGROUND & OBJECTIVE: In low and middle-income countries (LMICs), living donor liver transplant (LDLT) is common due to limited cadaveric grafts. Cholecystectomy during donor hepatectomy allows evaluation of gallbladder pathologies in healthy donors. This study examined the prevalence of gallbladder pathologies in healthy living liver donors. METHODOLOGY: This study used retrospective data from March 2019 to March 2023 and prospective data from March 2023 to July 2024, collected from consecutive liver donors at a single liver transplant center using a prospectively maintained database. Neuroendocrine Tumors of the Gallbladder: A Multicenter Case Series and Systematic Literature Review Indicating Predominantly Non-Aggressive Tumor Behavior and a Common Association with Cholesterol Polyps and Cholesterolosis.

Current data on neuroendocrine tumors (NETs) of the gallbladder and cystic duct (GB-NETs) are highly limited, and the available evidence, largely derived from cancer registry data, suggests that these tumors exhibit a substantially more aggressive clinical behavior than NETs arising at other anatomical sites. We analyzed 26 GB-NETs. Female-to-male ratio: 1.9:1; median age: 50 years. They were typically incidental small (median: 0.8 cm, range: 0.08-2.3 cm) tumors, with 81% classified as grade 1 (G1), either pT1 or pT2, and 38% showing a paraganglioma-like pattern at least focally. Severe Gallbladder Dyskinesia With 2% Ejection Fraction: A Comprehensive Clinicoradiologic and Pathologic Case Correlation.

Gallbladder dyskinesia (GBD) is a functional biliary disorder characterized by biliary colic in the absence of cholelithiasis or mechanical obstruction and is identified by reduced gallbladder ejection fraction (EF) on cholecystokinin (CCK)-stimulated hepatobiliary scintigraphy. Patients may experience significant symptoms due to impaired gallbladder contractility and bile stasis despite the absence of gallstones or inflammatory structural abnormalities on imaging. We present a 42-year-old male with a three-month history of episodic right upper quadrant (RUQ) pain triggered by fatty meals. Abd

Biliary Dyskinesia - Is It Real?

INTRODUCTION: Biliary dyskinesia is a functional gallbladder disorder characterized by altered gallbladder motility. This often presents as history of biliary symptoms without imaging evidence of gallstones, sludge, or other structural pathology. The diagnosis is made by demonstration of abnormal gallbladder emptying on hepatic iminodiacetic acid analogue scan with cholecystokinin administration (HIDA-CCK). The aim of this study was to determine if patients with biliary dyskinesia who undergo cholecystectomy have abnormal pathology and resolution of symptoms. METHODS: This is a retrospective

Gallbladder pathologies through a decade: Insights into emerging trends and premalignant lesions from a single-center experience: An observational study.

Gallbladder diseases, including chronic cholecystitis and cholesterolosis, impose a significant burden on global health. Monitoring their evolving trends is vital to refine diagnostic strategies and improve clinical outcomes. This study evaluates a decade of gallbladder pathology data to identify epidemiological shifts and associated risk factors. This retrospective study analyzed 6665 cholecystectomy specimens from January 2014 to December 2023. Diagnoses included chronic and acute cholecystitis, cholesterolosis, metaplasia, dysplasia, and adenocarcinoma. Annual trends, demographic patterns,

Clinicopathological Evaluation of Cholesterolosis in Cholecystitis: Histopathological Patterns and Metabolic Correlates.

Background/Objectives: Cholesterolosis is a relatively common, yet often incidental, histopathological finding in cholecystectomy specimens, frequently associated with chronic cholecystitis. Although its clinical significance remains unclear, possible associations with glycemic regulation have been proposed. This study aimed to examine the relationship between cholesterolosis, glycemic parameters, and inflammatory markers in patients with chronic cholecystitis. Methods: We retrospectively analyzed gallbladder specimens from patients who underwent cholecystectomy for chronic cholecystitis betwe

Heterotopic Pancreas in the Gallbladder: An Incidental Finding Following Laparoscopic Cholecystectomy in a Patient With Cholelithiasis.

Heterotopic pancreas (HP)-defined as ectopic pancreatic tissue lacking vascular or ductal connections to the main organ-represents an exceedingly rare anatomical variant in the gallbladder. This developmental anomaly warrants careful clinical attention due to its potential surgical implications. We report a case of a 38-year-old female who presented to the emergency department with progressive abdominal pain, nausea, and vomiting. Ultrasonographic evaluation revealed a solitary 15-mm gallstone without evidence of gallbladder wall inflammation. Following laparoscopic cholecystectomy, histologic

Ectopic Pancreatic Tissue in the Gallbladder.

Background: Ectopic pancreatic tissue, also referred to as heterotopic pancreas, is defined as the presence of pancreatic tissue in an organ outside the pancreas. Ectopic pancreatic tissue, a rare embryologic abnormality, has been reported in the stomach, duodenum, colon, and Meckel diverticulum and is usually discovered incidentally. Case Report: We report a case of ectopic pancreatic tissue in the gallbladder of a 37-year-old female who underwent a cholecystectomy after a clinical diagnosis of chronic cholecystitis. Histopathologic findings were chronic cholecystitis with cholesterolosis and

Histopathological Evaluation of Gallbladder Specimens Obtained From Living Liver Donors.

BACKGROUND: Cholecystectomy is routinely performed during living donor hepatectomy both to see the structure of the biliary tract and to determine the demarcation line based on the biliary tract junction. This study aims to present the general histopathological features of the gallbladder specimen obtained from living liver donors (LLD). METHODS: Data from 2577 LLDs who underwent living donor hepatectomy (n = 2511) or aborted living donor hepatectomy (n = 66) in our Liver Transplantation Institute between September 2005 and June 2021 were analyzed retrospectively. Acute Recurrent Pancreatitis With Exclusion of Biliary Causes in a Young Female Patient.

Biliary pathologies are common causes of acute pancreatitis, including gallbladder cholesterolosis and gallstone pancreatitis. Nevertheless, after these two pathologies have been excluded, a broader differential and less common etiologies must be considered. Here we report an 18-year-old female with preliminary diagnosis of gallstone pancreatitis who underwent cholecystectomy and intraoperative cholangiogram resulting in uneventful recovery and resolution of symptoms. She returned three times within the month with symptoms of acute pancreatitis. We discussed alternative etiologies of her pancr

Traditional Serrated Adenoma of the Gallbladder, a Case Report.

While overwhelming majority of laparoscopic cholecystectomy specimens performed for gallstones or cholecystitis show rather typical findings, sometimes polypoid structures are also removed. These can be related to cholesterolosis or conventional adenomas, but occasionally extraordinary findings do emerge. In our case, a 67-year old lady with typical complaints of cholecystitis underwent routine laparoscopic cholecystectomy. Preoperative ultrasound revealed a polypoid mass with inflammation and without suspicion for malignancy. Microscopic examination showed partly conventional, low-grade dyspl

Pediatric Normokinetic Biliary Dyskinesia: Pain with Cholecystokinin on Hepatobiliary Iminodiacetic Acid Scan Predictive of Symptom Resolution After Cholecystectomy.

Introduction: Biliary dyskinesia is typically defined as a gallbladder ejection fraction (EF) <35% on hepatobiliary iminodiacetic acid scan with cholecystokinin stimulation (CCK-HIDA) testing. Cholecystectomy often leads to resolution of associated biliary colic symptoms. Alternatively, there is a subset of symptomatic patients with normal gallbladder EF on CCK-HIDA. It has been proposed that pain with CCK injection is more predictive of symptom resolution after cholecystectomy than low gallbladder EF. We reviewed our experience with pediatric patients with positive CCK provocation testing and

Spectrum of imaging findings in hyperplastic cholecystosis and potential mimics.

Adenomyomatosis and cholesterolosis of the gallbladder, collectively termed hyperplastic cholecystosis, are commonly encountered incidental findings on imaging studies performed for a variety of indications including biliary colic or nonspecific abdominal pain. These pathologies are rarely the source of symptoms, generally considered benign and do not require further work-up. However, their imaging characteristics can overlap with more sinister conditions that should not be missed. In this review, the imaging findings of adenomyomatosis and cholesterolosis will be reviewed followed by other ga

Gallbladder polyps ultrasound: what the sonographer needs to know.

Gallbladder polyps are protuberances of the gallbladder wall projecting into the lumen. They are usually incidentally found during abdominal sonography or diagnosed on histopathology of a surgery specimen, with an estimated prevalence of up to 9.5% of patients. Gallbladder polyps are not mobile and do not demonstrate posterior acoustic shadowing; they may be sessile or pedunculated. Gallbladder polyps may be divided into pseudopolyps and true polyps. Pseudopolyps are benign and include cholesterolosis, cholesterinic polyps, inflammatory polyps, and localised adenomyomatosis. True gallbladder p

Outcomes After Laparoscopic Cholecystectomy in Hyperkinetic Biliary Dyskinesia.

BACKGROUND: Biliary dyskinesia (BD) is a poorly understood functional gallbladder disorder. Diagnosis is made with abdominal pain and an intact gallbladder without signs of anatomical obstruction on imaging or pathology. Our aim was to assess whether laparoscopic cholecystectomy (LC) resolves hyperkinetic BD symptoms. METHODS: Records of patients ≥18 years of age, who underwent LC by four surgeons at a tertiary care center between 2012 and 2020, were retrospectively reviewed. Patients were excluded if they had a documented gallbladder ejection fraction (GBEF) <80% or had biliary stones or slu

Systematic review and meta-analyses of cholecystectomy as a treatment of biliary hyperkinesia.

Biliary hyperkinesia is typically diagnosed in patients with biliary-like pain and no evidence of gall stones on imaging modalities but who have had biliary scintigraphy scan (HIDA) that shows ejection fraction ≥ 80%. This study aims to identify whether the removal of the gall bladder can alleviate the symptoms associated with biliary hyperkinesia. Systematic search following PRISMA guidelines was done from inception to January 2020 using PubMed/Medline, OVID, Embase, Cochrane database of systemic reviews, Cochrane central register of controlled trials, The Database of Abstracts of Reviews of
