Establishment of In Vivo Mouse Models to Study Salmonella-Induced Chronic Infection and Gastrointestinal Cancer.

Salmonella infection increases the risk of chronic diseases, e.g., inflammatory bowel diseases, colon cancer, and gallbladder cancer. However, the mechanisms leading to the development of these conditions are still not completely understood. Here, we describe the methods to establish in vivo mouse models to study Salmonella-induced chronic infection and colon cancer. These models will allow us to investigate the initiation and progression of diseases.

Implementation of a Bail-out Strategy in Difficult Laparoscopic Cholecystectomy.

BACKGROUND AND OBJECTIVES: Laparoscopic cholecystectomy is the standard treatment for benign gallbladder disease; however, difficult cases may increase the risk of biliary and vascular injury. This study evaluated surgical approach patterns and major injury rates after adoption of a structured bail-out approach. METHODS: This retrospective cohort study included adult patients who underwent cholecystectomy between Sep 2012 and Jul 2025. Patients were divided into a preimplementation period before Jun 2022 and a bail-out period from Jun 2022 to Jul 2025. Demographic characteristics, surgical ap

Navigating the Pancreatic Duct: Up-to-Date Radiologic Expedition through Intraluminal Pathologic Entities.

Several congenital, traumatic, infectious, inflammatory, and neoplastic processes, as well as iatrogenic interventions, can alter intraluminal contents of the pancreatic duct, leading to adverse outcomes. Variant ductal anatomy may result in clinically significant complications such as duodenal obstruction, acute and chronic pancreatitis, and increased risk of developing gallbladder or biliary malignancies. The presence of calculi, blood, pus, and other unintended iatrogenic material in the pancreatic duct can be associated with a range of complications. Neoplastic involvement of the pancreati

Nutritional-inflammatory indices and gallstone risk: complementary insights from hospital-based and community-based cohorts in China.

BACKGROUND: Gallstone is a common digestive disorder closely associated with inflammatory responses and nutritional metabolic disturbances. However, evidence regarding the association between composite nutritional-inflammatory indices and gallstone risk remains limited. This study aims to investigate the association of albumin/neutrophil to lymphocyte ratio (ANLR) and neutrophil percentage to albumin ratio (NPAR) with gallstone prevalence using two independent cohorts from China. METHODS: This cross-sectional study analyzed two cohorts: a hospital-based cohort (Zibo Central Hospital, 2014-202

Total and beverage-specific alcohol use at mid-life and all-cause and cause-specific mortality: a prospective cohort study.

BACKGROUND: While detrimental health impacts of excessive alcohol use are well-established, potential effects of low-to-moderate drinking remain controversial. We examined the associations between total and beverage-specific alcohol intake and mortality, including cardiovascular disease (CVD), cancer (e.g., 20 site-specific cancers), and other mortality. METHODS: We included 340,924 participants from UK Biobank (2006-2022). Alcohol use was categorized as: never or occasional alcohol consumption (≤ 20 g/week), low consumption (> 20 g/week to ≤ 20 g/d for men, > 20 g/week to ≤ 10 g/d for women)

Reevaluating the Role and Timing of Fever in Acute Cholecystitis.

INTRODUCTION: Acute cholecystitis is diagnosed based on clinical and diagnostic findings. Fever is commonly considered a sign of systemic inflammation in acute cholecystitis, although its diagnostic performance may vary depending on the timing of presentation. This study analyzed the prevalence of fever as a diagnostic parameter for acute cholecystitis, according to the symptom onset at the time of medical consultation (≤ 24 hours versus > 24 hours), and its potential association with both medical and surgical complications. METHODS: We conducted a retrospective cohort study in two urban, ter

A cross-sectional analysis and forward projection of China's gallbladder and biliary-tract cancer burden: Insights from the 2021 Global Burden of Disease Study.

Gallbladder and biliary tract cancer (GBTC) remains highly lethal. We examined 32-year epidemiological trends in China and projected outcomes to 2050. Using 2021 Global Burden of Disease (GBD) data, we extracted annual GBTC incident and prevalent cases, deaths, and disability-adjusted life years (DALYs) for 1990 to 2021, stratified by sex, age, and socio-demographic index (SDI). The age-standardized rates (ASRs) were calculated, and ARIMA models forecast 2050 incidence and mortality. Globally, GBTC risk rises after age 60 and correlates with SDI (R = 0.588). In China, cases increased from 17,0

Radiographic Findings Concerning for Gangrenous Cholecystitis in an Asymptomatic Elderly Patient: A Case for Conservative Management.

Gallbladder necrosis is a rare but life-threatening complication of acute cholecystitis, likely resulting from chronic blood flow, inflammation, and intraluminal pressure. It most commonly occurs in elderly or critically ill patients with significant comorbidities and is often underreported due to its nonspecific presentation. This diagnosis is usually associated with right upper quadrant pain, leukocytosis, and fever. Here, we present an 80-year-old woman with metastatic lung cancer, COPD, and an extensive surgical and oncologic history who was incidentally found to have radiographic findings

PEGylated fucoxanthin liposomes remodel G0/G1 cell cycle homeostasis and suppress gallbladder cancer progression via modulation of NR2F1-mediated signaling networks.

PURPOSE: Gallbladder cancer (GBC) is a highly aggressive and refractory biliary malignancy with disordered proliferative homeostasis, rapid malignant progression and scarce effective clinical therapies. Marine fucoxanthin possesses prominent anti-tumor activity, whereas its poor water solubility and low bioavailability limit application, and its core biomolecular mechanism against GBC remains poorly clarified. This study aimed to construct PEGylated fucoxanthin liposomes and elucidate their anti-tumor mechanism targeting biomolecular dynamics and core regulatory targets in GBC. METHODS: The a

Clinical value of C-reactive protein as a predictor of conversion of laparoscopic cholecystectomy to open surgery: a systematic review and meta-analysis.

BACKGROUND: Laparoscopic cholecystectomy is the gold standard for gallbladder disease, yet conversion to open surgery remains a challenge. Unplanned conversion from laparoscopic cholecystectomy to open surgery is associated with increased morbidity and potential patient safety risks. Accurate preoperative predictors may improve surgical planning and enhance patient safety. This meta-analysis evaluates the utility of preoperative C-reactive protein (CRP) as an objective biomarker for risk stratification. METHODS: A systematic review was conducted according to PRISMA guidelines. The search was

Sex-stratified association between systemic immune-inflammation index and gallstone risk: a cross-sectional study in a Chinese adult population.

INTRODUCTION: Gallstone disease is a prevalent biliary condition with rising incidence in China, driven by changes in lifestyle, metabolism, and inflammation. The systemic immune-inflammation index (SII), a composite marker reflecting immune and inflammatory status, has been linked to various chronic diseases. However, its association with gallstone in the general Chinese population remains unclear. This study aimed to investigate the relationship between SII and gallstone risk in a large adult cohort. METHODS: A cross-sectional analysis was conducted among 55,241 adults undergoing health scr

PTCD and Peroral Electronic Cholangioscopy with Holmium Laser Lithotripsy for CBD Stones in a Super-Elderly Patient Post-Billroth II Gastrojejunostomy.

The management of impacted common bile duct stones in patients with prior Billroth II gastrectomy poses unique challenges. Altered gastrointestinal anatomy renders duodenoscope passage and biliary cannulation during conventional ERCP exceedingly difficult, resulting in a low success rate. Super‑elderly (over 90 years old) patients often have multiple comorbidities such as cardiovascular diseases and require rapid relief of biliary obstruction and infection control to create a subsequent therapeutic window. Meanwhile, traditional open biliary surgery and general anesthesia carry prohibitive ris

Robotic Cholecystectomy Remains Safe and Effective After Regular Hours: A 3-Year Update.

BACKGROUND AND OBJECTIVES: Despite growing popularity in robotic-assisted surgery, access during evenings and weekends for nonelective operations may be restricted. A prior smaller comparison study in 2024 found no basis for safety concerns in managing acute cholecystitis in this setting. We sought to provide additional data to further evaluate whether restriction of robotic cholecystectomy during "after-hours" is justified. METHODS: We performed a retrospective analysis of 161 patients undergoing robotic cholecystectomy for acute cholecystitis according to 2018 Tokyo criteria by a single sur

PSMC2 Promotes Gallbladder Carcinoma Progression by Regulating the AKT/Cyclin D1/CDK6 Signaling Axis.

Gallbladder cancer (GBC) is a prevalent and aggressive malignancy of the biliary system. Studies have revealed that Proteasome 26S subunit ATPase 2 (PSMC2) is overexpressed and associated with the development process of several cancers. However, the specific role in GBC remains unknown. This study aims to explore the association between PSMC2 and GBC pathogenesis. We assessed PSMC2 expression from normal and GBC patient samples. Using Cell Counting Kit-8 (CCK-8), colony formation, scratch, and Transwell migration assays, we investigated the effects of PSMC2 on the proliferation and migration o
