Classification of Biliary Strictures Using Real-Time Cholangioscopy Artificial Intelligence: The SMART-AI Trial.

BACKGROUND & AIMS: Sampling techniques have poor accuracy for classifying biliary strictures as benign or malignant. Previously, a cholangioscopy artificial intelligence (AI) outperformed sampling techniques based solely on analysis of previously recorded cholangioscopy footage. The aim of this single-center, prospective trial was to compare the performance of a real-time cholangioscopy AI to both sampling techniques and human observers for the task of biliary stricture classification. METHODS: A cholangioscopy AI computer connected directly to a cholangioscope console. The computer analyzed

Imaging review of aortic fistulas.

Aortic fistulas represent rare but life-threatening communications between the aorta and the adjacent structures, most commonly the gastrointestinal tract. They are classified as primary, arising spontaneously in the setting of a native, diseased aorta, or secondary to prior aortic surgery or endovascular repair. Clinical presentation is often variable and nonspecific-ranging from gastrointestinal bleeding, sepsis, abdominal pain to hemodynamic collapse-making imaging pivotal for diagnosis. Computed tomography angiography is the preferred imaging modality. Imaging features such as visualizatio

MRCP-derived radiomics features associated with biliary dilation: a multi-step feature reduction, multivariable regression, and diagnostic classification study.

PURPOSE: To identify MRCP-derived radiomics features associated with image-defined biliary dilation, evaluate their independence from demographic confounders, and assess diagnostic classification performance, including in borderline-caliber cases. METHODS: In this retrospective cross-sectional study, 135 patients (dilated: n = 60; non-dilated: n = 75) who underwent clinically indicated MRCP (September 2025-January 2026) were included. Two blinded radiologists independently classified biliary dilation (Cohen's kappa = 0.87). Three-dimensional CBD segmentation was performed in 3D Slicer (v5.10.

Is R1 Extended Liver Resection for Locally Advanced Intrahepatic Cholangiocarcinoma Justified? Nodal Status Not Margin Drives Prognosis.

BACKGROUND AND PURPOSE: Surgery for locally advanced intrahepatic cholangiocarcinoma (LAICC) requires extended liver resections, often associated with vascular and/or biliary reconstruction. The benefits of these high-risk operations are still debated. The objective of this study was to analyze short, long-term outcomes, and futility after surgery for LAICC. METHODS: A retrospective single-center study on the 2013-2024 period was conducted. LAICC was defined as mass-forming intrahepatic tumors 5 cm or more in size with hepatic vein/IVC contact or hepatic hilum contact or both and requiring ex

Tuning Excited-State TICT Dynamics in Hemicyanine Type I Photosensitizers for Hypoxia-Tolerant Photodynamic Therapy of Cholangiocarcinoma.

Although photodynamic therapy (PDT) has emerged as a promising treatment for unresectable cholangiocarcinoma, the hypoxic tumor microenvironment severely limits oxygen-dependent Type II photochemical processes, resulting in insufficient reactive oxygen species (ROS) generation and compromised therapeutic outcomes. Therefore, developing Type I photosensitizers (PSs) with optimized excited-state dynamics and efficient hypoxia-tolerant ROS generation is critical for improving PDT efficacy under hypoxic conditions. Herein, we developed three hemicyanine-based Type I PSs (HCy1-3) and systematically

Survival impact of involvement of lymph node stations 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic and periampullary cancers (PANODE): prospective multicentre study.

BACKGROUND: Current guidelines do not recommend routine sampling of lymph node stations (LNS) 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic ductal adenocarcinoma (PDAC) and other periampullary cancers because prospective multicentre studies are lacking. This study evaluated the association between the involvement of these LNS and overall survival (OS). METHODS: This prospective multicentre study included patients undergoing pancreatoduodenectomy for PDAC, distal cholangiocarcinoma, ampullary cancer, or duodenal cancer between October 2015 and April 2020. Associations between inv

AASLD AGA clinical Practice Guideline on management of primary biliary cholangitis.

BACKGROUND AIMS: Primary biliary cholangitis (PBC) is a chronic cholestatic liver disease associated with progressive fibrosis, cirrhosis, liver transplantation, and premature mortality. Despite effective therapies, important challenges remain in the diagnosis and management of PBC, including identification of antimitochondrial antibody-negative disease, assessment of response to ursodeoxycholic acid (UDCA), risk stratification, treatment of patients with inadequate response, management of cholestatic pruritus, and prevention of recurrent disease after liver transplantation. Preoperative Frailty and Risk of Post-ERCP Pancreatitis in Older Adults with Choledocholithiasis.

BACKGROUND: Frailty is increasingly considered when evaluating surgical risk in older adults. However, evidence regarding whether frailty contributes to the risk of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis in elderly patients is limited. OBJECTIVE: We aimed to evaluate whether preoperative frailty was associated with the development of post-ERCP pancreatitis (PEP) in elderly patients undergoing ERCP. METHODS: Patients aged 65 years or older who underwent ERCP for choledocholithiasis were retrospectively included. Frailty was assessed preoperatively using an ele

Implementation of a Bail-out Strategy in Difficult Laparoscopic Cholecystectomy.

BACKGROUND AND OBJECTIVES: Laparoscopic cholecystectomy is the standard treatment for benign gallbladder disease; however, difficult cases may increase the risk of biliary and vascular injury. This study evaluated surgical approach patterns and major injury rates after adoption of a structured bail-out approach. METHODS: This retrospective cohort study included adult patients who underwent cholecystectomy between Sep 2012 and Jul 2025. Patients were divided into a preimplementation period before Jun 2022 and a bail-out period from Jun 2022 to Jul 2025. Demographic characteristics, surgical ap

Endoscopic retrograde cholangiopancreatography-related pancreatitis and cholangitis differ according to biliary stricture anatomy: a cohort study.

OBJECTIVE: Post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) and cholangitis are common complications. We evaluated whether stricture anatomy affects ERCP-related complications in patients with biliary strictures. METHODS: From a prospectively maintained ERCP database, 1333 procedures with biliary strictures were screened. Patients with prior sphincterotomy, acute gallstone pancreatitis, chronic pancreatitis, postsurgical biliary strictures, or periampullary adenomas or carcinomas were excluded. Epidemiological, clinical, laboratory, radiological, and therapeutic d

Minimally invasive management of pediatric hereditary spherocytosis: severity-stratified biliary risk and indications for simultaneous cholecystectomy and biliary intervention at laparoscopic splenectomy - a single-center retrospective study.

BACKGROUND: Hereditary spherocytosis (HS) is an inherited hemolytic anemia in which splenectomy may be indicated in moderate to severe disease. Chronic hemolysis increases the risk of cholelithiasis and biliary complications, but optimal timing of cholecystectomy remains uncertain. METHODS: A retrospective single-center analysis of pediatric patients with moderate to severe HS who underwent laparoscopic splenectomy between January 2017 and January 2024. Evaluated variables included disease severity, demographics, cholelithiasis, biliary manifestations, requirement for endoscopic retrograde ch

Confluence Stones at the Junction of the Cystic Duct and an Aberrant Anterior Sectoral Bile Duct: A Case Report.

INTRODUCTION: Confluence stones usually arise when gallstones impacted in the cystic duct migrate toward the biliary confluence. Surgical treatment can be technically demanding because of chronic inflammation and distorted anatomy. Confluence stones at the junction between the cystic duct and an aberrant bile duct are extremely rare. We report a successfully treated case of confluence stones arising at the junction of the cystic duct and an aberrant anterior sectoral bile duct with a communicating accessory bile duct. CASE PRESENTATION: A 38-year-old woman presented with abdominal pain, and b

Decoding Atrial Fibrillation Heterogeneity: A Nationwide Phenome-Wide Association Study in England.

BACKGROUND: Age and sex influence atrial fibrillation (AF) prevalence and outcomes, yet comprehensive data on sex- and age-specific health care utilization in patients newly diagnosed with AF remain limited. OBJECTIVES: We aimed to address this gap using a phenome-wide association study (Phe-WAS) approach. METHODS: We conducted a retrospective cohort study using nationwide Clinical Practice Research Datalink data linked to Hospital Episodes Statistics. Adults diagnosed with AF between 1998 and 2016 (n = 199,433) were matched 1:1 by age and sex to non-AF control subjects. Primary and secondar

Nutritional-inflammatory indices and gallstone risk: complementary insights from hospital-based and community-based cohorts in China.

BACKGROUND: Gallstone is a common digestive disorder closely associated with inflammatory responses and nutritional metabolic disturbances. However, evidence regarding the association between composite nutritional-inflammatory indices and gallstone risk remains limited. This study aims to investigate the association of albumin/neutrophil to lymphocyte ratio (ANLR) and neutrophil percentage to albumin ratio (NPAR) with gallstone prevalence using two independent cohorts from China. METHODS: This cross-sectional study analyzed two cohorts: a hospital-based cohort (Zibo Central Hospital, 2014-202
