Tumor-related local complications as a major contributor to mortality after definitive radiotherapy for unresectable extrahepatic bile duct cancer.

PURPOSE: This study investigated oncologic outcomes and causes of death after radiotherapy in patients with unresectable extrahepatic bile duct cancer. METHODS: We retrospectively analyzed 74 patients with localized unresectable or inoperable extrahepatic bile duct cancer treated with conventionally fractionated radiotherapy between September 2005 and February 2019. Fifty-five patients (74.3%) underwent three-dimensional conformal radiotherapy, and 19 patients (25.7%) received intensity-modulated radiotherapy. Induction chemotherapy was administered in 18 patients and 57 patients received con

Recurrent primary sclerosing cholangitis after liver transplantation: Impact of inflammatory bowel disease on graft outcomes in a Southern European multicenter cohort.

Primary sclerosing cholangitis (PSC) is an established indication for liver transplantation (LT), but disease recurrence (rPSC) and the course of associated inflammatory bowel disease (IBD) may influence post-transplant outcomes. This study aimed to determine the prevalence of rPSC and evaluate associated risk factors in a Mediterranean population. We conducted a multicenter retrospective study including PSC patients who underwent LT across 14 transplant centers in Southern Europe. Clinical characteristics, IBD phenotype and activity, immunosuppressive therapy, colectomy status, and post-LT ou

Molecular biomarkers for diagnosis, prognosis, precision therapy, and immunotherapy in cholangiocarcinoma, cHCC-CCA, and HCC.

Primary liver cancers include hepatocellular carcinoma (HCC), intrahepatic cholangiocarcinoma (iCCA), combined hepatocellular-cholangiocarcinoma (cHCC-CCA), and rarer hepatic malignancies. Their biomarker landscapes differ substantially. HCC has many approved systemic therapies, yet most are still selected through clinical context, liver function, tumor burden, and treatment line. By contrast, iCCA and broader biliary tract cancers (BTCs) have fewer patients but multiple treatment-directing genomic alterations, including FGFR2 fusions or rearrangements, IDH1 mutations, HER2 positivity, BRAF V6

Biomarkers of subclinical cholestasis predict cancer immunotherapy outcomes.

In a recent paper published in Cancer Cell, we identified subclinical cholestasis as an accessible imprint of gut dysbiosis predicting resistance to immune checkpoint blockade. Our work connects γ-glutamyl transferase, bile-acid remodeling, and MAdCAM-1 loss to compromised antitumor immunity, opening avenues for novel biomarker-guided microbiota interventions.

Cynaroside Attenuates Ethinylestradiol-Induced Cholestatic Liver Injury by Modulating the FXR/BSEP/CYP7A1/SHP Axis.

Cholestatic liver diseases are characterized by disrupted bile acid homeostasis, oxidative stress, and inflammation, ultimately leading to progressive hepatic injury. Although ursodeoxycholic acid (UDCA) remains the standard pharmacological therapy, its therapeutic efficacy is limited in several cholestatic conditions. This study investigated the hepatoprotective effects of cynaroside (CYN), a naturally occurring flavonoid glycoside, against ethinylestradiol (EE)-induced cholestatic liver injury and explored its underlying molecular mechanisms. Female Wistar rats were randomly allocated into f

Lipid-Adjusted Vitamin E Status in Children with Chronic Liver Disease: Reclassification Patterns and Differences According to Cholestasis and Liver Transplantation.

BACKGROUND: Serum α-tocopherol depends on lipids, so absolute concentrations misclassify vitamin E status in children with chronic liver disease (CLD). The CALIPER program defined lipid-adjusted E:cholesterol (E:C) and E:triglyceride (E:TG) ratios, but their application in mixed-etiology pediatric CLD using immunoassay has not been reported. METHODS: Exploratory single-center cross-sectional study of 66 children (observed age range 4 months-17.9 years) with mixed-etiology CLD. Retinol and total vitamin E were measured by ELISA, whereas the CALIPER reference intervals applied here were establi

Integrative therapy with Qinghao Biejia Decoction improves survival after resection of intrahepatic cholangiocarcinoma.

Intrahepatic cholangiocarcinoma (ICC) is characterized by high postoperative recurrence and limited benefit from conventional adjuvant therapy. Qinghao Biejia Decoction (QHBJD), a classical traditional Chinese medicine formula, has demonstrated potential anticancer effects. This study investigated the molecular mechanisms of QHBJD in ICC and evaluated its impact on postoperative survival, with a focus on AKT1 expression. In this retrospective study of 60 patients with intrahepatic cholangiocarcinoma (ICC), network pharmacology and bioinformatics analyses were used to identify candidate targets

Durable Response to Vebreltinib in Heavily Pretreated MET-Amplified Intrahepatic Cholangiocarcinoma: A Case Report.

BACKGROUND: MET amplification, although detected in only 2% to 5% of intrahepatic cholangiocarcinomas (ICC), represents an actionable oncogenic driver. Treatment options after failure of first-line immunochemotherapy remain limited, and clinical data on vebreltinib, a novel highly selective MET inhibitor, in this molecular subset are scarce. CASE PRESENTATION: A 61-year-old woman with metastatic ICC harboring high-level MET amplification (tissue gene copy number, 26.79) experienced disease progression on first-line durvalumab plus gemcitabine/cisplatin, second-line liposomal irinotecan/fluoro

Real-world outcomes in circulating tumor DNA-detected isocitrate dehydrogenase 1-mutant cholangiocarcinoma: First-line gemcitabine and cisplatin with versus without immunotherapy.

BACKGROUND: Comprehensive genomic profiling (CGP) is recommended for advanced cholangiocarcinoma (aCCA). Circulating tumor DNA (ctDNA) testing provides a noninvasive alternative with concordance to tissue. Standard first-line (1L) therapy is gemcitabine and cisplatin (GemCis) with or without immune checkpoint inhibitors (ICIs). Data suggest that patients with isocitrate dehydrogenase 1 (IDH1) mutations may derive less benefit from ICIs. This study evaluated outcomes with 1L GemCis versus GemCis + ICI in ctDNA-identified IDH1-mutant (IDH1+) aCCA. METHODS: Real-world data were analyzed from the

[Modified Endoscopic Biliary Stent as a Bridging Strategy in Complex Biliary Strictures].

INTRODUCTION: Endoscopic therapy represents the first-line strategy for the management of biliary strictures. However, strictures associated with altered biliary anatomy (particularly those located near the hepatic hilum or with angulated courses) may hamper the placement of conventional stents, especially when dedicated devices are unavailable. OBJECTIVE: To evaluate the technical feasibility and early clinical outcomes of using a modified endoscopic biliary stent as a temporary drainage strategy for the management of complex biliary strictures. MATERIALS AND METHODS: A retrospective study
