Microbiome Targeted Therapies for Primary Sclerosing Cholangitis.

Primary sclerosing cholangitis is a rare, progressive cholestatic liver disease of the intrahepatic and extrahepatic bile ducts with poor prognosis. The only life-extending treatment for PSC is liver transplantation, with up to 25% of post-transplant patients developing recurrent disease, highlighting an extrahepatic influence on disease. The gut-liver axis has been widely studied in the development and pathogenesis of PSC, with preclinical and clinical work demonstrating that targeting gut microbiota and its metabolites can elicit therapeutic effects against PSC. This review describes various

Tocilizumab for active rheumatoid arthritis in a patient with concomitant primary biliary cholangitis: a case based review.

The coexistence of active rheumatoid arthritis and active primary biliary cholangitis represents a distinct therapeutic challenge. Evidence-based guidance for treatment selection in this clinical setting remains limited, and published experience with tocilizumab is restricted to isolated case reports. We report a 59-year-old woman with highly active, seropositive rheumatoid arthritis and marked cholestatic biochemical activity, in whom diagnostic evaluation revealed active primary biliary cholangitis in the absence of advanced liver fibrosis. Following multidisciplinary assessment, methotrexat

Noninvasive Biomarkers for Early Detection of Primary Sclerosing Cholangitis in Ulcerative Colitis.

Primary sclerosing cholangitis (PSC) is a progressive cholestatic liver disease strongly associated with ulcerative colitis (UC). Although uncommon, PSC-associated ulcerative colitis (PSCUC) represents a distinct subtype of inflammatory bowel diseases characterized by a higher risk of colorectal and hepatobiliary cancers. Early diagnosis remains a challenging issue because of the lack of specific biomarkers. Bioinformatic analysis of Gene Expression Omnibus (GEO) datasets identified potential, specific, and noninvasive biomarkers via differential gene expression and functional enrichment. Afte

Rethinking Pathogenesis of Primary Sclerosing Cholangitis.

Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease characterized by progressive fibroinflammatory injury of intrahepatic and extrahepatic bile ducts that may lead to cirrhosis, hepatobiliary malignancy, and death, with liver transplantation representing the only effective intervention in end-stage disease. Rather than a single dominant pathomechanism, the data support a model in which four axes - bile acid metabolism, the intestinal microbiome and gut barrier, biliary epithelial dysfunction, and Th17-skewed immune dysregulation - are reciprocally coupled and converge o

IgG N-glycosylation in autoimmune diseases.

IgG is the most abundant antibody in the human circulation. It carries a single conserved glycosylation site, Asparagine 297, in its Fc domain. This N-glycan is critical for the ability of the antibody to activate immune cells via binding to, and cross-linking Fc receptors in order to elicit functional responses. First documented in the context of rheumatoid arthritis (RA), the IgG N-glycan may undergo changes that are correlated to host health. We present a narrative in-depth comparison of changes incurred by IgG N-glycans in autoimmune diseases; RA, systemic lupus erythematosus, multiple scl

Inter-α-inhibitor heavy chain 4 levels in incident and prevalent patients with primary biliary cholangitis.

BACKGROUND: Primary biliary cholangitis (PBC) is an autoimmune liver disease characterised by progressive bile duct injury. We recently showed that inter-α-inhibitor heavy chain 4 (ITIH4) levels are elevated in patients with PBC. Here, we investigated the potential of ITIH4 as a biomarker for disease severity and treatment response. METHODS: We included 237 patients with primary biliary cholangitis, 80 incident (newly diagnosed), 157 prevalent (with established disease) and 100 healthy controls. All patients were followed for 12 months during initiation or continuation of ursodeoxycholic acid

Nationwide outcomes of biliary atresia after Kasai portoenterostomy in Taiwan: the impact of postoperative cholangitis and corticosteroid use.

PURPOSE: To provide updated nationwide outcomes of biliary atresia (BA) among patients in Taiwan, focusing on postoperative cholangitis, liver transplantation (LT), corticosteroid use, and renal outcomes. METHODS: Infants with BA who underwent Kasai portoenterostomy were identified from the Taiwan National Health Insurance Research Database (2008-2019). Postoperative outcomes were evaluated, and the association between cholangitis episodes and LT was analyzed using exploratory logistic regression analysis. Corticosteroid exposure was categorized by duration, and acute kidney injury was assess

Self-management strategies for symptom burden in primary biliary cholangitis are associated with improved health-related quality of life.

Introduction PBC patients can experience severe morbidity and extrahepatic symptoms leading to impaired HRQoL; it is proposed that this is due to symptom burdens rather than disease morbidity. There are few medical interventions for PBC symptoms. Many patients rely on self-management strategies to improve symptom burden. However, there is limited research exploring the prevalence and impact of PBC symptom burden on HRQoL. Study aims: 1. analyse the influence of extrahepatic symptoms on HRQoL; 2. measure the effect of self-management strategies on symptom burden and HRQoL. Methods PBC patients

Chemotherapy-induced secondary sclerosing cholangitis mimicking primary sclerosing cholangitis: a case report.

Secondary sclerosing cholangitis (SSC) is a rare condition that may mimic primary sclerosing cholangitis. Chemotherapy-related biliary injury is an emerging cause. We present a 72-year-old male with gastric carcinoma who developed cholestatic liver injury following oxaliplatin-based chemotherapy. Imaging revealed intrahepatic biliary dilatation without an obstructing lesion. Percutaneous transhepatic cholangiography confirmed biliary abnormalities. The patient was managed with biliary drainage and supportive care, resulting in gradual clinical and biochemical improvement. Chemotherapy-induced

Tumor-related local complications as a major contributor to mortality after definitive radiotherapy for unresectable extrahepatic bile duct cancer.

PURPOSE: This study investigated oncologic outcomes and causes of death after radiotherapy in patients with unresectable extrahepatic bile duct cancer. METHODS: We retrospectively analyzed 74 patients with localized unresectable or inoperable extrahepatic bile duct cancer treated with conventionally fractionated radiotherapy between September 2005 and February 2019. Fifty-five patients (74.3%) underwent three-dimensional conformal radiotherapy, and 19 patients (25.7%) received intensity-modulated radiotherapy. Induction chemotherapy was administered in 18 patients and 57 patients received con

Recurrent primary sclerosing cholangitis after liver transplantation: Impact of inflammatory bowel disease on graft outcomes in a Southern European multicenter cohort.

Primary sclerosing cholangitis (PSC) is an established indication for liver transplantation (LT), but disease recurrence (rPSC) and the course of associated inflammatory bowel disease (IBD) may influence post-transplant outcomes. This study aimed to determine the prevalence of rPSC and evaluate associated risk factors in a Mediterranean population. We conducted a multicenter retrospective study including PSC patients who underwent LT across 14 transplant centers in Southern Europe. Clinical characteristics, IBD phenotype and activity, immunosuppressive therapy, colectomy status, and post-LT ou

[Modified Endoscopic Biliary Stent as a Bridging Strategy in Complex Biliary Strictures].

INTRODUCTION: Endoscopic therapy represents the first-line strategy for the management of biliary strictures. However, strictures associated with altered biliary anatomy (particularly those located near the hepatic hilum or with angulated courses) may hamper the placement of conventional stents, especially when dedicated devices are unavailable. OBJECTIVE: To evaluate the technical feasibility and early clinical outcomes of using a modified endoscopic biliary stent as a temporary drainage strategy for the management of complex biliary strictures. MATERIALS AND METHODS: A retrospective study

Steatotic liver disease in autoimmune liver disease: Prevalence and clinical impact in a Southeast Asian cohort.

BACKGROUND: Steatotic liver disease (SLD) is increasingly common in patients with primary biliary cholangitis (PBC) and autoimmune hepatitis (AIH). We sought to determine the prevalence and clinical impact of SLD among Southeast Asian patients with PBC, AIH, and AIH/PBC overlap syndrome. METHODS: We included all patients diagnosed with PBC, AIH and AIH/PBC overlap syndrome between 2000 and 2024. SLD was defined based on either radiological or histological evidence of hepatic steatosis. The primary outcomes were all-cause mortality, treatment response, liver-related events (LRE), and major adv

Primary Hepatic Mucosa-Associated Lymphoid Tissue Lymphoma and Primary Biliary Cholangitis: A Case Report of a Rare Association.

INTRODUCTION: Primary hepatic mucosa-associated lymphoid tissue (MALT) lymphoma is an exceedingly rare subtype of extranodal marginal zone B-cell lymphoma within the spectrum of non-Hodgkin lymphomas. Chronic inflammatory and autoimmune conditions are recognized risk factors for lymphomagenesis; however, the association between primary hepatic MALT lymphoma and primary biliary cholangitis (PBC) has been rarely reported. CASE PRESENTATION: We report the case of a 50-year-old woman with a 9-year history of non-cirrhotic PBC, treated with ursodeoxycholic acid and with only partial biochemical re

The Great Imitator strikes again: a hepatobiliary puzzle unraveled by a skin biopsy.

Hepatic syphilis is a rare and often overlooked cause of cholestatic liver injury. A 65-year-old male presented with new-onset cholestasis, severe pruritus, weight loss, and marked elevation of CA 19-9. MRCP revealed irregular ductal calibers in a PSC-like pattern with biliary dilatation predominantly on the right side with stricture of the central bile ducts with a central filling defect, initially raising concern for occult cholangiocarcinoma. Histology from cholangioscopic bile duct biopsies demonstrated nonspecific inflammation. Concurrent dermatologic evaluation of a persistent rash led t
