Multimodal Imaging Demonstrates Antifibrotic Effects of Targeting α v β 6 /α v β 1 Integrins in Biliary Fibrosis.

OBJECTIVES: Development of molecular therapies for liver fibrosis is slowed by a lack of noninvasive methods addressing questions of target expression, target engagement, and treatment response. Integrin α v β 6 is a biomarker of liver fibrosis that is upregulated in livers of patients with primary sclerosing cholangitis. It activates latent TGF-β and plays a critical role in regulating extracellular matrix expression, especially collagen. In this study, our aim was to use combined α v β 6 integrin-targeted positron emission tomography (PET) and collagen-specific magnetic resonance imaging (MR

Comparing Laparoscopic and Open Choledochoduodenostomy At a Single Institution.

BACKGROUND: Choledochoduodenostomy (CDD) has been reconsidered as an alternative to choledocholithiasis, especially in cases where endoscopic clearance is difficult. This study aimed to evaluate the use of laparoscopic CDD and compare the results with open CDD, including long-term outcomes. METHODS: The medical records of 65 patients who underwent laparoscopic or open surgery due to the difficulty of endoscopic treatment between 2011 and 2021 were retrospectively analyzed. Laparoscopic CDD was performed in 20 cases during the latter period, from April 2016, and open CDD was performed in 45 ca

The Prognostic Utility of Quantitative Magnetic Resonance Cholangiopancreatography in Patients With PSC: A Systematic Review With Structured Evidence Synthesis.

BACKGROUND: Magnetic resonance cholangiopancreatography (MRCP) is used in the diagnosis and management of primary sclerosing cholangitis (PSC). However, MRCP is subjective, and guidelines state that there is insufficient evidence to recommend MRCP as a prognostic tool. EASL-PSC guidelines have identified quantitative MRCP (MRCP+) as having promising utility for risk assessment and prediction of clinical outcomes. We conducted a systematic review to determine the clinical utility of MRCP+ metrics for the management of patients with PSC. METHODS: We systematically searched PubMed and MEDLINE fo

Risk Factors for Primary Sclerosing Cholangitis Recurrence Following Liver Transplantation: A Multicenter Retrospective Analysis.

BACKGROUND AND AIMS: Primary sclerosing cholangitis recurrence (rPSC) after liver transplantation (LT) is common; however, the factors contributing to rPSC are poorly understood. This study aimed to identify the risk factors for rPSC after LT and determine whether donor type affects rPSC. METHODS: A multicenter retrospective cohort analysis was conducted on 174 patients with PSC who underwent LT between January 2000 and January 2024. Multivariable Cox models were used to evaluate risk factors for rPSC. The rPSC risk for living donor liver transplantation (LDLT) and deceased donor liver transp

DNA Methylation-mediated BTN3A2 Regulation via CD14+CD16+ Monocytes Protects Against Primary Sclerosing Cholangitis.

INTRODUCTION: Primary Sclerosing Cholangitis (PSC) remains a significant challenge in hepatology with an unclear pathogenesis and limited treatment options. This study employed Mendelian Randomization (MR) to explore novel pathogenic mechanisms of PSC. METHODS: We analyzed publicly available datasets, including cis-eQTL, cis-pQTL, 731 immune cell profiles, DNA methylation data, and PSC GWAS summary statistics. Using Inverse Variance Weighted (IVW) as our primary method, we identified genes causally associated with PSC. Subsequent mediation analyses elucidated how DNA methylation regulates gen

Prospective, randomised, placebo-controlled, phase 2 clinical trial assessing the efficacy and safety of oral vancomycin in patients with primary sclerosing cholangitis with/out inflammatory bowel disease in Italy: study protocol of VanC-IT trial.

BACKGROUND: Primary sclerosing cholangitis (PSC) is the classical hepatobiliary manifestation of inflammatory bowel disease (IBD). No therapy currently halts disease progression. The strong gut-liver axis implicated in PSC pathogenesis supports the investigation of microbiome-targeted treatments. Oral vancomycin (OV), an antibiotic with potential immunomodulatory properties, has shown encouraging results in improving clinical symptoms and liver biochemistry in PSC. However, prospective data on its safety and efficacy remain limited. METHODS AND ANALYSIS: Oral Vancomycin for primary sclerosing

Hepatoduodenal ligament teratoma presenting with fever in a young adult.

Hepatoduodenal ligament teratomas are very rare, and there are fewer than 20 cases reported in the literature. A 22-year-old woman with fever and acute abdominal pain due a mature cystic teratoma in the hepatoduodenal ligament. This occurrence when presented with acute cholangitis was the first case of this rare tumor. Previous cases seen in adults were asymptomatic and were diagnosed incidentally on investigations. The mass was successfully removed operation on this patient without affecting the portal triad structures. This case adds to our knowledge of how these rare tumors may present clin

Effect of hands-on versus hands-off assistance on trainee performance in ERCP training: a randomized controlled study.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is intricate and necessitates comprehensive training. The conventional hands-on approach in ERCP training may not be essential for improving trainee performance or ensuring procedure safety. We hypothesized that hands-off assistance would be non-inferior to hands-on assistance in terms of cannulation success by trainees during ERCP training. METHODS: A total of 770 patients with native papilla were randomized to either the hands-off (intervention, n = 389) group, where trainers offered verbal guidance via teleguidance using rea

Biliary disorders associated with immune checkpoint inhibitors: a pharmacovigilance analysis of the FDA adverse event reporting system (FAERS) database.

Immune checkpoint inhibitors (ICIs) are linked to severe biliary disorders (BDs). Using FAERS data (Q1 2015-Q1 2025), we identified 4,405 BD cases via standardized MedDRA queries (144 preferred terms). ICIs were significantly associated with BDs (overall ROR 3.37, 95% CI 3.28-3.47). Anti‑PD‑1/PD‑L1 monotherapy showed higher risk than anti‑CTLA‑4 (ROR 3.19-4.19 vs 2.57), while combination therapy displayed extreme risks for bile duct necrosis (ROR 530.58) and immune‑mediated cholangitis (ROR 550.54). Fatal outcomes occurred in 19.9% (N=875) and were more common in males, patients aged >65 years

Clip-Centered Common Bile Duct Stones Managed by Endoscopic Sphincterotomy Plus Endoscopic Papillary Large Balloon Dilation Years After Cholecystectomy.

Migration of clips used in laparoscopic cholecystectomy (LC) is rare, but when a clip migrates into the bile duct, it can serve as a nucleus for common bile duct stone formation and trigger acute cholangitis. An elderly woman in her 80s underwent LC for cholecystolithiasis at another hospital in year X. Abdominal computed tomography in year X+1 revealed that two clips had migrated into the bile duct, but this was not noted at the time, and she remained asymptomatic. In year X+5, she underwent bioprosthetic aortic valve replacement for severe aortic regurgitation. In year X+6, she presented to

FIRST BRAZILIAN CONSENSUS ON CHOLANGIOPANCREATOSCOPY BY THE BRAZILIAN SOCIETY OF DIGESTIVE ENDOSCOPY (SOBED).

BACKGROUND: Despite the widespread adoption of single-operator cholangiopancreatoscopy for the management of biliary and pancreatic diseases, this method was recently introduced in Brazil. This study aimed to develop a Brazilian consensus for the diagnostic and therapeutic use of single-operator cholangiopancreatoscopy for the management of pancreatobiliary diseases. METHODS: A working group from the Brazilian Society of Digestive Endoscopy (SOBED) specialists, using a Delphi methodology, formulated statements across six topics: indeterminate biliary strictures, complex biliary stones, primar

Fecal calprotectin in patients with concomitant primary sclerosing cholangitis and inflammatory bowel disease: a systematic review and meta-analysis.

BACKGROUND: Primary sclerosing cholangitis (PSC) is strongly associated with inflammatory bowel disease (IBD), with up to 70% of PSC patients having concomitant IBD (PSC-IBD). Fecal Calprotectin (FC) is a validated surrogate biomarker of intestinal inflammation in IBD. Emerging evidence suggests that FC may also reflect biliary inflammation in PSC. AIM: This study aimed to compare FC concentrations in patients with PSC-IBD versus IBD only. METHODS: A systematic literature search was performed (PROSPERO registration no. CRD42024600985). Studies reporting FC levels in both PSC-IBD and IBD-only

Decoding follicular cholangitis: A benign entity with a malignant façade.

Follicular cholangitis (FC) is a rare, benign inflammatory biliary disorder that can closely mimic malignancy. We present the case of a 34-year-old lady with an incidentally detected distal common bile duct (CBD) lesion. Preoperative imaging was suggestive of periampullary malignancy. However, serology and biopsy findings were inconclusive. Intraoperative frozen section analysis revealed cholangitis with prominent lymphoid aggregates, ruling out malignancy. These critical intraoperative histopathological findings guided toward a conservative surgical approach in the form of antegrade CBD excis

Radiomics-based diagnosis of acute cholangitis using non-contrast computed tomography: a preliminary study.

OBJECTIVES: We developed a radiomics model for the diagnosis of acute cholangitis using non-contrast computed tomography (NCCT). MATERIALS AND METHODS: We included 79 patients with acute cholangitis and 79 without known liver disease; all patients underwent NCCT and contrast-enhanced CT (CECT). Patients were randomly divided into a development dataset (n = 118) and a test dataset (n = 40). In total, 999 radiomics features were extracted from each segmented liver. Two experienced radiologists interpreted the NCCT for evidence of acute cholangitis by a consensus-based approach. Subsequently, bo

Successful Stepwise Endoscopic Ultrasound-Guided Cyst Drainage for a Giant Infected Hepatic Cyst: A Case Report.

A 79-year-old man presented with fever and jaundice. Laboratory tests revealed elevated inflammatory markers and hepatobiliary enzymes. Magnetic resonance imaging revealed a 20 × 16 cm giant hepatic cyst compressing the intrahepatic bile ducts. Emergency endoscopic retrograde cholangiopancreatography revealed intrahepatic bile duct dilatation secondary to cystic compression. An endoscopic nasobiliary drainage tube was inserted. After the cholangitis improved, the tube was replaced with a plastic stent. The patient was discharged but was readmitted 11 days later with recurrent fever and loss of
