BACKGROUND AND AIMS: Primary sclerosing cholangitis (PSC) lacks validated surrogate endpoints, impeding the development of new therapies. This study assessed the association between cholestatic biochemical markers and clinical symptoms to evaluate their construct validity as surrogate endpoints.
METHOD: Cholestatic biochemical markers and clinical symptoms measured by the simple cholestatic complaint score were collected from non-end-stage PSC patients that participated in the DILSTENT trial. Associations between each biochemical marker and clinical symptom were explored using a univariable a

Primary sclerosing cholangitis (PSC) is an immune-mediated disease of the biliary tract for which no prognosis-improving therapies are yet available. PSC is characterized by genetic risk factors, alterations in the microbiome and pathological immune activation leading to the development of biliary fibrosis. Cholangiocytes play an active role in the inflammatory processes, as they interact directly with the microbiome and immune cells. The concomitant chronic inflammatory bowel disease has a distinct phenotype and causes a reciprocal modulation of intestinal and hepatic disease activity via cha

Inflammatory bowel diseases (IBD) and primary sclerosing cholangitis (PSC) are chronic inflammatory conditions with limited biomarker-driven diagnostic tools. Proteomic profiling offers a promising approach to uncover specific biomarkers that could refine diagnostic accuracy, monitor disease activity, and guide therapeutic strategies. Our primary aim is to identify novel biomarkers for PSC-IBD and conventional ulcerative colitis (UC) via proteomic approach. The secondary aim is to advance the etiopathogenic understanding of the diseases by linking specific proteomic profiles with disease pheno

BACKGROUND: A robotic endoscopic system was developed to assist in endoscopic retrograde cholangiopancreatography (ERCP), enabling endoscopists to reduce occupational radiation exposure without the need for cumbersome protective gear. This pilot trial evaluated the procedural feasibility and clinical potential of this robotic system.
METHODS: Patients were prospectively recruited at a single tertiary hospital. The primary end points were technical and clinical success rates. The secondary end points included ERCP-related complications, convenience and stability of the robotic system, procedur

To identify novel diagnostic biomarkers for acute pancreatitis (AP) and facilitate the early prediction of severe AP (SAP), this investigation characterized the serum metabolomic profiles of patients across distinct disease phases and integrated metabolomics with artificial intelligence to construct bile acid-based predictive models. The observational protocol was registered with the Chinese Clinical Trial Registry (ChiCTR2000034117) on June 24, 2020. Comparative metabolomic analysis revealed significant alterations in 303 metabolites and 461 lipid species in AP. Subsequent weighted gene coexp

BACKGROUND: Solid organ transplant (SOT) recipients face increased colorectal cancer (CRC) risk due to chronic immunosuppression and comorbidities such as primary sclerosing cholangitis, inflammatory bowel disease, and cystic fibrosis. Despite this elevated risk, CRC screening and treatment guidelines specific to transplant recipients remain limited. This systematic review and meta-analysis evaluated the prevalence, clinical characteristics, risk factors, and outcomes of CRC in SOT recipients.
METHODS: A comprehensive search across major databases identified studies reporting CRC incidence an

BACKGROUND & AIMS: Alkaline phosphatase (ALP) normalization is increasingly recognized as the optimal treatment target in primary biliary cholangitis (PBC). We evaluated racial and ethnic differences in complete biochemical remission (CBR) to ursodeoxycholic acid (UDCA), with a focus on Hispanic populations.
METHODS: We conducted a retrospective study of the East Los Angeles Latinx PBC cohort, which includes predominantly Hispanic patients at the University of Southern California Medical Centers (2006-2022). PBC was defined using the American Association of Liver Disease criteria. Five-year t

BACKGROUND AND AIMS: Primary sclerosing cholangitis (PSC) is a pre-malignant condition with elevated risk of hepatopancreatobiliary cancers (HPBCa) and colorectal cancer (CRC) when inflammatory bowel disease (IBD) is present. We described cancer burden in a contemporary PSC-IBD cohort, and assessed rates of subsequent cancers, liver transplant, and death post-colectomy or post-cholecystectomy status.
METHODS: Using linked health administrative databases, we calculated cause-specific cumulative incidences of HPBCa-related outcomes (diagnoses/deaths) and non-HPBCa-related transplant/death among

BACKGROUND: Management of complete biliary obstruction remains challenging when conventional endoscopic or percutaneous approaches fail. Magnetic compression anastomosis (MCA) has emerged as a minimally invasive alternative; however, the evidence base is limited and methodologically heterogeneous. We conducted a systematic review and meta-analysis to evaluate the efficacy and safety of MCA in benign and malignant biliary obstruction.
METHODS: On August 27, 2025, we conducted a comprehensive search using PubMed, Scopus, Web of Science, and Cochrane Library for studies evaluating MCA in patient

Duodenal perforation is a rare but serious complication of endoscopic retrograde cholangiopancreatography (ERCP), particularly when anatomical distortions, such as large hiatal hernias, complicate endoscopic access. This case describes a 93-year-old patient with a type IV hiatal hernia who suffered an iatrogenic duodenal injury during a procedure for acute cholangitis. Due to the size of the defect and the patient's complex anatomy, an emergent laparoscopic repair was performed to reduce the hernia and repair the perforation. The patient's successful recovery highlights that prompt recognition
