Simple liver cyst complicated by fistula into abdominal wall: a case report.

UNLABELLED: Benign, fluid-filled lesions called simple liver cysts are commonly seen incidentally on imaging. While they are usually asymptomatic, they can cause complications including infection, rupture, or hemorrhage. The formation of a fistula into the abdominal wall is a rare complication, with diagnostic and therapeutic challenges. CASE PRESENTATION: In April 2024, a 29-year-old obese (body mass index 30.56) male presented to Madinat Zayed Hospital with left pneumonia, which was resolved with intravenous antibiotics. A subsequent CT scan, however, showed an 11×10×5 cm hypodense lesion i

Localized cystic dilatation of the intrahepatic bile ducts as a distinct subtype of Caroli disease: a case report.

BACKGROUND: Caroli disease encompasses multiple distinct entities. We report two cases of localized cystic dilatation of the intrahepatic bile ducts exhibiting a unique morphology. CASE PRESENTATIONS: Both patients were Japanese, diagnosed with a cyst in the liver prenatally (27 weeks' gestation) and at 3 months of age. The dilatation was limited to the anterior section branch and its first upstream branches, and the cystically dilated bile ducts abruptly connected to normal-sized ducts on both the upstream and downstream sides. One case transiently had debris in the dilated duct during the n

The Role of Endoscopic Ultrasound in the Diagnosis and Management of Congenital Gastrointestinal and Pancreaticobiliary Conditions.

PURPOSE OF REVIEW: This review synthesizes current literature on the use of Endoscopic ultrasound (EUS) in congenital gastrointestinal (GI) conditions. Special emphasis is placed on pediatric applications, where EUS is increasingly recognized as both feasible and safe. RECENT FINDINGS: EUS has transformed the evaluation and management of congenital anomalies affecting the GI tract and pancreaticobiliary system, and has been shown to be safe and effective in the pediatric population. By combining high-resolution imaging with the ability to perform real-time interventions, EUS addresses diagnost

Spontaneous rupture and hemorrhage of a large simple hepatic cyst following external electrical muscle stimulation: a case managed by emergency laparoscopy.

Hemorrhagic rupture of a simple liver cyst is a rare clinical occurrence. Whether external physical forces can trigger such rupture remains unclear. A 47-year-old woman presented with acute abdominal pain 72 h following an electrical muscle stimulation (EMS) session for weight reduction. Cross-sectional imaging demonstrated a 10-cm ruptured cyst in the right hepatic lobe with associated 1.5 l of hemoperitoneum. Her platelet count was 88 × 109/l. The patient successfully underwent emergency laparoscopic cyst deroofing with povidone-iodine sclerotherapy and did not require blood transfusion. His

Mucin-producing cysts of the liver. Radiologic-pathologic correlation.

Mucin-producing cysts of the liver - mucinous cystic neoplasm (MCN), intraductal papillary neoplasm of the bile duct (B-IPMN), and ciliated foregut cyst (CFC) - are rare lesions with variable biological behavior and significant malignant potential that pose diagnostic and therapeutic challenges.Gadolinium-enhanced MRI with MR cholangiography sequences is the modality of choice for characterizing these entities, offering unparalleled multiparametric capability that integrates morphological, functional, and anatomical biliary information in a single examination. Beyond diagnosis, accurate imagin

Early-onset extrahepatic cholangiocarcinoma in a young adult with a resected congenital biliary cyst: a rare case report.

INTRODUCTION: Congenital choledochal cysts are well-recognized risk factors for subsequent hepatobiliary malignancy, particularly cholangiocarcinoma, even after surgical excision. However, the development of extrahepatic cholangiocarcinoma (eCCA) in early adulthood following neonatal resection of a congenital biliary cyst is exceptionally rare. We report a case of early-onset eCCA in a young woman who was previously treated surgically in the neonatal period. CASE PRESENTATION: A 25-year-old woman with a history of neonatal resection of a congenital biliary cyst, cholecystectomy, and biliary-e

A Biliary Behemoth: A Report of a Rare Case of a Giant Choledochal Cyst.

Choledochal cysts are very uncommon congenital anomalies of the biliary tract, most commonly diagnosed in childhood. Adult presentation is uncommon and often atypical. Giant choledochal cysts, defined as cysts measuring more than 10 cm, are exceptionally rare and present significant diagnostic and surgical challenges. A 19-year-old female presented with a two-month history of dull-aching abdominal pain, progressive right upper quadrant abdominal swelling, and features of obstructive jaundice. Physical examination revealed deep jaundice and a large, non-tender abdominal mass. Imaging studies, i

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Type 6B Choledochal Cyst Unveiled: A Rare Entity in Pediatric Surgery - A Case Report and Review.

Choledochal cyst(CDC) originating from the cystic duct (Type VI A) is uncommon. Dilatation of both the common bile duct (CBD) and cystic duct (Type VI B) is exceptionally rare in children, with only a handful of adult cases documented in medical literature to date. We report a case of a 1-year-old child who was diagnosed with a Type VI B CDC involving dilation of both the cystic and CBDs, confirmed by imaging and histopathology following cholecystectomy, cyst excision, and hepaticoduodenostomy. While recent medical literature has begun to explore Type VI CDCs, documented cases, particularly in

Isolated Right Hepatic Duct Biliary Cyst With a Literature Review: Is It Time to Expand the Todani Classification?

Biliary tract cysts are congenital intrahepatic and/or extrahepatic cystic dilatations of the biliary system. Todani classified the biliary tract cysts into five categories. However, solitary intrahepatic biliary tract cysts are rare. With the advancement of better imaging modalities over the decades, new cases have been reported that do not fit into the original Todani classification of biliary duct cysts. Hence, we propose an expansion of the Todani classification to include the uncommon variants.

Benign Choledochal Cyst Management in Adults: General Surgeons' Perspective in a Non-Tertiary Centre.

BACKGROUND: Choledochal cysts (ChDCs) in adults are rare and mostly benign. Their management involves having either extensive surgery in tertiary centres or conservative long-term surveillance in general hospitals. However, there is a gap in the literature regarding the ideal management approach to adopt in practice. This study explored our experience in this context. METHODS AND ANALYSIS: This three-year retrospective study was conducted in a district general hospital in the West. The database of magnetic resonance scans of the pancreaticobiliary territory in adults over the study period was

Choledococele as a cause of recurrent abdominal pain and biliary sepsis.

We report the case of a 91-year-old female with acute cholangitis and long-standing symptoms of recurrent colic- related abdominal pain after cholecystectomy. She was diagnosed by abdominal CT of saccular dilation of the intramural bile duct in the duodenum suggesting choledococele. ERCP was performed in which the presumptive diagnosis was confirmed, and choledochotomy and choledochoplasty were done with excellent subsequent progress. Choledococele is the least common type of biliary cyst. It consists of a cystic dilation of the intramural portion of the common bile duct that protrudes into th

Incidental findings during donor liver assessment: Single center experience.

Intraoperative consultation of donor liver is an important part of transplant evaluation and determination of liver eligibility. In this study, we describe incidental pathologic findings discovered during the pretransplant evaluation of liver donors in our Institution from 1/2010 to 12/2022. During this 13-year period 369 intraoperative consultations from 262 liver donors were performed. Of those cases, incidental findings were identified in 22 cases (5.9 %) from 19 donors (7.3 %); two donors had more than one lesion. The median age of this subset of patients was 53 years (range: 18-70) and fe

Biliary Cyst: An Unusual Cause of Cholestasis Post Cholecystectomy.

Biliary cysts are relatively uncommon and they can be congenital or acquired and can have various presentations such as cholelithiasis, cholangitis, jaundice, and pancreatitis. Biliary cysts are associated with a high risk of biliary cancers and such risk increases with age. Identification of biliary cysts warrants an aggressive approach to lower cancer risk. Surgical management has a high success rate and it lowers morbidity, mortality, and cancer risk. We present a 40-year-old female who had a cholecystectomy in 2016. She presented with obstructive jaundice and was found to have a class I bi

Adipose tissue torsion with localized lymphangiectasia in a Rio Cauca caecilian Typhlonectes natans.

OBJECTIVE: A female Rio Cauca caecilian Typhlonectes natans (estimated as between 10 and 18 years of age) housed at the Smithsonian National Zoological Park in Washington, D.C., developed progressive severe coelomic effusion over a 4-week period. The coelomic effusion was diagnosed via radiographs and ultrasound, and a sample of the fluid was obtained for analysis, which revealed a low-protein transudate suggestive of inflammation. As the coelomic effusion progressed, the caecilian became tachypneic, hyporexic, and lethargic. The caecilian was started on antibiotics and a diet trial, but signs

Choledochal Cyst and Pancreas Divisum: A Case Report.

BACKGROUND A choledochal cyst (CC), or biliary cyst, is a congenital or acquired anomaly of the biliary tree. Pancreas divisum (PD) is a rare congenital anomaly due to incomplete fusion of pancreatic ducts, which can complicate the clinical course of choledochal cysts. This rare combination is a surgical management challenge. This report presents the diagnosis and management of a 23-year-old woman with a combined choledochal cyst and pancreas divisum treated with pancreaticoduodenectomy. CASE REPORT This article presents the case of a 23-year-old woman who presented with severe, stabbing abdom

Autoimmune Pancreatitis Type 2, Biliary Cysts and Giardia lamblia.

Autoimmune pancreatitis type 2 is a relatively novel entity with some still controversial issues. The current diagnostic algorithm relies on imaging studies and histology. Therapy includes corticosteroids with consequently low risk of relapse in the following year. However, the pathogenesis remains unclear, and data are insufficient for long-term prognosis. We have treated a 17-year-old boy whose autoimmune pancreatitis type 2 was revealed during surgery for a pre-existing biliary tract anomaly with concurrent protozoal infection. We discuss the co-occurrence of these conditions in terms of ev
