PURPOSE: The aim of this study was to understand the level of bile flow by perinatal transitions of the hepatobiliary cyst size in biliary cystic malformation (BCM) cases and to examine the association between the transitions of the cyst size, cholangiographic images of the intrahepatic bile ducts (IHBDs), and histological features of the liver specimens in BCM.
METHODS: We measured perinatal size of the hepatobiliary cyst in 7 BCM cases, composed of 1 case with choledochal cyst and 6 cases with biliary atresia (BA). Bile excretion was inferred from the transition of cyst size and the postope

The main target of our investigations was the development of antireflux, bilio-cholecysto (cystico) enteral anastomosis method for the surgical treatment of extrahepatic biliary duct obstruction. We were using the cholecystic duct spiral valve (Lutkens valve, Haister valve) as the antireflux structure. In this case the bile drains into the biliary cyst not through the cholecystic duct but through the extrahepatic biliary duct - the bilio-cholecystic anastomosis. Such an interposition of cholecystic and spiral valves promotes inhibition of chemical reflux between extrahepatic ducts and gastroin

BACKGROUND: The purpose of this study was to delineate the long-term prognosis and factors we have noted in our 25 years of experience treating patients suffering from biliary atresia (BA) who have undergone Kasai's operation.
METHODS: We studied 141 patients (69 male and 72 female infants) who underwent Kasai's operation at the National Taiwan University Hospital between 1976 and 2000. Factors analyzed included age at time of surgery, postoperative bile flow, frequency of cholangitis, prophylactic long-term oral antibiotics given, and intrahepatic biliary cyst formation.
RESULTS: The 5-year

The polycystic kidney (PCK) rat represents a liver and kidney cyst pathology corresponding to Caroli's disease with congenital hepatic fibrosis and autosomal recessive polycystic kidney disease. We previously reported that an epidermal growth factor receptor tyrosine kinase inhibitor, gefitinib (Iressa), significantly inhibited the abnormal growth of biliary epithelial cells of PCK rats in vitro. This study investigated the effects of gefitinib on cyst pathogenesis of the PCK rat both in vitro and in vivo. A three-dimensional culture model of biliary epithelial cells in the collagen gel matrix

Sclerosing cholangitis is a rare but dreadful complication of liver hydatid surgery. Almost all scolocidal agents are associated clinically or experimentally with this complication. Cetrimide-chlorhexidine combination is the most potent scolocidal agent, and this study was designed to examine its effect on the hepato-pancreatico-biliary system. Forty Wistar-albino rats were divided into two groups. 0.5% cetrimide-0.05% chlorhexidine was injected into the biliary tract of study group animals and 0.9% saline ( NaCl ) into the control group through a 3-mm duodenotomy. The animals were sacrificed

OBJECTIVE: Among the various organs that may be affected by hereditary hemorrhagic telangiectasia (HHT), the liver can show various degrees of vascular and parenchymal involvement. The purpose of this prospective study comprising a large series of patients was to reassess the computed tomography (CT) features of hepatic involvement in HHT using helical CT.
METHODS: Twenty-four consecutive patients with HHT had prospective helical CT of the liver, including noncontrast, arterial-dominant, and portal-dominant phases. The CT images were analyzed by 2 readers in consensus to determine the presenc

In adulthood, choledochal cysts are often surprisingly discovered during cholecystectomy. We report here on a case of an unsuspected congenital type-IA biliary cyst, according to Todani's classification encountered during laparoscopic cholecystectomy in a 30-year-old woman complaining of acute abdominal pain with an unremarkable preoperative workup. The well-known risk of developing cystic cancer, mainly in the adult, means that an excisional operation is indicated to prevent such complications. Cyst excision and Roux-en-Y hepaticojejunostomy is the definitive treatment of choice, performed at

BACKGROUND & AIMS: Recent studies have showed that proteins associated with polycystic kidney disease (PKD) are expressed in cilia, linking this organelle and cyst formation in the kidney, but involvement of cilia in PKD-related biliary cystogenesis has not been shown. We investigated: (1) the expression of fibrocystin (a product of PKHD1, the autosomal-recessive PKD [ARPKD] gene) in cholangiocyte cilia; (2) biliary cyst formation in an orthologous rat model, PCK; and (3) the effect of Pkhd1 mutation on ciliary structure.
METHODS: Biliary cystogenesis was assessed by microcomputed tomography.

AIM: To investigate the differences in clinical and pathological manifestations between biliary atresia with extrahepatic biliary cyst and choledochal cyst in neonates.
METHODS: Symptoms and clinical signs in 5 neonates with biliary atresia with extrahepatic biliary cyst (4 of type I and I of type III) and 17 neonates with choledochal cyst were recorded. The levels of serum alkaline phosphatase, bilirubin, direct bilirubin, transaminase, gamma-glutamyl transpeptidase were analysed. Width and length of gallbladder and choledochal cyst were measured on B-mode ultrasound before surgery. Intrahep

OBJECTIVE: To analyze the complications of diagnostic and therapeutic sonographically guided interventional procedures of focal liver lesions observed during a 22-year period in a single center.
METHODS: Complications of sonographically guided diagnostic and therapeutic procedures on focal liver lesions, observed during a 22-year period in a single center, were reviewed. From 1979 to 2001, 13,222 patients (age range, 7-89 years; mean, 59 years; 8,688 male and 4,534 female) with 13,777 focal liver lesions underwent 16,648 sonographically guided biopsies and 3,035 therapeutic procedures: pyogen

We report the second case of squamous cell carcinoma arising in a hepatic foregut cyst (CHFC) in a 40-year-old woman. Microscopically, the lining of the cyst was composed of ciliated columnar epithelium, gastric and squamous epithelium. The squamous epithelium showed areas with dysplastic changes and other areas with carcinomatous transformation. In this congenital lesion, it was not surprising to find squamous and gastric mucosa because oesophagus, stomach, and tracheobronchic tree derive from the embryologic foregut. Squamous carcinoma might develop in a context of inflammation as in biliary

The hepatopulmonary syndrome (HPS) results from pulmonary microvascular dilatation in cirrhosis and is associated with increased pulmonary endothelial nitric oxide synthase (eNOS) levels. In the common bile duct ligation (CBDL) model, endothelin-1 (ET-1) released from the liver contributes to the rise in pulmonary eNOS and intrapulmonary vasodilatation. Whether substances, including ET-1, are found in the biliary tree and selectively enter the circulation after CBDL to influence the pulmonary vasculature is unknown. We assessed if control bile and fluid obtained from the obstructed biliary tre

UNLABELLED: With widespread use of ultrasonography, fortuitous discovery of benign tumors of the liver raises the question of surgical management in a large number of young subjects. In order to obtain certain diagnosis and determine appropriate management, avoiding unnecessary surgery for asymtomatic tumors with a benign evolution or inversely delaying surgical treatment of malign lesions, the surgeon must be aware of the different features of benign tumors, their expected course, and the capacity of imaging techniques to provide positive diagnosis.
DIAGNOSIS: The most frequent benign tumors

BACKGROUND: The purpose of this study was to develop a new concept of the embryonic etiology of pancreaticobiliary maljunction (PBM) based on cholangiopancreatograms.
METHODS: The subjects were 202 patients with PBM (60 men and 142 women) in whom the junction of the pancreatic and bile ducts was radiologically diagnosed as being located outside of the duodenal wall; 133 of the 202 patients also had congenital cystic dilatation of the bile duct (CCBD).
RESULTS: The length of the duct from the junction to the orifice of the major papilla (the common channel) ranged from 0.5 to 5 cm on the chol

BACKGROUND: Embryonic sarcoma of the liver is uncommon in adults. We report an unusual case presenting as a cyst-like formation of the liver.
CASE REPORT: The pre-operative and also the peroperative histology diagnosis was remodelled biliary cyst. The patient was treated accordingly. The final diagnosis of embryonic sarcoma of the liver was made when a local recurrence developed. The patient underwent hepatic resection followed by a chemotherapy and radiotherapy protocol. Tumor markers were measured within the tumor and gave a positive result for CA-125.
DISCUSSION: Diagnosis of cystic forms

Focal nodular hyperplasia (FNH) is a benign lesion of the liver which usually presents with one or two localizations. We report a patient with history of resection of a biliary cyst, and who had been taking oral contraceptives for the past 18 years, who had multiple localizations of FNH (more than 30 lesions). The largest lesion measured 10.5 x 11 x 12cm. The imaging characteristics of our patient were atypical. A central scar could be demonstrated only in the largest lesion, in an eccentric location. In the other lesions, no scar formations could be detected. Furthermore, imaging characterist
