OBJECTIVES: Differential diagnosis between a benign cystic hepatic lesion, biliary cyst, and a potentially malignant lesion or biliary cystadenoma, is difficult. The aim of this study was to evaluate imaging features of atypical cystic liver lesions and the role of imaging techniques in determining a specific diagnosis.
METHODS: Twenty-six patients with atypical cystic hepatic lesions were included in this study. All patients underwent surgery and pathological diagnosis was atypical hepatic cyst (n = 18), biliary cystadenoma (n = 4), hydatic cyst (n = 3), and ciliated hepatic foregut cyst (n

A 60-year-old man underwent resection of a sigmoid colon adenocarcinoma, left hepatic lobe, part of segment 6 and 8, due to its metastasis. He received hepatic artery infusions of 5-FU total amount, 15.5 g. There was no recurrence in the liver, but he developed sclerosing cholangitis, and a large biliary cyst formed in segment 8. After drainage of the cyst by percutaneous-trans-hepatic approach, bile leakage persisted, and liver dysfunction was progressive. Sclerosing cholangitis-induced biliary cyst formation is rare. Occlusion, or necrosis of the peripheral bile duct may be the cause.

Ten cases of complications of simple biliary cysts were reported: 4 intracystic haemorrhages including 1 intraperitoneal rupture, 3 bacterial infections including 1 case with invalidating pain. Complications of biliary cysts raises the problem of the differential diagnosis with cysts caused parasites, neoplastic processes (cystadenomas and pseudocysts of the liver) and with intrahepatic collections of pancreatic or traumatic origin. Imaging and echo-guided puncture are the most appropriate exploration techniques. MRI has been particularly helpful to evaluate bleeding in an intrahepatic cyst. A

Recurrent angiocholitis attacks occurred in an 80-year-old woman, and repeated investigations could not provide any diagnosis. Subsequently, the patient developed haematemesis. Angiography revealed haemobilia from an intrahepatic cause. Surgical constatations and pathological examination of the resected specimen revealed that haemobilia was due to a non-parasitic cyst of the liver. In this case, haemobilia resulted from two unusual complications of this hepatic lesion: intra-cystic bleeding and communication with an intrahepatic duct. Haemobilia has never been reported as a complication of non

OBJECTIVES: We retrospectively examined our clinical data on hydatic cysts of the liver collected over the last 10 years in order to determine a classification which could be used to guide future therapeutic decisions.
METHODS: From 1982 to 1992, hydatic cysts of the liver were surgically removed in 378 patients (mean age 38 +/- 12 years). Echography had been performed in all patients. Other preoperative investigations included computerized tomodensitometry (n = 57) and arteriography of the caeliomesenteric arteries (n = 12). Histological examination of surgical specimens included research fo

Twelve patients (11 female) with an extrahepatic biliary cyst (six type I, three type II and three type III according to the classification of Todani) are reviewed with emphasis on aetiology, clinical features and long-term results at follow-up of 3-10 years. The clinical manifestations were abdominal pain, cholestasis with jaundice, fever and episodes of pancreatitis. The diagnosis was established before surgery in all cases by ultrasonography, endoscopic retrograde cholangiopancreatography, percutaneous transhepatic cholangiography and computed tomography. An abnormally long common channel w

Five cases of intrahepatic biliary cyst or cystic dilatation and one suspicious case were found (6.4%) among 93 survivors of biliary atresia. The age at manifestation of such cysts ranged from 6 months to 12 years. These cysts or cystic dilatations were treated surgically in two cases, by percutaneous transhepatic cholangiodrainage (PTCD) in two, and not treated in one. Four patients are alive; one died of biliary atresia. Twenty-nine well-documented cases of intrahepatic biliary cyst were found in the literature, including five treated by the authors. The modes of dilatation were as follows:

The authors report about 2 cases of Budd-Chiari syndrome occurring during the immediate postoperative period of extensive hepatectomy, after exeresis of a large biliary cyst (20 cm in diameter) through 6/7 sectoriectomy and after exeresis of a large sarcomatous tumor preserving only sectors 2 and 3 of the liver using an "ex-vivo/in-situ" technique. These 2 cases lead to discuss the diagnosis, treatment and, most importantly, the prevention of acute suprahepatic obstruction after extensive right hepatectomy. Similar cases are reported in the literature after partial liver transplantation. Preve

A case of type-IA biliary cyst, according to the Todani classification, is reported in a 44-years-old man admitted to hospital with epigastric pain. Ultrasound was misleading; CT scanning and preoperative cholangiography confirmed the biliary duct's dilatation. Cyst excision and hepaticojejunostomy were performed with a view to avoiding possible cyst cancerization.

The experience with surgical treatment of 21 patients with a biliary cyst who underwent 25 operations has been analysed. The authors came to a conclusion about unexpediency to use the walls of a cyst in creation of the cystodigestive anastomosis. The operations with complete removal of the cystic wall are recommended.

Biliary cysts are uncommon and often asymptomatic. Once symptoms occur, treatment is needed. Though percutaneous drainage is possible, the recurrence rate is high. Open drainage is associated with the morbidity of laparotomy. We report a case of laparoscopic treatment of a large biliary cyst.

A 26 year-old man presented a hepatic cyst complicated by intracystic bleeding. Clinical and biological features (chronic pancreatitis), ultrasound and computed tomography findings apparent hepatic and splenic location), macroscopic aspects (inflammatory changes, brown-colored fluid) as well as microscopic characteristics (thickened cyst-wall, absence of epithelium) of both cysts suggested the diagnosis of pancreatic pseudocysts. The diagnosis of hepatic cyst was carried out on histological examination showing Meyenburg and neuro-fibrillar complexes. Occurrence of complications (bleeding, size

The levels of tumor markers in cystic fluid and serum were measured in six patients with benign biliary cyst of the liver. AFP in the cystic fluid was lower than the upper normal limit for serum in all cases, and CEA in the cystic fluid was higher than the upper normal limit for serum in one of the six cases. CA19-9, DU-PAN 2, and SPAN 1 in cystic fluid were much higher than the upper normal limit for serum in all cases (more than 100-fold for CA19-9, twofold for DU-PAN 2, and ninefold for SPAN 1). CA19-9, DU-PAN 2, and SPAN 1 in cystic fluid were significantly higher than the levels in the co

Epithelial cells of several types of hepatic cysts were examined by mucin histochemistry and immunohistochemically. There were some differences in mucus and antigenic expression among the hepatic cysts examined. Epithelial cells of non-parasitic simple cysts and adult-type polycystic liver showed similar mucin-histochemical and immunohistochemical features, and were characterized by little mucin and weak immunoreactivities to several antibodies examined. Epithelial cells of hepatic hilar cysts were characterized by much mucin and moderate immunoreactivities to carbohydrate antigen 19-9 (CA 19-

A case of a patient with a congenital cyst that was localized solely to the cystic duct is presented. We propose including this anomaly in the biliary cyst classifications, which currently include choledochal and intrahepatic and extrahepatic biliary cysts. The diagnosis and management of this new type of cyst in addition to other types of biliary cysts are discussed.

We describe a new technique of biliary cyst fenestration using a videolaparoscopic approach. From June 1989 to October 1990, laparoscopic fenestration was performed for four symptomatic solitary cyst and one polycystic liver disease patients, without any morbidity. Mean hospital stay was short: 6.8 days (range of 3-14 days). No recurrence was observed on ultrasound controls with a mean follow-up of 6.4 months (range of 2-15 months). Laparoscopic fenestration for symptomatic biliary cysts is a simple, effective, and minimally invasive procedure.

The authors report on 9 cases of cystic lesion detected by fetal ultrasound echography, uropathies and bowel dilatations having been excluded. The lesions were detected at a fairly advanced stage of pregnancy, during the 3rd trimester and involved 8 girls and 1 boy. Investigation of possible chromosomal and associated anomalies was carried out in each case. Analysis confirmed the frequency of ovarian cysts in the female subjects (6), but also demonstrated the following: 1 case of bowel duplication, 1 cystic Meckel diverticulum, 1 liver biliary cyst (boy). The etiology and diagnostic difficulti
