[Management of a biliary cyst disclosed prenatally. Apropos of 2 cases].

Two cases of prenatal detection of congenital biliary cysts are reported. Prenatal and postnatal ultrasonography demonstrate a solitary sonolucent cystic lesion. The differential diagnosis in fetus and infant includes any abdominal cystic lesions: ovarian cyst, duplication cyst, cystic lymphangioma, choledochal cyst and other congenital cysts of the liver. In opposite to the difficulty of diagnosis, the choice for treatment is easy: surgery is the only way to allow histologic diagnosis and to avoid any relapse or complications.

[The validity of selected nuclear medicine imaging procedures (brain, skeletal, and hepatobiliary diagnosis)].

In a retrospective study we tested several scintigraphic investigations for a total of 2,014 patients. As reference we used the results of biopsy, autopsy or the epicritic final diagnosis before emission, summing all clinical signs as well as the results of additional alternative methods and other imaging methods. Criteria of validity (sensitivity, specificity, diagnostic accuracy) are used in a consistent manner. Brain perfusion scintigraphy detected acute cerebral perfusion disorder with a sensitivity of 0.87, differentiating for reversible and irreversible lesions. Carotid angiography had t

[Extrahepatic biliary tract atresia and a common biliopancreatic canal. Review of the literature apropos of a case].

The authors report the case of an 3 months old caucasian boy which had an extrahepatic biliary atresia associate with a congenital biliary cyst and an anomalous pancreaticobiliary junction. The patency between gallbladder and duodenum permits the operative opacification of this long common pancreaticobiliary channel. The congenital biliary cyst was located on the upper part of the common bile duct and was obstructed by biliary concretions. Unfortunately, no patency between intrahepatic bile ducts and this cyst was found. Because this long common pancreaticobiliary channel may cause biliary dil

[Solitary hepatic cysts--a therapeutic problem].

The authors give an account of the therapeutic procedure in 22 patients with solitary cysts of the right liver lobe. In 13 pyogenic cysts (abscesses) drainage of the abscess was made under CT control, in four cysts anatomical or non-anatomical resection of the liver was performed, in three cysts enucleation, in one biliary cyst cystojejunostomy with a Roux-Y loop of the jejunum. Aetiopathogenesis of the cysts: pyogenic cysts 14, echinococcal cysts 2, inborn parenchymatous cysts 3, posttraumatic cysts 3.

Congenital extra-hepatic biliary cyst in a Congo African Grey Parrot (Psittacus erithacus erithacus).

An 8-week-old Congo African Grey Parrot (Psittacus erithacus erithacus) was presented with a history of abdominal enlargement since hatch. Trans-illumination of the abdomen suggested that the enlargement was due to marked hepatomegaly. This was supported by radiographic and ultrasonic examination. Postmortem examination revealed an extra-hepatic biliary cyst 4 cm in diameter.

Solitary intrahepatic biliary cyst: diagnostic and therapeutic strategy.

This paper describes a newborn infant in whom an intrahepatic biliary cyst was successfully demonstrated by antenatal ultrasound. Postnatally, percutaneous cyst aspiration and computed tomography enhanced with intravenous cholangiographic contrast proved extremely helpful in the selection of surgical procedure. The lesion was completely removed at 12 weeks of age without complications.

[Partial rupture of biliary cyst of large dimensions. Clinical description and ultrasonic diagnosis].

The clinical case of an elderly patient hospitalised for acute abdomen type syndrome is reported. Investigation, particularly echography, pointed to a diagnosis of partially ruptured giant biliary cyst of the liver. The clinical pictures and the unusual features of this rare condition are discussed. Differential diagnosis with respect to echinococcosis and polycystic liver is examined.

[Cystic dilatation of the choledochus. 9 cases].

A retrospective study of 9 cases of extra-hepatic biliary cyst is presented. Prior to admission to our hospital, 5 patients had been operated upon elsewhere. This initial operation proved to be of great importance. In 4 patients initial excision of the cyst was performed resulting in complete regression of symptoms, and no further surgery was required. In 5 patients the cyst was not excised initially and cysto-enterostomy was performed. Recurrence of symptoms and complaints, with major morbidity, occurred in all but one of these patients. After several operations, only final excision of the cy

[Pathology of the bile ducts in children with homozygous sickle-cell anemia. Apropos of 5 recent cases].

Five cases of biliary complications in childhood sickle cell disease are reported. In four cases, the pathology was gall stones causing recurrent abdominal pain in a 10 year old boy, a 13 year old girl and a 2 year old infant, and responsible for a "Salmonella septicaemia" in a 17 year old adolescent. In one case, a biliary cyst was diagnosed at 3 years of age. Four children underwent successful surgery. The complications of gall stones are difficult to distinguish from episodes of vasoocclusive abdominal pain. Ultrasonography is an easy method of detecting gall stones and may be repeated regu

Surgical treatment of congenital cystic dilation of the biliary tract.

Cystic dilation of the biliary tract in 38 Vietnamese children is reported. Most had symptoms and signs of cholangitis and biliary obstruction. The surgical management is discussed, and for choledochal cyst resection of the cyst with hepaticojejunostomy Roux-en-Y is recommended. Postoperative cholangitis occurred in 16% of the children. The overall mortality was 18% after a median follow-up time of 28 months. Cystic dilation of the biliary tract is suggested to be a congenital entity, resulting from two pathogenetic factors--an unexplained structural anomaly of the biliary duct wall and an abn

Bile-duct cyst with cystlithiasis: a case report.

Cystic dilatation of the biliary tree is an uncommon congenital condition, most often discovered in infancy. It should be suspected in an adult who has a history, over a long period, of intermittent pain in the right upper quadrant, episodes of jaundice or cholangitis, or both, and has a palpable mass in the right hypochondrium. Ultrasonography and especially endoscopic retrograde cholangiopancreatography allow the biliary cyst to be diagnosed preoperatively more often than previously. The authors describe the case of a 60-year-old woman with congenital cystic dilatation of extra- and intrahep

[The post-traumatic biliary cyst (author's transl)].

The case report communicated here illustrates a late complication subsequent to an emergency operation for liver rupture. This complication may result from ligating a branch of the hepatic artery to stop bleeding. If the necessary second operation is omitted for any reason, the hepatic sequester can develop into an abscess or biliary cyst. It is to be expected that these late complications will occur more frequently in future owing to a probable increase in the incidence of the so-called blunt abdominal traumas. The reason for this is, on the one hand, the growing number of traffic accidents a

Gray scale ultrasonography of a traumatic biliary cyst.

Traumatic hepatic biliary cysts, which occur when there is leakage after traumatic disruption of the biliary system, can be readily detected with hepatic sonography. Differentiation from other hepatic fluid collections can be made by percutaneous thin needle aspiration. Aspiration may have possible utility as a therapeutic modality. A case is presented in which Gray scale ultrasonography revealed a biliary cyst which was drained operatively and resolved.

Management of adult choledochal cysts.

A review of the English literature reveals a total of 1,337 patients with choledochal cysts. Improved diagnostic techniques to visualize the biliary system are demonstrating an increasing number of unsuspected choledochal cysts in adult patients. Either choledochal cysts remain clinically silent until adulthood or may develop in later life. Experience is reported with adult patients having type I, II, III, and IV choledochal cysts. Type I cysts are preferably managed by excision but cyst anatomy may necessitate choledochoenteric drainage. Type II cysts are treated by excision except for those

[Single centro-hepatic biliary cyst (author's transl)].

This is a rare condition, often presenting late as a result of compression of adjacent organs, as demonstrated by radiological examinations. Laparoscopy reveals only hepatic hypertrophy whilst the intrahepatic lesion is seen on isotope scan, its irregular appearance sometimes leading to confusion with metastatic disease. Study by scanner confirms its cystic nature. This dysembryoplasic nature of this lesion is now generally accepted, its origins lying in the vestiges of the hepatic ducts. Resection of the overling dome is the usual treatment of exteriorised biliary cysts. The evacuation of lar
