The Crosstalk Role of Intestinal Barrier Damage in the Gut-Organ Axis and its Potential Therapeutic Implications.

Intestinal barrier damage is a pivotal mediator of gut-organ crosstalk and a key contributor to systemic disease. This review systematically summarizes the structural organization of the intestinal barrier, including the mucus layer, epithelial tight junctions, and the lamina propria immune network, and highlights their coordinated roles in regulating intestinal barrier dysfunction and immune homeostasis. Disruption of barrier integrity facilitates the translocation of microbial products, cytokines, and metabolites, thereby promoting systemic inflammation and the pathogenesis of multi-organ di

Dramatic regression of mass-forming gastric IgG4-related disease with inebilizumab: a case report.

BACKGROUND: Gastric immunoglobulin G4-related disease (IgG4-RD) is exceedingly rare, with fewer than 50 cases reported worldwide. Its mass-forming presentation frequently mimics malignancy, leading to high rates of unnecessary surgical resection. Inebilizumab, a CD19-directed monoclonal antibody and the first FDA-approved therapy for IgG4-RD, has not previously been described in gastric IgG4-RD. We report the first case of mass-forming gastric IgG4-RD treated with inebilizumab, achieving clinical, endoscopic, and histologic remission without surgical intervention. CASE PRESENTATION: A 52-year

Epigenetic regulation of INPP5D and its downstream molecular network in autoimmune pancreatitis.

AIMS: Autoimmune pancreatitis (AIP) is a representative manifestation of IgG4-related disease characterized by lymphoplasmacytic inflammation and storiform fibrosis. Although epigenetic dysregulation has been implicated in autoimmune diseases, its contribution to the pathogenesis of AIP remains unclear. METHODS AND RESULTS: Genome-wide DNA methylation profiling was performed using the Infinium MethylationEPIC array in tissues affected by AIP and obstructive pancreatitis (OP). Differentially methylated regions (DMRs) were identified and subjected to pathway enrichment analysis. Among the signi

Octreotide attenuates pancreatic fibrosis through modulation of the HMGB1-Wnt/β-catenin axis in experimental chronic pancreatitis.

Chronic pancreatitis is a progressive fibroinflammatory disease in which activated pancreatic stellate cells and extracellular matrix deposition drive irreversible tissue remodeling. Octreotide has anti-secretory and anti-inflammatory properties, but its anti-fibrotic mechanism remains unclear. To determine whether octreotide attenuates DBTC-induced pancreatic fibrosis through suppression of high-mobility group box 1 (HMGB1) and downstream Wnt/β-catenin signaling. Male Sprague-Dawley rats received dibutyltin dichloride (DBTC, 7 mg/kg) to induce chronic pancreatitis and were assigned to seven g

Efficacy, safety, and cost of immediate versus on-demand endoscopic necrosectomy in infected necrotizing pancreatitis: a systematic review and meta-analysis.

BACKGROUND: Endoscopic ultrasound (EUS)-guided drainage is the standard of care for walled-off necrosis (WON); however, the optimal timing of subsequent endoscopic necrosectomy remains uncertain. We performed a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing immediate versus on-demand necrosectomy following EUS-guided drainage. METHODS: We searched PubMed, Embase, Web of Science, and the Cochrane Library through March 2026 for RCTs comparing immediate (≤ 72 h) versus on-demand necrosectomy in adults with necrotizing pancreatitis. Primary outcomes were tech

Clinical and Genetic Interplay Between Chronic Pancreatitis, Cystic Fibrosis and CFTR-Related Disorders.

Chronic pancreatitis (CP) is a clinically heterogeneous condition that may occur with a broad clinical spectrum, including cases of cystic fibrosis (CF), cystic fibrosis transmembrane regulator (CFTR)-related disorder (CFTR-RD), acute recurrent pancreatitis (ARP), and CP, including cases of idiopathic CP (iCP) and hereditary pancreatitis (HP). The specific contribution of CFTR variants and pancreatitis-predisposing genes (PPG), i.e., genes responsible for intrapancreatic activation of trypsinogen or pancreatic secretion pathways to CP susceptibility in these different contexts remains incomple

Gut Microbiota and Nutritional Aspects in Pancreatic Diseases: A Narrative Review.

Pancreatic diseases, including acute pancreatitis (AP) and chronic pancreatitis (CP), pancreatogenic diabetes mellitus (T3cDM), autoimmune pancreatitis (AIP), and pancreatic cancer (PDAC), are associated with metabolic, inflammatory, and nutritional disturbances. Increasing evidence indicates a bidirectional relationship between pancreatic diseases and the gut microbiota. On the one hand, alterations in microbial composition and function, intestinal barrier dysfunction, and bacterial translocation may contribute to their pathogenesis and progression. On the other hand, pancreatic diseases and

Association of 24 hour mean blood glucose levels with mortality in patients with acute pancreatitis: A retrospective cohort study.

This study aims to investigate the impact of 24 hour mean blood glucose (24h MBG) levels on the mortality of patients hospitalized with acute pancreatitis (AP) and to explore the clinical implications of this association. A retrospective examination of clinical datasets of AP patients was performed using the Medical Information Mart for Intensive Care-IV database. The optimal 24h MBG levels threshold was determined by the X-tile tool. Univariate and multivariate logistic regression analyses were used to assess the prognostic significance of 24h MBG levels. A survival analysis was carried out,

Primetime for the Gasdermins Seesaw in Intestinal Homeostasis and Digestive Disorders.

The five pore-forming gasdermins (GSDMs, namely GSDMA-E) are best known for executing pyroptosis, a lytic and highly inflammatory form of programmed cell death. Accumulating evidence over the past decade has unveiled diverse nonlytic functions of GSDMs in digestive homeostasis and disease. GSDMs preserve intestinal barrier integrity and mucosal immunity under physiological conditions, whereas their dysregulated activation fuels tissue injury across a spectrum of digestive diseases, including inflammatory bowel disease, acute intestinal injury, radiation enteritis, acute pancreatitis, and liver

Fibroblast growth factor in pancreatic development, homeostasis, and disease.

Pancreatic development is a tightly orchestrated process driven by the concerted action of multiple signaling pathways. Fibroblast growth factors (FGFs) serve as pivotal cues across its stages: they engage FGF receptors (FGFRs) to activate downstream ERK/MAPK, PI3K-AKT, and PLCγ cascades, thereby governing pancreatic progenitor proliferation, differentiation, and functional maturation. The spatiotemporal expression of FGFs is precisely coupled to developmental milestones and intertwined with Notch, Wnt, and Hedgehog signaling in a complex regulatory network. This review systematically summariz

Usefulness of Endoscopic Ultrasound-Guided Pancreatogastrostomy and the Related Trans-Endosonographically Created Route Procedure for the Management of Refractory Pancreatic Stones (With Video).

Endoscopic management of symptomatic pancreatic stones (P-Stones) with main pancreatic duct (MPD) strictures due to chronic pancreatitis remains challenging. For refractory P-Stones, high-quality evidence for endoscopic treatment remains limited, and endoscopic ultrasound-guided pancreatogastrostomy (EUS-PGS) is considered a potential salvage option. This single-center retrospective study enrolled 16 consecutive patients with symptomatic refractory P-Stones who underwent EUS-PGS between June 2017 and June 2025, after failed transpapillary access or, in cases of surgically altered anatomy (SAA)
