Alterations of platelet function and coagulation parameters during acute pancreatitis.

Vascular thrombosis and systemic hypercoagulable states are known complications of pancreatitis. Higher levels of mean platelet volume (MPV) have been associated with thrombotic diseases. However, a few studies have investigated the association between acute pancreatitis and MPV. We aimed to investigate whether there is a difference of MPV and coagulation parameters in patients with active and remission in acute pancreatitis. We included 24 consecutive patients with biliary acute pancreatitis and 24 consecutive healthy age-matched and sex-matched controls. Full blood counts and other laborator

Ansa pancreatica: an anatomic variation and a rare cause of acute pancreatitis.

INTRODUCTION: The ansa pancreatica is a rare anatomic variation of the pancreatic ducts. It is a communication between the main pancreatic duct (Wirsung) and the accessory pancreatic duct (Santorini). Recently, the ansa pancreatica has been considered as a predisposing factor in patients with idiopathic acute pancreatitis. CASE REPORT: \We report a case of non-alcoholic and non-biliary acute pancreatitis, in a 53-year-old patient. An ansa pancreatica was discovered in a post-operative cholangiography. CONCLUSION: A pancreatic duct variation, as the ansa pancreatica, can be diagnosed during a

Long term outcome of acute pancreatitis in Italy: results of a multicentre study.

BACKGROUND: In Italy, no long-term studies regarding the natural history of acute pancreatitis have been carried out. AIM: To report the results of a follow-up on a large series of patients hospitalised for pancreatitis. METHODS: Data of 631 patients admitted to 35 Italian hospitals were retrospectively evaluated 51.7±8.4 months after discharge. RESULTS: The average recovery time after mild or severe pancreatitis was 28.2 and 53.4 days respectively. Fourteen sequelae were not resolved and 9 cases required late surgical intervention. Eighty patients (12.7%) had a second hospital admission. O

Review of experimental animal models of biliary acute pancreatitis and recent advances in basic research.

Acute pancreatitis (AP) is a formidable disease, which, in severe forms, causes significant mortality. Biliary AP, or gallstone obstruction-associated AP, accounts for 30-50% of all clinical cases of AP. In biliary AP, pancreatic acinar cell (PAC) death (the initiating event in the disease) is believed to occur as acinar cells make contact with bile salts when bile refluxes into the pancreatic duct. Recent advances have unveiled an important receptor responsible for the major function of bile acids on acinar cells, namely, the cell surface G-protein-coupled bile acid receptor-1 (Gpbar1), locat

Early proteome analysis of rat pancreatic acinar AR42J cells treated with taurolithocholic acid 3-sulfate.

BACKGROUND: Bile acids are the initiating factors of biliary acute pancreatitis. Bile acids can induce the activation of intracellular zymogen, thus leading injury in pancreatic acinar cells. Pathological zymogen activation in pancreatic acinar cells is a common feature of all types of acute pancreatitis. The proteins expressed in pancreatic acinar cells during the activation of zymogen may determine the severity of acute pancreatitis. The present study aims to determine the differentially expressed proteins in taurolithocholic acid 3-sulfate-stimulated pancreatic acinar cells as an in vitro m

[Role and indications of ERCP in treatment of acute biliary pancreatitis].

Results of endoscopic treatment of patients with biliary acute pancreatitis are presented. Initially this form of treatment was rather performed in the late period of the disease. After publication of the controlled randomized study by Neoptolemos and Carr-Locke'a, this method of the treatment was recommended in the early phase of the disease within the first 72 hours after hospital admission and only in patients with severe acute biliary pancreatitis. Recently, meta-analysis of controlled randomized studies restricted indications of this procedure only in patients with associated with acute c

[Analysis of clinical course of severe acute biliary and non biliary pancreatitis: a comparative study].

UNLABELLED: The acute pancreatitis is a relative common disease with incidence of 5-80 per 100000 people of the population. The number of new cases has steadily increased in recent years. The two main etiological factors are alcohol and cholelithiasis. The incidence of alcoholic pancreatitis is higher in male, and the incidence of gallstone pancreatitis is higher in female. AIM: To summarize the difference between the clinical course of biliary and not biliary type of severe acute pancreatitis by analyzing the data of these patients. METHODS: 139 patients treated with severe acute pancreatit

Alanine transaminase rather than abdominal ultrasound alone is an important investigation to justify cholecystectomy in patients presenting with acute pancreatitis.

OBJECTIVES: The aims of this study were to investigate the predictive value of an elevated level of alanine transaminase (ALT) for biliary acute pancreatitis (AP) and to reconsider the role of abdominal ultrasound (AUS). METHODS: All patients admitted to Christchurch Public Hospital with AP between July 2005 and December 2008 were identified from a prospectively collected database. Peak ALT within 48 h of presentation was recorded. Aetiology was determined on the basis of history, AUS and other relevant investigations. RESULTS: A total of 543 patients met the inclusion criteria. Patients wit

Early versus delayed cholecystectomy in patients with biliary acute pancreatitis.

BACKGROUND: In patients with biliary acute pancreatitis (AP), cholecystectomy is mandatory to prevent further biliary events, but timing of cholecystectomy remains a subject of ongoing debate. The objective of the present, retrospective study was to compare the outcomes of early (within 2 weeks after onset of disease) versus delayed cholecystectomy in patients with biliary AP. METHODS: Between January 2000 and December 2005, 112 patients underwent cholecystectomy because of biliary AP. Thirteen patients were excluded from analysis because of necrotizing pancreatitis on the initial computed to

The use of antibiotics for acute pancreatitis: is there a role?

Infections due to pancreatic necrosis and abscesses are observed in one third of patients with severe acute pancreatitis (SAP). Based on results of double-blind, randomized, placebo-controlled trials, antibiotic prophylaxis in SAP is ineffective for reducing the frequency of infected necrosis and to decrease hospital mortality. Antibiotic treatment using carbapenems and quinolones is indicated on demand in patients with SAP and multiorgan failure at admission and in those with hemodynamic shock. Patients with biliary acute pancreatitis (AP) and clinically acute cholecystitis and/or cholangitis

Biliary acute pancreatitis in mice is mediated by the G-protein-coupled cell surface bile acid receptor Gpbar1.

BACKGROUND & AIMS: The mechanisms by which reflux of bile acids into the pancreas induces pancreatitis are unknown. We reasoned that key events responsible for this phenomenon might be mediated by Gpbar1, a recently identified and widely expressed G-protein-coupled, cell surface bile acid receptor. METHODS: Acute pancreatitis was induced in wild-type and Gpbar1(-/-) mice by either retrograde ductal infusion of taurolithocholic acid-3-sulfate (TLCS) or supramaximal secretagogue stimulation with caerulein. In vitro experiments were performed in which acini obtained from wild-type and Gpbar1(-/-

[Changes in diagnostic and therapeutic standards of acute pancreatitis in clinical practice. Epidemiologic analysis of data from a regional center of internal medicine and surgery].

UNLABELLED: Epidemiological data analysis of a tertiary (regional) medical and surgical center. Diagnostic and therapeutic standards of patients with acute pancreatitis have changed significantly in the last few decades. Progress in laboratory and imaging diagnostics and achievements in experimental research resulted in a significant modification of the guidelines related to the care of pancreatitic patients. The aim was to analyse and compare the data of patients with acute pancreatitis treated in 1996 (period I) and 2004 (period II) at the Departments of Internal Medicine and Surgery, Univer

Acute pancreatitis: management update and future directions of pancreatic imaging.

Essential management points for AP are: 1. Intravenous fluids and adequate hydration are the mainstay of therapy. Antibiotics are not routinely indicated in most cases. 2. The most reliable marker for diagnosing biliary AP is a greater than threefold elevation of the serum alanine amniotrasferase (ALT), which has a positive predictive value of 95% for biliary acute pancreatitis. 3. Early ERCP with ES for stone extraction and biliary decompression has proved beneficial for patients with biliary pancreatitis and evidence of persistent or progressive biliary obstruction with elevated serum total

[Lipase/amylase ratio and liver function tests for the etiological assessment of acute pancreatitis: facts and fiction].

Acute pancreatitis represents the 0.15-1.5% of all diagnoses in the Emergency Room. Biliary diseases and alcohol abuse are the two mainly etiological factors of this illness in Italy. From a clinical point of view, the course of alcoholic and biliary acute pancreatitis is the same; however, because the endoscopic retrograde cholangiopancreatography associated with endoscopic sphincterotomy can prevent further complications in patients with severe biliary pancreatitis, it is important to early recognize the biliary origin of the disease. On the other hand, the identification of alcoholic origin
