Case Report: Vincristine-induced acute pancreatitis in pediatric Wilms tumor: first reported case challenging previous risk classifications and proposing vigilant monitoring protocols.

Vincristine, a cornerstone vinca alkaloid in pediatric oncology, has historically been regarded as a low-risk agent for drug-induced acute pancreatitis (DIAP). We report the first documented pediatric case of vincristine monotherapy-associated acute pancreatitis, challenging existing toxicity paradigms. A 3.25-year-old boy with stage IV Wilms tumor developed acute-onset fever and localized periumbilical pain 48 h after vincristine infusion (0.9 mg), administered within standard dosing. Laboratory testing confirmed marked pancreatic hyperenzymemia (serum α-amylase 2,022 U/L, 12.5 × ULN; lipase

Valproic Acid-Induced Pancreatitis in a Pediatric Patient With 17q12 Duplication Syndrome.

Drug-induced acute pancreatitis is an uncommon cause of acute pancreatitis. Among the common culprits is valproic acid (VPA) and its derivatives, i.e., Depakote. These medications are commonly prescribed for epilepsy, bipolar disorder, and migraines. Although VPA-induced pancreatitis is rare, it has a variable disease course and may progress to severe disease, which can become fatal. Our case reports a 14-year-old male with a history of 17q.12 duplication, focal epilepsy, autism spectrum disorder (ASD), and attention-deficit hyperactivity disorder (ADHD), who presented to the emergency departm

Azathioprine-Associated Acute Pancreatitis Followed by a Probable Autoimmune Hepatitis Flare After Thiopurine Withdrawal: A Case Report.

Azathioprine is commonly used in the management of inflammatory bowel disease and autoimmune hepatitis (AIH). Acute pancreatitis is a recognized idiosyncratic adverse effect that necessitates drug discontinuation. In patients with immune-mediated liver disease, abrupt withdrawal of immunosuppression may predispose to disease flares. We present a case of probable azathioprine-associated acute pancreatitis, followed by biochemical hepatitis consistent with a probable AIH flare after thiopurine cessation. This case highlights the clinical challenge of balancing adverse drug reactions with mainten

A Case of Recurrent Acute Pancreatitis After Gemcitabine + Nab-Paclitaxel and Modified FOLFIRINOX Therapy for Advanced Recurrent Pancreatic Cancer.

The incidence of drug-induced acute pancreatitis is an important consideration when developing treatment strategies for patients with pancreatic ductal adenocarcinoma. We report a case of recurrent acute pancreatitis that developed following both gemcitabine plus nab-paclitaxel (GnP) chemotherapy and modified FOLFIRINOX (mFFX) therapy. The clinical stage at initial diagnosis was cT3N1M0 stage IIB pancreatic cancer, with subsequent progression to metastatic disease over approximately 1.5 years. Treatment was prioritized at each stage of disease progression despite the occurrence of acute pancre

Acute Pancreatitis in a Young Patient With Type 2 Diabetes Mellitus Taking Sitagliptin and the Combined Oral Contraceptive Pill.

Acute pancreatitis is a common cause of hospital admission; however, cases secondary to medication use are rare. We report a case of drug-induced mild acute pancreatitis in a 33-year-old female with type 2 diabetes mellitus. The patient presented with a one-day history of vomiting and lower chest and epigastric pain after five months of starting sitagliptin therapy. She was also taking the combined oral contraceptive pill. Clinical examination revealed epigastric tenderness and signs of dehydration. Laboratory findings showed elevated white cell count, C-reactive protein, and triglyceride leve

Quetiapine-Induced Acute Pancreatitis in a 78-Year-Old Woman.

Quetiapine-induced pancreatitis is a rare but serious adverse effect of second-generation antipsychotics, often linked to metabolic derangements such as hypertriglyceridemia and diabetic ketoacidosis. We report a 78-year-old woman found unresponsive at home, with a history of nausea and vomiting in the preceding days. Laboratory evaluation revealed significantly elevated lipase and liver enzymes, and imaging confirmed acute pancreatitis. She had experienced multiple prior episodes of pancreatitis during long-term quetiapine therapy. Common causes, including alcohol use, gallstones, hypertrigly

Probable Vildagliptin-Associated Acute Pancreatitis in a Patient With Well-Controlled Type 2 Diabetes Mellitus: A Case Report.

Drug-induced acute pancreatitis (DIAP) is rare, accounting for approximately 0.1%-5% of all cases, and the association between dipeptidyl peptidase-4 (DPP-4) inhibitors and pancreatitis remains controversial despite postmarketing safety signals. We report the case of a 46-year-old man with well-controlled type 2 diabetes mellitus who presented with severe epigastric pain radiating to the back after six months of therapy with vildagliptin and metformin. Laboratory evaluation showed markedly elevated serum amylase (1,220 U/L) and lipase (2,334 U/L), exceeding three times the upper limit of norma

Acute pancreatitis following semaglutide therapy: a case report.

BACKGROUND: Acute pancreatitis (AP) is a potentially life-threatening condition that can result from various aetiologies. While gallstones and alcohol consumption are the most common causes, medications can also induce AP. In the absence of a clear cause, drug-induced acute pancreatitis (DIAP) may pose challenges for clinicians. This is especially true when addressing unusual and rare causes such as glucagon-like peptide-1 receptor agonists. CASE PRESENTATION: A 41-year-old Saudi man was admitted to the hospital with epigastric pain after receiving subcutaneous semaglutide for weight loss. La

Black Elderberry Exposure in Apparent Idiopathic Mild Acute Pancreatitis: A Case Report.

Drug-induced acute pancreatitis is uncommon and remains difficult to confirm, particularly when the suspected exposure is an herbal or over-the-counter supplement. Black elderberry, derived from Sambucus nigra, is widely marketed for immune support, but published literature linking it to acute pancreatitis is limited. We report a 75-year-old woman with hypertension, hyperlipidemia, and prior cholecystectomy who presented with abrupt severe epigastric pain radiating to the back, diaphoresis, and lipase 3016 U/L. Computed tomography of the abdomen and pelvis on day 0 showed a normal-appearing pa

Stem Cell-Derived Extracellular Vesicles for Acute Pancreatitis: a Systematic Review and Meta-analysis of Preclinical Studies.

BACKGROUND: Several studies have reported the effectiveness of stem cell-derived extracellular vesicles (SC-EVs) in disease treatment. However, the efficacy of SC-EVs for severe acute pancreatitis (SAP) remains uncertain. This systematic review aimed to analyze and evaluate the effect of SC-EVs in the treatment of SAP in animal models by summarizing data from published studies. METHODS: We searched Pubmed, Embase, and Web of Science databases to identify preclinical studies investigating the therapeutic effect of SC-EVs on SAP. The primary outcome was the histopathological scores of pancreati

Rare Case of Tacrolimus-Induced Acute Pancreatitis in a Liver Transplant Recipient.

Tacrolimus-induced acute pancreatitis (TAP) is rare and requires thorough evaluation, including genetic testing, to rule out other causes. While TAP has been documented in a few cases following kidney transplantation, we report the first case of TAP in an adult after liver transplantation, with a noteworthy feature of late-onset TAP occurring more than 12 months after initiating tacrolimus therapy. This case highlights the potential for delayed onset of TAP, and we suggest that tacrolimus may warrant reclassification from category III to Ic in the drug-induced pancreatitis classification. In a

Thiazide-Induced Acute Interstitial Pancreatitis in a 55-Year-Old Male: A Rare Adverse Drug Reaction.

Acute pancreatitis is inflammation of the pancreas and is most commonly caused by gallstones, alcohol use, hyperlipidemia, or idiopathic. Drug-induced acute pancreatitis is relatively rare and often under-recognized; it accounts for 0.1%-2% of all cases. Among many drugs, thiazide diuretics are infrequently implicated but remain a known cause. We report a case of a 55-year-old male who presented with acute epigastric pain, nausea, and vomiting 2 weeks after adding thiazide for persistent secondary hypertension. Laboratory investigations revealed elevated pancreatic enzyme levels, and contrast-
