Perioperative chemotherapy and stereotactic body radiotherapy versus perioperative chemotherapy alone in patients with high-risk, localised pancreatic cancer (MASTERPLAN): an open-label, phase 2, multicentre, randomised trial of the Australasian Gastro-Intestinal Trials Group.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) has poor outcomes and high rates of locoregional recurrence. MASTERPLAN evaluated whether stereotactic body radiotherapy (SBRT) reduced locoregional recurrence in patients with high-risk, localised PDAC. METHODS: MASTERPLAN was an open-label, randomised, phase 2 trial conducted across 14 hospitals in Australia and New Zealand. Eligible participants were aged 18 years or older with an Eastern Cooperative Oncology Group performance status of 0-1, and had previously untreated localised PDAC with at least one high-risk feature, including tumour

Preoperative Frailty and Risk of Post-ERCP Pancreatitis in Older Adults with Choledocholithiasis.

BACKGROUND: Frailty is increasingly considered when evaluating surgical risk in older adults. However, evidence regarding whether frailty contributes to the risk of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis in elderly patients is limited. OBJECTIVE: We aimed to evaluate whether preoperative frailty was associated with the development of post-ERCP pancreatitis (PEP) in elderly patients undergoing ERCP. METHODS: Patients aged 65 years or older who underwent ERCP for choledocholithiasis were retrospectively included. Frailty was assessed preoperatively using an ele

Endoscopic retrograde cholangiopancreatography-related pancreatitis and cholangitis differ according to biliary stricture anatomy: a cohort study.

OBJECTIVE: Post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) and cholangitis are common complications. We evaluated whether stricture anatomy affects ERCP-related complications in patients with biliary strictures. METHODS: From a prospectively maintained ERCP database, 1333 procedures with biliary strictures were screened. Patients with prior sphincterotomy, acute gallstone pancreatitis, chronic pancreatitis, postsurgical biliary strictures, or periampullary adenomas or carcinomas were excluded. Epidemiological, clinical, laboratory, radiological, and therapeutic d

Navigating the Pancreatic Duct: Up-to-Date Radiologic Expedition through Intraluminal Pathologic Entities.

Several congenital, traumatic, infectious, inflammatory, and neoplastic processes, as well as iatrogenic interventions, can alter intraluminal contents of the pancreatic duct, leading to adverse outcomes. Variant ductal anatomy may result in clinically significant complications such as duodenal obstruction, acute and chronic pancreatitis, and increased risk of developing gallbladder or biliary malignancies. The presence of calculi, blood, pus, and other unintended iatrogenic material in the pancreatic duct can be associated with a range of complications. Neoplastic involvement of the pancreati

Minimally invasive management of pediatric hereditary spherocytosis: severity-stratified biliary risk and indications for simultaneous cholecystectomy and biliary intervention at laparoscopic splenectomy - a single-center retrospective study.

BACKGROUND: Hereditary spherocytosis (HS) is an inherited hemolytic anemia in which splenectomy may be indicated in moderate to severe disease. Chronic hemolysis increases the risk of cholelithiasis and biliary complications, but optimal timing of cholecystectomy remains uncertain. METHODS: A retrospective single-center analysis of pediatric patients with moderate to severe HS who underwent laparoscopic splenectomy between January 2017 and January 2024. Evaluated variables included disease severity, demographics, cholelithiasis, biliary manifestations, requirement for endoscopic retrograde ch

Discovery of Novel STING Inhibitors for the Treatment of Severe Acute Pancreatitis.

Stimulator of interferon genes (STING) is a promising therapeutic target for severe acute pancreatitis (SAP), but effective small-molecule inhibitors remain limited. Here, structure-activity relationship studies identified compound D4 as a potent STING inhibitor with favorable cellular activity and safety. D4 directly engaged STING, inhibited its oligomerization and downstream TBK1/IRF3 signaling, and restored macrophage phagocytosis, autophagic flux, and mitochondrial function while suppressing NOD-like receptor family pyrin domain containing 3(NLRP3)-associated pyroptosis. D4 showed oral exp

Venous Thromboembolism in Acute Necrotizing Pancreatitis Is Associated With Worse Outcomes, With Subtype-Specific Patterns: A Nationwide Analysis, 2016-2022.

OBJECTIVES: Venous thromboembolism (VTE) complicates acute necrotizing pancreatitis (ANP), but outcomes across extra-splanchnic deep vein thrombosis (DVT), portal vein thrombosis (PVT), and splenic vein thrombosis (SVT) remain poorly defined. We evaluated national trends and in-hospital outcomes in ANP with versus without VTE and across VTE subtypes. METHODS: Using the National Inpatient Sample (2016-2022), adults hospitalized with principal ANP were identified. Secondary diagnosis codes captured VTE subtypes. Primary analyses compared mortality and major complications in ANP with versus with

Endoscopic ultrasound in hepatology: A European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review.

BACKGROUND: Endoscopic ultrasound (EUS)-guided techniques are increasingly applied in hepatology, including for tissue acquisition, vascular access, pressure measurement, and selected therapeutic interventions. However, adoption remains heterogeneous because of variability in technical approaches, devices, outcome definitions, operator expertise, and the quality of available evidence. METHODS: This European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review summarizes the current evidence on selected EUS-guided techniques in liver disease and portal hypertension, inc

Mechanistic insights into TLR2-mediated acute pancreatitis exacerbation via macrophage-ferroptotic acinar cell crosstalk.

Ferroptosis is closely linked to increased inflammatory responses and plays a vital role in the development of acute pancreatitis. We investigated how this process contributes to pancreatic injury in AP and examined the interactions between ferroptotic acinar cells and macrophages. Our study profiles the crosstalk mechanisms between ferroptotic acinar cells and macrophages. These findings, derived from human peripheral blood cohorts, were validated in vivo and in vitro. Multi-omics analyses revealed a reduction in pancreatic acinar cells owing to abnormal ferroptosis activation, as confirmed i

Impact of Obesity on Chemotherapy Pharmacokinetics, Toxicity, and Survival Outcomes in Pediatric Acute Lymphoblastic Leukemia.

Obesity is increasingly prevalent among children and adolescents and may influence chemotherapy pharmacokinetics, treatment-related toxicity, and clinical outcomes in pediatric acute lymphoblastic leukemia (ALL). This narrative review evaluates current evidence regarding the effects of obesity on drug disposition, chemotherapy-associated toxicity, dosing considerations, and survival outcomes in pediatric ALL. Obesity alters body composition and may affect hepatic and renal physiology; however, its pharmacokinetic effects vary among therapeutic agents. Studies demonstrate associations between g

A gastric dieulafoy lesion presenting with life-threatening upper gastrointestinal hemorrhage during recovery from hypertriglyceridemic acute pancreatitis: a case report.

BACKGROUND: Dieulafoy lesions are an uncommon but potentially life-threatening cause of non-variceal upper gastrointestinal bleeding, caused by rupture of a large-caliber submucosal artery through a minute mucosal defect; diagnosis is difficult because bleeding is intermittent and the surrounding mucosa appears nearly normal. Hemorrhage from a gastric Dieulafoy lesion during recovery from hypertriglyceridemic acute pancreatitis (HTG-AP) has rarely been reported. CASE PRESENTATION: A man in his twenties with severe obesity presented with acute epigastric pain after alcohol and a high-fat meal.

Metabolic Syndrome Is Associated With Worsened Clinical Outcomes in Individuals With Necrotizing Pancreatitis.

GOALS: Evaluate the relationship between metabolic syndrome (MetS) and clinical outcomes in individuals with necrotizing pancreatitis (NP) and infected pancreatic necrosis (IPN). BACKGROUND: MetS affects a significant proportion of the global population and is associated with low-grade systemic inflammation and immune dysregulation. This has implications for several disease processes, but the impact of MetS on patients with NP is not well characterized. STUDY: A retrospective cohort study of patients admitted with NP at a single tertiary center was performed. MetS was defined as meeting any

A novel dual-epitope ELISA reveals heterogeneity of circulating lipoprotein lipase: distinguishing intact LPL from total LPL.

BACKGROUND: Conventional immunoassays measure total circulating lipoprotein lipase (LPL) but do not distinguish molecular species retaining both terminal regions. We developed a dual-epitope E LISA that measures LPL simultaneously recognized through defined N- and C-terminal epitopes, operationally termed "intact LPL." METHODS: The F1A/R23B ELISA was evaluated using recombinant full-length LPL, analytical validation, immunoblotting, reciprocal immunoprecipitation-immunoblotting, post-heparin kinetics, LPL activity measurements, and ex vivo tetrahydrolipstatin treatment. Clinical analyses incl

Assessment of serum micronutrient concentration in chronic pancreatitis: insights from the nationwide COMBO randomised trial.

INTRODUCTION: In patients with chronic pancreatitis (CP), pancreatic exocrine insufficiency (PEI) and poor dietary intake may cause malnutrition. International guidelines recommend monitoring key micronutrients (MN), however clinical implications remain uncertain. This study aims to evaluate the prevalence, risk factors, complications, and longitudinal course of low serum MNs in patients with CP. METHODS: This is a predefined analysis of the COMBO trial, a nationwide stepped-wedge cluster randomised trial on evidence-based CP management. Nutritional biochemical assessment included vitamins A,

Evaluation of Pirfenidone for Recurrent Acute Pancreatitis: Protocol for a Randomized Trial.

INTRODUCTION: Recurrent acute pancreatitis (RAP) is defined as a syndrome of two or more episodes of acute pancreatitis (AP), with a period of resolution in between. Management of RAP is largely supportive, with no available drug therapy to prevent recurrence or progression to chronic pancreatitis (CP). Our preclinical data suggest that pirfenidone can improve parameters of RAP and decrease progression to CP. Therefore, we decided to conduct a pilot trial to evaluate pirfenidone in patients with RAP. METHODS: We have designed a double-blinded, multicenter, randomized, pilot clinical trial inv

Falciform Ligament Inflammation Complicated by Umbilical Vein Thrombophlebitis, Left Portal Vein Thrombosis, and Multiple Hepatic Abscesses: A Case Report.

Falciform ligament pathology is a rare cause of acute upper abdominal pain and may resemble more common pancreatic and hepatobiliary disorders. A woman in her 50s presented with sudden severe epigastric pain radiating to her back and marked hyperamylasaemia of 1,747 U/L. This combination fulfilled clinical and biochemical diagnostic criteria for acute pancreatitis, although computed tomography (CT) demonstrated no pancreatic inflammation and instead showed severe inflammatory change centred on the falciform ligament, with associated focal fat infarction, left portal vein thrombosis, and gas lo

Mixed Presentation of Hyperglycemic Hyperosmolar State and Diabetic Ketoacidosis in a Pediatric Patient: a Case Report and Literature Review.

The coexistence of diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar state (HHS), known as mixed hyperglycemic crisis, is a rare but potentially fatal metabolic emergency in pediatrics, and is frequently underrecognized because it is misclassified as isolated DKA. It carries a higher risk of complications than either entity alone, particularly in patients with complex comorbidities. We describe the case of a 16-year-old female with myelomeningocele, neurogenic bladder, obesity, and insulin resistance, who had self-discontinued treatment with metformin, atorvastatin, and allopurinol on

Not Everything Has a Single Diagnosis: A Case of Hematemesis, Pneumothorax, and Acute Pancreatitis Mimicking Boerhaave Syndrome.

Patients presenting with multiple unrelated complaints are a common occurrence in the emergency department and one that poses significant difficulty. While we as physicians are trained to look for a unifying diagnosis, we must never forget "Hickam's dictum," stating that a patient may have multiple simultaneous diagnoses. A 48-year-old man presented to the emergency department with a history of hematemesis, acute shortness of breath, epigastric pain, and right-sided chest pain, after a bout of alcohol binge. On examination, the patient had reduced air entry on the right side, raising suspicion

Merkel Cell Carcinoma Masquerading as Dermatitis.

Merkel cell carcinoma (MCC) is a rare, highly aggressive neuroendocrine skin carcinoma that typically presents as a rapidly enlarging, firm, painless nodule on sun-exposed areas in elderly patients. This malignancy is characterized by high rates of local recurrence, regional spread, and distant metastasis. We report a case of a woman in her 80s who presented with an erythematous patch on the left anterior thigh, a non-sun-exposed site, initially diagnosed as inflammatory dermatitis and treated with topical corticosteroids. After two months of failed therapy and interval growth, a punch biopsy
