Efficacy and safety of sivelestat sodium hydrate adjunct to remifentanil for analgesia in patients under mechanical ventilation (SIDING trial): protocol of a multicenter, randomized, double-blinded, placebo-controlled trial.

INTRODUCTION: Inadequate analgesia is associated with worse outcomes in patients under mechanical ventilation. Current guidelines recommend opioids as the first-line analgesics. However, prolonged use of opioids is associated with numerous adverse effects. Preclinical studies have shown that sivelestat sodium hydrate attenuates pain-related behaviours in rodent models; however, its analgesic and opioid-sparing effects in humans remain unknown. This study aims to investigate whether sivelestat reduces remifentanil requirements while maintaining adequate analgesia in mechanically ventilated pati

Red Cell Distribution Width-to-Platelet Count Ratio as an Early Marker of Severity in Acute Pancreatitis: A Diagnostic Accuracy Study at a Tertiary Care Center.

INTRODUCTION: Early identification of clinically significant acute pancreatitis is essential for timely escalation of care, yet established scoring systems are multiparametric, and several require 48-hour data. The red cell distribution width (RDW)-to-platelet count ratio is a composite index derived from the routine complete blood count that may reflect the systemic inflammatory burden of the disease. This study assessed its accuracy for identifying moderately severe or severe disease at admission. METHODS: Consecutive adults admitted with acute pancreatitis were enrolled on accrual at a ter

Impact of Glucagon-Like Peptide 1 (GLP-1) and Gastric Inhibitory Polypeptide (GIP) Receptor Agonists on Intraocular Pressure in Glaucoma: A Case Series.

PURPOSE: Glucagon-Like Peptide 1 Receptor Agonists (GLP-1RAs) are antidiabetic and weight management medications that bind to GLP-1 receptors throughout the body. Growing interest in this class has led to several retrospective cohort studies reporting a decreased occurrence of glaucoma in diabetic and non-diabetic patients with the use of GLP-1RAs, suggestive of anti-inflammatory and neuroprotective effects. Here we report five real world cases evaluating the impact of GLP-1RA and dual GLP-1/Gastric Inhibitory Polypeptide (GIP) receptor agonist use on Intraocular Pressure (IOP) measurements an

GLP-1 receptor agonists in elective spine surgery: a systematic review of fusion outcomes, perioperative safety, and procedure-specific risk.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are increasingly used in patients with obesity and type 2 diabetes mellitus undergoing elective spine surgery. Although these agents may improve metabolic risk profiles, their associations with spinal fusion, perioperative complications, and procedure-specific outcomes remain incompletely defined. METHODS: We conducted a systematic review and qualitative synthesis of comparative clinical studies evaluating adults undergoing spine surgery with documented preoperative or perioperative GLP-1 RA exposure. Eligible studies assessed

Indication-specific adverse event profiles of semaglutide and liraglutide: a FAERS disproportionality analysis.

OBJECTIVE: By 2021, regulatory agencies had approved two glucagon-like peptide-1 receptor agonists (GLP-1RAs), semaglutide and liraglutide, for the treatment of type 2 diabetes and obesity. Since then, concerns have emerged about whether adverse events (AEs) differ by GLP-1RA or indication. RESEARCH DESIGN AND METHODS: We analyzed the Food and Drug Administration Adverse Event Reporting System (FAERS) reports (2021-2025) to compare adverse events associated with semaglutide and liraglutide indications. Comparisons with metformin and orlistat helped distinguish drug-related signals from diseas

Risk of recurrent acute pancreatitis with GLP-1 receptor agonists after prior pancreatitis: a systematic review.

INTRODUCTION: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are widely used for patients with type 2 diabetes (T2D) and obesity. However, their safety in patients with prior pancreatitis remains uncertain. This systematic review aims to evaluate the risk of recurrent acute pancreatitis (RAP) associated with GLP-1 RA therapy. METHODS: We systematically searched for studies evaluating the safety of GLP-1 receptor agonists in adults with type 2 diabetes mellitus (T2DM) or obesity and a history of acute pancreatitis. The primary outcome was recurrent acute pancreatitis. We included studie

Effects of Febuxostat, a Non-purine Xanthine Oxidase Inhibitor, on Cerulein-induced and L-Arginine-induced Acute Pancreatitis in Rats.

OBJECTIVES: The effects of xanthine oxidase (XO) inhibitors on acute pancreatitis (AP) have only been reported for allopurinol, with controversial results. We investigated the effects of febuxostat, a potent and selective XO inhibitor, in two AP rat models and compared the results with those of allopurinol. METHODS: Using cerulein- and L-arginine-induced AP models, we evaluated the effects of the two XO inhibitors based on histopathological and biochemical parameters, including serum amylase, lipase, blood urea nitrogen, pancreatic malondialdehyde, glutathione peroxidase, reduced-to-total glu

Integrative single-cell and bulk transcriptomic analysis identify R-loop-associated gene signatures driving immune-inflammatory networks in acute pancreatitis.

BACKGROUND: R-loop dysregulation has been implicated in chronic inflammation via DNA damage and cytoplasmic ssDNA release, yet its role in acute pancreatitis (AP) remains to be systematically investigated. This study aimed to identify R-loop-related biomarkers in AP and elucidate their potential roles in immune-inflammatory networks through integrative bioinformatics analysis and in vitro experimental validation. METHODS: Single-cell and bulk transcriptomic datasets from the GEO database were integrated to calculate R-loop-related gene (RLRG) scores across cell types using multiple scoring al

Asperginase-based chemotherapy regimen for acute lymphoblastic leukemia: A cohort study with promising outcomes on adults.

BACKGROUND: Acute lymphoblastic leukemia (ALL) is a malignancy of lymphoid progenitor cells. Asparaginase-based regimens are highly effective in pediatric ALL, but concerns over toxicity have limited their use in adults. This study evaluates the incidence of adverse events following asparaginase treatment in the adult ALL population. MATERIALS AND METHODS: In this retrospective cohort study, we assessed drug-related adverse events among 67 adults with ALL treated with either a hyper-central vascular access device (CVAD) (n = 34) or an asparaginase-containing regimen (n = 33). Those under aspa

CFTR variant-associated hereditary pancreatitis complicated by diabetes mellitus: a case report and literature review.

Hereditary pancreatitis (HP) is a rare autosomal dominant disorder characterized by recurrent episodes of pancreatic inflammation leading to chronic pancreatic damage. We report a 38-year-old Chinese male with a heterozygous cystic fibrosis transmembrane conductance regulator (CFTR) intronic variant (c.1210-11T>G) who presented with pancreatic diabetes (type 3c diabetes) and an otherwise "silent" pancreatic phenotype. He had a family history of similar manifestations (father and brother with pancreatogenic diabetes). Genetic testing identified the CFTR c.1210-11T>G variant, which is known to a
