Targeting TGF-βR1 Signaling Pathway in Pancreatic Cancer: A Potential Approach with Synthetic Flavanols.

INTRODUCTION: Pancreatic adenocarcinoma is a highly aggressive cancer with a poor prognosis and a five-year survival rate of just 13%. Its asymptomatic onset, rapid progression, and resistance to therapy make it challenging to treat. Transforming Growth Factor-β (TGF-β) signaling, particularly through TGF-β Receptor 1 (TGF-βR1/ALK-5), plays a major role in tumor progression by inducing Epithelial-Mesenchymal Transition (EMT), immune evasion, and apoptosis resistance. Targeting ALK-5 is a promising strategy for therapeutic intervention. METHODS: Twenty-nine synthetic flavonols were designed to

Integrated screens reveal that guanine nucleotide depletion, which is irreversible via targeting IMPDH2, inhibits pancreatic cancer and potentiates KRAS inhibition.

BACKGROUND: Over 90% pancreatic cancers harbour activating kirsten rat sarcoma viral oncogene homolog (KRAS)mutations. However, monotherapies targeting the KRAS vertical pathway, with recently developed KRAS inhibitors or rapidly accelerated fibrosarcoma (RAF)/MEK/ERK inhibitors, have demonstrated limited clinical benefit. Therefore, there is an urgent need to identify novel therapeutic targets and combination strategies with KRAS inhibition. OBJECTIVE: This study aims to identify pharmaceutical targets whose inhibition suppresses pancreatic ductal adenocarcinoma (PDAC) or potentiates KRAS in

Lipid signaling networks in pancreatic cancer progression and therapeutic perspectives.

Pancreatic ductal adenocarcinoma (PDAC) remains one of the deadliest cancers, driven by aggressive biology, profound therapy resistance, and scarce treatment options. Beyond classical metabolic rewiring, recent discoveries reveal that lipid metabolism plays a central and previously underappreciated role in PDAC progression. Emerging evidence shows that fatty acids, cholesterol, lysophospholipids, sphingolipids, and oxidized lipid species actively shape oncogenic signaling, influence stromal and immune interactions, and contribute to metastasis and treatment evasion. In this review, we highligh

Pharmacogenomic profiling of the efficacy of gemcitabine monotherapy in metastatic pancreatic cancer: Subgroup analysis of the GENESECT study.

AIM: In the GENESECT study, no significant gemcitabine (GEM) metabolism-related germline genetic polymorphisms (GPs) were identified because approximately 70% of patients received combination therapy with nab-paclitaxel, which has metabolic pathways different from GEM. The present study was a sub-analysis including only GEM monotherapy patients. METHODS: Of 159 patients analysed in the GENESECT study, only GEM-monotherapy patients were selected. The endpoints included carbohydrate antigen 19-9 (CA19-9) response (reduction ≥ 50% from the pretreatment level at 8 weeks), progression-free surviva

Translation of a novel EphA2-targeted PET tracer from preclinical models to first-in-human studies in pancreatic cancer.

PURPOSE: Ephrin type-A receptor 2 (EphA2) is a promising target in pancreatic ductal adenocarcinoma (PDAC). However, the clinical potential of targeting EphA2 requires further imaging evaluation. This study developed and preliminarily evaluated a novel 68Ga-labeled radiotracer for PET imaging of EphA2 expression in PDAC patients. METHODS: 68Ga-FZEAR-1, 68Ga-FZEAR-2, and 68Ga-FZEAR-3 were synthesized, and their stability, affinity, pharmacokinetics were evaluated in vitro and in vivo. Further biological evaluation was performed in EphA2-positive and EphA2-negative tumor xenografts. A pilot fir

MYC-Mediated USP39 Upregulation Stabilizes SRSF1 in Pancreatic Cancer.

UNLABELLED: Pancreatic ductal adenocarcinoma (PDAC) is a highly lethal malignancy primarily driven by oncogenic KRAS signaling. The splicing factor serine/arginine-rich splicing factor 1 (SRSF1) plays a key oncogenic role in PDAC through reciprocal cross-interactions with KRAS signaling. However, the mechanisms regulating SRSF1 protein stability remain poorly understood. In this study, we identify the deubiquitinase USP39 as a critical regulator of SRSF1 stability. It interacts with SRSF1 in an RNA-independent manner and suppresses its ubiquitination. USP39 is upregulated in PDAC and correlate

Comparison of [111In]octreotide Scintigraphy and [68Ga]DOTATOC PET in Gastroenteropancreatic Neuroendocrine Tumors: Concordance of Krenning Scores and Implications for Peptide Receptor Radionuclide Therapy Eligibility.

BACKGROUND: Peptide receptor radionuclide therapy (PRRT) is an established treatment for somatostatin receptor (SSTR)-positive gastroenteropancreatic neuroendocrine tumors (GEP-NETs), with eligibility determined by the Krenning score. While SSTR PET provides greater sensitivity than octreotide scintigraphy, its impact on PRRT eligibility remains unclear. We compared Krenning scores between [111In]octreotide scintigraphy and [68Ga]DOTATOC PET and assessed eligibility under NETTER-1 and NETTER-2 criteria. METHODS: We retrospectively assessed tumor Krenning scores in 115 patients with well-diffe

Improving pancreatic adenocarcinoma prognosis models through ligand receptor interactions and histopathological integration.

BACKGROUND: Pancreatic adenocarcinoma (PAAD) has an extremely poor prognosis, and existing prognostic markers fail to fully capture the complex heterogeneity of the tumor microenvironment. This study aimed to integrate ligand-receptor (L-R) interactions, multi-omics data, and deep learning-based pathological images to construct an interpretable multimodal prognostic model and to elucidate the mechanisms underlying the cancer-associated fibroblast (CAF) microenvironment. METHODS: Significant L-R interactions were identified using BulkSignalR, followed by sequential Cox, least absolute shrinkag

Long-term disease control of GP2-NTRK1 fusion-positive pancreatic adenocarcinoma with liver metastases using sequential tyrosine receptor kinase inhibitors.

We report an extremely rare case of GP2-NTRK1 fusion-positive pancreatic ductal adenocarcinoma (PDAC) with liver metastases in a 78 year-old woman. PDAC is an aggressive cancer that presents with locally advanced or metastatic disease in more than 50% of patients at the time of diagnosis. This is the first case of GP2-NTRK1 fusion-positive PDAC that was successfully treated with gemcitabine and nab-paclitaxel followed by entrectinib and larotrectinib, which resulted in long-term survival of more than 54 months from diagnosis and 34 months from initiation of TRK inhibitor therapy.

Single-cell and bulk transcriptomics identify senescence-related EMT transcriptional programs and a prognostic framework in pancreatic ductal adenocarcinoma.

BACKGROUND: Senescence-related transcriptional programs and epithelial-mesenchymal transition (EMT) are implicated in pancreatic ductal adenocarcinoma (PDAC) progression and therapy resistance. However, the single-cell distribution, functional heterogeneity, and clinical relevance of EMT-associated senescence-like programs remain incompletely defined. METHODS: We integrated single-cell RNA sequencing (scRNA-seq) data from 10 treatment-naive PDAC tumors with bulk transcriptomic profiles from TCGA-PAAD and an independent validation cohort. Senescence-related transcriptional activity was estimat

Lactylation-driven stabilization of the S100A11/ANXA2 complex promotes pancreatic cancer metastasis.

Pancreatic ductal adenocarcinoma (PDAC) is characterized by a pronounced Warburg effect and high lactate levels. While lysine lactylation (Kla) is an emerging post-translational modification, its role in regulating the S100A11/ANXA2 complex-a critical driver of membrane repair and metastasis-remains unexplored. In this study, we identified that S100A11 (K3, K55) and ANXA2 (K49) are significantly lactylated in PDAC tissues. Lactate-induced lactylation of S100A11 at K3 and K55 inhibits FBXW11-mediated ubiquitination and proteasomal degradation, thereby stabilizing the protein. This stabilized S1

ALKBH5 in Cancer-Associated Fibroblasts Governs an Epitranscriptomic Axis That Drives Pancreatic Cancer Metastasis.

UNLABELLED: Cancer-associated fibroblasts (CAF) are principal determinants of pancreatic ductal adenocarcinoma (PDAC) progression. CAFs can shape tumor behavior via multiple pathways, underscoring the need for a complete understanding of the regulatory mechanisms that govern CAF function. In this study, we identified N6-methyladenosine (m6A) remodeling as a hallmark of CAF activation and defined a critical role for the m6A demethylase ALKBH5 in PDAC metastasis. Activated CAFs exhibited a global reduction in m6A abundance, with ALKBH5 emerging as a key regulator of the CAF epitranscriptome. Fun

Cell-Selective Delivery of RIBOTACs via an Anti-EGFR Nanobody for Pancreatic Cancer Treatment.

Pancreatic ductal adenocarcinoma (PDAC) remains a therapeutic challenge due to its dense stroma and lack of druggable targets. Through integrated bioinformatics analysis of in-house and public datasets, we systematically identified miR-21 as the most significantly upregulated oncomiR in PDAC, showing strong correlation with poor patient prognosis. RNA-targeted degradation has emerged as a promising strategy for cancer treatment, enabling precise disruption of oncogenic signaling by degrading disease-driving non-coding RNAs. Nevertheless, poor tissue penetration and insufficient tumor specifici

Combining an αvβ6-Targeted 177Lu-Based Peptide Receptor Radionuclide Therapy with Olaparib to Boost Therapeutic Efficacy in Pancreatic Cancer.

Pancreatic ductal adenocarcinoma is one of the most lethal malignances worldwide, and there remains an urgent need for more effective and less toxic treatment strategies. Integrin αvβ6 is a cell-surface receptor that is overexpressed in several malignancies, including pancreatic ductal adenocarcinoma, and plays a key role in invasion and metastasis, making it a promising molecular target for the detection and treatment of many cancers. We investigated a molecularly targeted approach that exploits the overexpression of αvβ6 using peptide receptor radionuclide therapy (PRRT) in combination with

Redefining Oligometastatic Pancreatic Ductal Adenocarcinoma: Biology, Diagnosis, and Precision Multimodal Therapy.

BACKGROUND: Given the dismal prognosis of metastatic pancreatic ductal adenocarcinoma, oligometastatic disease (OM-PDAC) has emerged as a distinct clinical entity, challenging systemic-only paradigms via advanced multimodal therapies. RECENT FINDINGS: Retrospective data indicate that anatomical burden alone inadequately guides patient selection. Optimal management requires integrating high-quality imaging, liquid biopsies, and molecular profiling to identify biologically favorable disease. Patients achieving durable responses to induction chemotherapy show improved survival following local in

Trans-anastomotic stent does not impact the predictive value of drain fluid amylase after pancreatoduodenectomy.

BACKGROUND: Drain fluid amylase (DFA) guides risk stratification and drain management after pancreatoduodenectomy (PD), but its interpretation in patients with trans-anastomotic stents (TS) remains poorly defined. METHODS: We retrospectively analyzed multicenter PD data, focusing on TS-managed patients. DFA was measured on postoperative days (POD) 1, 3, and 5. The primary outcome was clinically relevant postoperative pancreatic fistula (CR-POPF), with major morbidity (Clavien-Dindo ≥ IIIa) as a secondary outcome. Discrimination was evaluated by adding log-transformed DFA to a baseline model i

Periductal Fibroblast Density Defines Lymphocyte Exclusion via a CD44-Dependent Stromal Checkpoint in Pancreatic Cancer.

Pancreatic ductal adenocarcinoma (PDAC) exhibits dense fibrosis and immune exclusion. Although fibrosis has been studied globally and at the region-of-interest level, its impact on stromal-ductal architecture and immune cell localization remains unknown. In this study, we establish cancer-associated fibroblast (CAF)-stratified ductal spatial architecture as a fundamental determinant of immune exclusion in PDAC. Focusing on malignant PDAC epithelial ductal regions, the critical interface in which immune cells must access tumor epithelium, we demonstrate that periductal fibroblast organization d
