Discovery of Novel STING Inhibitors for the Treatment of Severe Acute Pancreatitis.

Stimulator of interferon genes (STING) is a promising therapeutic target for severe acute pancreatitis (SAP), but effective small-molecule inhibitors remain limited. Here, structure-activity relationship studies identified compound D4 as a potent STING inhibitor with favorable cellular activity and safety. D4 directly engaged STING, inhibited its oligomerization and downstream TBK1/IRF3 signaling, and restored macrophage phagocytosis, autophagic flux, and mitochondrial function while suppressing NOD-like receptor family pyrin domain containing 3(NLRP3)-associated pyroptosis. D4 showed oral exp

A Near-Infrared-II Antibody-Photosensitizer Conjugate for Photoimmunotherapy of Advanced Pancreatic Cancer.

Pancreatic cancer (PC) is a highly lethal malignancy, characterized by frequent late-stage diagnosis and limited response to available treatments. Photoimmunotherapy, which combines the localized tumor ablation of phototherapy with the systemic immune activation of immunotherapy, presents a promising strategy. However, its success is often hampered by insufficient accumulation of therapeutic agents at the tumor site. To address this challenge, we developed a novel antibody-photosensitizer conjugate, named αPD-L1@AIE, by conjugating a multifunctional phototheranostic agent with αPD-L1 antibody

Biomimetic Dual-Driven Janus Nanorobots for Enhanced Penetration and Combined Gas/Photothermal/Photodynamic/Catalytic Therapy of Pancreatic Cancer.

Pancreatic cancer (PC) remains an extremely lethal malignancy due to its dense desmoplastic stroma and aberrant tumor microenvironment (TME), which severely impedes the efficacy of conventional therapies. Herein, a biomimetic dual-driven Janus nanorobot (rCeGLI@PM) is strategically constructed via the asymmetric decoration of l-arginine (l-Arg)-functionalized and indocyanine green (ICG)-loaded poly(amidoamine) dendrimers onto CeOx nanozymes, followed by homologous Panc02 cell membrane camouflage. The formed nanorobots exhibit precise tumor accumulation via homologous targeting and deep penetra

Albumin-Binding Modification Improves Tumor Retention of Hydrophilic αvβ6-Targeting Radiotheranostic Tracers.

Integrin αvβ6 is an attractive theranostic target for pancreatic ductal adenocarcinoma (PDAC). Existing αvβ6-targeting tracers are limited by relatively low tumor uptake and rapid tumor washout, compromising theranostic efficacy. In this study, a series of αvβ6-targeting ligands with different hydrophilic linkers and/or albumin-binding moiety were designed and evaluated with molecular docking and MD simulations. Among them, NOTA-Asp2-IPB-αvβ6L was selected for further theranostic evaluation due to its favorable targeting capability, enhanced tumor uptake and favorable pharmacokinetic propertie

Comprehensive insight into the anti-dyslipidemic and pancreatic lipase inhibition activities of Clematis flammula L. and Fraxinus angustifolia Vahl. extracts using two animal models.

Clematis flammula and Fraxinus angustifolia leaves are commonly used in Algeria to treat inflammatory diseases, with potential hypolipidemic effects that have not been investigated so far. The present study explored the in vitro, ex vivo, and in vivo hypolipidemic effects of ethanolic leaf extracts from both plants using two animal models (mice and rabbits) with distinct lipid metabolism. Dyslipidemia was induced by oral administration of Triton X-100, once daily for three consecutive days. Immediately after each Triton X-100 administration, animals received plant extracts (100 and 200 mg/kg b

MAF Transcription Factors: Emerging Key Players in Cardiovascular Health and Disease.

Cardiovascular diseases are a leading cause of morbidity and mortality worldwide, and their complex pathophysiological mechanisms remain incompletely understood. The regulatory role of transcription factors provides a crucial avenue for investigating the mechanisms underlying these diseases. The MAF transcription factor family is a key member of the AP-1 superfamily, comprising two subfamilies: large MAFs (including MAFA, MAFB, MAF [c-MAF], and NRL) and small MAFs (including MAFK, MAFG, and MAFF). Different members exhibit distinct cellular expression patterns and functions within tissues: sma

Mechanistic insights into TLR2-mediated acute pancreatitis exacerbation via macrophage-ferroptotic acinar cell crosstalk.

Ferroptosis is closely linked to increased inflammatory responses and plays a vital role in the development of acute pancreatitis. We investigated how this process contributes to pancreatic injury in AP and examined the interactions between ferroptotic acinar cells and macrophages. Our study profiles the crosstalk mechanisms between ferroptotic acinar cells and macrophages. These findings, derived from human peripheral blood cohorts, were validated in vivo and in vitro. Multi-omics analyses revealed a reduction in pancreatic acinar cells owing to abnormal ferroptosis activation, as confirmed i

Pancreatic Neuroendocrine Microtumors: Etiology, Diagnostic Criteria, and Differential Diagnosis.

BACKGROUND: Pancreatic neuroendocrine microtumors represent small, well-differentiated neuroendocrine neoplasms (<5 mm) that are widely regarded as early lesions within the spectrum of pancreatic neuroendocrine tumorigenesis; however, their biological behavior remains a matter of an ongoing debate. SUMMARY: Although frequently associated with hereditary tumor syndromes such as multiple endocrine neoplasia type 1 and von Hippel-Lindau disease, pancreatic neuroendocrine microtumors also occur sporadically and are increasingly detected incidentally. In this review, we summarize their histopathol

Metabolic Barriers to Cancer Immunotherapy: Multiscale Mechanisms from Organelle Dysfunction to Organ-Specific Constraints.

Immune checkpoint blockade (ICB) has reshaped cancer therapy, yet primary resistance and acquired relapse remain common despite therapeutic targeting of inhibitory receptor pathways. This limitation suggests that checkpoint release may remain insufficient when antitumor lymphocytes lack the metabolic, organelle-level, spatial, or systemic conditions required for tissue entry, functional recovery, and durable effector activity. In this review, we organize metabolic barriers to ICB into a multiscale framework comprising four interconnected axes. The Local Resource Axis captures how nutrient comp

Surgery as Metabolic Therapy: Expanding the Role of Gastrointestinal Surgery Beyond Anatomical Reconstruction.

Gastrointestinal surgery comprises procedures used to treat diseases and injuries of the digestive tract. Evidence now shows that some gastrointestinal procedures are also systemic metabolic interventions. Bariatric and metabolic surgery intentionally treats obesity and associated metabolic disease and can improve type 2 diabetes, metabolic dysfunction-associated steatotic liver disease, and cardiometabolic risk, sometimes before substantial weight loss occurs. Other procedures, including hepatectomy, pancreatic resection, and bowel resection, are generally performed for non-metabolic indicati

Ginsenoside Re attenuates ferroptosis induced by hepatic ischemia-reperfusion injury via the BAP1/SLC7A11 axis.

Hepatic ischemia-reperfusion injury (HIRI) is a complex clinical complication involving inflammation and multiple forms of regulated cell death, among which ferroptosis plays a critical role. Ginsenoside Re (Re) exhibits prominent antioxidant capabilities, but its exact molecular targets in HIRI are unidentified. Here, using in vivo HIRI and in vitro hypoxia/reoxygenation (H/R) models, we found that Re administration exhibited an optimal therapeutic window, significantly alleviating hepatocellular damage, inflammatory responses, and oxidative stress. Sequentially, Re suppressed ferroptosis, as

Protective Effects of PD-L1-Overexpressing MSCs and Their Derived Extracellular Vesicles in Type 1 Diabetes.

This study was conducted to address the urgent need for novel therapies for type 1 diabetes. We investigated whether programmed death-ligand 1 (PD-L1)-overexpressing mesenchymal stromal/stem cells (PD-L1-MSCs) and their extracellular vesicles (PEVs) can modulate immune responses and protect pancreatic islets, and whether these effects depend on PD-L1 signaling. Our results demonstrate that both PD-L1-MSCs and PEVs suppress T-cell activity, enhance regulatory T-cell populations, delay the onset of diabetes, and reduce immune cell infiltration in pancreatic islets. These findings suggest that PD

Exploiting Acquired Drug Resistance in Pancreatic Cancer through Selective Covalent Targeting of MAP2K4/7.

Pancreatic ductal adenocarcinoma (PDAC) is highly lethal and remains largely refractory to targeted therapy despite its near-universal dependence on oncogenic KRAS. Single-agent KRAS inhibitors often fail due to adaptive resistance mechanisms that emerge under pharmacologic pressure. Previous studies combining MAP2K4 inhibition with KRASG12C-targeted therapy determined that partial JNK suppression can enhance anticancer responses. Here, we extend this concept to PDAC, which is predominantly driven by KRASG12D mutations, and demonstrate that full JNK pathway inactivation restores sensitivity to

A Dual-Action CXCR4-Targeted Peptide-Drug Conjugate Engineered to Enhance Antitumor Efficacy and Mitigate Myelosuppression in Pancreatic Cancer.

Gemcitabine (GEM) is a first-line therapeutic option for pancreatic cancer; however, it has low efficacy due to rapidly developed drug resistance and severe dose-limiting myelosuppression. To enhance its therapeutic effect, this study developed a novel peptide-drug conjugate using a CXCR4 antagonistic peptide as the targeting head and gemcitabine as the drug payload, based on the characteristics of pancreatic tumor cells highly expressing CXCR4 that mediates immunosuppression and tumor progression through interaction with its specific ligand CXCL12. The therapeutic effect of the conjugate (P12
