Home Microbiological Sampling in a Pediatric Cystic Fibrosis Population: Pandemic Implementation and Ongoing Use.

BACKGROUND: During the coronavirus disease 2019 (COVID19) pandemic, restrictions on in-person care threatened to disrupt in-clinic airway sampling for microbiological surveillance, a vital aspect of cystic fibrosis (CF) care. In response, institutions developed home airway sampling strategies to allow continued guidelines-based microbiological surveillance. The validity of this sampling technique and its ongoing use has not been reviewed. Our aim is to characterize the frequency of home versus in-clinic airway sampling at our institution before, during, and after COVID19 and to compare the pos

Pancreatic panniculitis associated with presumed advanced pancreatic tumor: A case report and literature review.

Pancreatic tumor-associated panniculitis is a rare paraneoplastic syndrome, primarily caused by systemic release of pancreatic enzymes (e.g., lipase) from malignancies like acinar cell carcinoma (ACC), leading to necrotizing inflammation in subcutaneous and other adipose tissues. This report details an 87-year-old female presenting with progressive skin lesions (erythematous rash → indurated swelling → ulceration with purulent drainage), lower limb pain, fever, and markedly elevated lipase (3686.2 U/L). Skin biopsy confirmed lobular panniculitis with characteristic "ghost cells". Imaging revea

Comparative assessment of physiochemical metrics for the clustering of adaptive immune receptor repertoires.

A continued challenge in the analysis of adaptive immune receptor repertoires (AIRRs) is the prediction of antigen reactivity from primary sequence data. Many algorithms infer antigen-specific responses by measuring sequence similarity between receptors. Similarity is often scored using tools for protein alignment such as the BLOSUM matrices. However, these metrics were designed to identify homology in genomic proteins, not VDJ-recombined immune receptors. Comparison of these metrics against other approaches is underexplored. We used matrix factorization to make physiochemical-based alternativ

Predicting textbook outcome after pancreaticoduodenectomy for invasive intraductal papillary mucinous neoplasm: An international validation cohort study.

BACKGROUND: Textbook outcome, a composite measure of optimal recovery, remains poorly defined for invasive intraductal papillary mucinous neoplasm after pancreaticoduodenectomy. This study aimed to develop and validate the first nomogram predicting textbook outcome in this distinct cohort, addressing gaps in tailored prognostic tools for patients meeting contemporary surgical criteria on the basis of high-risk features. METHODS: Using multicenter data from 327 US patients (training cohort) and 152 Chinese patients (external test cohort), we evaluated 43 preoperative and intraoperative variabl

Duct-to-mucosa versus other types of pancreaticojejunostomy for the prevention of postoperative pancreatic fistula following pancreaticoduodenectomy.

RATIONALE: Postoperative pancreatic fistula (POPF) is a common and serious complication following pancreaticoduodenectomy. Duct-to-mucosa pancreaticojejunostomy has been used in many centers to reconstruct pancreatic digestive continuity following pancreatoduodenectomy. There is currently uncertainty about the benefits and harms of duct-to-mucosa pancreaticojejunostomy. This is an update of a Cochrane review first published in 2022 with three new studies added. OBJECTIVES: To evaluate the benefits and harms of duct-to-mucosa pancreaticojejunostomy versus other types of pancreaticojejunostomy

When Fever Strikes Twice: A Case Report of Streptococcus pneumoniae Myelitis with Delayed-Onset Reactive Arthritis.

Background:Streptococcus pneumoniae is a well-known pathogen responsible for respiratory and invasive diseases; however, central nervous system (CNS) involvement in the form of bacterial myelitis is exceedingly rare, particularly in immunocompetent adults. Moreover, the association between pneumococcal infections and reactive arthritis is scarcely documented. We report an unusual case of pneumococcal myelitis complicated by reactive arthritis in an elderly patient with no evident immunosuppression. Case Presentation: A 68-year-old man with a medical history of hypertension, benign prostatic hy

De novo formation of tuft and goblet cells protects the intrapancreatic biliary duct system from inflammatory injury.

The biliary tree is susceptible to inflammatory injury that can result in severe liver damage. However, the intrapancreatic biliary duct system is largely refractory to damage caused by pancreatitis; the mechanisms that protect these ducts from injury remain unknown. Here, we demonstrate that, although serine/threonine-protein kinase DCLK1 (DCLK1) + tuft cells and mucin-2 (MUC2) + goblet cells were largely restricted to the ampulla of non-injured mice, they exhibited a marked increase in the ampulla and a progressive appearance in the intrapancreatic biliary duct system after injury. The incre

Assessment of pancreatic volumes and secretory function by magnetic resonance cholangiopancreatography (MRCP) in children with suspected exocrine pancreatic insufficiency.

AIM: Exocrine pancreatic insufficiency (EPI) can be assessed noninvasively using magnetic resonance cholangiopancreatography (MRCP) with secretin stimulation. This pilot study measured pancreatic volumes in children with suspected EPI and quantified total secreted fluid volume after secretin stimulation. MATERIALS AND METHODS: We retrospectively reviewed MRCPs performed between 2011 and 2021. Pancreatic volume was quantified using parametric magnetic resonance imaging (MRI) software on axial T2-weighted images. Secreted volume after secretin administration was measured with regions of interes

Epidemiology, classification and clinical burden of pancreatic cystic lesions.

Pancreatic cystic neoplasms (PCNs), once considered rare, are increasingly detected due to advances in imaging, with prevalence estimates ranging from 13 % to nearly 50 % and rising with age. They comprise a heterogeneous group, including intraductal papillary mucinous neoplasms (IPMNs), mucinous cystic neoplasms (MCNs), serous cystic neoplasms (SCNs), solid pseudopapillary neoplasms (SPNs), cystic neuroendocrine tumors, and pseudocysts. Their biological behavior differs substantially: SCNs and pseudocysts are usually benign, MCNs and IPMNs may progress to cancer, and SPNs generally require re

You Survived Your Whipple Operation, Now What? Long-Term Consequences of Anemia, Incisional Hernia, Osteopenia, and Pathologic Fractures.

Survivorship after pancreaticoduodenectomy (PD) has substantially increased over the past 3 decades and this has resulted in more patients manifesting with long-term sequelae from the surgery. PD results in notable changes to the endocrine and exocrine functions of the pancreas. Additionally, the resection of the duodenum and proximal jejunum impacts the intestinal absorption of key micronutrients resulting in iron-deficiency anemia and accelerated osteoporosis. In this article, we explore several long-term complications of PD and review potential monitoring and management strategies.

STAndardization of Reports (the STAR project) Upper gastrointestinal endoscopy: European Society of Gastrointestinal Endoscopy (ESGE) Quality Improvement Initiative.

Upper gastrointestinal (UGI) endoscopy is the mainstay for the diagnosis and staging of precancerous and cancerous conditions of the esophagus, stomach, and duodenum. Despite development of several validated classifications for UGI diseases, endoscopy reports differ between endoscopists and endoscopy departments. This document proposes a standardized high quality UGI endoscopy report, including preprocedural, periprocedural, endoscopic findings, and postprocedural information. Preprocedural details should comprise data on the patient, exam, use of antithrombotic drugs, fasting, and premedicati

In silico evaluation of the potential of very high energy electrons delivered in both conventional and FLASH regimes for the SBRT treatment of pancreatic cancer: A report of three case studies.

BACKGROUND AND PURPOSE: As the incidence and mortality rates of pancreatic cancer continue to rise, the search for effective treatments is becoming increasingly urgent. Among the therapeutic approaches, highly hypofractionated stereotactic treatments are being explored. This paper explores the potential of Very High Energy Electrons (VHEE) in the range of 80-130 MeV, in light of recent advances in compact accelerator technology and its compatibility with ultra-high dose rate (UHDR) delivery. MATERIALS AND METHODS: Using dose-volume histograms and dose maps, we compared VHEE treatment plans si

Current status and further directions of endoscopic ultrasound-directed transgastric ERCP and endoscopic ultrasound-directed transenteric ERCP in the management of pancreaticobiliary diseases in surgically altered anatomy: a comprehensive review.

Endoscopic ultrasound-directed transgastric endoscopic retrograde cholangiopancreatography (EDGE) and endoscopic ultrasound-directed transenteric endoscopic retrograde cholangiopancreatography (EDEE) are innovative endoscopic techniques developed to overcome the challenges of biliary access in patients with surgically altered gastrointestinal anatomy. EDGE facilitates the creation of a gastro-gastric anastomosis, enabling endoscopic access to the excluded stomach and subsequent duodenum for endoscopic retrograde cholangiopancreatography (ERCP) procedures. Similarly, EDEE involves creating a ga

An eHealth Delivery Alternative for Cancer Genetic Testing for Hereditary Predisposition in Patients With Metastatic Cancers: Protocol for a Randomized Trial.

BACKGROUND: Germline BRCA1 and BRCA2 testing is a standard evidence-based practice, with established risk reduction and cancer screening guidelines for genetic carriers. With Food and Drug Administration approval for poly (adenosine diphosphate ribose) polymerase (PARP) inhibitors in patients with metastatic breast, ovarian, pancreatic, and prostate cancer, there is an additional therapeutic rationale for testing all patients with these cancers for germline BRCA1 and BRCA2 mutations. However, many at-risk patients do not have access to genetic services, leaving many genetic carriers unidentifi
