Clinicopathological characteristics and prognosis analysis of gastrinoma based on the SEER database.

Gastrinoma, distinguished by Zollinger-Ellison Syndrome, is a highly unusual neoplasm. However, understanding of its clinicopathological characteristics and survival at the population level is inadequate. This study aimed to examine the clinicopathological features and survival of gastrinoma patients utilizing the Surveillance, Epidemiology, and End Results (SEER) database. Patients diagnosed with gastrinoma from 2000 to 2020 were included in the study. Multiple imputation was used to handle missing data. Chi-square test was employed to analyze the clinicopathological features. The Kaplan-Meie

[A Case of an Elderly Patient with Locally Advanced Pancreatic Head Cancer in Which a Metallic Stent Was Removed Endoscopically after Tumor Shrinkage Was Achieved with Gemcitabine Monotherapy].

An 86-year-old woman was referred from another hospital for gemcitabine monotherapy for locally advanced pancreatic head cancer. After 8 courses of gemcitabine monotherapy, follow CT showed partial response, and biliary metallic stent which had been placed 8 months ago displaced into the duodenum. An upper gastrointestinal endoscopy was performed and the stent was retrieved. There has been no subsequent elevation in hepatic and biliary enzymes, and she is currently under observation without re-stenting. There have been reports of displaced biliary stents causing gastrointestinal perforation an

Viral Infection Correlates with a Better Clinical Outcome than Pulmonary Exacerbation of Bacterial Origin in Paediatric Patients with Cystic Fibrosis.

Cystic fibrosis is a progressive disease affecting various organs of the human body, including the respiratory system. While the effect of bacterial infection on cystic fibrosis outcome has been comprehensively described, little is known about the impact of viruses. We collected 49 nasopharyngeal swabs derived from cystic fibrosis paediatric patients during pulmonary exacerbation and tested them for the presence of respiratory viruses to elucidate the influence of the viral infection on their clinical outcome. We found that patients infected with a virus, compared to those in whom molecular te

Molecular diagnosis of respiratory viral infections in cystic fibrosis, sputum versus nasopharyngeal samples.

BACKGROUND: Respiratory viral infections in people with Cystic Fibrosis (pwCF) significantly impact disease progression, and their molecular diagnosis should be part of the recommendations. Sputum, the routinely used specimen for bacterial culture, could also be used for viral molecular diagnosis, avoiding an additional invasive nasopharyngeal swab or aspirate. Our primary objective was to evaluate whether invasive nasopharyngeal samples (NP) and non-invasive sputum specimens are equally accurate for the diagnosis of viral infections in pwCF. METHODS: We conducted a prospective and multicentr

Prediction model for mutation detection via circulating tumor DNA analysis in pancreatic cancer.

BACKGROUND: Comprehensive genomic profiling (CGP) is essential for precision medicine in pancreatic cancer (PC) patients. In unresectable cases, obtaining sufficient tissue is challenging, making plasma-based liquid CGP a common choice. The successful detection of KRAS mutations, which are present in more than 90 % of pancreatic cancers, indicates that liquid CGP has successfully detected circulating tumor DNA. This study aimed to develop a prediction model for detecting KRAS mutations in liquid CGP among patients with PC using the Japanese National Database. METHODS: We evaluated the detecti

Oligometastatic disease in pancreatic cancer: is there a role for curative-intent surgery? A narrative review.

Current standards of clinical practice recommend systemic chemotherapy but no surgical approach for Stage IV pancreatic ductal adenocarcinoma (PDAC), with expected overall median survival between 3 and 6 months. In the last 20 years, a subset of patients with a limited number of metastases to just one or two organs (liver and/or lungs) and a less aggressive biology and growth ability have been identified and defined as "oligometastases" (OM). The continuous improvement in systemic therapy with the arrival of the multiagent FOLFIRINOX and gemcitabine + nab-paclitaxel and the refinements and adv

Safety and efficacy of KRAS antisense oligonucleotides and RIG-I agonists delivered by extracellular vesicles for pancreatic cancer peritoneal metastasis treatment.

Pancreatic ductal adenocarcinoma (PDAC) often metastasizes to the peritoneum and is highly resistant to treatments due to its immunosuppressive microenvironment. In this study, we evaluate the safety and efficacy of a novel therapeutic strategy that combines KRAS-targeting antisense oligonucleotides (ASOs) with immunomodulatory RNA (immRNA), a RIG-I agonist, both delivered by extracellular vesicles (EVs), in preclinical models using PDAC patient-derived organoids and mice bearing PDAC peritoneal metastasis. Our data demonstrate that the combination of KRAS ASO and immRNA synergistically activa

Updates in endoscopic management of pain in chronic pancreatitis.

PURPOSE OF REVIEW: This manuscript reviews recent updates on the management of pain in chronic pancreatitis, an entity that remains difficult to manage. In a time when opioid use disorder is on the rise, advanced endoscopists should be aware of the nonopioid options available to patients to help manage their pain. RECENT FINDINGS: Although there is no standardized approach in the management of pain in chronic pancreatitis, societal guidelines and recommendations have recently been updated to help guide physicians in this matter. However, the available endoscopic approaches have remained relat

Proximity ligation strategy-mediated cross-modal logic-gate sensing platform: Towards dual biomarkers detection for complex disease.

The development of a dual-target detection platform with independent signal output can effectively improve the accuracy and confidence of early diagnosis of complex diseases. Herein, a proximity ligation strategy-mediated cross-modal logic-gate sensing platform was constructed with colorimetric and electrochemiluminescence (ECL) as dominant output while photothermal as an auxiliary mode for human epididymis protein 4 (HE4) and ribonuclease A (RNase A) detection. Novel murine partial tracheal occlusion model with altered respiratory dynamics.

The respiratory system is integrated to optimize efficiency. Dysfunction of one element often impacts others. For example, in chronic obstructive pulmonary disease (COPD), both obstructive sleep apnea and small airways dysfunction are associated with worse emphysema. In bronchopulmonary dysplasia (BPD), cystic lung disease and tracheobronchomalacia are often comorbid. Furthermore, childhood asthma predisposes to COPD. Although mouse models have elucidated key mechanisms in respiratory disease, to date, no models have accounted for how conducting airway dysfunction impacts alveolar structure an

Porcine airway tissue explant culture at a hydrogel-air interface to model submucosal glands in cystic fibrosis progression and therapy.

Submucosal glands (SMGs) are critical for airway health; however, replicating their complex structure and function outside the body remains challenging. Here, we establish a method of airway tissue explant culture at a hydrogel-air interface (i.e., AirTECH) using trachea from newborn pigs. After 14 days, AirTECH tissues maintain their natural SMG structures, cell types, gene expression, and defense functions, along with intact surface epithelia, smooth muscle, and cartilage. Using AirTECH tissue as a model, we evaluated SMGs in cystic fibrosis (CF) airway disease progression, where exposure to

Pancreatoscopy in the evaluation and management of pancreatic disorders.

Pancreatoscopy is an advanced endoscopic technique that enables high-resolution imaging of the main pancreatic duct. Its relevance has grown in recent years with the introduction of novel technologies, allowing for both diagnosis and treatment within a single procedure. In therapeutic applications, it facilitates interventions such as stone fragmentation, stone retrieval, and tumor-related obstruction management. In diagnostic applications, it improves the accuracy of biopsies for suspicious lesions, particularly in cases of cystic neoplasms, indeterminate strictures, and pancreatic fistula as
