Risk Management in ERCP and Endoscopic Ultrasound.

Endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic ultrasound (EUS) are essential for the diagnosis and treatment of pancreatobiliary disease but also carry risks of clinical adverse events (AEs). Acute pancreatitis, bleeding, perforation, and cholangitis are the main AEs associated with ERCP. Acute pancreatitis is the most frequent AE and can become severe and even fatal. Diagnostic EUS is relatively safe, although events such as pancreatitis, bleeding, infection, and needle tract seeding via the puncture site have been reported with EUS-guided tissue acquisition. Hypoglossal nerve palsy secondary to cervical spondylosis mimicking angio-oedema.

A man in his 50s, with a history of cervical spondylosis, presented with acute right-sided tongue swelling, dysarthria and dysphagia. Although tongue deviation was present, unilateral tongue oedema led to initial suspicion of angio-oedema and malignancy; however, blood tests and tongue MRI did not lead to a definitive diagnosis. Tranexamic acid was prescribed for presumed angio-oedema; however, at follow-up, rightward tongue deviation persisted without any other neurological deficits. Localised tenderness was noted over the right occipital area, corresponding to the atlanto-occipital joint. Im

Against the Grain: Navigating Challenges in Celiac Disease Pathology.

The diagnosis of celiac disease requires integration of clinical features, serologic testing, and histopathologic findings. Duodenal biopsy remains essential, with characteristic findings that include villous atrophy, crypt hyperplasia, and intraepithelial lymphocytosis. This article outlines key considerations in the assessment of each histologic feature and examines ongoing controversies in pathologic interpretation, including the value of formal classification systems and the role of immunohistochemistry in routine practice. It also discusses issues with the evaluation of follow-up biopsies

Genetic and clinical spectrum of progressive familial intrahepatic cholestasis type 1: novel ATP8B1 variants and genotype-phenotype correlations.

Progressive familial intrahepatic cholestasis type 1 (PFIC1) is a rare genetic disorder caused by variants in ATP8B1. The aim of this study was to characterize the clinical and genetic spectrum of PFIC1 and to investigate genotype-phenotype correlations in affected individuals. Clinical manifestations and laboratory findings from 15 newly identified individuals with PFIC1, together with data from 70 previously reported cases retrieved from the PubMed and CNKI databases, were collected and analyzed. A total of 10 novel pathogenic or likely pathogenic ATP8B1 variants were identified. Clinical ma

Atraumatic Knee Marrow Edema-Like Signal Preceding a Low-Energy Bicondylar Tibial Plateau Fracture in a Patient with Crohn Disease: A Case Report.

Bone marrow edema-like signal is a nonspecific magnetic resonance imaging finding whose clinical significance depends on its cause, distribution, associated structural abnormalities, symptoms, and skeletal risk. A 44-year-old construction engineer with Crohn disease and previous systemic corticosteroid exposure developed several weeks of atraumatic, load-related right-knee pain. Available prefracture radiographs showed no visible acute fracture. Noncontrast MRI on 28 July 2025 was reported as showing a medial femoral condyle marrow contusion without a fracture line or subchondral collapse; ho

Clinical profile of gastro-oesophageal reflux disease and associated comorbidities in children aged 1-60 months: a prospective observational study.

BACKGROUND: Gastro-oesophageal reflux disease (GERD) in young children causes troublesome symptoms and complications affecting growth, feeding and quality of life, but symptom-based diagnosis correlates poorly with objective reflux severity. Contemporary clinical-profile data from South Indian children under 5 years and the accompanying nutritional and comorbidity burden are sparse. METHODS: Prospective observational study at a tertiary-care paediatric centre in South India (June 2023 to March 2025). Children aged 1-60 months with clinically suspected GERD were enrolled consecutively. Clinica

Associations of Meal-Related and Somatic Symptoms With Mucosal Eosinophil and Mast Cell Counts in Youth With Disorders of Gut-Brain Interaction.

INTRODUCTION: Meal-related symptoms are commonly identified in disorders of gut-brain interaction (DGBIs). Gastroduodenal low-grade inflammation involving mucosal eosinophils and/or mast cells has been previously described in patients with DGBIs. In children with DGBIs, we sought to determine potential relationships between gastric and duodenal mucosal eosinophils and mast cells with specific meal-related and non-GI somatic symptoms. METHODS: Two hundred eight youth with AP-DGBI were evaluated. All patients underwent esophagogastroduodenoscopy (EGD) with biopsies taken from the gastric antrum

Disrupting the vicious cycle of malnutrition and enteropathy: advancing strategies to improve child physical and neurocognitive outcomes.

BACKGROUND: Recognition of the synergistic cycle of malnutrition, immune dysfunction, and recurrent enteric infection in children and its role in linear growth stunting and impaired cognitive development (the "stunting syndrome"), dates back to the 1960s. Early hypotheses emphasized reduced intestinal absorption associated with diarrhea as the primary enteropathy driving malnutrition and developmental deficits. However, recent evidence indicates that a subclinical condition termed environmental enteropathy (EE), or environmental enteric dysfunction (EED), may be a more critical contributor. Mi

Duodenal wall hematoma imitating a pancreatic tumor as a rare cause of obstructive jaundice - a case report.

Obstructive jaundice is mostly caused by malignant lesions such as pancreatic head cancer and cholangiocarcinoma. Atypical benign conditions such as intramural duodenal hematoma (IDH), can imitate malignant processes in both clinical and radiological presentation. The diagnostic workup included computed tomography (CT), magnetic resonance cholangiopancreatography (MRCP), endoscopic retrograde cholangiopancreatography (ERCP), and endoscopic ultrasound (EUS) with biopsy. A 65-year-old patient was referred for assessment of obstructive jaundice and endoscopic retrograde cholangiopancreatography (

Antifungal therapy improves microbiome dynamics in inflammatory bowel disease.

Gut fungal dysbiosis has been implicated in inflammatory bowel disease (IBD), yet strategies for targeting the gut mycobiota in IBD remain unexplored. Here we leveraged the observation that Candida albicans strains are shared between the oral cavity and gut in patients with IBD with mild oral thrush, a condition caused by Candida overgrowth, to design a prospective observational study comparing oral antifungal therapy (swish-and-spit nystatin; oral nystatin fungal targeting (ORNT); n = 18) with orogastrointestinal antifungal therapy (fluconazole; gastrointestinal and oral fluconazole fungal ta

Multidisciplinary management of Epstein-Barr virus-associated atraumatic splenic rupture incorporating splenic artery embolisation and intraoperative cell salvage.

A young woman presented with abdominal pain and jaundice secondary to Epstein-Barr virus-induced hepatitis and splenomegaly. She collapsed and CT imaging showed a large haemoperitoneum secondary to atraumatic splenic rupture. Splenic artery embolisation was attempted without success, and subsequently she underwent an open splenectomy. A collaborative multidisciplinary approach and adequate hospital set-up allowed for an initial attempt at non-operative intervention with the aim of splenic preservation, with rapid conversion to open surgery when necessary. Total blood loss of 7.5 litres was rep

Type 3 resistant starch attenuates concanavalin A-induced immune-mediated hepatitis by preserving intestinal barrier integrity and remodeling the gut microbiota.

BACKGROUND: Autoimmune hepatitis (AIH) is a chronic immune-mediated liver disorder characterized by loss of immune tolerance and progressive hepatocellular injury. Although corticosteroids remain standard therapy, incomplete responses and adverse effects necessitate safer adjunctive strategies. Growing evidence indicates that intestinal barrier disruption and gut microbiota dysbiosis contribute to AIH pathogenesis. Resistant starch (RS), a dietary fiber with immunomodulatory, anti-inflammatory, and microbiota-regulating properties, benefits metabolic liver diseases; however, its effect on ConA

Utilization of Des-Gamma-Carboxy-Prothrombin in Estimating Vitamin K Status in People With Cystic Fibrosis: A Single Center Retrospective Review.

BACKGROUND: People with cystic fibrosis (CF) are at increased risk of fat-soluble vitamin (A, D, E, and K) deficiencies secondary to exocrine pancreatic insufficiency (EPI). Estimation of vitamin K deficiency in routine clinical practice is challenging, and no uniform consensus exists due to lack of an accurate clinical test for the evaluation of its status. We aimed to estimate vitamin K status in people with CF using des-gamma-carboxy-prothrombin (DCP). METHODOLOGY: A single-center retrospective chart review was done between 2022 and 2024. Ninety-four (39 pediatric and 55 adult) people with

Limited effect of surfactant protein D ablation on doxorubicin-induced intestinal toxicity in mice.

INTRODUCTION: Surfactant protein (SP-D) is an anti-inflammatory host defense molecule, produced by epithelial cells and present on mucosal surfaces throughout the human and mouse body. In the present study, we investigated whether SP-D attenuates chemotherapy-induced gastrointestinal toxicity and inflammation in mice. METHODS: SP-D knockout (KO, Sftpd -/-) mice and wild type (WT, Sftpd +/+) littermates were treated with doxorubicin (20mg/kg) or saline and sacrificed at day 3 o rat day 7, post-administration. Gastrointestinal toxicity was evaluated by weight change, citrulline levels, and hist
