A chimeric rather than native secretion signal elicits stress-related transcriptional adaption during 75k γ-secalin production in Komagataella phaffii.

Komagataella phaffii (formerly Pichia pastoris) is a popular host for protein production, enabling their targeted application and in-depth study. Among relevant targets are gluten proteins, which are essential for cereal-based food properties, but can also trigger immune reactions such as coeliac disease. Previously, production of rye (Secale cereale) prolamin 75k γ-secalin in K. phaffii was shown to yield the highest titres with the native secretion signal compared to co- and post-translational signals commonly used for enhanced production. Here, we compared the host's transcriptomic response

Recurrent Projectile Vomiting as the Sole Presenting Feature of Coeliac Disease With Marsh-Oberhuber Grade 3C Enteropathy in a 13-Year-Old Girl: A Case Report.

A 13-year-old girl presented with 4 months of recurrent projectile vomiting and weight loss without diarrhea. Investigations confirmed hypochloraemic metabolic alkalosis, strongly positive coeliac serology, nutritional deficiency, and biopsy-proven Marsh-Oberhuber Grade 3C total villous atrophy. A strict gluten-free diet resulted in clinical resolution and histological recovery to Marsh Grade 1.

Developing Glow: A Co-Designed Support Framework Integrating Behavioural and Psychological Needs in Coeliac Disease.

BACKGROUND: Strict management of a gluten-free diet (GFD) is the only effective treatment for coeliac disease; support in current healthcare frameworks focususes primarily on dietary support and intestinal recovery. However, psychological and social processes that shape gluten-free living are less often addressed, despite high levels of emotional burden among people with coeliac disease. There is limited guidance on how these needs should be met in routine support. OBJECTIVE: To co-design an evidence-informed framework integrating dietary, behavioural and psychological support for adults livi

Irritable bowel syndrome.

Irritable bowel syndrome (IBS) is one of the most prevalent disorders of gut-brain interaction, characterized by recurrent abdominal pain or abdominal discomfort associated with altered bowel habits in the absence of identifiable structural disease. IBS affects ~4-11% of the global population and is associated with substantial healthcare utilization, impaired quality of life and considerable socioeconomic burden. IBS is increasingly recognized as a heterogeneous condition caused by dysregulated bidirectional communication within the gut-brain axis. Altered gastrointestinal motility, visceral h

Immunoglobulin a Deficiency in Coeliac Disease: A Scoping Review.

Coeliac disease (CeD) and immunoglobulin A deficiency (IgAD) frequently co-occur and share a genetic background within the HLA-DQ2 region. Although this has been known for decades, the underlying mechanisms linking the two conditions remain unclear. We conducted a scoping review to evaluate the prevalence, clinical characteristics, diagnostic challenges and histopathological features of IgAD in patients with CeD. Thirty-one studies published between 1985 and 2024 were included. IgAD was more prevalent among individuals with CeD, 1.4%-4.8% than in the general population, and CeD occurred in 5.9

Interobserver agreement among pathologists in the histological diagnosis of coeliac disease: a systematic review supports the British Society of Gastroenterology guidelines' integrated approach to diagnosis.

AIMS: Histological assessment of duodenal biopsies remains central to the diagnosis of coeliac disease (CD), as detailed in the 2026 British Society for Gastroenterology guidelines on the diagnosis and management of CD, which recommend them as part of an integrated diagnostic approach. However, duodenal biopsies demonstrate a spectrum of mucosal changes, making interpretation inherently subjective. The reproducibility of histological diagnosis and classification has not been previously systematically evaluated. METHODS: A systematic review was registered with PROSPERO (CRD42021257108) and con

Microscopic Colitis Beyond Histology: Biomarkers and Candidate Endotypes.

Microscopic colitis (MC) is a chronic inflammatory bowel disorder characterised by chronic watery diarrhoea in the setting of a normal or near-normal endoscopic appearance, with diagnosis relying on histologic evaluation of colonic biopsies. It is traditionally divided into lymphocytic colitis (LC), defined by an increased intraepithelial lymphocyte count, and collagenous colitis (CC), characterised by a thickened subepithelial collagen band. However, these categories do not fully capture the biological heterogeneity of the disease. Conventional histology remains crucial for diagnosis yet prov

MicroRNA-652-3p Drives Chondrocyte Dysfunction in Post-Traumatic Osteoarthritis by Targeting γ-Glutamyl Carboxylase (GGCX).

BACKGROUND: The aim of the present study was to provide an insight into medical treatment practices among patients with chronic rhinosinusitis (CRS) in Germany. An investigation of ICD codes and ATC classes of CRS patients in general and otolaryngology offices in Germany should reveal the prevalent treatment behaviors of German physicians. METHODS: The present study used data from the Disease Analyzer database (IQVIA). The study sample included patients from 940 general (GP) and 106 otolaryngology (ENT) practices who were coded as having chronic sinusitis (ICD-10: J32) or nasal polyps (ICD-10

Can flow cytometry be a key in the difficult diagnosis of coeliac disease? A case report.

We present the clinical case of a 39-year-old woman with a medical history of juvenile idiopathic arthritis and uveitis, evaluated for abdominal pain and distension, difficulty gaining weight, and diarrhea. Blood tests were requested, including antibodies which were negative and common genetic markers, showing positivity for HLA DQ 7.5. A gastroscopy with duodenal biopsies revealed findings consistent with stage 3b of the modified Marsh classification. Due to diagnostic uncertainty, an IEL study was performed, showing a characteristic CD pattern, leading to the initiation of a strict gluten-fr

Follow-up in coeliac disease: evidence and future challenges.

It is estimated that Coeliac Disease has a prevalence of 1% of the global population, and many cases remain undiagnosed, either because patients do not have proper health monitoring or, conversely, due to the asymptomatic nature of the disease in some cases. Furthermore, there is no population wide screening program, although suspicion of the disease has significantly improved. Consequently, it is expected that the incidence will continue to rise in the upcoming years. With this letter, we aim to raise awareness of the imminent challenge for gastroenterologists with expertise in Coeliac Diseas

Comparative analysis of surgical costs vs. theoretical tiopronin therapy in pediatric cystinuria: a single-center experience from Türkiye.

BACKGROUND: Cystinuria is a rare autosomal recessive disorder leading to recurrent cystine stone formation, often necessitating repeated surgical interventions. In Türkiye, tiopronin-a proven medical therapy-has become inaccessible since 2019, raising concerns about its economic and clinical consequences. This study aimed to compare the annual hospital costs of surgical management with the theoretical cost of tiopronin therapy in pediatric cystinuria patients. METHODS: This single-center, retrospective study included 10 consecutive pediatric patients (median age: 12 years; range: 3-17 years)

Enterocyte proliferation as a new biomarker in potential coeliac disease.

BACKGROUND: Potential coeliac disease is an increasingly diagnosed condition, and the dilemma whether start a gluten-free diet is still unsolved. AIMS: We hypothesized that an exaggerated enterocyte turnover is responsible for an impairment of gut function leading to the development of symptoms and/or progression to villous atrophy. METHODS: The proliferation and apoptotic rates of duodenal enterocytes, assessed by anti-Ki-67 and -Caspase-3 antibodies at immunohistochemistry, of 36 adult patients with potential coeliac disease were compared to those found in a group of 32 active coeliac pati

Key Roles of B Cells and B-Cell Receptors in HLA Class II-Associated Diseases: Insights From Coeliac Disease.

When an HLA association is observed for a disease, it is typically to one or a few HLA class II allotypes. Considering that the HLA association is mechanistically tied to a preferential presentation of antigenic peptides to T cells, its presence suggests that only one or a few HLA-binding peptides are likely to contribute to the pathogenesis. By contrast, if many antigenic peptides are involved, the likelihood for involvement of several HLA allotypes increases and, consequently, the disease will not appear as an HLA associated disease even though T cells could play equally important roles.
