Malabsorption syndromes.

BACKGROUND/AIMS: Malabsorption syndromes commonly result from a pathological interference of the normal digestive process. There have been major advances in the last 4 years. The purpose of this review is to highlight in the form of a brief summary the most outstanding information available. METHODS: The review was performed based on a medical literature search using MEDLINE (1993-1997), bibliographic reviews of book chapters and review articles. As a consequence of the extensive information incorporated in the period and the limited scope of this review, the review will focus in three aspect

[Tropical sprue as the cause of chronic diarrhea after travel to Southeast Asia].

A 43-year-old woman developed abdominal pain and diarrhea following a travel to Malaysia. Examinations in another hospital proved no evidence of infection, an empirical antibiotic therapy with ciprofloxacin yielded no benefit. One and a half year later, the patient was admitted to our department because of persistent diarrhea and wasting. Laboratory tests showed megaloblastic anemia, folate deficiency and steatorrhea. Stool specimens for bacterial pathogens and parasites were negative. Endoscopy and biopsy from the distal portion of the duodenum revealed broadening and shortening of the villi

Persistent diarrhea in the returned traveler.

In conclusion, the causes of chronic diarrhea in the returned traveler are protean. Careful evaluation requires an understanding of where the traveler has been, when they were there, the type of diarrheal illness, medications taken, and knowledge of the patients' other medical problems. Protozoa, particularly G. lamblia, C. parvum, and C. cayatenensis, are among the more commonly identified agents. If the patient is immunocompromised, microsporidia and Isospora become more likely, and a prior history of antimicrobial use raises the possibility of C. difficile colitis. Occasionally helminths, w

Diarrhoeal diseases.

Diarrhoea is a symptom of the response of the intestinal tract to a variety of primary diseases. It can be clinically classified based on the duration of the symptom and other associated features. In tropical regions, acute infective diarrhoea is widely prevalent and is an important factor contributing to morbidity and mortality, especially in children. The strategy of maintaining hydration by oral rehydration solutions has contributed significantly to reduction of mortality. Antibiotics are indicated only under specified circumstances. Persistent diarrhoea with prolonged symptoms increases th

[Endoscopic study of the small intestine with push enteroscopy. A prospective study].

OBJECTIVE: To evaluate whether push enteroscopy of the small bowel (PES), undertaken after extensive previous investigations in suspected intestinal bleeding from an uncertain site, chronic diarrhoea or lymphoma of the small intestine, contributes to the diagnosis, and to ascertain the results of PES and its clinical significance. PATIENTS AND METHODS: 56 consecutive patients (29 men, 27 women; mean age 63 years) were investigated prospectively. The main indications for PES were the search for the source of intestinal bleeding in 79% of patients (group A), chronic diarrhoea or tropical sprue

Tropical sprue and subclinical enteropathy: a vision for the nineties.

Aside from infectious intestinal diseases with known etiology, there is a group of gastrointestinal disorders mainly affecting the small intestine of individuals predominantly living in and less often visiting or returning from the Third World, usually the tropics, and ranging from asymptomatic structural and/or functional abnormalities of the gastrointestinal mucosa (subclinical enteropathy, SE) to a fully symptomatic condition highlighted by malabsorption of nutrients with associated nutritional deficiencies responsive to folate and broad spectrum antibiotic treatment (tropical sprue, TS). Combined magnification endoscopy with chromoendoscopy in the evaluation of patients with suspected malabsorption.

BACKGROUND: Magnification endoscopy and chromoendoscopy together have been used to evaluate mucosal detail in a number of conditions, including Barrett's esophagus and flat colonic polyps, but they have not been used to evaluate villous atrophy in the proximal small intestine. METHODS: Thirty-four patients suspected of having a malabsorption syndrome (either celiac disease or tropical sprue) were evaluated using an Olympus magnification gastroscope in both normal and high magnification settings. Indigo carmine dye spraying techniques were used to assist in evaluating duodenal mucosa for evide

Coeliac disease in adults. Presentation and management.

Coeliac disease, also known as gluten-sensitive enteropathy or non-tropical sprue, is a relatively uncommon condition. The dietary presence of gliadin, an alcohol-soluble subfraction of gluten, in immunologically susceptible hosts will lead to small intestinal mucosal inflammation and subsequent mucosal villous atrophy which results in nutrient and vitamin malabsorption. The symptomatic presentations of patients with coeliac disease are related to this malabsorption process which can be reversed in the vast majority of patients with a gluten-free diet.

Peripheral T-cell lymphomas respond well to vincristine, adriamycin, cyclophosphamide, prednisone and etoposide (VACPE) and have a similar outcome as high-grade B-cell lymphomas.

Peripheral T-cell lymphomas (PTCL) represent a heterogeneous group of T-cell malignancies including subentities with favourable (large cell anaplastic) or unfavourable (pleomorphic) prognosis. The clinical outcome of PTCL has been controversially discussed, but a worse prognosis than high-grade B-cell Non-Hodgkin's lymphomas (NHL) has been postulated by most authors. In this report we summarize the results of a prospective comparative study investigating the therapy outcome of 27 patients (pts) with PTCL and 55 pts. with high grade B-cell NHL and give an overview of therapy studies in PTCL. Vitamin E in intestinal fat malabsorption.

This study was done on 5 patients suffering from tropical sprue (8-10 years' duration). Serum vitamin E level was significantly low in these patients. Cell-mediated immune mechanism assessed by using 2,4-dinitrochlorobenzene, a skin-sensitizing hapten, was also depressed. Polyneuropathy was also observed. Repletion of vitamin deficiency was associated with marked improvement in T-cell functions and neuropathy.

Association of lymphocytic (microscopic) colitis with tropical sprue.

Lymphocytic (microscopic) colitis is a disease of unknown aetiology which manifests as long-standing intermittent diarrhoea. Diagnosis is confirmed on histological examination of the colon. An association of this uncommon disease with tropical sprue is described. Tetracycline therapy resulted in a favourable clinical response.

[Tropical sprue].

We report the case of a 60 year old female who presented with diarrhoea and malabsorption syndrome, confirmed by the laboratory data and the radiologic examination, which developed after a journey to Egypt. The mainly finding was flattening of the villi in the jejunal biopsy; the syndrome recovered after treatment with antibiotics and folic acid. The case fulfills the diagnostic criteria of tropical sprue although this country is not considered as an endemic area.

Reconstitution of cultured intestinal epithelial monolayers with a mucosal-derived T lymphocyte cell line. Modulation of epithelial phenotype dependent on lymphocyte-basolateral membrane apposition.

In vivo, epithelial cells which line the intestine are intimately associated with lymphocytes, termed intraepithelial lymphocytes. Previous studies have demonstrated that intraepithelial lymphocytes are present in the uninflamed mucosa, and become especially prominent in various human enteropathies including coeliac disease, tropical sprue, dermatitis herpetiformis, and giardiasis. Using the intestinal crypt cell line T84, and a previously well-defined human mucosa-derived lymphocyte (MDL) line with phenotypic features similar to (but not specific for) intraepithelial lymphocytes, we describe

[Megaloblastic anemia associated with chronic diarrhea. A prospective and multicenter study in Lima].

Since 1986 we have been observing an increased number of patients with megaloblastic anaemia (MA) associated to chronic diarrhea. In 60% of the cases we could not identify any etiologic factor. In the last three years a prospective study in Lima (Peru) has been carried on aimed to investigate this aspect; patients with diseases recognized to be associated to MA were excluded. 45 patients were included age average 37.5 years, all of them have a confirmed diagnosis by bone marrow; 64% with low serum B12 and folic acid, 20% with low serum B12, and 16% with low serum folic acid. Fifty years after: some experiences of the medical care of prisoners of war.

Mention is made of Italian prisoners-of-war treated in Catterick military hospital but in October 1943 Italy joined the Allied side. An account is given of the medical condition of 78 Japanese POWs and of 228 members of the Japanese Indian Forces (JIFs) in a transit hospital in eastern India (now Bangladesh). Tropical sprue appeared to occur in an epidemic form each June and July in Burma and eastern India, but did not seem to have affected the Japanese or JIFs. A brief account is given of the condition of 180 European or Australian service men who had survived harsh conditions in Japanese POW
