Global Protective Ventilator Settings Are Not Regional Protection: A Compartment-Resolved Framework for Personalizing Mechanical Ventilation in Acute Respiratory Distress Syndrome.

Background: Lung-protective ventilation in acute respiratory distress syndrome (ARDS) is guided by whole-lung measures, although the same acceptable breath may stabilize regions undergoing repeated alveolar collapse and expansion (RACE) while overdistending tissue already open. Fatigue-Aware Alveolar Stress and Strain Tracking (FAAST) is a clinician-in-the-loop framework designed to make this regional tradeoff observable. Methods: We first constructed a deterministic, twenty-compartment, phenotype-conditioned model from published observations of acute lung injury. With FAAST excluded, the test

Gastrointestinal Manifestations of Systemic Sclerosis: Screening and Management to Improve Outcomes.

Gastrointestinal (GI) involvement is among the most common and clinically important manifestations of systemic sclerosis (SSc), affecting up to 90% of patients and substantially contributing to morbidity and reduced quality of life. SSc-related GI disease results from microvascular injury, enteric nervous system dysfunction, and progressive smooth muscle atrophy with fibrosis, producing widespread dysmotility throughout the gastrointestinal tract. Clinical involvement can extend from the oral cavity to the anorectum, with esophageal dysfunction, gastroesophageal reflux disease (GERD), gastropa

Sustained normalization of hydrogen-positive small intestinal bacterial overgrowth after polyethylene glycol bowel preparation.

Small intestinal bacterial overgrowth (SIBO) is a frequent complication of chronic liver disease with portal hypertension that often persists or recurs despite standard treatments. Polyethylene glycol (PEG)-based bowel preparations are routinely used before colonoscopy; however, their effects on SIBO have not yet been systematically evaluated. We report the case of a 44-year-old man with metabolic dysfunction-associated steatohepatitis, complicated by intrahepatic portal vein thrombosis and esophagogastric varices. Before colonoscopy, hydrogen and methane breath testing revealed hydrogen-posit

Possible Crosstalk Between Small Intestinal Bacterial Overgrowth (SIBO) and Atopic Manifestations-A Short Overview.

Small intestinal bacterial overgrowth (SIBO) is an increasingly recognized condition that influences immune responses. It may be linked to atopic disorders such as bronchial asthma (BA), food allergies (FA), chronic spontaneous urticaria (CSU), and mast cell activation syndrome (MCAS). The aim of our study was to perform a structured literature search to assess the possible correlation between SIBO and the presentation of atopic disorders. The prevalence of SIBO was highest in patients with BA (60-100%) and FA (50-87.5%), followed by MCAS (30.9%) and CSU (27.9%). The diagnosis of SIBO was base

Chronic, Noninfectious Diarrhea: A Review.

IMPORTANCE: Chronic diarrhea is defined as loose or watery stools lasting longer than 4 weeks and affects approximately 6% to 7% of adults in the US. More than 90% of patients with chronic diarrhea have a noninfectious etiology. OBSERVATIONS: The most common causes of chronic, noninfectious diarrhea are irritable bowel syndrome with diarrhea (IBS-D) and functional diarrhea. IBS-D typically presents with recurrent abdominal pain relieved or worsened after defecation. Functional diarrhea is a condition in which more than 25% of bowel movements in the preceding 3 months are loose or watery, but

Spontaneous transanal evisceration of the small bowel through perforation of the sigmoid colon: a case report and new surgical approach.

INTRODUCTION AND IMPORTANCE: Transanal evisceration of the small intestine is a rare surgical emergency that requires immediate intervention. Since its initial description in 1827, only three cases of evisceration secondary to perforation of the sigmoid colon have been documented. This case report describes the fourth case worldwide. PRESENTATION OF CASE: An 85-year-old woman presented with transanal small bowel evisceration after straining during defecation. Examination revealed prolapsed, congested but viable small bowel loops through the anal canal, without uterine or rectal prolapse. Emer

Unraveling the Complexities of Small Intestinal Bacterial Overgrowth.

This narrative review explores the intricate dynamics of Small Intestinal Bacterial Overgrowth (SIBO), a condition marked by an excessive growth of bacteria in the small intestine. We delve into the multifaceted etiology of SIBO, including its associations with gastrointestinal disorders, motility issues, and alterations in gut architecture. The review examines current diagnostic methodologies, emphasizing the distinctions between breath testing and direct aspiration techniques. Furthermore, we assess the various treatment approaches, ranging from antibiotics and dietary modifications to emerg

Closed-loop synchronization versus conventional synchronization in spontaneously breathing pediatric patients (CHESTSIPP) - a randomized controlled cross-over study.

BACKGROUND: Patient-ventilator asynchrony (PVA) is common during invasive mechanical ventilation in children and may increase respiratory workload and discomfort. The IntelliSync+ (IS+) algorithm provides closed-loop synchronization by continuously analyzing airway pressure and flow waveforms to optimize inspiratory and expiratory cycling in real time. This study evaluated the efficacy and safety of IS+ compared with conventional physician-tailored synchronization in spontaneously breathing pediatric patients. METHODS: Multicenter, prospective, randomized controlled, single-blind crossover tr

Small intestinal microbiome, the underrated maestro of SIMO disease.

Small intestinal microbial overgrowth (SIMO) results from a breakdown in the delicate equilibrium between luminal environment, gut motility, and microbial ecology. Despite extensive research, these factors have largely been investigated as separate entities, with limited integrative insights into their interplay. This review is the first comprehensive synthesis of physicochemical, mechanical, and microbial parameters shaping SIMO pathogenesis. By reviewing both clinical and experimental data, we reveal how alterations in pH, transit time, digestive secretion dynamics, bile acid composition and

Laparoscopic Roux-en-Y Duodenojejunostomy for Duodenal Obstruction after Endovascular Aneurysm Repair Mimicking Superior Mesenteric Artery Syndrome: A Case Report.

INTRODUCTION: Superior mesenteric artery (SMA) syndrome is a rare cause of duodenal obstruction, typically associated with weight loss-related narrowing of the aortomesenteric angle. In contrast, fixed mechanical compression after endovascular aneurysm repair (EVAR) may produce duodenal obstruction that mimics SMA syndrome through a different mechanism. CASE PRESENTATION: A 79-year-old man developed recurrent postprandial abdominal pain and vomiting 4 years after EVAR. CT performed during symptomatic episodes showed marked compression of the horizontal duodenum by the aneurysm sac, whereas th

Small Intestinal Bacterial Overgrowth: Microbiome Dysregulation, Gut-Brain Axis Disruption, and Systemic Consequences.

Small intestinal bacterial overgrowth (SIBO) is a gastrointestinal disorder characterized by excessive bacterial colonization in the small intestine, leading to impaired digestion and nutrient malabsorption. Increasing evidence indicates that SIBO exerts systemic effects beyond the gut, contributing to metabolic, neurological, cardiovascular, dermatological, and autoimmune conditions. Current management strategies include nonsystemic antibiotics such as rifaximin, dietary interventions (low-FODMAP and biphasic diets), and nutraceuticals including berberine, oregano oil, peppermint oil, garlic

Ingested twin ball magnets in a Nepalese boy: swallowed but not forgotten!

INTRODUCTION AND IMPORTANCE: Swallowed foreign body in children is a common and frequent emergency, with most of them passing through the gut without complications. Ingesting multiple magnets or a magnet along with another metallic object, however, poses a significant risk to the bowel due to potential complications such as obstruction, pressure necrosis, perforation, and fistulation. CASE PRESENTATION: We report a 5-year-old boy presenting with an alleged history of swallowing twin ball magnets a month prior to presentation, remaining largely asymptomatic until imaging revealed their presenc

Endoscopic salvage for afferent loop syndrome: the antegrade lumen-apposing metal stent technique.

BACKGROUND AND AIMS: Afferent loop syndrome (ALS) is a rare postgastrectomy condition with biliopancreatic limb obstruction. Revision surgery carries high morbidity in patients with hostile anatomy. We aimed to demonstrate the feasibility of a double-scope, antegrade EUS-guided lumen-apposing metal stent (LAMS) enteroenterostomy as an endoscopic alternative in the management of complex ALS. METHODS: A patient with extensive prior gastric surgery presented with ALS. With a double-scope antegrade technique, a pediatric gastroscope (GIF-XP190 N; Olympus, Tokyo, Japan) was used to cannulate the a

Early effects of bolus versus continuous infusion furosemide in acute decompensated heart failure: a randomized double-blind study.

BACKGROUND: Acute decompensated heart failure (ADHF) is a clinical syndrome associated with high morbidity and mortality. Current guidelines emphasize the importance of oxygenation and diuresis; however, there is no clear recommendation regarding whether loop diuretics should be administered as bolus or continuous infusion. This study aimed to compare the early effectiveness of bolus versus continuous infusion diuretic therapy in the emergency department (ED). METHODS: This was a single-center, prospective, randomized, double-blind, parallel-group clinical trial. Patients aged ≥18 years with

Clinical Symptoms Do Not Distinguish Sucrose Malabsorption From SIBO.

BACKGROUND: Sucrose malabsorption and small intestinal bacterial overgrowth (SIBO) present with symptoms that overlap with those of disorders of gut-brain interaction (DGBI). Recent studies suggest that sucrose malabsorption is more prevalent than previously thought. This study aims to determine the prevalence of sucrose malabsorption based on 13C-sucrose breath test (SBT) in patients undergoing SIBO breath testing and assess whether symptoms distinguish sucrose malabsorption from SIBO. METHODS: Three hundred patients referred for SIBO breath testing between August 2020 and March 2022 were re
