Nutritional Approach to Small Intestinal Bacterial Overgrowth: A Narrative Review.

Small intestinal bacterial overgrowth (SIBO) is a functional digestive disorder whose incidence has been acknowledged by several medical associations, such as the American Gastroenterological Association. It is estimated that between 14% and 40% of patients diagnosed with irritable bowel syndrome also have SIBO, highlighting the importance of accurate diagnosis to enable effective treatment plans. Nutrition and diet therapy play a pivotal role in SIBO management, not only in alleviating symptoms but also in preventing relapses. The objective of this review is to gather updated information on d

[The influence of dietary pattern on the production of volatile organic compounds in patients with hydrogen-producing small intestinal bacterial overgrowth syndrome].

Antibacterial therapy of the small intestine bacterial overgrowth (SIBO) is not effective enough and has a high relapse rate after treatment. Diet is a well-modifiable factor that plays a major role in shaping the composition, diversity, metabolic activity and stability of the intestinal microbiota. Intestinal bacteria ferment undigested food residues to form species-specific carbon-chain volatile organic compounds (VOCs). The aim of this research was to study possible correlations between dietary variables and the level of stool VOCs in patients with hydrogen-producing SIBO. Material and meth

Frequent errors in the interpretation of breath tests in patients with suspected small intestinal bacterial overgrowth (SIBO).

The letter highlights the growing issue of misinterpretation of the breath test in diagnosing SIBO, particularly in patients with slow intestinal transit. It warns of the risk of false negatives when key clinical factors are overlooked and gas curves are not properly interpreted. Additionally, it calls for a broader therapeutic approach beyond antibiotics, including alternatives such as phytotherapy, and emphasizes the need for improved clinical training in the interpretation of these tests.

The intricate interplay between acute pancreatitis and small intestinal bacterial overgrowth: Unraveling the unknown.

Small intestinal bacterial overgrowth (SIBO) refers to the overcolonization of bacteria in the small intestine. Multiple studies have shown a correlation between SIBO and the occurrence and development of various diseases. This review focuses on the relationship between SIBO and acute pancreatitis (AP), summarizing the current research on the interaction and development between SIBO, AP, and gut microbiota translocation. It is emphasized that AP may lead to the generation of SIBO by prolonging the migrating motor complex (MMC) time and weakening the intestinal barrier. SIBO also plays a critic

Modern concepts of small intestinal bacterial overgrowth.

PURPOSE OF REVIEW: Small intestinal bacterial overgrowth (SIBO) has been a recognized condition for more than half a century. Early descriptions of SIBO were based on the concept of colonic bacteria "backing up" into the small intestine. This was based on techniques using unprotected aspiration catheters and earlier culture techniques. Recent advances in breath testing, small bowel sampling, culture techniques, and next generation sequencing have helped expand our understanding of SIBO. RECENT FINDINGS: "SIBO" is now understood to encompass at least three different types of overgrowth includi

Small bowel obstruction by a giant fecal bezoar in a blind loop of small intestine: a case report.

Blind loop syndrome (BLS) is a clinical condition characterized by bacterial overgrowth and stasis within intestinal blind loops, which may result from anatomical abnormalities such as diverticula, fistulae, or surgical anastomoses. While end-to-side and side-to-side intestinal anastomoses are common surgical techniques, the latter has been associated with a higher risk of BLS due to the potential formation of stagnant segments. This case report presents a rare instance of small intestinal obstruction caused by large fecalith formation within a blind loop 1 year after side-to-side anastomosis.

Diagnosis and Management of Small Intestinal Bacterial Overgrowth in Pediatric Short Bowel Syndrome.

This article explores the diagnosis and management of small intestinal bacterial overgrowth in children with short bowel syndrome, a population affected by altered anatomy, dysmotility, and medication-related risk factors. Diagnostic accuracy is limited by the feasibility, and specificity of breath testing and small bowel aspirates. While antibiotics are commonly used, concerns about recurrence and antibiotic resistance remain significant. Emerging nutritional strategies and precision-based, microbiome-targeted therapies offer promising adjunctive treatment options.

Afferent limb percutaneous jejunostomy to treat pancreatic leak after pancreaticoduodenectomy: a case report.

Although the mortality after a pancreaticoduodenectomy (PD) is low, it increases with post-operative pancreatic leaks. In this case, a 76-year-old male with pancreatic head adenocarcinoma after successful PD developed a 6 cm fluid collection by the pancreaticojejunostomy (PJ). Interventional radiology proceeded with percutaneous drainage. Unfortunately, the drain color changed from purulent to bilious. A contrast study confirmed the drain's position within the bowel lumen of the afferent limb. Although not the original intent, the patient progressed. Few cases describe percutaneous drainage of

Dysbiosis and nutrition in steatotic liver disease: addressing the unrecognized small intestinal bacterial overgrowth (SIBO) challenge.

Steatotic liver disease (SLD) is characterized by hepatic fat accumulation, potentially causing major consequences such as liver decompensation. Currently, we lack medications for the treatment of SLD. Therapeutic recommendations for patients include a hypocaloric diet, weight loss, and physical activity. In particular, the Mediterranean diet is frequently recommended. However, this diet might exacerbate intestinal problems in a subset of patients with coexisting small intestinal bacterial overgrowth (SIBO). Previous studies have reported that SIBO is more predominant in patients with fatty li

Glucose breath test for the detection of small intestine bacterial overgrowth: Impact of diet prior to the test.

BACKGROUND: Glucose breath test (GBT) is used for the diagnosis of small intestine bacterial overgrowth. A restrictive diet without fibers and/or fermentable food is recommended on the day before the test. The aim of our retrospective study was to evaluate the impact of two different restrictive diets on the results of GBT. METHODS: A change of the pretest restrictive diet was applied in our lab on September 1, 2020. The recommended diet was a fiber-free diet before this date, and a fiber-free diet plus restriction of all fermentable food afterward. We thus compared the results of GBT perform

Small intestinal bacterial overgrowth in diabetic gastroenteropathy.

BACKGROUND: Although diabetic gastroenteropathy (DGE) is associated with small intestinal bacterial overgrowth (SIBO), most studies have evaluated SIBO with a hydrogen breath test, which may be affected by altered transit in DGE. The risk factors for the consequences of SIBO in DGE are poorly understood. We aimed to evaluate the prevalence of, risk factors for, and gastrointestinal symptoms associated with SIBO in patients with DGE. METHODS: In 75 patients with DGE and dyspepsia, we tested for SIBO (≥105 colony forming units /mL of aerobic and/or anaerobic bacteria in a duodenal aspirate) and

The importance of food quality, gut motility, and microbiome in SIBO development and treatment.

The prevalence of small intestinal bacterial overgrowth (SIBO) is rising worldwide, particularly in nations with high rates of urbanization. Irritable bowel syndrome, inflammatory bowel illnesses, and nonspecific dysmotility are strongly linked to SIBO. Moreover, repeated antibiotic therapy promotes microorganisms' overgrowth through the development of antibiotic resistance. The primary cause of excessive fermentation in the small intestine is a malfunctioning gastrointestinal motor complex, which results in the gut's longer retention of food residues. There are anatomical and physiological fa
