In pancreatic steatorrhoea due to cystic fibrosis (CF) a major proportion of the meal enters the jejunum below the critical pH of 5, causing bile acid (BA) precipitation and limiting aqueous solubilisation of lipid. Treatment with pancreatin alone results only in a small increase in lipolysis as the lipase is largely inactivated; aqueous lipid solubilisation is not improved as BA precipitation remains a limiting factor. The clinical value of estimation of serum concentrations of immunoreactive trypsin was evaluated by studying 46 healthy controls, 23 controls in hospital, 44 patients with chronic pancreatic disease, and 184 patients with non-pancreatic conditions in which pancreatic disease commonly enters into the differential diagnosis. The absorption of oral 14C-glyceryl tripalmitate was tested in 77 subjects by hourly interval sampling of breath 14CO2 for up to 6 h. A highly significant correlation was obtained between maximum breath 14CO2 activity and faecal fat excretion. The test was very effective in differentiating between patients with and without steatorrhoea. Urinary indican excretion was studied in 5 patients with steatorrhoea of pancreatic origin, 4 patients with steatorrhoea due to other causes, and 5 normal subjects. This report describes a man who died at the age of 43 of overwhelming pulmonary suppuration in association with pancreatic steatorrhoea. Autopsy revealed lipomatous pseudohypertrophy of the pancreas. The patient is the longest documented survivor with pulmonary complications of lipomatous pseudohypertrophy of the pancreas. The literature on this rare condition is reviewed.

The absorption of (14)C-labelled glyceryl tripalmitate was tested in 62 patients by determining the maximum specific activity of CO(2) in the breath after an oral dose. A significant correlation with faecal fat excretion was found. The absorption of (14)C labelled palmitic acid was tested subsequently in 54 patients. In cases of pancreatic steatorrhoea, normal absorption of palmitic acid with impaired absorption of glyceryl tripalmitate was found, whereas in most patients with coeliac disease the absorption of both substances was depressed to the same extent.

The effect of a whole pancreas preparation (Viokase) has been investigated in 11 patients with pancreatic steatorrhoea and in five patients with steatorrhoea due to other causes.
