Failure to track a stable AMOC state under rapid climate change.

Persistent olfactory dysfunction (OD) is the second most common symptom of post coronavirus disease-19 (COVID-19) now being termed long-COVID. Its prevalence after recovery from COVID-19 is estimated to be 12% after nearly 6 months of follow-up. It thus becomes imperative for the treating clinicians to update themselves with the pathophysiology of this post COVID disability as well as the tools for diagnosis and the available treatment options. A systematic literature search was performed as per PRISMA guidelines in MEDLINE, Cochrane Library, LILACS, Google Scholar, ClinicalTrials.gov, and med

PCK2-Mediated PQBP1 Lactylation Promotes Asthmatic Inflammation through PRMT5 Inhibition.

Based on their immunodominant nature and ability to induce appropriate immune responses in the host, several antigens of Mycobacterium tuberculosis have shown promise of protection. However, most of the candidate vaccines developed by employing various strategies have afforded protection that is at best comparable with bacillus Calmette-Guérin (BCG) in animal models. Due to the inherent ability of BCG to prime cellular responses in the host, it has become an attractive vehicle for development of a vaccine against intracellular infections. In this study, we have cloned the genes of three immuno

Depressive and anxiety symptoms in patients with celiac disease - co-occurrence and mutual dependencies.

It is estimated that approximately 1% to 3% of people in the world suffer from celiac disease, which mainly affects Caucasians. The HLA-DQ2 and HLA-DQ8 haplotypes present in the majority of the European population diagnosed with celiac disease are helpful in diagnosing the disease. The main symptom is chronic inflammation of the small intestine and atrophy of the intestinal villi, but neurological and psychopathological symptoms, such as depression or severe anxiety, may also occur. Numerous studies show that a gluten-free diet alleviates the psychological symptoms associated with celiac disea

Iron Therapy in Pediatric Iron Deficiency and Iron-Deficiency Anemia: Efficacy, Safety, and Formulation-Specific Trade-Offs-A Narrative Review.

Background/Objectives: Iron deficiency (ID) is the most common nutritional disorder in childhood worldwide. It has profound consequences for growth, neurodevelopment, behaviour, and overall health. Despite the long-standing efficacy of oral ferrous salts, their poor gastrointestinal tolerability and adherence challenges have spurred the development of alternative formulations and innovative dosing strategies. Methods: We conducted a narrative review of national and international guidelines, pediatric randomized controlled trials, observational and cohort studies, cost-effectiveness analyses, d

Novel homozygous variant in ACSL5 gene causing Congenital Diarrhea and Enteropathy (CODE) with sustained therapeutic success: a case report.

BACKGROUND: Congenital Diarrhea and Enteropathies (CODEs) are rare monogenic disorders with early-onset diarrhea, feeding intolerance, and malabsorption causing secondary failure to thrive (FTT). ACSL5 deficiency is an emerging CODE subtype linked to defective intestinal fatty acid metabolism and dysregulated satiety signaling. CASE PRESENTATION: A female infant presented with chronic diarrhea, postprandial vomiting, and severe growth faltering. First-line evaluations were unrevealing. Proband-only whole-exome sequencing (WES) identified a novel homozygous missense variant in ACSL5, NM_016234.

Exocrine Pancreatic Insufficiency Treated With Pancreatic Replacement Enzyme in a Premature Neonate: A Case Report.

Exocrine pancreatic insufficiency (EPI) in neonates is rare and challenging to diagnose, particularly in extremely preterm infants in whom physiologic pancreatic immaturity may mimic disease. Persistent malabsorption, poor growth, and repeatedly abnormal fecal elastase-1 (FE1) levels can help differentiate true EPI from transient developmental insufficiency. An extremely preterm female born at 25.0 weeks adjusted gestational age (AGA), weighing 775 g, developed feeding intolerance, direct hyperbilirubinemia, and growth failure during her neonatal intensive care unit course. Serial FE1 testing

Frequency and predictors of fructose malabsorption in patients with functional abdominal pain syndrome: A case-control study.

BACKGROUND: Functional abdominal pain syndrome (FAPS) is a subset of functional gastrointestinal disorder characterized by chronic recurrent abdominal pain condition that is not attributable to structural, organic, or metabolic diseases. The etiology and pathophysiology of FAPS remain poorly understood; however, brain-gut interactions and food intolerance, particularly to fermentable oligo-, di-, and monosaccharides and polyols, have been implicated. AIM: To determine the frequency of fructose malabsorption (FM) in patients with FAPS and to evaluate the occurrence of resultant symptoms. METH

IS THERE A RELATIONSHIP BETWEEN INTESTINAL FRUCTOSE MALABSORPTION, GASTROINTESTINAL SYMPTOMS AFTER FRUCTOSE OVERLOAD, AND BODY ADIPOSITY IN ADOLESCENTS?

BACKGROUND: Over the past several decades, dietary fructose intake has increased significantly, along with the prevalence of obesity. Fructose malabsorption is associated with gastrointestinal symptoms and may lead to fructose intolerance. Evidence suggests a probable association between intestinal fructose malabsorption and increased body adiposity. OBJECTIVE: To analyze the prevalence of intestinal fructose malabsorption and intolerance in asymptomatic adolescents and to investigate their relationship with fructose intake, body adiposity, systemic metabolic indicators, and intestinal permea

A national consensus guideline on the performance and interpretation of hydrogen- and methane-based breath tests for carbohydrate malabsorption, small intestinal bacterial overgrowth, and intestinal methanogen overgrowth.

Abdominal bloating, distension, excessive gas, abdominal pain, diarrhea and constipation are common symptoms that may arise from carbohydrate malabsorption, carbohydrate intolerance, small intestinal bacterial overgrowth (SIBO), intestinal methanogen overgrowth (IMO), or disorders of gut-brain interaction. Hydrogen- and methane-based breath tests are safe, noninvasive, inexpensive and widely used, but differences in indications, patient preparation, test performance and interpretation can lead to inconsistent diagnoses and management. At the recommendation of the Israeli Gastroenterology Assoc

Parental perspectives concerning gluten-free food insecurity in households with children with coeliac disease: take away messages for policy makers.

Coeliac disease (CD) is an autoimmune disease resulting in lifelong intolerance to gluten. Treatment of CD is a gluten-free diet (GFD). High gluten-free (GF) food costs contribute to GF-food insecurity (GF-FI) in households with children with CD. The study objective was to describe parental perceptions of the barriers/facilitators of GF-food security (GF-FS) and the health policies parents deem necessary to combat GF-FI. A subset analysis of a cross-sectional study examining household and child socio-demographic, child diet quality and GFD adherence, food environment, and GF-FI in parent-child

Multifocal motor neuropathy secondary to gluten intolerance: a case report.

BACKGROUND: Peripheral nervous system manifestations of gluten sensitivity usually present as distal symmetric axonal polyneuropathy, small fiber neuropathy, or sensory neuropathy, often accompanied by ataxia or painful symptoms. By contrast, motor neuropathies are extremely rare, with only a few cases of multifocal motor neuropathy reported. Notably, the most recent guidelines of the European Society for the Study of Coeliac Disease do not recognize multifocal motor neuropathy as a classical neurological manifestation of gluten-related disorders. The main differential diagnoses include amyotr

The interplay between malnutrition, persistent diarrhea, lactose intolerance, and food associated gut dysbiosis in children; a comprehensive review.

BACKGROUND: Persistent diarrhea, lactose intolerance, and gut dysbiosis are major gastrointestinal conditions that worsen malnutrition in children, especially in low and middle-income countries. Due to these factors, there is a vicious cycle in which diarrhea worsens malnutrition by reducing the gut’s capacity to absorb nutrients. Persistent diarrhea is defined by WHO, diarrhea (≥ 3 loose/watery stools daily) lasting 14 days or longer posing higher risk of malnutrition than acute diarrhea. METHODS: This review explores the interconnected nature of these conditions and their cumulative impact o

Comparative effects of conventional cow's milk versus milk free of A1-type beta-casein on gastrointestinal physiology and symptoms of digestive discomfort in participants intolerant to conventional cow's milk: a pilot randomised controlled trial.

BACKGROUND: Cow’s milk is an important source of nutrients supporting human growth and development as well as adult health, but it can be associated with unwanted gastrointestinal (GI) symptoms. These symptoms are often attributed to lactose, but may instead be partly due to peptides released during digestion. This exploratory study aimed to compare the effects of consuming milk free of A1-type beta-casein (A1PF milk) with those of conventional cow’s milk (containing both A1- and A2-types of beta-casein) on GI symptoms in individuals who usually suffer from mild-to-moderate digestive discomfor

Misdiagnosis of alpha-gal syndrome as non-celiac gluten sensitivity or lactose intolerance: A diagnostic blind spot for clinicians.

Alpha-gal syndrome (AGS), an emerging tick-borne carbohydrate hypersensitivity, has gained increasing recognition for its atypical presentation and delayed food-related allergic reactions. While urticaria and anaphylaxis dominate the clinical narrative, gastrointestinal (GI) manifestations-including abdominal pain, diarrhea, bloating, and cramping-are common and often underrecognized. These nonspecific GI symptoms create significant diagnostic overlap with prevalent functional and food-related disorders such as non-celiac gluten sensitivity (NCGS) and lactose intolerance. Consequently, many pa

Refractory hypothyroidism: Diagnostic evaluation and management strategies.

The term refractory hypothyroidism is used to describe the condition in which patients require levothyroxine doses higher than expected for their age and weight (> 1.9-2.5μg/kg/day). Its prevalence is not well defined. Refractory hypothyroidism is not related to thyroid hormone resistance, while most common causes include non-compliance (also known as pseudo-malabsorption) and malabsorption. Malabsorption can be caused by food and/or medication interactions as well as digestive pathologies such as gastritis, coeliac disease, lactose intolerance, or a history of digestive surgery. Less commonly

Atopy and Other Sensitivities in Non-Celiac Wheat Sensitivity: Is There an Associated Hypersensitivity Background? †.

BACKGROUND: A hypersensitivity reaction has been hypothesized as one of the possible pathophysiological mechanisms involved in non-celiac wheat sensitivity (NCWS). Some studies have reported a high frequency of atopic diseases in NCWS patients. OBJECTIVES: This study aimed (A) to define the presence and features of atopic diseases and other hypersensitivities in NCWS patients and (B) to search for possible allergic features which could address a NCWS diagnosis. METHODS: Clinical, laboratory and histological data from NCWS patients before the start of a wheat-free diet were retrospectively an
