Novel Treatment of Irritable Bowel Syndrome (IBS) and Other Disorders of the Peripheral Nervous System Using Peripherally Restricted Positive Allosteric Modulators of GABA‑A Receptor.

The invention in this patent application relates to 6,9-dihydro-5H-imidazo-[1,2-a]-[1,4]-diazepine derivatives (or their N-oxides) represented herein by formula 1. These compounds are peripherally restricted, positive allosteric modulators of one or more GABA-A receptors. They selectively target the peripheral nervous system and body organs and do not substantially penetrate the blood-brain barrier. Thus, they may provide useful treatment for irritable bowel syndrome and other related peripheral disorders, including, but not limited to, functional abdominal pain, chronic idiopathic diarrhea, v

Neurological Manifestations in Whipple's Disease: A Scoping Review of Clinical Features, Laboratory Findings, and Neuroimaging Characteristics of 111 Cases.

BACKGROUND: Whipple disease (WD) is a rare, potentially fatal infection caused by Tropheryma whipplei (TW). Central Nervous System involvement (CNS-WD) is a severe complication of WD with a challenging diagnosis, due to limited data integrating neurological features, imaging, cerebrospinal fluid analysis, PCR, and intestinal histopathology. Prompt recognition and treatment are essential to improve patient outcomes. METHODS: A scoping review of the literature from 2000 to 2025 was conducted, including all CNS-WD case reports and series, to outline its clinical spectrum and diagnostic workup.

HLA-DQB1*02, HLA-DRB1*03 alleles and HLA-DQB1*02-HLA-DRB1*03 haplotype confer a significant risk of celiac disease in the Mauritanian population.

BACKGROUND AND AIM: Celiac disease (CD) is an autoimmune disorder primarily caused by an abnormal immune response to dietary gluten in human leucocytes class II antigens carriers. This preliminary study aimed to assess the specific risk conferred by DQB1 and DRB1 alleles to celiac disease development in the Mauritanian population. MATERIALS AND METHODS: Frequencies of HLA-DQB1and HLA-DRB1alleles and haplotypes were evaluated in 50 celiac disease (CD) patients. The association of HLA-DQB1 and HLA-DRB1with the disease was tested using a control group of 50 non-CD healthy individuals. Genotyping

Gliadin-specific CD4+ T cells exhibit functional diversity independent of TCR usage or epitope specificity.

Celiac disease (CeD) is driven by pathogenic CD4+ T cell responses, primarily targeting four HLA-DQ2.5-restricted immunodominant epitopes derived from α-gliadin and ω-gliadin. While gliadin-specific CD4+ T cells have been investigated, how transcriptional and effector phenotypes in CeD vary across distinct epitope specificities remains unclear, as do the signature TCR motifs governing gliadin epitope specificity and cross-reactivity. Single-cell transcriptomic and TCR profiling of gliadin epitope-specific peripheral blood CD4+ T cells from patients with CeD after gluten challenge revealed T ce

Gastrointestinal Manifestations of Systemic Sclerosis: Screening and Management to Improve Outcomes.

Gastrointestinal (GI) involvement is among the most common and clinically important manifestations of systemic sclerosis (SSc), affecting up to 90% of patients and substantially contributing to morbidity and reduced quality of life. SSc-related GI disease results from microvascular injury, enteric nervous system dysfunction, and progressive smooth muscle atrophy with fibrosis, producing widespread dysmotility throughout the gastrointestinal tract. Clinical involvement can extend from the oral cavity to the anorectum, with esophageal dysfunction, gastroesophageal reflux disease (GERD), gastropa

Association between hyperhomocysteinemia and frailty in older adults with acute ischemic stroke: A cross-sectional study.

PURPOSE: Frailty is common among patients with acute ischemic stroke (AIS) and is associated with adverse health outcomes. Although hyperhomocysteinemia (Hhcy) is a recognized biomarker in AIS, its association with frailty remains unclear. This study aimed to examine the relationship between Hhcy and frailty and assess whether Hhcy is independently associated with frailty in AIS patients. PATIENTS AND METHODS: We conducted a cross-sectional study involving 397 AIS patients admitted between April 2023 and April 2024. Frailty was assessed using the Tilburg Frailty Indicator (TFI). Clinical and

Polyunsaturated Fatty Acids in Celiac Disease: Intake Patterns While on Gluten-Free Diet.

Celiac disease (CD), an autoimmune disorder triggered by gliadin, is treated with a strict gluten-free diet. While omega-3 intake is known to reduce inflammation in other autoimmune diseases, its role in CD remains unclear due to limited data on fatty acid intake in this population. To assess the dietary intake of polyunsaturated fatty acids (PUFAs) in patients with CD compared with healthy controls. In this cross-sectional study, patients with CD and age- and sex-matched healthy controls were evaluated. Sociodemographic data and dietary intake were assessed using a validated food frequency qu

Psychological intervention for parents of children with celiac disease: A pilot and feasibility randomized controlled trial.

OBJECTIVES: Children with celiac disease (CD) must follow a lifelong gluten-free diet (GFD). This can create psychosocial challenges for the family, making caregiver support essential. This study evaluated the feasibility and acceptability of a self-help psychological intervention designed to empower parents in supporting their own well-being, and their child's quality of life. METHODS: Parents of children with CD (8-11 years) took part in an online self-help intervention, including psychoeducation, and family-based activities, to support well-being and quality of life (QoL) in the context of

Acute gluten-induced inflammatory response highlights CCL20 as a potential biomarker for celiac disease.

BACKGROUND: Diagnosis of celiac disease (CD) remains challenging in individuals already on a gluten-free diet (GFD). Although several alternative methods have been proposed, they have limitations. Identifying inflammation-related proteins that rapidly respond to gluten exposure in blood may offer diagnostic alternatives. We aimed to characterize the inflammatory protein response to gluten in patients with CD on a GFD, and to assess the diagnostic potential of candidate biomarkers and their association with clinical symptoms. METHODS: Seventeen patients with CD and 15 non-CD individuals on a G

The HUNT study identifies host genetic factors reproducibly associated with human gut microbiota composition.

The gut microbiota is associated with human health and disease. Here we conducted a genome-wide association study of host genetic factors influencing gut microbiota composition in 12,652 individuals from the Trøndelag Health Study (HUNT), with replication in Nordic cohorts (n = 16,017-21,976). We identified 12 reproducible SNP-species associations across six genomic loci, including known (LCT, ABO) and novel (HLA-DQB1, MUC12, SLC37A2, FUT2) regions. Additionally, we detected genetic signals associated with gut microbiota functional modules at three loci (LCT, ABO, FUT2). Follow-up analyses sug

Association Between Celiac Disease and Uncontrolled Hemoglobin A1c Levels in Type 1 Diabetes Pediatric Patients.

BACKGROUND: Celiac disease (CD) occurs in ~6% of individuals with type 1 diabetes (T1D) and may complicate glycemic control due to conflicting dietary needs. Prior studies show mixed results regarding the impact of CD on Hemoglobin A1c (HbA1c), especially in pediatric populations. This study evaluates whether CD is associated with suboptimal glycemic control in pediatric patients with T1D. METHODS: This retrospective chart review analyzed pediatric patients (<18 years) diagnosed with T1D between 2012 and 2023 across Corewell Health East. Patients were identified via ICD-10 codes and stratifie

Impaired Intestinal Function Among Indigenous Shuar Children of Amazonian Ecuador: Evidence From Lactulose:Mannitol Tests of Absorption and Permeability.

OBJECTIVES: Intestinal function is an important but often overlooked aspect of human biological variation, with the intestines serving as both a barrier against external pathogens/contaminants and the primary conduit of nutrient extraction from food. Impaired intestinal function among children is viewed as a key contributor to growth faltering globally. However, few studies have investigated children's intestinal function in rural settings or beyond infancy. To address this limitation, we conducted a pilot study to describe the intestinal function of school-age Indigenous Shuar children of Ama
