Inflammatory bowel diseases, comprising Crohn's disease and ulcerative colitis, represent chronic inflammatory disorders with rising global incidence, underscoring the pivotal role of modifiable environmental factors in disease pathogenesis. Diet and intestinal microbiota have emerged as critical bidirectional therapeutic targets through complex interactions with host immune responses. Epidemiological evidence demonstrates that healthy and high fiber diets reduce disease risk, while ultra-processed foods and inflammatory dietary patterns increase susceptibility. Therapeutic nutritional interve

Celiac disease (CD) is a T-cell-immune mediated enteropathy triggered by gluten ingestion in genetically susceptible individuals. The estimated global prevalence is 1% (range 0.7-1.4%), and the clinical manifestation results from a complex interplay of genetic, immunologic, and environmental factors. Although it occurs predominantly in predisposed individuals with the genetic haplotypes HLA-DQ2, HLA-DQ8, or DQA1*05 haplotypes, several other predisposing genes have been described. Clinical presentations are variable and include both gastrointestinal and extraintestinal manifestations, contribut

BACKGROUND: Hyperhomocysteinemia (HHcy) is an important risk factor for Alzheimer's disease (AD), but its differential effects on tau pathology and beta-amyloid (Aβ) deposition, as well as the key mediating molecules involved, remain unclear. This study investigates how HHcy influences AD pathology and examines whether interleukin-1β (IL-1β) neutralization can mitigate HHcy-accelerated neurodegeneration.
METHODS: Female 12-month-old 3 × Tg-AD mice were supplemented with methionine water for 7 weeks to induce HHcy. Brain tissues were analyzed for Aβ deposition, tau phosphorylation, oligomeriza

Celiac disease (CeD) is an autoimmune-mediated disorder triggered by gluten ingestion. Strict adherence to a gluten-free diet (GFD) is currently the only available treatment, yet it presents nutritional and lifestyle challenges. Polyphenols and other bioactive food compounds have shown potential in modulating inflammation and gut health. This study evaluated the impact of cocoa, a polyphenol-rich food, on CeD pathogenesis in a preclinical model. DQ8-Dd-villin-IL-15tg mice with predisposition to CeD were fed either a GFD (REF), a gluten-containing diet (GLI), or a gluten-containing diet supplem

OBJECTIVES: Despite a well-defined diagnostic work-up, uncertainties persist regarding celiac disease (CeD) detection strategies in the general population. Machine learning (ML) algorithms offer promise in aiding medical decision-making on clinical data. This study aimed to utilize ML prediction models to identify uncommon features indicating the necessity for CeD screening in children.
METHODS: The discovery cohort comprised children with CeD exhibiting nonspecific clinical features, drawn from a single referral center, along with gender- and age-matched controls. Data collected included dem

Patients with celiac disease have lower bone mineral density (BMD) and higher incidence of fractures compared to age- and sex-matched controls. There are no studies of denosumab, an antiresorptive drug, which is a fully human monoclonal antibody that binds the receptor activator of NFκB ligand (RANKL) in celiac patients with osteoporosis. The aim is to study the long-term effect of denosumab on BMD in celiac patients with osteoporosis on a gluten-free diet (GFD), compared to non-celiac osteoporotic patients. Fifteen celiac patients with osteoporosis and control subjects of the same age, sex, B

Insights gained through interpretation of models trained on the T-cell receptor (TCR) repertoire contribute to advances in understanding of immune-mediated disease. This has the potential to improve diagnostic tests and treatments, particularly for autoimmune diseases. However, TCR repertoire datasets with samples from donors of known autoimmune disease status generally include orders of magnitude fewer samples than TCR sequences. Promising TCR repertoire classification approaches consider relationships between non-identical TCR sequences. In particular, kmer methods demonstrate strong and sta

UNLABELLED: This observational retrospective study describes the rate and time of symptom resolution in celiac disease (CD) in a cohort of pediatric patients following a gluten-free diet (GFD). We evaluated the rate and time to symptom resolution after starting a GFD, as well as associated predictive factors (age, gender, comorbidities, familial history, IgA anti-transglutaminase antibodies (TGA-IgA) at diagnosis, dietary adherence), in children diagnosed with CD between 2014 and 2022 at the Gastroenterology Unit of Maggiore Hospital, Bologna. The minimum follow-up period was 6 months. Of 421

Gut microbiota, the community of microorganisms in the gastrointestinal (GI) tract, plays many critical roles in the maintenance of an individual's health by regulating bone metabolism. Bone mass has been linked to the GI system because nutrients (minerals) are absorbed to maintain bone homeostasis. Dysbiosis of gut microbiota due to metabolic deregulation associated with obesity, fatty liver, and inflammatory status of an individual promotes altered gut microbial diversity, gut-epithelial integrity, releases of harmful gut metabolites those could mediate malabsorption of nutrients such as cal

Coeliac disease (CD) is an autoimmune enteropathy triggered by proline-rich gluten immunogenic peptides (GIPs), for which no curative therapy exists. We developed celiacase (Clc), a recombinant prolyl endopeptidase engineered from pitcher plant neprosin to enhance expression, stability, and activity. Clc showed maximal activity at gastric pH, synergized with and resisted pepsin, and efficiently cleaved GIPs, including the highly immunogenic 33-mer, outperforming Aspergillus niger prolyl endopeptidase in degrading GIPs from wheat flour and gliadin. At an enzyme-to-gliadin ratio of 1:250, Clc re

BACKGROUND & AIMS: Intestinal adaptation in patients with chronic intestinal failure (CIF) due to short bowel syndrome (SBS) may allow weaning from parenteral support (PS). The contribution of gut microbiota to this process remains unclear. This observational study compared gut microbiota profiles in strictly matched CIF-SBS patients with identical intestinal anatomy and comparable anthropometric and clinical characteristics, stable dietary intake, and differing PS dependence.
METHODS: Adult SBS patients under follow-up at our national tertiary referral center were screened. Inclusion criteri

Jejunal diverticulosis (JD) is a rare condition of the small intestine characterized by acquired pseudodiverticula arising along the mesenteric border. Although most cases remain asymptomatic, complications may occur in a minority of patients and include diverticulitis, perforation, intestinal obstruction, malabsorption, and, more rarely, GI hemorrhage. Bleeding originating from jejunal diverticula is an uncommon but potentially life-threatening cause of obscure GI bleeding (OGIB). We report the case of a 59-year-old woman who presented to the ED with melena associated with symptomatic anemia.

Sprue-like enteropathy associated with angiotensin II receptor blockers (ARBs) is a rare cause of chronic diarrhea and malabsorption, typically characterized by villous atrophy and intraepithelial lymphocytosis mimicking celiac disease. Although olmesartan is the most frequently implicated agent, cases associated with other ARBs, including losartan, have increasingly been recognized. A 61-year-old woman with hypertension treated with losartan for three years presented with a three-month history of abdominal pain and chronic diarrhea. Upper endoscopy revealed a macroscopically and histologicall

Exocrine pancreatic insufficiency (EPI) is a clinically significant disorder characterized by inadequate secretion or activity of pancreatic digestive enzymes, leading to maldigestion, malabsorption, and adverse nutritional and metabolic consequences. The causes of EPI can be divided into loss of pancreatic parenchyma, inhibition or inactivation of pancreatic secretion, and postcibal pancreatic asynchrony. Although most associated with intrinsic pancreatic diseases such as cystic fibrosis, chronic pancreatitis or pancreatic cancer, EPI is increasingly recognized in a range of other conditions

Alterations in gut microbiota have been reported in coeliac disease (CeD). However, longitudinal evidence distinguishing disease-related effects from diet-driven changes after gluten-free diet initiation remains scarce, particularly in pediatric cohorts. The present study aimed to evaluate time-dependent changes in selected intestinal microorganisms in children with CeD before and during adherence to a gluten-free diet and to compare these findings with those of healthy controls. Microbial DNA isolates from stool samples of pediatric patients with CeD (n = 24) and healthy children (n = 24) wer

Restrictions or prohibitions on the use of antibiotics in pig feed for growth promotion have been introduced in many countries. Bioactive compounds (BCs) as functional additives are emerging as an alternative to overcome these restrictions. These compounds include natural extracts from animals, plants, and microorganisms, as well as their chemically synthesized counterparts. Their growing popularity is due to their abundant sources, safety, and multiple biological functions, including antibacterial, antioxidant, anti-inflammatory, immunomodulatory, and intestinal health-promoting effects. BACKGROUND: Population-based screening for coeliac disease could reduce underdiagnosis, but benefits and harms remain debated.
AIM: We studied health outcomes in screen-detected coeliac disease patients at diagnosis and following a long-term gluten-free diet.
METHODS: Data were collected from Health 2000 and 2011 studies representing the Finnish population. Coeliac disease was screened in 6339 individuals by transglutaminase and endomysial antibodies, with dual seropositivity as the diagnostic criterion, histologically validated in this cohort. Survey-weighted analyses enabled population-lev

This cross-sectional based market surveillance study examined the level of gluten contamination in commercially manufactured products labelled 'gluten-free' in Jordan and compared the results to the globally recognised 20 mg/kg gluten safety limit for people with coeliac disease and other gluten-related disorders. From August 2022 to November 2024, 182 goods from 25 different food categories were tested in retail locations in Jordan's main governorates. A validated monoclonal-based antibody-sandwich ELISA was used to determine the levels of gluten concentration, and a 95% CI was used to descri
