Clinical Trial: Multi-Strain Probiotic Improves Bile Acid Profile, Microbiome, and Metabolomic Parameters in Patients With History of Bile Acid Malabsorption-A Randomized, Controlled Trial.

BACKGROUND: Bile acid (BA) malabsorption (BAM) is a common cause of chronic diarrhoea and may occur in some patients due to abnormalities of the gut microbiota. Effects of probiotics on faecal secretory BAs, particularly the primary BA, chenodeoxycholic acid, are unclear. AIM/METHODS: We conducted a randomized, double-blind, placebo-controlled trial of the De Simone formulation 8-strain probiotic in 24 patients previously diagnosed with BAM. Patients were randomized to 3 weeks of the probiotic (900 billion bacteria) or placebo (maltose), both administered three times daily. Symptoms, serum 7α

Evaluation and interpretation of Hawthorne and other trial-related effects in Celiac Disease: A systematic review and meta-analysis.

BACKGROUND AND AIMS: The Hawthorne effect describes outcome changes driven by awareness of observation rather than active intervention. In celiac disease (CeD), improvements reported in placebo or control arms suggest that observation may influence objective outcomes. We aimed to analyze the Hawthorne and other trial-related effects in treated CeD, focusing on histological changes of the small intestinal mucosa. METHODS: We conducted a systematic review and meta-analysis following PRISMA 2020 guidelines. PubMed and ClinicalTrials.gov were searched from inception to October 2025 for studies en

Intestinal lipase inhibition uncouples diet-induced obesity from MASH by remodeling the hepatic lipidome in mice.

Dietary fatty acids exert distinct effects on hepatic metabolism. Intestinal lipase inhibition, used clinically for obesity, promotes weight loss by reducing dietary fat absorption, yet its impact on hepatic lipid metabolism in metabolic dysfunction-associated steatohepatitis (MASH) remains poorly defined. Here, we examined the metabolic consequences of lipase inhibition on fatty acid absorption in MASH. To induce MASH, C57BL/6J mice were fed the high-fat Gubra-Amylin NASH diet with or without the lipase inhibitor orlistat for 32 weeks. Orlistat protected against diet-induced obesity by promot

Review Article: Accelerating Coeliac Disease Clinical Trials-Beyond Celiac Meeting With Industry, Academics, Patients and the FDA.

BACKGROUND: Coeliac disease is due to an acquired, lifelong gluten-specific immune response resulting in a gluten-dependent chronic immune-mediated enteropathy associated with gastrointestinal and extraintestinal morbidity. Strictly avoiding dietary gluten is the only available management but has never been subjected to rigorous efficacy assessment now required for a pharmaceutical agent. Gluten-free diet (GFD) is partially and inconsistently effective, but it places a high burden on patients, families, the food industry, and regulators and is defined inconsistently around the world, highlight

Protein source and redox-mediated structural modifications, bioactivity, and celiac-related peptides in high-moisture extruded gluten-based meat analogs.

This study examined how protein source and redox treatment influence structural properties and peptide profiles of high-moisture extruded (HME) meat analogs. Wheat gluten (WG) was blended with soy protein (SP) or pea protein (PP) and L-cysteine/ascorbic acid (LC/AA) followed by HME. The optimum feed moisture content (FMC) was selected based on the degree of texturization, which was subsequently analyzed for structural, thermal, digestibility, bioactivity, and celiac-related peptide. WG-SP formed stronger β-sheet-rich networks, whereas WG-PP showed more molecular flexibility with higher β-turn

Antibodies to Transglutaminase-6 Identify Ataxia Responsive to Gluten-Free Diet.

Background: Gluten ataxia (GA) is the most common neurological manifestation of gluten sensitivity. GA is identified by serological evidence of gluten sensitivity when other defined causes for ataxia have been excluded. Whilst there are several serological markers for gluten sensitivity, antibodies against transglutaminase enzyme 6 (anti-TG6) are of particular interest as it is the autoantigen in GA. Our objective was to assess the value of a commercial anti-TG6 ELISA assay in routine clinical practice for the investigation and management of patients with progressive ataxia. Methods: Patients

[Use of teduglutide in short bowel syndrome in infants, children and adolescents: Position paper of the working group "Chronic intestinal failure" of the Society for Paediatric Gastroenterology and Nutrition (GPGE)].

Paediatric short bowel syndrome (SBS) with chronic intestinal failure (IF) is a rare, complex and potentially life-threatening condition. The main causes in children are necrotizing enterocolitis, volvulus, congenital malformations, and other enteropathies. The resulting reduced intestinal absorption capacity often requires parenteral nutrition (PN), which is associated with complications such as catheter infections, liver disease, and thrombosis, as well as significant limitations in quality of life. The goal of multidisciplinary treatment is to achieve enteral autonomy, prevent complications

Revisional Surgery After Bilio-Pancreatic Diversion: Indications, Outcomes, and Complications in a High-Volume Center.

BACKGROUND: Bilio-pancreatic diversion (BPD), once a prominent bariatric procedure, has seen a marked decline in use due to high rates of nutritional deficiencies and frequent need for surgical revisions. Despite this, BPD remains in practice in select centers, leading to a persistent cohort of patients requiring revisional surgery. METHODS: We conducted a retrospective review at a high-volume bariatric surgery center to identify all patients who underwent revisional surgery following primary BPD between January 2015 and December 2025. Inclusion criteria were age ≥ 18 years, documented primary

Measurement of Fecal Energy Loss Using Bomb Calorimetry in Adult Patient Populations: A Systematic Review and Meta-Analysis.

Background/Objectives: Accurate assessment of nutritional adequacy requires insight not only into energy intake but also into the proportion of nutrients absorbed. Stool bomb calorimetry quantifies fecal energy content and estimates intestinal energy absorption, but its application and reporting vary. We aimed to evaluate its application in adult patient populations and to quantify intestinal energy absorption and its variability. Methods: PubMed, Embase, ClinicalTrials.gov, BASE and NDLTD were searched without date restriction (PROSPERO CRD420261389214). Two reviewers screened independently,

Targeting redox imbalance through Nrf2 activation in the inflamed coeliac duodenum.

Coeliac Disease (CeD) is a chronic gastrointestinal inflammatory disease initiated by dietary gluten in genetically predisposed individuals. While the inflammatory processes which drive tissue destruction in the coeliac duodenum have been extensively characterized, an increased oxidative stress (OS) response has also been suggested to contribute to CeD pathogenesis. However, the precise mechanisms which regulate OS in the coeliac mucosa and whether they impact inflammation remain ill defined. The master anti-oxidant transcriptional regulator Nuclear factor erythroid 2-related factor 2 (Nrf2),

Eosinophilic Colitis Presenting as Hemoperitoneum: A Report of a Rare Case.

Eosinophilic colitis (EC) is a rare clinical entity. Advanced cases with severe inflammation may develop severe abdominal pain and distension, malabsorption, intestinal obstruction, and even GI hemorrhage. Once diagnosed early, EC responds to steroid therapy. We report the case of a 46-year-old male who presented with acute-onset left upper abdominal pain, enlargement of the right scrotal sac, and hemoperitoneum. A contrast-enhanced CT scan of the abdomen revealed an irregular hypoechoic lesion measuring approximately 6.5 × 4.8 cm, likely a transverse mesocolon hematoma in the epigastric regio
