BACKGROUND: Recognition of the synergistic cycle of malnutrition, immune dysfunction, and recurrent enteric infection in children and its role in linear growth stunting and impaired cognitive development (the "stunting syndrome"), dates back to the 1960s. Early hypotheses emphasized reduced intestinal absorption associated with diarrhea as the primary enteropathy driving malnutrition and developmental deficits. However, recent evidence indicates that a subclinical condition termed environmental enteropathy (EE), or environmental enteric dysfunction (EED), may be a more critical contributor. Mi

Hypothyroidism is a common condition that can have serious consequences if left untreated. Adrenal insufficiency, on the other hand, is a rare condition that can be life-threatening if left untreated. Early initiation of treatment with L-thyroxine and hydrocortisone supplementation helps restore quality of life by eliminating the risks associated with these deficiencies. Managing this type of hormonal imbalance in patients with short bowel syndrome is a rare situation, and there are few clear guidelines regarding its treatment.

PURPOSE OF REVIEW: Immune-related enteropathies (IREs) represent a heterogeneous group of disorders characterised by immune-mediated small bowel injury. While coeliac disease is the most common cause, widespread use of small bowel endoscopy has expanded the recognition of rarer causes. This review aims to summarise recent advances in diagnosis, classification and management of IRE.
RECENT FINDINGS: Significant overlap exists between coeliac disease, autoimmune enteropathy, collagenous sprue, drug-induced enteropathies, common variable immunodeficiency (CVID) associated enteropathy and other I

BACKGROUND: The role of homocysteine (Hcy) in immunoglobulin A nephropathy (IgAN) remains unclear. This study investigates the association between Hcy levels, clinical symptoms, and pathological changes in patients with IgAN to elucidate Hcy's clinical significance.
METHODS: 256 IgAN patients were included and categorized into two groups : normal Hcy and hyperhomocysteinemia (HHcy). Clinical and pathological characteristics were compared between these groups. Logistic regression was used to analyze the association between clinicopathological factors and Hcy levels, while the predictive capabi

Dermatitis herpetiformis (DH) shares a pathophysiological basis with celiac disease (CD), but its epidemiology remains poorly defined. This systematic review and meta-analysis estimated the prevalence of DH among CD patients. Following the PRISMA 2020 guidelines, PubMed, Embase, Web of Science, and Scopus were searched. Observational studies with extractable DH data in CD patients were pooled using a random-effects model, with subgroup and meta-regression analyses. The risk of bias was assessed using the JBI Checklist. Of 7271 records, 24 studies published between 1996 and 2023, comprising 73,

BACKGROUND: Mitochondrial neurogastrointestinal encephalomyopathy (MNGIE) is an extremely rare autosomal recessive disease caused by variants in the thymidine phosphorylase gene (TYMP), primarily characterized by severe gastrointestinal and neurological symptoms. The complete phenotype of MNGIE has not been linked to any gene other than TYMP.
METHODS: We describe two identical twins who exhibited delayed psychomotor development, infantile bilateral cataract, congenital demyelinating polyneuropathy, and severe progressive gastrointestinal dysmotility with recurrent pseudo-obstruction episodes,

Thiamine (vitamin B1) deficiency is a reversible, yet potentially fatal condition that affects both the central and peripheral nervous systems. Although commonly linked to chronic alcoholism, non-alcoholic causes, including malabsorption syndromes, are increasingly being recognized. We report the case of a 22-year-old woman who presented with progressive lower limb weakness, gait disturbances, and confusional symptoms, following a history of intermittent diarrhea and vomiting. Neurological examination revealed ascending motor deficits, areflexia, cerebellar syndrome, and internuclear ophthalmo

GOALS: This study aimed to assess the seroprevalence of celiac disease in Mexican patients with autoimmune thyroid diseases, hypothesizing that prevalence would align with rates observed in other populations.
BACKGROUND: The association between celiac disease and autoimmune thyroid diseases has been documented globally, with varying seroprevalence rates. Historically, Mexican Mestizos were considered at low risk for celiac disease, yet recent findings suggest similar prevalence to other regions. However, data regarding Hispanic patients with autoimmune thyroid diseases are limited.

Celiac disease (CeD) is a gluten-induced immune-mediated enteropathy that preferentially involves the proximal duodenum. Consequently, iron deficiency is common in CeD, impacting at least 10% of newly diagnosed individuals. In this narrative review, we aim to investigate the mechanisms, prevalence, treatment, and monitoring of iron deficiency and the impacts of a gluten-free diet (GFD) on iron deficiency in individuals with CeD. We will also review the role of and approach to iron supplementation in this population. Specifically, we will explore whether mucosal healing on a GFD is sufficient f

BACKGROUND: Diabetic retinopathy (DR) is a major microvascular complication of diabetes mellitus and a leading cause of visual impairment. While dyslipidemia has been implicated in DR pathogenesis, the relationship between high-density lipoprotein cholesterol (HDL-C) and DR remains controversial. This study aimed to investigate the association between HDL-C levels and DR prevalence in diabetic patients type 2 diabetes mellitus (T2DM).
METHODS: This study is the second analysis based on a cross-sectional studv. A total of 2001 (858 men and 1143 women) diabetic patients who visited the diabetic

Celiac disease (CD) and Hashimoto's thyroiditis (HT) frequently coexist, suggesting that shared mechanisms of autoimmunity extend beyond the intestine. In CD, the pathogenic role of gluten is firmly established; in HT, however, gluten is better viewed as a candidate modifier rather than a proven universal trigger. This review synthesizes current evidence on how gluten may influence the CD-HT axis through gut dysbiosis, epithelial barrier dysfunction, immune cross-reactivity, and epigenetic regulation. We also examine the clinical evidence for gluten-free diet (GFD) use in three settings: class

AIMS: Type 1 diabetes mellitus (T1DM) is frequently associated with other autoimmune disorders, particularly autoimmune thyroid disease (AITD) and celiac disease. Guidelines recommend screening for thyroid and celiac disease in T1DM patients. Nevertheless, in case of their positivity, indicating the presence of an autoimmune polyendocrine syndrome (APS), there is a lack of guidance on how to proceed. Should we screen these patients for other autoimmune diseases? This study's aim was to evaluate the prevalence of additional autoimmune conditions in a cohort of T1DM patients with AITD and/or cel

BACKGROUND: First-degree relatives (FDRs) of individuals with type 1 diabetes (T1D) carry a significantly elevated risk of developing autoimmune diseases, including celiac disease (CD) and autoimmune thyroid disease (AITD). The DiaUnion 1.0 study aimed to characterize one-time autoantibodies, genetic risk profiles and long-term progression to T1D, CD or AITD in siblings of children newly diagnosed with T1D.
METHODS: Between 1997 and 2010, 1427 Danish siblings provided plasma and DNA samples to the DanDiabKids biobank. Autoantibodies associated with T1D (GADA, IAA, IA2A, ZnT8A), CD (tTGA) and

BACKGROUND: Vitamin B12 deficiency is a reversible cause of hematologic and neurologic morbidity, but its etiologic spectrum in Latin American referral centers remains poorly defined.
METHODS: We retrospectively studied consecutive adults with vitamin B12 deficiency evaluated in hematology and oncology clinics at a Mexican tertiary referral center from January 2013 to July 2023. Etiologies were classified as pernicious anemia, non-surgical malabsorption, post-surgical malabsorption, reduced intake, drug-related causes, or unknown, and pernicious anemia was compared with all other etiologies.

Anemia due to micronutrient deficiencies has received little attention in public health and modern-day clinical practice. Increasing numbers of persons, mainly in low- and middle-income nations, are faced with malnutrition and malabsorption syndromes, giving rise to various micronutrient deficiencies that can lead to anemias refractory to treatment with iron, folate or vitamin B12. Though relatively uncommon, such underlying nutrient deficiencies may be difficult to recognize as they can coexist with more common causes and there may be few or no specific clinical characteristics pinpointing a

BACKGROUND: Homocysteine (Hcy) is a sulfur-containing intermediate metabolite during methionine metabolism. Studies indicate that individuals with polycystic ovary syndrome (PCOS) have higher circulating Hcy levels relative to unaffected populations. Hyperhomocysteinemia (HHcy) has been implicated in multisystem cytotoxicity. This study was designed to evaluate the association between Hcy levels and embryo quality and pregnancy outcomes in PCOS patients undergoing assisted reproductive technologies (ART). Mechanistic insights were explored through in vitro assays and combined network pharmacol

Hookworm infects over 400 million people globally and causes gastrointestinal morbidity, yet its physiological effects remain poorly defined. Controlled human hookworm infection is also being explored as a therapy for gut diseases. We performed an exploratory study to evaluate the impact of experimental Necator americanus infection on gastrointestinal transit, motility, and luminal pH in 10 healthy adults (mean age 41 yr, 60% females) infected with 30 larvae via skin application. Assessments using the SmartPill Wireless Motility Capsule were performed at baseline, week 6 (acute infection), and

BACKGROUND: Giardia duodenalis is globally distributed intestinal protozoan parasite responsible for giardiasis causing various clinical manifestations ranging from asymptomatic to chronic diarrhea and malabsorption. Accurate diagnosis is crucial for effective clinical management and prevention of transmission especially in cases with low-parasite burden where false-negative results remain a challenge.
METHODS: In this study, the diagnostic performance of four commonly used molecular markers: triosephosphate isomerase (tpi), small subunit ribosomal RNA (ssu-rRNA), glutamate dehydrogenase (gdh

Giardia duodenalis (synonyms G. lamblia and G. intestinalis) and Blastocystis sp. are prevalent zoonotic intestinal protozoans with significant public health and economic importance. Both parasites infect various hosts, including humans and most livestock. G. duodenalis can cause infections ranging from asymptomatic carriage to clinical manifestations such as acute diarrheal disease, growth stunting and malabsorption, whereas Blastocystsi sp. infection is often asymptomatic and its pathogenicity remains debated. Following the rapid expansion of modern equine industries (horse racing, equestria

BACKGROUND: Dupuytren's disease (DD) is a common fibrotic disorder of the hand, characterized by progressive thickening and contracture of the palmar and digital fascia. Surgical excision remains the primary treatment; however, there are currently no therapies to prevent disease progression or recurrence. This study aims to develop a 3D in vitro model to test novel antifibrotic therapies. The model is based on decellularized pathological DD tissue seeded with patient-derived fibroblasts, capturing the role of both cellular and extracellular matrix components in disease progression.
