The alleviation of oxidative damage in celiac disease by black soybean peptides and their mechanism.

BACKGROUND: Celiac disease (CD) is a chronic autoimmune condition that occurs in genetically predisposed individuals when they consume gluten. The α-gliadin peptide exacerbates the CD pathological process by inducing oxidative stress, intestinal cell damage, and apoptosis. Addressing oxidative stress in CD may offer a valuable strategy for supplementary treatment. RESULTS: Black soybean peptides (BSPs) have been shown to inhibit oxidative stress and inflammation, although the specific mechanisms are not fully understood. This study investigated the effects of BSPs on Caco-2 cytotoxicity induc

Novel Treatment of Irritable Bowel Syndrome (IBS) and Other Disorders of the Peripheral Nervous System Using Peripherally Restricted Positive Allosteric Modulators of GABA‑A Receptor.

The invention in this patent application relates to 6,9-dihydro-5H-imidazo-[1,2-a]-[1,4]-diazepine derivatives (or their N-oxides) represented herein by formula 1. These compounds are peripherally restricted, positive allosteric modulators of one or more GABA-A receptors. They selectively target the peripheral nervous system and body organs and do not substantially penetrate the blood-brain barrier. Thus, they may provide useful treatment for irritable bowel syndrome and other related peripheral disorders, including, but not limited to, functional abdominal pain, chronic idiopathic diarrhea, v

From analytical bias to threshold-based discordance: An ID-LC-MS/MS reference method reveals and corrects hyperhomocysteinemia classification discordance in China.

BACKGROUND: Homocysteine (Hcy) is a critical biomarker for cardiovascular risk stratification, but significant between-method variability in China can result in classification discordance relative to candidate reference measurement procedure (cRMP), including both positive discordance and negative discordance of hyperhomocysteinemia. We aimed to establish a cRMP and evaluate its ability to correct method-dependent classification discordance. METHODS: An isotope-dilution liquid chromatography-tandem mass spectrometry (ID-LC-MS/MS) cRMP for serum Hcy was developed and validated. Five routine me

Neurological Manifestations in Whipple's Disease: A Scoping Review of Clinical Features, Laboratory Findings, and Neuroimaging Characteristics of 111 Cases.

BACKGROUND: Whipple disease (WD) is a rare, potentially fatal infection caused by Tropheryma whipplei (TW). Central Nervous System involvement (CNS-WD) is a severe complication of WD with a challenging diagnosis, due to limited data integrating neurological features, imaging, cerebrospinal fluid analysis, PCR, and intestinal histopathology. Prompt recognition and treatment are essential to improve patient outcomes. METHODS: A scoping review of the literature from 2000 to 2025 was conducted, including all CNS-WD case reports and series, to outline its clinical spectrum and diagnostic workup.

Presumptive paroxysmal dyskinesia in 41 dogs: is the diet change enough?

BACKGROUND: Paroxysmal dyskinesia (PD) in dogs comprises episodic, involuntary, nonepileptic movement disorders. Recent evidence suggests that PD may be associated with gluten sensitivity, paralleling human gluten-related neurologic syndromes. HYPOTHESIS/OBJECTIVES: Characterize clinical features, gastrointestinal comorbidities, gluten-related serology, and response to a gluten-free diet (GFD) in dogs with presumptive gluten-sensitive PD. We hypothesized that dietary modification would decrease episode frequency, particularly in dogs with gastrointestinal signs, seropositivity for anti-gliadi

Developing Glow: A Co-Designed Support Framework Integrating Behavioural and Psychological Needs in Coeliac Disease.

BACKGROUND: Strict management of a gluten-free diet (GFD) is the only effective treatment for coeliac disease; support in current healthcare frameworks focususes primarily on dietary support and intestinal recovery. However, psychological and social processes that shape gluten-free living are less often addressed, despite high levels of emotional burden among people with coeliac disease. There is limited guidance on how these needs should be met in routine support. OBJECTIVE: To co-design an evidence-informed framework integrating dietary, behavioural and psychological support for adults livi

Irregular follow-up is associated with poorer dietary adherence and iron deficiency in children with celiac disease.

The purpose of this study is to evaluate the association between follow-up regularity and dietary adherence, growth, and micronutrient status in children with celiac disease (CD). In this single-center, prospective observational study, children on a gluten-free diet (GFD) for ≥ 1 year were classified over a fixed 24-month window as regular follow-up (RFU; ≥ 4 visits) or irregular follow-up (IFU; < 4 visits) according to the number of outpatient visits. Adherence was assessed by parent- and child-reported modified Biagi scores and by anti-tissue transglutaminase IgA (TGA) normalization (< 20 RU

Lengthening Bowel Endoscopically: Hydrogel-Enabled Endoscopic Spring-Mediated Distraction Enterogenesis.

We evaluated the feasibility to lengthen small bowel endoscopically utilizing a spring device constrained by an engineered, slow biodegrading, hyaluronan and elastin-like protein (HELP) hydrogel as a potential treatment for short bowel syndrome. While we previously demonstrated that the HELP hydrogel could provide device confinement and enable non-invasive device removal, here we demonstrate that the HELP hydrogel can also be designed to be catheter-injectable while still being stiff enough to achieve device confinement. Six pigs underwent endoscopic spring-mediated distraction enterogenesis (

Coexistence of Chiari 1 Malformation and Marsh 3b Celiac Disease.

This case report presents the first simultaneous occurrence of Chiari malformation type 1 and celiac disease in a 27-year-old male patient. To the best of our knowledge, there are no previously published cases in the literature. This rare association suggests that nutrient malabsorption caused by celiac disease may exacerbate the symptomatic severity of Chiari malformation. We recommend screening for celiac disease in patients diagnosed with Chiari malformation who present with anaemia, vitamin deficiency or unexplained gastrointestinal complaints. Conversely, in the presence of atypical neuro

One threshold does not fit all: assay-specific validation of anti-tTG IgA tests for biopsy-free diagnosis of pediatric celiac disease.

BACKGROUND: The ESPGHAN biopsy-free diagnostic algorithm for pediatric celiac disease (CeD) requires serum IgA anti-tissue transglutaminase antibody (anti-tTG IgA) concentrations ≥10 times the upper limit of normal (ULN) in the first blood sample, together with positive IgA endomysial antibodies (EMA) in a second blood sample. However, the transferability of the 10×ULN anti-tTG IgA threshold across different analytical platforms remains uncertain owing to the lack of assay harmonization. METHODS: Eighty-four children (6-10 years) who underwent duodenal biopsy during the diagnostic work-up for

Fat malabsorption and reduced exocrine pancreatic function after gastroesophageal cancer surgery.

BACKGROUND: Patients post-gastroesophageal cancer surgery frequently present with clinical features of malabsorption and are often treated empirically with pancreatic enzyme replacement therapy (PERT). Primary aim was to objectify the presence of fat malabsorption in patients after gastroesophageal surgery and, secondary, to measure exocrine pancreatic function. METHODS: Eligible patients were those who underwent gastroesophageal cancer surgery (2017-2021), were at least six months post-surgery, maintained an oral diet, and showed no evidence of recurrent disease. Treatment with PERT was ceas

Validation of a rapid test for celiac disease detection in first-degree relatives - A prospective study.

Celiac disease (CD) is an autoimmune disorder triggered by gluten in genetically predisposed individuals, often diagnosed by detecting anti-tTG2 antibodies. First-degree relatives of CD patients are at high risk and require careful evaluation. We conducted a prospective study to validate CeliacDetect, a rapid immunochromatographic test that detects anti-tTG2 IgA antibodies in capillary blood within 10 minutes. Involving 95 first-degree relatives, the study showed that CeliacDetect had 100% sensitivity, specificity, positive predictive value, and negative predictive value. These results suggest

Gluten-Free Diet in the Context of Food and Nutrition Security, Food Safety, and Sustainability.

A gluten-free diet (GFD) is the only safe treatment for people with gluten-related disorders (GRDs). However, adherence to a GFD is challenging in different interconnected aspects related to food security, safety, and sustainability. These obstacles are critically contextualized in this review. In general, a GFD may increase the risk of food and nutritional insecurity, especially in populations with lower educational levels and purchasing power. Furthermore, avoiding cross-contact with foods containing gluten is challenging, reducing the options available for consumption and increasing fear an

Potassium-Rich, Gluten-Free Diets for Patients with Sjögren's Syndrome: A Mini-Review.

Sjögren's syndrome (SS) is an autoimmune disease and its management is palliative. There is no specific dietary protocol for SS patients. A gluten-free diet has been tested in SS patients with celiac disease (CD) and indicated modest improvements. Whether gluten-free diets per sè could alleviate autoimmune inflammatory processes in the salivary glands of SS patients with associated CD or even in SS patients without CD is an interesting hypothesis and warrants clinical studies. Hypokalemia in SS patients is among the most frequent sequelae of renal tubular acidosis. Supplementation with potassi

Unlocking New Treatment Horizons for Celiac Disease: PRKCD Revealed as a Promising Target through Mendelian Randomization.

INTRODUCTION: Celiac Disease (CeD) is a serious, lifelong autoimmune condition. There remains a significant unmet medical need for effective pharmacological treatments for CeD. METHODS: We utilized summary statistics for 2,888 druggable genes from the eQTLGen Consortium and the FinnGen Consortium for CeD. In our Mendelian Randomization (MR) analysis, we identified genes associated with CeD that had a false discovery rate (FDR) < 0.05 using the Inverse Variance Weighted (IVW) method. To enhance the reliability of the results, we validated them through colocalization analysis and Summary-data-b

Development of a consensus parenteral solution weaning algorithm for children with short-bowel syndrome associated intestinal failure responding to teduglutide therapy.

BACKGROUND: Teduglutide is a glucagon-like peptide 2 analogue with demonstrated efficacy in reducing parenteral solution (PS) requirements to facilitate enteral autonomy in children with short bowel syndrome-associated intestinal failure (SBS-IF). There is currently no clear consensus on how to wean PS and monitor children once treatment is commenced, with variable regimens utilised in published paediatric and adult studies. We aimed to develop a practical algorithm for PS weaning in children with SBS-IF responding to teduglutide for use in the real-world setting. METHODS: A modified Delphi a
