Treatment Outcomes of Short Bowel Syndrome in Paediatric Patients.

OBJECTIVE: This study evaluated the association between intestinal anatomical characteristics-including the resected bowel segment, residual small bowel length (RSBL), and preservation of the ileocecal valve (ICV)-and treatment outcomes in children with short bowel syndrome (SBS). METHODS: A retrospective study was conducted of 35 paediatric SBS patients who underwent small bowel resection at the study site from July 2021 to July 2024. SBS was defined by an RSBL ≤ 25% of the expected length for age or dependence on parenteral nutrition ≥ 60 days. Treatment outcomes were categorised as success

Protective Effects of Probiotic Mediated Digested Gliadin and 33-mer Peptide in Caco-2 Cell Cultures.

Gluten protein generates an immunogenic 33-mer peptide upon incomplete digestion, leading to induced cytotoxic and inflammatory responses in intestinal epithelial cells of susceptible individuals with Celiac disease (CeD). The present study investigates the protective effects of gluten hydrolyzing probiotic strains-Bacillus subtilis AJG10 and Bacillus tequilensis AJG23 on gliadin and 33-mer peptide in the Caco-2 cell model. The effect of probiotics on further degradation of 33-mer peptide and pepsin-trypsin digested gliadin was analysed using SDS-PAGE and mass spectrometry. The effect of all f

Updates on intestinal failure management.

INTRODUCTION: Chronic intestinal failure (CIF) is a rare condition characterized by inadequate intestinal absorption requiring long-term parenteral nutrition (PN). Given etiological shifts and expanding therapeutic options, we provide an updated review covering CIF definitions, epidemiology and multidisciplinary management in adults, based on recently published studies (2012-2025). AREAS COVERED: We conducted a scoping review of recent literature, registry data and guideline recommendations to summarize contemporary trends in CIF.Short bowel syndrome remains the leading cause of CIF, though t

Follow-up in coeliac disease: evidence and future challenges.

It is estimated that Coeliac Disease has a prevalence of 1% of the global population, and many cases remain undiagnosed, either because patients do not have proper health monitoring or, conversely, due to the asymptomatic nature of the disease in some cases. Furthermore, there is no population wide screening program, although suspicion of the disease has significantly improved. Consequently, it is expected that the incidence will continue to rise in the upcoming years. With this letter, we aim to raise awareness of the imminent challenge for gastroenterologists with expertise in Coeliac Diseas

Unmasking Celiac Disease Through Chronic Urticaria: Case Report and Scoping Review.

Background: Celiac disease (CD) is an immune-mediated, gluten-induced enteropathy with intestinal and extraintestinal manifestations. Chronic urticaria (CU) is a heterogeneous inflammatory skin disorder often considered idiopathic, but emerging evidence suggests possible autoimmune causes. Methods: We describe a pediatric case in which CU and angioedema were the sole clinical expressions of CD. We also conducted a scoping review of the literature to assess the prevalence of CD in CU patients and the therapeutic impact of a gluten-free diet (GFD). Results: The child's CU resolved rapidly after

The shared mechanisms and potential diagnostic markers for IgA nephropathy and celiac disease.

IgA nephropathy (IgAN) and celiac disease (CeD) are autoimmune disorders characterized by dysregulated immune responses; however, the molecular mechanisms underlying their comorbidity remain incompletely understood. Here, we integrated transcriptomic datasets from IgAN and CeD to perform differential expression analysis, weighted gene co-expression network analysis (WGCNA), functional enrichment analysis, and machine learning-based hub gene identification. The expression profiles and diagnostic performance of the identified hub genes were validated across multiple independent cohorts using rec

Myosin VB is critical for progenitor cell identity and function in the intestine.

Microvillus inclusion disease (MVID) is a congenital diarrheal disorder, caused by inactivating mutations in myosin Vb (MYO5B). MYO5B-deficient mice and cell lines have demonstrated the importance of MYO5B in brush border development; however, the previous models lacked specificity to test intestinal stem cell functions. In the present study, we investigated the effects of progressive MYO5B deficiency originating in intestinal crypt cells utilizing mouse models. Our transcriptomic and multiplex immunostaining datasets demonstrate that MYO5B is critical for intestinal stem cell function. MYO5B-

Placental extracellular vesicles stimulate intestinal re-growth after massive small bowel resection in a rat model.

Extracellular vesicles (EVs) are released by most cells, including stem cells, into the extracellular environment and regulate cell function and tissue regeneration through cell-cell communication. The EVs derived from human placenta have a crucial role in regulating the maternal immune response for a successful pregnancy outcome. The purpose of the current study was to evaluate the effects of human villous trophoblasts (HVT)-derived EVs on structural intestinal adaptation, cell proliferation and apoptosis in a rat model of short bowel syndrome (SBS). HVT-EVs were isolated from cell media afte

Structural Insights into HLA-DQ-Associated Susceptibility to Celiac Disease Through an Integrated Genetic and In Silico Approach in a Sardinian Population.

BACKGROUND: Celiac disease (CD) is a multifactorial autoimmune disorder strongly associated with specific HLA class II molecules, particularly HLA-DQ-encoding haplotypes. Although the genetic contribution of these loci is well established, the structural features accompanying allele-specific disease susceptibility remain incompletely explored. METHODS: In this study, molecular HLA typing was integrated with in silico secondary structure analysis to examine the relationship between genetic predisposition and structural organization of HLA class II molecules in a Sardinian population. [Diagnostic challenges in celiac disease].

Celiac disease (CeD) is caused by an autoimmune reaction triggered by dietary intake of gluten. CeD affects approximately 1% of the population, although many cases remain undiagnosed. CeD is diagnosed by testing for CeD antibodies, and if positive, referral to a gastroenterologist or pediatrician is indicated. Treatment of CeD is a lifelong gluten-free diet. Guidance from a dietitian is needed. Symptoms of CeD vary and are often non-specific, which can cause diagnostic challenges and often a significant diagnostic delay. Therefore, increased awareness of CeD symptoms and knowledge of diagnosti

Rivaroxaban vs. apixaban in short bowel syndrome: a focused clinical pharmacology review.

INTRODUCTION: Short bowel syndrome (SBS) substantially alters nutrient and drug absorption and frequently necessitates long-term parenteral nutrition, increasing thrombotic risk. Indications for long-term anticoagulation such as atrial fibrillation or venous thromboembolism are common in this population. Because gastrointestinal anatomy and physiology are markedly distorted, the pharmacokinetics of direct oral anticoagulants (DOACs) may become unpredictable, creating uncertainty in drug selection and dosing. AREAS COVERED: This review compares pharmacokinetic and clinical evidence for rivarox

Investigation of the stool microbiome of short bowel syndrome patients before and after a 24-week treatment with the glucagon-like peptide 2 analog glepaglutide.

BACKGROUND AND AIMS: Glepaglutide is a long-acting glucagon-like peptide 2 (GLP-2) analog under development for the treatment of short bowel syndrome (SBS). Glepaglutide enhances intestinal absorption, which may hypothetically lead to changes in the intestinal microbiota, mainly by slowing gastrointestinal transit time. This study evaluated whether glepaglutide affects bacterial load and composition of the intestinal microbiota in SBS patients enrolled in a phase 3b trial assessing its 24-week efficacy on intestinal wet weight and energy absorption. METHODS: In this single-center, open-label,

Surgical reconstruction in a 100-year-old patient with short bowel syndrome under palliative care: A case report.

We present the case of a 99-year-old woman who developed functional short bowel syndrome after an emergency laparotomy for mesenteric ischemia. Her initial management was palliative care because of hemodynamic instability and subsequently because of advanced age. Following 3 months of recurrent infections and 30% weight loss, her dependence on intravenous hydration and her declining quality of life led to a reassessment of treatment goals. Ultimately, she underwent successful intestinal reconstruction with a side-to-side jejuno-jejunal anastomosis after reaching 100 years of age during the pre
