Nutritional surveillance and diet adherence in pediatric celiac disease: A multidisciplinary case-control study within a clinical nutrition model.

BACKGROUND & AIMS: Adherence to a gluten-free diet (GFD) is the cornerstone of celiac disease (CD) management, yet maintaining long-term adherence remains challenging, particularly in adolescence. Evidence on the nutritional profile and dietary adherence of pediatric CD populations in Southern Europe is scarce. This study aimed to compare the nutritional status of children and adolescents with CD to healthy peers and to explore the relationship between GFD duration, age at diagnosis, and dietary adherence. METHODS: This observational case-control study included 80 biopsy-confirmed pediatric C

Factors associated with adherence to gluten-free diet among celiac patients in Palestine: a cross-sectional study.

Celiac disease (CD) is an autoimmune disease. Since the only treatment is a lifelong strict gluten-free diet (GFD), good adherence is the cornerstone. No research has examined the degree of GFD adherence among CD patients in Palestine and how it impacts the disease activity and prognosis. Therefore, this study aims to determine the level of adherence to GFD among adult patients with (CD) in Palestine, and its association with disease activity, mental health, patients' awareness, attitude, barriers, lifestyle, and sociodemographic variables. The study involved 137 patients with celiac disease u

Enterocyte proliferation as a new biomarker in potential coeliac disease.

BACKGROUND: Potential coeliac disease is an increasingly diagnosed condition, and the dilemma whether start a gluten-free diet is still unsolved. AIMS: We hypothesized that an exaggerated enterocyte turnover is responsible for an impairment of gut function leading to the development of symptoms and/or progression to villous atrophy. METHODS: The proliferation and apoptotic rates of duodenal enterocytes, assessed by anti-Ki-67 and -Caspase-3 antibodies at immunohistochemistry, of 36 adult patients with potential coeliac disease were compared to those found in a group of 32 active coeliac pati

Key Roles of B Cells and B-Cell Receptors in HLA Class II-Associated Diseases: Insights From Coeliac Disease.

When an HLA association is observed for a disease, it is typically to one or a few HLA class II allotypes. Considering that the HLA association is mechanistically tied to a preferential presentation of antigenic peptides to T cells, its presence suggests that only one or a few HLA-binding peptides are likely to contribute to the pathogenesis. By contrast, if many antigenic peptides are involved, the likelihood for involvement of several HLA allotypes increases and, consequently, the disease will not appear as an HLA associated disease even though T cells could play equally important roles. Cultural Adaptation and Reliability Testing of the Coeliac Disease Food Attitudes and Behaviours Scale in Brazil.

Background: Coeliac disease (CD) is an autoimmune chronic condition that requires a strict diet without gluten as a single effective treatment. However, adherence to a life-long gluten-free diet (GFD) may promote psychological suffering and disordered eating. The Coeliac Disease Food Attitudes and Behaviours Scale (CD-FAB) is the only instrument specifically designed to evaluate these aspects in individuals with CD. Objective: This study aimed to translate the CD-FAB and test its reliability in the Brazilian population. Methods: The research was conducted in three stages: (i) translation and c

A comparative study of cognitive function among children with coeliac disease and healthy controls.

Coeliac disease (CD) is an immune-mediated systemic disorder triggered by gluten in genetically predisposed patients. The only available treatment is a strict life long gluten-free diet (GFD), which has been linked to a reduced quality of life (QOL) and causes alterations in the gastrointestinal microbiome. Abnormal compositions of the microbiome are now recognized as factors in the pathogenesis of neuropsychological disorders via gut-brain-axis. The aim of this study was to assess the QOL and the mental performance of children and teenagers with CD and compare it to healthy controls (HC). Chi

Diagnostic delay in children with coeliac disease: a national multicentre study in Turkey.

OBJECTIVES: There are few studies conducted investigating diagnostic delay in children with coeliac disease (CD). A better understanding of the factors associated with diagnostic delay and the development of strategies to reduce it are important in preventing long-term complications related to the disease. This study aimed to determine the median diagnostic delay in children with CD and to investigate possible underlying factors associated with diagnostic delay. METHODS: A prospective screening program was performed in children with CD at the seven clinical centers operating in Turkey within t

How to diagnose coeliac disease in 2026?

Coeliac disease (CD) is a chronic, immune-mediated enteropathy triggered by gluten ingestion in genetically predisposed individuals carrying HLA-DQ2 and/or -DQ8 alleles. Its diagnosis traditionally combines serology and confirmatory duodenal biopsy. IgA anti-tissue transglutaminase is the first-line test, supported by endomysial antibody. Duodenal histology remains the gold standard, though limitations include patchy involvement and variability in interpretation. Recent advances have proposed biopsy-sparing approaches, whereas applicability in adults remains debated. Moreover, CD is increasing

Limited Availability and Higher Cost of Gluten-Free Foods Continue in the United Kingdom: A Comparative Follow-Up Over More Than a Decade.

BACKGROUND: Coeliac disease requires strict lifelong adherence to a gluten-free diet, although adherence is challenged by the limited availability and higher cost of gluten-free foods. The demand for gluten-free foods has increased over recent years, yet its impact on availability and cost is unknown. This study aims to assess the availability and cost of gluten-free food across diverse retail outlets and compare changes over 11 years. METHODS: Replicating the same methodology as our previous study in 2010, the availability and cost of 20 foods (10 wheat-based, 10 everyday foods) were assesse

Olmesartan-related sprue-like enteropathy presenting as severe diarrhoea.

A woman in her 60s with a history of atrial fibrillation, hypertension and dyslipidaemia was admitted to the Intensive Care Unit with hypotension, dehydration and acute kidney injury after 20 days of watery diarrhoea. Although initial investigations did not detect an evident infectious aetiology, a diagnosis of infectious colitis was hypothesised. After supportive care and quick recovery, she was discharged and instructed to resume her prior medications, including olmesartan (used for 1 year) for hypertension. Diarrhoea and renal failure recurred promptly, leading to readmission. Gastroscopy r

Modernising Coeliac Disease Dietitian Follow-Up: Engagement and Functionality of a Digital Annual Review.

INTRODUCTION: UK guidance recommends adults living with coeliac disease (CD) receive annual care from a dietitian. However, many CD provisions have difficulties meeting these demands. The study aimed to evaluate a digital virtual annual review (VAR) tool as an alternative method of reviewing clinically stable adults with CD annually. METHODS: This single-centre retrospective study analysed data between September 2022 and March 2024. Adults living with CD, diagnosed for > 3 years and deemed clinically stable by a specialist CD dietitian, received a link to complete the digital VAR tool. Chi-sq

Performance of large language models in answering frequently-asked questions on celiac disease.

OBJECTIVES: Celiac disease (CeD) is a common autoimmune condition requiring lifelong adherence to a gluten-free diet (GFD). Patients and caregivers increasingly seek information online, and large language models (LLMs) have emerged as potential educational tools. However, their reliability in CeD remains uncertain. This study aimed to evaluate the performance of three popular LLMs in answering frequently asked questions (FAQs) about CeD and GFD management. METHODS: We conducted a cross-sectional comparative evaluation in which 12 FAQs were submitted to three LLMs: ChatGPT-4 (OpenAI), Gemini F

Factors Affecting Gluten-Free Dietary Adherence in Patients with Neurological Gluten-Related Disease.

Background/Objectives: The gluten-free diet (GFD) is the primary treatment for patients with neurological gluten-related disease, which may occur with or without coeliac disease (CD). Dietary adherence is arguably most important in such patients, as ongoing gluten exposures have been shown to exacerbate irreversible neurological deterioration. We utilised a cross-sectional postal questionnaire to explore factors affecting dietary adherence in a large sample of such patients, highlighting potential areas of dietetic need. Methods: Patients returned a postal questionnaire (N = 225), which assess

Immunoglobulin gene polymorphisms shape the naïve B-cell receptor repertoire; relevance to celiac disease.

Genetic variation in the loci encoding immunoglobulin genes (IGH, IGK and IGL) affects the repertoire of B-cell receptors (BCRs). Such effects were previously demonstrated for total peripheral blood B cells, but so far have not been investigated at scale for isolated naïve B cells. As B cells are implicated in the pathogenesis of autoimmune diseases, genetically encoded features of the naïve BCR repertoire may affect disease risk, for instance in celiac disease (CeD) which is hallmarked by stereotyped disease-specific antibodies that recognize antigen in their germline configuration. Here we h
