Massive Dilation of the Cystic Duct Stump With a Retained Stone Two Years After Complete Cholecystectomy Managed With Robotic-Assisted Resection.

Retained stones within the cystic duct stump are an uncommon but clinically important cause of postcholecystectomy syndrome. Dilation of the remnant duct secondary to an occluding stone is exceedingly rare, with only isolated reports of massive dilation in the literature. We describe a 67-year-old male (BMI 31.35) who presented two years after laparoscopic cholecystectomy with recurrent right upper quadrant biliary colic. Cross-sectional imaging demonstrated progressive enlargement of the cystic duct stump from mild dilation at 241 days to marked dilation of greater than 40 mm containing a 9 m

Completion cholecystectomy for remnant gall bladder stones: A tertiary centre experience.

INTRODUCTION: Remnant gall bladder and cystic duct stump calculi are uncommon but significant causes of post-cholecystectomy syndrome, often presenting after a prolonged interval and posing diagnostic and technical challenges. Evidence from contemporary Indian cohorts remains limited and heterogeneous. PATIENTS AND METHODS: This retrospective observational study analysed 22 consecutive patients who underwent completion cholecystectomy for symptomatic remnant gall bladder or cystic duct stump calculi between January 2015 and December 2024 at a tertiary referral centre. Patients with radiologic

Duodenal obstruction from superior mesenteric artery syndrome with nutcracker phenomenon in an adolescent: a case report.

INTRODUCTION AND IMPORTANCE: Superior mesenteric artery (SMA) syndrome is a rare cause of duodenal obstruction. Its diagnosis is often delayed due to nonspecific symptoms that mimic common functional gastrointestinal disorders. This case highlights the diagnostic challenge and the critical role of imaging, further complicated by an incidental, anatomically related finding. PRESENTATION OF CASE: A 13-year-old male presented with a 2-month history of post-prandial vomiting, epigastric pain relieved by a prone position, and unintentional weight loss of 9.3% of total body weight, having previousl

Langerhans cell histiocytosis presenting as a cecal mass in an adolescent: a rare surgical case report.

INTRODUCTION AND IMPORTANCE: Langerhans cell histiocytosis (LCH) is a rare, idiopathic, multifocal disease that commonly occurs in the skin and skeletal system, although involvement of the gastrointestinal tract is exceptional. Only a limited number of cases have been reported in the literature, particularly at the intestinal level, often with incidental diagnosis after colonoscopic polypectomy, and presentation as a cecal mass is extremely uncommon. CASE PRESENTATION: A 15-year-old boy presented with abdominal pain and a non-tender right iliac fossa mass. Hematological and biochemical tests

Isolated jejunal amyloidosis presenting as acute mechanical small bowel obstruction: a rare diagnostic challenge - a case report.

INTRODUCTION: Gastrointestinal (GI) involvement in amyloidosis is uncommon and usually occurs as part of systemic disease. Localized jejunal amyloidosis is exceptionally rare and may mimic neoplastic or inflammatory disorders because of its nonspecific presentation. CASE PRESENTATION: A 77-year-old man with no significant medical history presented with acute abdominal pain, distension, and bilious vomiting. Contrast-enhanced CT demonstrated jejunal obstruction associated with circumferential wall thickening and multiple intraluminal polypoid lesions. Enteric magnetic resonance imaging confirm

Bouveret Syndrome Mimicking Gallbladder Malignancy: A Diagnostic Challenge in an 85-Year-Old Woman.

Bouveret syndrome is a rare complication of cholelithiasis in which a gallstone enters the duodenum via a cholecystoenteric fistula, causing gastric outlet obstruction. Because of chronic inflammation and nonspecific symptoms, the condition can easily be misdiagnosed as a gastrointestinal malignancy. An 85-year-old woman presented with severe weight loss (10 kg), night sweats, and non-bloody vomiting. CT imaging revealed massive aerobilia, a thickened gallbladder wall, and an ill-defined mass at the gastroduodenal junction. These findings raised strong suspicion for a malignant process in the

Case Report: Recurrent peptic ulcer perforations-a possible role of median arcuate ligament syndrome.

INTRODUCTION: Median arcuate ligament syndrome (MALS), also known as Dunbar syndrome, is a rare vascular compression disorder caused by extrinsic compression of the celiac artery by the median arcuate ligament, affecting 2/100,000 people. Clinical presentation is often non-specific and may include postprandial abdominal pain, nausea, vomiting, diarrhea, and weight loss. Impaired splanchnic perfusion may contribute to gastrointestinal ischemia and, in rare cases, recurrent peptic ulcer disease. We report a patient with multiple episodes of perforated peptic ulcer disease associated with previou

EORTC QLQ-C30 symptom components and their functional correlates during adjuvant chemotherapy for early breast cancer: a multivariate analysis. Data from GEICAM/2003-02 study.

PURPOSE: Understanding how multiple symptoms jointly impact functioning is essential for interpreting patient-reported outcomes (PROs) in oncology. We aimed to characterize the main dimensions underlying change-from-baseline (CFB) in symptom scores during adjuvant chemotherapy for early breast cancer, and to examine their associations with functional outcomes. PATIENTS AND METHODS: We analyzed EORTC QLQ-C30 CFB scores from 820 patients enrolled in the GEICAM/2003-02 trial. Principal component analysis (PCA) was used to summarize correlated symptom changes into a smaller number of components,

Jejunal Leiomyoma Arising in a Diverticulum Mimicking a Hollow Viscus Perforation: A Diagnostic Challenge.

Small intestinal leiomyomas are rare benign smooth muscle tumors that are now infrequently encountered following the reclassification of many mesenchymal tumors as gastrointestinal stromal tumors (GISTs). They are usually asymptomatic or present with nonspecific symptoms such as abdominal pain or gastrointestinal bleeding. Acute presentation with perforation peritonitis is exceedingly rare and poses a diagnostic challenge. We report the case of a 50-year-old female who presented with fever, vomiting, and acute abdominal pain with features of localized peritonitis. Contrast-enhanced computed to

Duodenum inversum presenting with intestinal obstruction in a 40-day-old infant: a case report.

BACKGROUND: Duodenum inversum is a rare congenital anomaly characterized by an abnormal course of the duodenum with preservation of a normally positioned duodenojejunal junction. Clinical presentation is highly variable, ranging from incidental findings to symptoms of upper gastrointestinal obstruction. Due to its rarity, diagnosis and management remain challenging, particularly in pediatric patients. CASE PRESENTATION: We report the case of a 40-day-old female infant presenting with recurrent postprandial vomiting and poor weight gain since the first days of life. Initial clinical, metabolic

EXPRESS: Transcutaneous Electrical Acupoint Stimulation for Postoperative Gastrointestinal Recovery: A Systematic Review and Meta-Analysis Using a Composite Stimulation-Pattern Framework.

BACKGROUND & AIMS: Transcutaneous electrical acupoint stimulation (TEAS) promotes postoperative gastrointestinal recovery. However, existing protocols lack standardization due to unaddressed methodological collinearity between frequency and waveform in previous analyses. We aimed to evaluate the efficacy of TEAS following abdominal surgery and examine whether treatment effects varied across stimulation parameters. METHODS: We searched four major databases from inception to April 2026 for randomized controlled trials (RCTs) comparing TEAS with sham/standard care. Frequency and waveform were sy

Cecal cyst as a lead point for intussusception in children: a case report.

Enteric duplication cysts are rare congenital malformations that can occur anywhere along the gastrointestinal tract and, although usually asymptomatic, may occasionally act as a pathological lead point for intussusception. We report a case of a cecal duplication cyst presenting as the lead point for ileocecal intussusception in a previously healthy 4-month-old female infant, who presented with acute-onset recurrent vomiting. Abdominal examination revealed a firm, palpable 5-cm mass in the epigastric region, and imaging confirmed intussusception at the ileocecal region with an associated cysti

Muir-Torre syndrome in a patient with advanced duodenal carcinoma.

Muir-Torre syndrome (MTS) is a rare autosomal dominant disorder characterized by sebaceous neoplasms and visceral malignancies and is regarded as a phenotypic variant of Lynch syndrome. Although colorectal and genitourinary cancers are the most common visceral malignancies in MTS, duodenal cancer is extremely rare. We report a case of advanced duodenal cancer associated with MTS treated with multidisciplinary therapy. A 62-year-old man with a history of multiple colorectal cancers and sebaceous carcinomas presented with vomiting and epigastric pain. Upper gastrointestinal endoscopy revealed a

Incision and plasty of the descending duodenal diverticulum using an ESD technique in a child with acute pancreatitis: a case report.

A 10-year-old boy presented with recurrent abdominal pain and vomiting for over a 20-month period. He had previously been diagnosed with recurrent acute pancreatitis at local hospitals and treated conservatively, but the symptoms continued to recur. Upon admission, a comprehensive diagnostic evaluation was undertaken, which ruled out the majority of established causes of pediatric pancreatitis. It was postulated that the diverticulum itself may have led to intermittent duodenal obstruction and contributed to the recurrent episodes of pancreatitis. The patient subsequently underwent incision an

Cholecystocolonic Fistula With Acute Cholangitis: A Case Report.

Cholecystocolonic fistula (CCF) is a rare type of cholecystoenteric fistula, which are sometimes diagnosed incidentally in patients undergoing cholecystectomy. They are usually a complication of chronic cholecystitis, often secondary to gallstones, and they are difficult to diagnose due to their rarity and vague clinical presentation. If untreated, they can lead to complications such as acute cholangitis, biliary peritonitis, and biliary cirrhosis. This highlights the importance of early recognition of CCF and timely treatment. While the generally accepted management is surgical, there are no

Outcomes Following Gastric Bypass Reversal for Refractory Complications: A UK Tertiary Centre Retrospective Case Series.

BACKGROUND: Gastric bypass is an effective and safe metabolic bariatric surgery, yet patients develop refractory long-term complications, post-bypass problems that persisted despite appropriate medical, endoscopic and/or surgical treatment, and that resulted in ongoing symptom burden and nutritional compromise, prompting consideration of reversal. Outcomes following reversal remain poorly characterised. The aim of this study is to describe indications, operative techniques and short- to medium-term nutritional and symptomatic outcomes following gastric bypass reversal in a UK tertiary bariatri

[Day-case management of minimally invasive liver resection: a path construction from concept to reality].

With the convergence of minimally invasive surgery and enhanced recovery after surgery, day-case surgery minimally invasive liver resection (DCS-MILR) has emerged as a potential care model. Available evidence, derived predominantly from single-center retrospective case series and supplemented by database analyses, suggests that DCS-MILR may be feasible and achieve acceptable short-term safety outcomes in carefully selected patients when supported by experienced surgical teams and standardized perioperative care. It may also shorten hospital stay and provide potential health-economic benefits.
