Comparing the diagnostic performance of 19-gauge and 22-gauge needles in endoscopic ultrasound-guided liver biopsy: a systematic review and meta-analysis.

BACKGROUND: Liver biopsy is the gold standard for histologic diagnosis of parenchymal liver disease. Endoscopic ultrasound-guided liver biopsy (EUS-LB) is increasingly being performed for tissue acquisition. However, the needle size to perform the procedure is still debated. We performed a systematic review and meta-analysis comparing outcomes and safety of two different needles, 19 gauge (G) and 22 G, for EUS-LB. METHODS: A comprehensive search of multiple databases was conducted to identify studies comparing 19 G and 22 G needles for liver biopsy. Studies reporting tissue adequacy, histolog

Intraluminal Migration of Surgical Mesh With Retained Metallic Tacks Causing Small Bowel Obstruction After Ventral Hernia Repair: A Case Report.

Mesh repair is commonly used for abdominal wall hernias. However, mesh-related complications, including infection, adhesion formation, and migration, remain important considerations. Mesh migration into the gastrointestinal tract represents an uncommon delayed postoperative complication that can manifest long after the initial surgical repair. Patients may experience a range of gastrointestinal symptoms depending on the location and extent of migration. We report a case of small bowel obstruction caused by intraluminal migration of surgical mesh with retained metallic fixation tacks. A 60-year

A multicenter parallel-group double-blind randomized placebo-controlled phase III clinical trial of generic aprepitant QLG2174 for the prevention of postoperative nausea and vomiting.

BACKGROUND: Postoperative nausea and vomiting (PONV), a common gastrointestinal disorder following surgery, primarily occurs within 6-24 hours postoperatively, with an incidence rate of approximately 30% in general surgical patients. QLG2174 (generic aprepitant) is a neurokinin-1 (NK-1) receptor antagonist exerting antiemetic effects by blocking the binding of substance P to NK-1 receptors. This study aimed to evaluate the efficacy and safety of QLG2174 for preventing PONV in Chinese patients. MATERIALS AND METHODS: This was a multi-center, parallel-group, double-blind, randomized, placebo-co

Acute Postoperative Intestinal Obstruction Due to Abdominal Drains: A Case Report.

Early small bowel obstruction caused by a drain in the postoperative period is a rare complication, with few cases reported in the literature. Its diagnosis is difficult, and its complications can be severe. The objective of this case report is to describe the clinical and radiological features suggestive of intestinal occlusion associated with abdominal drains and to summarize the management strategies reported in the literature for this rare complication. We report a case of acute intestinal obstruction caused by an abdominal drain after rectal surgery, documented in the oncology surgery dep

[Endoscopic totally extraperitoneal (eTEP) versus intraperitoneal onlay mesh (IPOM) repair for ventral hernias: a systematic review and meta-analysis].

OBJECTIVE: To compare safety and efficacy of eTEP and IPOM in endoscopic treatment of ventral hernias. MATERIAL AND METHODS: A systematic review and meta-analysis of RCTs for 2019-2025 in PubMed, EMBASE, Cochrane Library, Web of Science, eLibrary.Ru was performed. Fixed-effect (dichotomous outcomes) and random-effect (continuous outcomes) models were used. RESULTS: Twelve RCTs (694 patients: 348 eTEP, 346 IPOM) were included. Surgery time was shorter in IPOM (89.8±28.5 vs. 134.9±39.4 min; p<0.001). The risk of bowel injury was 7.73 times higher [1.73; 34.61], and risk of postoperative intest

Early Postoperative Intestinal Obstruction from Retained Myoma Fragment After Laparoscopic Myomectomy- A Case Report.

Laparoscopic myomectomy is increasingly used for uterine fibroid excision in women needing fertility preservation. However, as complex laparoscopic procedures become common and widespread, new and novel complications are being reported. Retained fibroid pieces are an uncommon but possibly dangerous consequence. We describe a case of laparoscopic myomectomy involving multiple fibroids of varying sizes which were enucleated and electrosurgically cut into smaller pieces. This case is unique because a residual fibroid fragment got buried in the mesentery causing early postoperative intestinal obst

One Patient, Two Problems: Applying Short Bowel Syndrome Management Principles in Inflammatory Bowel Disease patients.

PURPOSE OF REVIEW: Surgery for inflammatory bowel disease (IBD) often results in alterations in gut function beyond the anatomical changes. While some patients develop short bowel syndrome (SBS), a significant proportion of IBD patients benefit from pharmacologic and dietary interventions for SBS even in the absence of true SBS or IF. RECENT FINDINGS: While no formal criteria exist to clearly define the presence of SBS in IBD patients, the presence of specific features in the surgical and clinical history are crucial to the rapid identification of SBS or IF in IBD patients. Anatomy specific d

Clinical presentation and outcomes of revisional Roux-en-Y gastric bypass after failed gastric banding: direct versus sleeve-assisted approach.

BACKGROUND: Following failed adjustable gastric banding (AGB), patients may undergo direct conversion to Roux-en-Y gastric bypass (RYGB) or progress through an intermediate sleeve gastrectomy (SG) prior to revisional bypass. The impact of these differing revisional pathways on clinical presentation and postoperative outcomes remains unclear. METHODS: A retrospective cohort study was performed of adult patients undergoing revisional RYGB after prior AGB between 2013 and 2025 at a single academic center. Patients were categorized into direct AGB-RYGB and two-step AGB-SG-RYGB conversion groups.

Idiopathic pneumatosis cystoides intestinalis of the small bowel with perforation and congenital duodenal membranous stenosis and malrotation of intestine. A case report.

INTRODUCTION: Pneumatosis cystoides intestinalis (PCI) in children is a disorder characterized by gas-filled cystic lesions located in the submucosa and subserosa of any segment of the gastrointestinal tract, with pneumoperitoneum occurring in partial cases. This disease is well recognized as an early imaging sign of necrotizing enterocolitis secondary to intestinal ischemia in preterm neonates, yet PCI is rare in older children. Unlike pneumatosis intestinalis associated with life-threatening conditions such as neonatal necrotizing enterocolitis and mesenteric ischemia, PCI is generally regar

A four-year analysis of the predictors of overnight stay in day-case laparoscopic cholecystectomy and the prevention strategies within two centers in the UK: a double-centric retrospective observational study.

BACKGROUND: Laparoscopic cholecystectomy (LC) is the mainstay treatment of benign gallbladder disease, most commonly gallstones. Although the recent guidelines of British Association of Day-Surgery (BADS) and World Society of Emergency Surgery (WSES) recommend same-day discharge (i.e. day-case surgery), there is still a considerable proportion of cases that get admitted unexpectedly overnight. The primary aim of this study is to determine the predictors of overnight stay and consequently develop prevention strategies. MATERIALS AND METHODS: This was a retrospective study involving 913 patient

Application of robotic-assisted hepatopancreatoduodenectomy in non-jaundiced gallbladder cancer patient with invasion of the common bile duct: A case report.

RATIONALE: Hepatopancreatoduodenectomy (HPD) is a surgical procedure utilized for the curative-intent resection of biliary tract malignancies. However, there is a paucity of literature documenting the application of robotic-assisted HPD in case of gallbladder cancer (GBC) with invasion into the common bile duct (CBD) in patients who had no jaundice. PATIENT CONCERNS: One week before admission, a 58-year-old female suffered from paroxysmal diarrhea and vomiting without any apparent cause. DIAGNOSES: Preoperative computed tomography (CT) and magnetic resonance imaging revealed GBC with dilatio

Early-onset extrahepatic cholangiocarcinoma in a young adult with a resected congenital biliary cyst: a rare case report.

INTRODUCTION: Congenital choledochal cysts are well-recognized risk factors for subsequent hepatobiliary malignancy, particularly cholangiocarcinoma, even after surgical excision. However, the development of extrahepatic cholangiocarcinoma (eCCA) in early adulthood following neonatal resection of a congenital biliary cyst is exceptionally rare. We report a case of early-onset eCCA in a young woman who was previously treated surgically in the neonatal period. CASE PRESENTATION: A 25-year-old woman with a history of neonatal resection of a congenital biliary cyst, cholecystectomy, and biliary-e

Case report of pyloric obstruction caused by giant gallbladder compression with literature review.

Giant gallbladder is a rare clinical condition characterized by abnormal gallbladder enlargement, typically defined as a longitudinal diameter exceeding 14 cm or a volume surpassing 1.5 L. The most common pathological etiologies are cholelithiasis and chronic cholecystitis, followed by neoplastic factors, while congenital developmental anomalies and other causes are relatively uncommon. Herein, we report a case of giant gallbladder-induced pyloric obstruction in a patient who presented with nausea and vomiting for two weeks-symptoms that recurred after initial resolution of diabetic ketoacidos

Superior mesenteric artery syndrome in a 26-year-old male presenting as acute pancreatitis and portal venous gas: a case report and review of the literature.

BACKGROUND: Superior mesenteric artery syndrome is a rare form of duodenal obstruction that usually requires nonoperative management. While individual complications such as acute pancreatitis, portal venous gas, and gastric emphysema have been reported, the simultaneous occurrence of this triad in a single patient with superior mesenteric artery syndrome has never been documented. This case is novel as it highlights the successful nonoperative management of superior mesenteric artery syndrome despite the presence of these severe, potentially life-threatening complications. CASE PRESENTATION:

Radiological Imaging in Gastroparesis: A Narrative Literature Review of Diagnostic and Evaluation Techniques.

Gastroparesis is a gastrointestinal disorder characterised by delayed gastric emptying without mechanical obstruction, leading to symptoms such as nausea, vomiting, bloating, and epigastric discomfort. Accurate diagnosis and evaluation of this condition are critical, as untreated gastroparesis can result in complications including malnutrition, electrolyte imbalances, and mucosal injury. Radiological imaging plays a central role in both diagnosing and assessing the severity of gastroparesis. This narrative review provides an overview of current imaging modalities, including gastric emptying sc

Spontaneous Right-Sided Diaphragmatic Rupture With Liver Herniation After Bilateral Lung Transplantation.

Spontaneous diaphragmatic rupture is a rare but life-threatening complication. We report the case of a 53-year-old female individual who developed acute right-sided diaphragmatic rupture with liver, gallbladder, and transverse colon herniation 6 days after bilateral lung transplantation for end-stage chronic obstructive pulmonary disease. The event followed forceful vomiting and caused cardiac tamponade due to compression. Emergency surgical repair required a combined thoracic and abdominal approach with mesh reconstruction of the diaphragm. Prompt recognition, careful perioperative positionin
