Gastric coconut bezoar following Roux-en-Y gastric bypass.

Coconut bezoars are rare causes of gastrointestinal obstruction, particularly in patients who have undergone Roux-en-Y gastric bypass (RYGB). Altered gastric motility and anatomical changes after bariatric surgery predispose patients to the accumulation of indigestible fibers. We report the case of a 45-year-old female who presented one year after RYGB with nausea, vomiting, and abdominal pain. Upper endoscopy revealed a coconut bezoar in the gastric pouch, which was successfully treated with endoscopic mechanical fragmentation and extraction. The patient recovered fully without surgical inter

Preduodenal Portal Vein With Midgut Non-rotation Causing Duodenal Obstruction in a Neonate: A Case Report.

Preduodenal portal vein (PDPV) is a rare congenital vascular anomaly that may cause duodenal obstruction, either in isolation or in association with other gastrointestinal malformations such as intestinal malrotation. We report a neonate who presented on the seventh day of life with bilious vomiting. Imaging studies suggested proximal duodenal obstruction due to extrinsic compression, with findings consistent with a preduodenal portal vein and associated midgut non-rotation. The diagnosis was confirmed intraoperatively, and the infant was successfully managed with duodenoduodenostomy and Ladd'

Beyond the Scope: Unmasking Appendicitis Following Colonoscopy.

Colonoscopy is a key diagnostic and therapeutic procedure in gastrointestinal practice, although rare complications such as post-colonoscopy appendicitis (PCA) may occur. We report the case of a 61-year-old woman who presented 24 hours after a screening colonoscopy with severe diffuse abdominal pain, fever, nausea, vomiting, and watery diarrhea. The procedure had been complicated by poor bowel preparation, with a Boston Bowel Preparation Score of 1-2-1 (right colon, transverse colon, and left colon, respectively; scores range from 0 to 3 per segment, with 0 indicating an unprepared colon and 3

Effect of Chewing Gum on Postoperative Gastrointestinal Recovery in Patients Undergoing Abdominal Surgery: A Randomized Controlled Study.

Background Postoperative ileus is a common complication following abdominal surgeries, leading to delayed recovery, prolonged hospital stay, and increased patient discomfort. Chewing gum, a form of sham feeding, has been proposed as a simple, non-pharmacological intervention to stimulate gastrointestinal motility and hasten postoperative recovery. This study aimed to evaluate the effect of chewing gum on postoperative recovery of gastrointestinal function in patients undergoing abdominal surgery. Methods This single-blinded randomized controlled study included 198 adult patients undergoing ele

Primary Enterolithiasis Associated With Severe Hypothyroidism and Ileal Stricture Presenting As Small Bowel Obstruction: A Case Report.

Enterolithiasis is an uncommon condition characterized by the formation of stones within the gastrointestinal tract and may rarely present as small bowel obstruction, typically developing in areas of intestinal stasis such as strictures, diverticula, or blind loops. Severe hypothyroidism can contribute to intestinal dysmotility and predispose to such stasis. We report a case of a 76-year-old woman who presented with abdominal pain, constipation, vomiting, and inability to pass stool or flatus for four days. Imaging demonstrated dilated small bowel loops with an intraluminal radio-opaque lesion

Shoulder-Tip Pain After Laparoscopic Surgery: A Narrative Review of Intraperitoneal Anaesthesia.

BACKGROUND AND AIMS: Shoulder-tip pain is a common and distressing side effect following laparoscopic surgery. Standard systemic analgesic regimens are used to manage postoperative pain but are often associated with side effects such as nausea and vomiting. There is some evidence to support a reduction in insufflation pressure to lessen shoulder-tip pain. This narrative review examined the efficacy of intraperitoneal anaesthesia on shoulder-tip and overall pain after laparoscopic surgery. METHODS: Relevant literature was identified through database searches containing terms related to intrape

Laparoscopic Reversal of One-Anastomosis Gastric Bypass After Sleeve Gastrectomy: A Case Report.

BACKGROUND One-anastomosis gastric bypass (OAGB) is an effective bariatric surgery; however, there may be serious instances of metabolic and nutritional complications that may require complete reversal. Reversal of complete OAGB to near-normal physiology, especially with a background of sleeve gastrectomy, is extremely rare. This case report discusses the complications associated with reversal of OAGB. CASE REPORT Our patient was a 28-year-old woman who had a history of a sleeve gastrectomy procedure and later underwent a revisional OAGB procedure for poor weight loss/weight regain. She later

A Rare Presentation of Cecal Adenocarcinoma Causing Adult Ileocolic Intussusception: A Case Report.

Intussusception occurs when a segment of the bowel telescopes into an adjacent segment, potentially leading to obstruction and intestinal ischemia. Intussusception in adults is likely caused by a pathological lead point (PLP), and it can be challenging to diagnose due to its broad clinical presentation, which requires a high degree of clinical suspicion. This case report describes a rare presentation of cecal adenocarcinoma in a young patient that caused ileocolic intussusception. A 31-year-old woman presented to the emergency department with progressively worsening intermittent abdominal pain

Recurrent Small Bowel Obstruction in an Adult Male Patient Due to Complicated Meckel's Diverticulum: A Case Report.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract, although it remains asymptomatic in most cases and is rarely diagnosed in adulthood. When symptomatic, it may present with complications such as intestinal obstruction, which can pose a diagnostic challenge due to its nonspecific clinical features. We report the case of a 31-year-old man with a history of multiple prior episodes of small bowel obstruction managed conservatively without an established etiology, who presented with a five-day history of progressive abdominal pain, abdominal distension, non-

Patient-reported outcomes and measures applied in individuals undergoing colonoscopy surveillance for colorectal cancer: a scoping review.

PURPOSE: Clinical guidelines recommend regular colonoscopy surveillance for individuals at elevated risk for colorectal cancer (CRC). While colonoscopy surveillance is proven to reduce the incidence of CRC, colonoscopy is an invasive procedure that can impact patient-reported outcomes (PROs). Assessment of PROs is recommended as a key indicator of the quality of health service delivery. However, there is no standard set of PROs and PRO measures (PROMs) to be applied in individuals undergoing regular surveillance colonoscopy. The aim of this scoping review was to identify PROs and PROMs applied
