Synchronous ileo-ileal and ileocolic intussusception caused by an inverted giant Meckel's diverticulum in an adult: a rare case report.

INTRODUCTION: Adult intussusception is a rare clinical entity, typically associated with a pathological lead point. Meckel's diverticulum, although the most common congenital gastrointestinal anomaly, seldom presents with complications in adulthood. Synchronous intussusception involving multiple bowel segments is exceptionally uncommon. PRESENTATION OF CASE: A 21-year-old man presented with a 2-week history of intermittent colicky abdominal pain, abdominal distension, bilious vomiting, and constipation. Imaging revealed features of both ileo-ileal and ileocolic intussusception with small bowe

Optimising Non-operative Management of Inoperable Malignant Bowel Obstruction: A Clinical Review for Surgical Practice.

Inoperable malignant bowel obstruction (MBO) is a frequent and distressing complication of advanced intra-abdominal malignancy, particularly colorectal and ovarian cancer, and is associated with substantial morbidity and limited survival. Management is primarily palliative, with emphasis on symptom control rather than disease modification. For the practising surgeon, MBO represents a common clinical scenario requiring structured non-operative decision-making. This structured narrative review synthesises current evidence on non-operative management strategies for inoperable MBO, based on a syst

Incidentally Identified Pneumatosis Cystoides Intestinalis in Patients With Gastric Outlet Obstruction From Addis Ababa, Ethiopia: A Case Report.

Pneumatosis cystoides intestinalis (PCI) is a rare condition characterized by the presence of multiple gas-filled cysts within the submucosal or subserosal layers of the intestinal wall. Although often asymptomatic and detected incidentally, PCI may be associated with significant underlying gastrointestinal or systemic pathology. We report two young adults from Addis Ababa, Ethiopia, who presented with long-standing upper gastrointestinal symptoms, including early satiety, postprandial epigastric pain, reflux, vomiting, and marked weight loss. Clinical, laboratory, and endoscopic evaluations c

A Case of Laparoscopic Distal Gastrectomy for Early Gastric Cancer Associated With an Upside-Down Stomach: Sac-First Dissection Before Lymphadenectomy.

Gastric cancer associated with an upside-down stomach, an advanced form of paraesophageal hiatal hernia, is rare, and laparoscopic surgery is technically demanding because of marked anatomical distortion. We report an 88-year-old woman with marked kyphosis who presented with vomiting and was diagnosed with early gastric cancer on the lesser curvature of the gastric angle. Computed tomography revealed a giant hiatal hernia with an upside-down stomach. Laparoscopic distal gastrectomy with D1 plus lymphadenectomy and concomitant hiatal hernia repair were performed. The herniated stomach was first

Laparoscopic cholecystectomy in the paediatric population: A quality-of-life study.

INTRODUCTION: Cholelithiasis is uncommon in childhood but is more likely to present with symptoms that adversely affect the quality of life (QoL). This study assessed patient-reported outcomes after laparoscopic cholecystectomy (LC) using a paediatric-specific gastrointestinal (GI) health-related quality-of-life tool at a tertiary care centre in northern India. PATIENTS AND METHODS: A retrospective study of a cohort of children aged <12 years who underwent LC between 2016 and 2022 was performed. The PedsQL™ GI module questionnaire was administered to the patients or their parents, and the cha

Intrathoracic Pouch Migration after Gastric Bypass: A Systematic Review.

BACKGROUND: Intrathoracic pouch migration (ITM) is an uncommon but increasingly recognized complication after Roux-en-Y (RYGB) or one-anastomosis gastric bypass (OAGB), often associated with hiatal hernia. This systematic review summarizes incidence, clinical presentation, diagnostic modalities, management strategies, and risk factors for recurrence. METHODS: This systematic review followed PRISMA guidelines and was registered on PROSPERO. A comprehensive search of PubMed, Embase, Scopus, and Web of Science through November 2025 identified full-text studies on ITM after RYGB or OAGB. Eligible

Gastric slow-wave morphology in health and disease.

PURPOSE: Underlying bioelectrical slow waves are critical for regulating gastric motility, and abnormal spatiotemporal slow-wave dysrhythmias are associated with a range of gastrointestinal disorders. However, the definition and role of the morphology of gastric slow-wave signals have remained limited. This study aimed to define gastric slow-wave morphology in cases of health and disease. METHODS: Data were repurposed from a study where, following ethical approval, a control cohort ( n = 9 ) and a pathological cohort of patients with chronic unexplained nausea and vomiting (CUNV; n = 8 ) un

Microcystic lymphatic malformation of the small intestine presenting as acute abdominal pain in a 5-year-old boy: a case report and review of the literature.

BACKGROUND: Lymphatic malformations are rare benign congenital anomalies of the lymphatic system, most commonly affecting the head and neck. Gastrointestinal involvement is uncommon, and lymphatic malformations of the small intestine are particularly rare. Microcystic lymphatic malformations infiltrating the bowel wall may present with nonspecific clinical and radiological features, making preoperative diagnosis challenging. Reporting such cases is important to raise awareness of this rare entity as a potential cause of acute abdominal pain in children. CASE PRESENTATION: A 5-year-old Vietnam

Warm staple-line irrigation (37-40 °C) before omentopexy reduces early bleeding after laparoscopic sleeve gastrectomy: a prospective, randomized trial.

BACKGROUND: Early bleeding remains a common concern after laparoscopic sleeve gastrectomy (LSG), despite improvements in stapling technology and reinforcement techniques. This study examined whether using warm saline to irrigate the staple line before performing omentopexy could enhance intra-operative hemostasis and reduce early postoperative bleeding. METHODS: In this prospective randomized trial conducted between December 2024 and June 2025, 200 adults undergoing primary LSG were allocated equally to warm saline irrigation (37-40 °C) or conventional room-temperature irrigation (22-24 °C),

Robotic management of superior mesenteric artery syndrome after weight loss: a case report.

Superior mesenteric artery (SMA) syndrome is a rare cause of proximal intestinal obstruction resulting from compression of the third portion of the duodenum between the aorta and the SMA. It most commonly occurs in slender individuals or those experiencing rapid weight loss, but presentations can be atypical and diagnostically challenging. We report the case of a 28-year-old female with history of postural orthostatic tachycardia syndrome and gastroparesis who presented with recurrent nausea and intractable vomiting following a 110-lb weight loss after prior obesity. Upper gastrointestinal end

Comparing Efficacy of Oxycodone versus Sufentanil for Postoperative Recovery in Patients Receiving Bilateral Modified Thoracoabdominal Nerves Block Through Perichondrial Approach (M-TAPA) Following Laparoscopic Radical Distal Gastrectomy: A Study Protocol for a Prospective, Blinded, Randomized Controlled Trial.

INTRODUCTION: While laparoscopic radical distal gastrectomy (LRDG) has been widely adopted for its minimally invasive benefits in the treatment of gastric cancer, acute postoperative pain, comprising both somatic incisional pain and visceral discomfort, remains a clinical challenge that can affect patients' perioperative comfort and the rehabilitation process. Consequently, optimizing postoperative analgesic strategies remains essential for enhanced recovery. The modified thoracoabdominal nerves block through a perichondrial approach (M-TAPA) has been introduced as an emerging regional analges

Comparison of oliceridine and sufentanil in postoperative pain management for postoperative nausea and vomiting after bimaxillary orthognathic surgery: an exploratory randomized controlled trial.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a prevalent adverse event following bimaxillary orthognathic surgery, with opioids being a significant contributing factor. Oliceridine, a G protein-biased μ-opioid receptor agonist, reduces activation of the β-arrestin pathway and may decrease gastrointestinal adverse events relative to traditional opioids. This study aimed to evaluate the efficacy and safety of oliceridine relative to sufentanil for postoperative patient-controlled intravenous analgesia (PCIA) in this population, with a particular emphasis on PONV. METHODS: This prospe

Cross-Modal Assessment of Post-Cholecystectomy Symptoms: Integrating MRCP Metrics with Upper Endoscopy.

BACKGROUND/OBJECTIVES: Post-cholecystectomy syndrome (PCS) remains diagnostically challenging due to overlapping biliary and non-biliary causes. This study aimed to evaluate whether common bile duct (CBD) diameter measured by MRCP can serve as a practical triage parameter in symptomatic PCS patients and to define a data-supported threshold for predicting clinically relevant biliary pathology. Secondary objectives included assessing correlations between MRCP findings and upper endoscopic features. METHODS: In this retrospective single-center study, symptomatic adults undergoing upper endoscopy

Titanium Allergy: A Case of Foreign Body Reaction Following Laparoscopic Cholecystectomy.

Titanium is widely regarded as a biocompatible and inert material commonly used in surgical implants. Despite this reputation, hypersensitivity reactions to titanium, though rare, can lead to persistent and unexplained postoperative symptoms. We report the case of a 76-year-old woman who developed progressive right upper quadrant abdominal pain, burning sensations, neuropathic symptoms, and fatigue three months after laparoscopic cholecystectomy in which titanium clips were used. Her history included documented systemic intolerance to a titanium dental implant 10 years earlier, which had requi
