Acute mesenteric ischemia complicated by short bowel syndrome: a rare case report from Syria.

BACKGROUND: Acute mesenteric ischemia (AMI) is a life-threatening pathological condition characterized by obstruction of blood flow to the intestine, it is rare and usually presents with non-specific gastrointestinal symptoms. Embolic, thrombotic and non-obstructive pathology are the main causes of this disease. Treatment varies from case to case depending on clinical and imaging findings. Open surgery is often required in advanced cases. CASE PRESENTATION: A 79-year-old female presented with sudden-onset severe generalized abdominal pain associated with nausea, vomiting, and poor oral intake

Clinical characteristics and therapeutic experiences of magnetic foreign body ingestion in children: a single-center retrospective study.

BACKGROUND: The ingestion of multiple magnetic foreign bodies (MMFBs) poses a growing risk of severe gastrointestinal injury in children, yet optimal management strategies remain debated due to the lack of largescale studies and international consensus. The study aims to summarize the clinical features and treatment experiences associated with the ingestion of MMFB in children. METHODS: We retrospectively reviewed 160 pediatric patients hospitalized with MMFB ingestion at our institution from June 2016 to August 2025. Clinical data on demographics, symptoms, imaging, treatment modalities, and

The Effectiveness of Perioperative Intravenous Lidocaine for Postoperative Analgesia in Video-Assisted Thoracic Surgery: A Systematic Review and Meta-Analysis.

BACKGROUND: Acute pain after video-assisted thoracic surgery (VATS) remains a significant challenge. While intravenous lidocaine is increasingly used for analgesia, its efficacy for VATS remains uncertain. This meta-analysis aimed to evaluate the impact of perioperative intravenous lidocaine on postoperative pain in patients undergoing VATS. METHODS: We systematically searched PubMed, Web of Science, Embase, Cochrane Library, and Chinese databases (CNKI, WANFANG, SinoMed) from inception to December 31, 2025, for randomized controlled trials (RCTs) comparing intravenous lidocaine with placebo

Comparative analgesic outcomes of anterior quadratus lumborum block and transversus abdominis plane block in patients undergoing laparoscopic cholecystectomy: A prospective observational study.

Pain following laparoscopic cholecystectomy often necessitates substantial opioid use. This study aimed to compare postoperative analgesic outcomes among anterior quadratus lumborum block (QLB), transversus abdominis plane block (TAPB), and intravenous paracetamol within routine perioperative management of laparoscopic cholecystectomy. A total of 126 patients (42 per group: QLB, TAPB, IVA) undergoing elective laparoscopic cholecystectomy were evaluated. Postoperative outcomes recorded during the first 24 hours included Visual Analogue Scale (VAS) scores, rescue opioid consumption, Richmond Agi

Organ-Preserving Gastric Wedge Resection for a Giant Posterior Gastric Gastrointestinal Stromal Tumor: A Case Report.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract, with the stomach representing the predominant site of origin. Although complete surgical resection remains the standard treatment for localized disease, the management of giant gastric GISTs can be challenging because of their size, anatomical relationships, and the risk of tumor rupture during surgery. We report the successful organ-preserving resection of a giant posterior gastric GIST using a wedge resection technique. A 49-year-old woman presented with a two-year history of abdo

Functional fatty acid-based dietary therapy as a new strategy for small gallstone expulsion: study protocol for a prospective, single-center, double-blind, randomized controlled trial.

BACKGROUND: Gallstone disease is a major global health concern with a steadily increasing prevalence, affecting approximately 7% of the global population and imposing substantial health and economic burdens. Globally, gallstone disease accounts for 10-15% of all gastrointestinal hospital admissions, with annual direct medical costs exceeding $6 billion in the United States and ¥20 billion in China, primarily driven by cholecystectomy and post-surgical complications. According to clinical observations, oral administration of olive oil may facilitate the expulsion of small gallstones; however, h

Adult Annular Pancreas Presenting With Gastric Outlet Obstruction Following Semaglutide Initiation: A Case Report.

Annular pancreas is a rare congenital anomaly that may remain clinically silent until adulthood. Its symptoms can be masked or exacerbated by glucagon-like peptide-1 receptor agonists (GLP-1 RAs), which delay gastric emptying. We report a 55-year-old male with type 2 diabetes who developed progressive postprandial vomiting and nine-kg weight loss following semaglutide initiation. Imaging revealed a partial annular pancreas causing second-part duodenal stenosis without malignancy. Symptoms persisted despite semaglutide discontinuation, supporting a fixed mechanical obstruction as the primary ae

Early Presentation of Internal Hernia After Laparoscopic Roux-en-Y Gastric Bypass.

Internal hernia is a potentially serious complication of laparoscopic Roux-en-Y gastric bypass (LRYGB), usually presenting months to years after surgery. Early postoperative internal hernia that occurs within the first week is exceptionally rare and may be difficult to diagnose due to nonspecific symptoms and misleading initial imaging. We report a case of a 58-year-old female who developed acute epigastric pain and vomiting on the sixth postoperative day following revisional LRYGB with hiatal hernia repair. The initial upper gastrointestinal contrast study showed normal contrast passage. As t

Ga-68 FAPI-04 PET/CT detection of repeatedly biopsy-negative poorly differentiated gastric adenocarcinoma: a case report.

False-negative endoscopic biopsy may delay the diagnosis of infiltrative gastric cancer, particularly in poorly differentiated or signet-ring cell phenotypes. We report a 56-year-old man with progressive abdominal distension, nausea, vomiting, weight loss, and epigastric pain. Three endoscopic biopsies performed before and after transfer to our hospital showed chronic inflammation without definite malignancy. F-18 FDG PET/CT demonstrated slightly increased and indistinct uptake compared with the liver in the thickened gastric antrum wall, whereas subsequent Ga-68 FAPI-04 PET/CT showed intense

Gastroduodenal Perforation Secondary to Heterotopic Pancreas in a Patient With Systemic Amyloidosis and End-Stage Renal Disease on Hemodialysis: A Case Report.

Heterotopic pancreas (HP) is a rare congenital anomaly characterized by pancreatic tissue lacking anatomical and vascular continuity with the orthotopic pancreas. Although usually asymptomatic and incidentally diagnosed, HP may rarely become clinically significant due to inflammation, ulceration, bleeding, obstruction, or gastrointestinal perforation. Systemic conditions such as amyloidosis, chronic kidney disease (CKD), and chronic hemodialysis may further increase gastrointestinal vulnerability through vascular fragility, impaired mucosal healing, and recurrent splanchnic hypoperfusion. Laparoscopic resection of a small intestinal venous malformation presenting as acute bowel obstruction without hemorrhage in a child: a case report.

BACKGROUND: Small intestinal venous malformation is a rare vascular anomaly that most commonly presents with gastrointestinal bleeding. Its manifestation as acute bowel obstruction without hemorrhage is exceedingly uncommon, especially in children, posing a diagnostic dilemma. CASE DESCRIPTION: A previously healthy 9-year-old boy presented with acute abdominal pain and vomiting. Contrast-enhanced computed tomography revealed a homogeneously enhancing lesion with persistent enhancement on portal venous phase in the left pelvic small bowel, raising suspicion of a vascular malformation. Diagnost

Postoperative intestinal obstruction caused by staple-related internal hernia after laparoscopic appendectomy: a case report.

Postoperative intestinal obstruction is most commonly associated with adhesions but may also arise from unusual causes such as surgical staples adhering to adjacent structures. Although the use of endoscopic staplers in laparoscopic appendectomy is effective and generally safe, it can occasionally result in complications, including intestinal obstruction. We report the case of a 41-year-old woman who underwent surgical treatment for pelvic endometriosis, including appendectomy, and subsequently developed intestinal obstruction caused by an internal hernia formed by the entrapment of a surgical

A novel endoscopic approach for refractory gastroesophageal reflux disease after laparoscopic sleeve gastrectomy: endoscopic anti-reflux mucosectomy.

INTRODUCTION: Gastroesophageal reflux disease (GERD) is a relatively common complication following laparoscopic sleeve gastrectomy (LSG), with refractory GERD often necessitating revisional surgery. We report two cases of refractory GERD after LSG that were managed with endoscopic anti-reflux mucosectomy (ARMS). CASE PRESENTATION: Case 1: A 58-year-old woman with morbid obesity (BMI 42.2 kg/m2) complicated by hypertension, dyslipidemia, and sleep apnea syndrome underwent LSG. Postoperatively, vomiting symptoms developed approximately two weeks after LSG and worsened over the subsequent year.

The Gamboa-Hoil Point for Primary Perineal Closure During Total Pelvic Exenteration.

Introduction Total pelvic exenteration (TPE) is a radical procedure for selected patients with advanced pelvic malignancies and is associated with significant perineal morbidity. Despite multiple reconstructive strategies, no consensus exists regarding the optimal strategy for perineal closure. The Gamboa-Hoil point is a standardized primary closure technique proposed by the authors, designed to reinforce perineal wound closure using a simple suture-based approach. Methods We conducted a single-center, retrospective descriptive study including patients with colorectal cancer (rectal, sigmoid c
