Efficacy and safety of split-dose ultra-low-volume polyethylene glycol with ascorbic acid versus sodium picosulfate with magnesium citrate for bowel preparation: a systematic review and meta-analysis.

Colonoscopy remains the gold standard for early detection and prevention of colorectal cancer, with adequate bowel cleansing being pivotal for its success. This systematic review aims to evaluate the efficacy and safety of split-dose ultra-low-volume polyethylene glycol (PEG) with ascorbic acid (Asc) versus sodium picosulfate magnesium citrate for bowel preparation. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, we conducted a comprehensive literature search across Embase, PubMed, Web of Science, and Scopus from inception to July 2025. Transversus abdominis plane block versus port site anesthetic in adult laparoscopic appendectomy: a systematic review and meta-analysis.

BACKGROUND: Despite advances in minimally invasive surgery, pain after appendectomy remains common. The transversus abdominis plane (TAP) block has been proposed as an alternative to port site local anesthetic. We aimed to synthesize randomized controlled trials (RCTs) comparing the efficacy and safety of 1) TAP block versus port site local anesthetic, and 2) ultrasound to laparoscope-guided TAP blocks following adult minimally invasive appendectomy. METHODS: Relevant databases were searched for RCTs until December 2024. Two authors independently identified trials, extracted data, assessed ri

A randomized evaluation of early oral feeding after ambulatory non-gastrointestinal surgical patients under general anesthesia.

BACKGROUND: The optimal timing and safety of early oral feeding after non-gastrointestinal surgery under general anesthesia remain unclear, particularly in ambulatory settings. This study aims to evaluate the impact of an evidence-based early oral feeding strategy on postoperative adverse events, patient comfort, and gastrointestinal recovery in patients undergoing day case non-gastrointestinal surgery. METHODS: A total of 117 patients were allocated to an intervention group (n = 62) receiving an early oral feeding protocol based on Steward Recovery Score and Apfel PONV risk assessment, or a c

Appropriate Combination of Nalbuphine Combined With Esketamine for Weakly Opioid Anesthesia in Postlaparoscopic Cholecystectomy Analgesia: A Randomized Controlled Trial.

CONTEXT: Laparoscopic cholecystectomy (LC) often leads to abdominal wall incision pain, visceral pain, and shoulder and neck pain after surgery. Nalbuphine provides strong sedative and analgesic effects with minimal risk of adverse reactions. Esketamine, with its higher receptor affinity, significantly alleviates both visceral and somatic pain. OBJECTIVES: Our objective was to establish the optimal dosage of nalbuphine with esketamine for patient-controlled intravenous analgesia (PCIA) following LC, enhancing postoperative pain management for these patients. METHODS: This research employs a

Anastomotic-Transverse Colon Fistula Following Gastrectomy: A Case Report of Delayed Diagnosis 14 Years After Surgery.

BACKGROUND Gastrocolic fistula is a rare complication of partial gastrectomy that can present with weight loss, diarrhea, and feculent vomiting. Most cases documented in the literature have occurred following non-physiological anastomoses, such as Billroth II procedures. This report describes the case of a 60-year-old man with a gastrocolic fistula from the gastrectomy anastomosis to the transverse colon 14 years after a Billroth I (gastroduodenal) anastomosis. CASE REPORT A 60-year-old male patient presented with a 6-year history of recurrent diarrhea, which had worsened over the past 6 month

Recurrent Cyclic Vomiting and Gastroparesis-Like Symptoms in a Patient With Mast Cell Activation Syndrome (MCAS): A Case of Gastrointestinal-Predominant MCAS.

Mast cell activation syndrome (MCAS) remains an underrecognized cause of gastrointestinal morbidity, frequently presenting with symptoms indistinguishable from functional disorders. Such diagnostic ambiguity can misdirect management and prolong patient symptoms. Thus, we present a case of a 36-year-old male with MCAS who presented with acute, intractable nausea, vomiting, and left-sided back pain. Examination revealed tachycardia and tenderness in the left costovertebral angle. Laboratory evaluation revealed mild anemia, thrombocytopenia, and slightly elevated bilirubin levels. Abdominal and p

Health-Related Quality of Life Post Gastrectomy: A Quantitative and Qualitative Cross-Sectional Analysis.

INTRODUCTION: Approximately eight per 1000 patients develop gastric cancer in the US. Surgery serves as part of curative intent treatment, but subsequent changes to diet, digestion, and financial burden may impact lifestyle for many. Health-related quality of life (HRQoL) changes following gastric cancer surgery have not been fully explored. METHODS: A cross-sectional study design explored patients' HRQoL measures after undergoing total and subtotal gastrectomy. Participants were surveyed using European Organisation for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaires (

A 31-Year-Old Primigravida Woman Presenting at 18 Weeks of Gestation With Right Ovarian Torsion Successfully Managed With Laparoscopic Adnexectomy.

BACKGROUND Ovarian torsion is a gynecological emergency characterized by rotation of an ovary around the infundibulopelvic and utero-ovarian ligaments, which leads to impaired venous and lymphatic drainage and, if prolonged, arterial obstruction with potential ischemic necrosis. The clinical presentation is often nonspecific, typically including acute lower abdominal pain, nausea, and vomiting, which can overlap with other abdominal or obstetric conditions, particularly during pregnancy. Imaging modalities, such as ultrasound with Doppler flow assessment, play a supportive role but are not def

Management of Gastrointestinal Function in Patients After Laparoscopic Surgery for Benign Gynaecological Diseases: A Best Practice Implementation Project.

INTRODUCTION: Laparoscopy is widely used in benign gynaecological surgeries. However, postoperative gastrointestinal dysfunction, abdominal distension, abdominal pain, nausea, vomiting and delayed defecation are often negatively affected after surgery. At present, the management of gastrointestinal function in these patients is not standardised; therefore, this project used the available evidence to improve awareness and practice of standardised management of postoperative gastrointestinal function recovery in patients undergoing laparoscopic surgery for benign gynaecological diseases. Texture-optimized early enteral nutrition enhances gastrointestinal recovery and caloric intake after general surgery: A randomized controlled trial.

BACKGROUND & AIMS: Early oral feeding is recommended in enhanced recovery protocols; however, the impact of diet texture on postoperative gastrointestinal recovery remains insufficiently studied. We evaluated whether a standardized semi-solid, nutritionally complete postoperative diet improves gastrointestinal recovery and caloric delivery compared with conventional stepwise feeding after general surgery. METHODS: In this single-center, prospective, single-blind randomized controlled trial, 120 patients undergoing noncardiothoracic surgery under general anesthesia were assigned (1:1) to recei
