Laparoscopic cholecystectomy in the paediatric population: A quality-of-life study.

INTRODUCTION: Cholelithiasis is uncommon in childhood but is more likely to present with symptoms that adversely affect the quality of life (QoL). This study assessed patient-reported outcomes after laparoscopic cholecystectomy (LC) using a paediatric-specific gastrointestinal (GI) health-related quality-of-life tool at a tertiary care centre in northern India. PATIENTS AND METHODS: A retrospective study of a cohort of children aged <12 years who underwent LC between 2016 and 2022 was performed. The PedsQL™ GI module questionnaire was administered to the patients or their parents, and the cha

Intrathoracic Pouch Migration after Gastric Bypass: A Systematic Review.

BACKGROUND: Intrathoracic pouch migration (ITM) is an uncommon but increasingly recognized complication after Roux-en-Y (RYGB) or one-anastomosis gastric bypass (OAGB), often associated with hiatal hernia. This systematic review summarizes incidence, clinical presentation, diagnostic modalities, management strategies, and risk factors for recurrence. METHODS: This systematic review followed PRISMA guidelines and was registered on PROSPERO. A comprehensive search of PubMed, Embase, Scopus, and Web of Science through November 2025 identified full-text studies on ITM after RYGB or OAGB. Eligible

Gastric slow-wave morphology in health and disease.

PURPOSE: Underlying bioelectrical slow waves are critical for regulating gastric motility, and abnormal spatiotemporal slow-wave dysrhythmias are associated with a range of gastrointestinal disorders. However, the definition and role of the morphology of gastric slow-wave signals have remained limited. This study aimed to define gastric slow-wave morphology in cases of health and disease. METHODS: Data were repurposed from a study where, following ethical approval, a control cohort ( n = 9 ) and a pathological cohort of patients with chronic unexplained nausea and vomiting (CUNV; n = 8 ) un

Microcystic lymphatic malformation of the small intestine presenting as acute abdominal pain in a 5-year-old boy: a case report and review of the literature.

BACKGROUND: Lymphatic malformations are rare benign congenital anomalies of the lymphatic system, most commonly affecting the head and neck. Gastrointestinal involvement is uncommon, and lymphatic malformations of the small intestine are particularly rare. Microcystic lymphatic malformations infiltrating the bowel wall may present with nonspecific clinical and radiological features, making preoperative diagnosis challenging. Reporting such cases is important to raise awareness of this rare entity as a potential cause of acute abdominal pain in children. CASE PRESENTATION: A 5-year-old Vietnam

Warm staple-line irrigation (37-40 °C) before omentopexy reduces early bleeding after laparoscopic sleeve gastrectomy: a prospective, randomized trial.

BACKGROUND: Early bleeding remains a common concern after laparoscopic sleeve gastrectomy (LSG), despite improvements in stapling technology and reinforcement techniques. This study examined whether using warm saline to irrigate the staple line before performing omentopexy could enhance intra-operative hemostasis and reduce early postoperative bleeding. METHODS: In this prospective randomized trial conducted between December 2024 and June 2025, 200 adults undergoing primary LSG were allocated equally to warm saline irrigation (37-40 °C) or conventional room-temperature irrigation (22-24 °C),

Robotic management of superior mesenteric artery syndrome after weight loss: a case report.

Superior mesenteric artery (SMA) syndrome is a rare cause of proximal intestinal obstruction resulting from compression of the third portion of the duodenum between the aorta and the SMA. It most commonly occurs in slender individuals or those experiencing rapid weight loss, but presentations can be atypical and diagnostically challenging. We report the case of a 28-year-old female with history of postural orthostatic tachycardia syndrome and gastroparesis who presented with recurrent nausea and intractable vomiting following a 110-lb weight loss after prior obesity. Upper gastrointestinal end

Comparing Efficacy of Oxycodone versus Sufentanil for Postoperative Recovery in Patients Receiving Bilateral Modified Thoracoabdominal Nerves Block Through Perichondrial Approach (M-TAPA) Following Laparoscopic Radical Distal Gastrectomy: A Study Protocol for a Prospective, Blinded, Randomized Controlled Trial.

INTRODUCTION: While laparoscopic radical distal gastrectomy (LRDG) has been widely adopted for its minimally invasive benefits in the treatment of gastric cancer, acute postoperative pain, comprising both somatic incisional pain and visceral discomfort, remains a clinical challenge that can affect patients' perioperative comfort and the rehabilitation process. Consequently, optimizing postoperative analgesic strategies remains essential for enhanced recovery. The modified thoracoabdominal nerves block through a perichondrial approach (M-TAPA) has been introduced as an emerging regional analges

Comparison of oliceridine and sufentanil in postoperative pain management for postoperative nausea and vomiting after bimaxillary orthognathic surgery: an exploratory randomized controlled trial.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a prevalent adverse event following bimaxillary orthognathic surgery, with opioids being a significant contributing factor. Oliceridine, a G protein-biased μ-opioid receptor agonist, reduces activation of the β-arrestin pathway and may decrease gastrointestinal adverse events relative to traditional opioids. This study aimed to evaluate the efficacy and safety of oliceridine relative to sufentanil for postoperative patient-controlled intravenous analgesia (PCIA) in this population, with a particular emphasis on PONV. METHODS: This prospe

Effect of Oliceridine Fumarate for Patient-Controlled Intravenous Analgesia on Gastrointestinal Function in Laparoscopic Radical Gastrectomy Surgery: A Randomized, Double-Blind, Controlled Trial.

BACKGROUND: Postoperative ileus and opioid-related side effects are major barriers to recovery following laparoscopic gastrectomy. Oliceridine, a novel G-protein-biased μ-opioid receptor agonist, aims to offer effective analgesia with fewer adverse effects. This study evaluated the efficacy of oliceridine for patient-controlled intravenous analgesia (PCIA) on gastrointestinal (GI) recovery and analgesic outcomes. METHODS: This single-center, prospective, randomized, double-blind, controlled trial enrolled 106 patients undergoing elective laparoscopic radical gastrectomy. Participants were ran

Probing the gut barrier: epithelial permeability as a dynamic marker in eosinophilic gastroenteritis via pCLE.

Eosinophilic gastroenteritis (EGE) is a rare, chronic inflammatory condition characterized by symptoms and eosinophilic infiltration of the gastrointestinal tract in the absence of secondary causes of gastrointestinal eosinophilia. The advent of probe-based confocal laser endomicroscopy (pCLE) has introduced the possibility of real-time, in vivo microscopic imaging, enabling dynamic assessment of mucosal architecture and barrier function. We report the case of a 32-year-old woman with an atypical manifestation of mucosal EGE, initially presenting with pseudothrombotic microangiopathy secondary

Spontaneous intramural small-bowel hematoma secondary to coagulopathy in patients receiving vitamin K antagonists: A case series report.

RATIONALE: Spontaneous intramural small-bowel hematoma (SISBH) is a rare but potentially life-threatening complication of anticoagulant therapy and severe coagulopathy. Because it may mimic acute abdomen or intestinal obstruction, delayed recognition can lead to unnecessary surgery. This study aimed to describe the clinical characteristics, imaging findings, and outcomes of patients with SISBH secondary to vitamin K antagonist-related coagulopathy. PATIENT CONCERNS: We retrospectively analyzed 11 consecutive patients with SISBH secondary to vitamin K antagonist-related coagulopathy, including
