Pancreatogenic Diabetes Mellitus - Type 3c (T3cDM).

Pancreatogenic diabetes mellitus (type 3c diabetes, T3cDM) is a distinct form of diabetes resulting from primary pancreatic disorders, most commonly chronic pancreatitis. It involves both - endocrine and exocrine dysfunction, leading to unstable glycemia with frequent hypo-and hyperglycemia. The condition is often underdiagnosed despite its notable prevalence in patients with pancreatic disease, including malignancy and post-surgical states. Clinically, it overlaps with type 1 and type 2 diabetes but is distinguished by malabsorption and weight loss. Diagnosis requires standard glycemic criter

One Patient, Two Problems: Applying Short Bowel Syndrome Management Principles in Inflammatory Bowel Disease patients.

PURPOSE OF REVIEW: Surgery for inflammatory bowel disease (IBD) often results in alterations in gut function beyond the anatomical changes. While some patients develop short bowel syndrome (SBS), a significant proportion of IBD patients benefit from pharmacologic and dietary interventions for SBS even in the absence of true SBS or IF. RECENT FINDINGS: While no formal criteria exist to clearly define the presence of SBS in IBD patients, the presence of specific features in the surgical and clinical history are crucial to the rapid identification of SBS or IF in IBD patients. Anatomy specific d

Pernicious anemia predominates among Mexican adults with vitamin B12 deficiency: clinical and laboratory findings from a tertiary referral center.

BACKGROUND: Vitamin B12 deficiency is a reversible cause of hematologic and neurologic morbidity, but its etiologic spectrum in Latin American referral centers remains poorly defined. METHODS: We retrospectively studied consecutive adults with vitamin B12 deficiency evaluated in hematology and oncology clinics at a Mexican tertiary referral center from January 2013 to July 2023. Etiologies were classified as pernicious anemia, non-surgical malabsorption, post-surgical malabsorption, reduced intake, drug-related causes, or unknown, and pernicious anemia was compared with all other etiologies.

Social and Financial Costs of Neonatal Intestinal Failure.

IMPORTANCE: Neonatal short bowel syndrome with intestinal failure is rare, and observational studies are limited to small cohorts. Health disparities exist in long-term management with little known about the initial hospitalization. OBJECTIVE: To identify children with neonatal short bowel syndrome with intestinal failure and to determine whether race, ethnicity, and neighborhood opportunity are associated with length of stay during their initial hospitalizations. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study of children with neonatal short bowel syndrome and intestinal failu

Spontaneous ileitis and postsurgical murine models of enteric hyperoxaluria.

Enteric hyperoxaluria, a risk factor for kidney stone disease, often arises from malabsorptive bariatric surgeries or inflammatory bowel diseases. Current murine models for studying this condition are limited, necessitating new approaches. This study aims to establish two novel and distinct mouse models to investigate enteric hyperoxaluria: one simulating Roux-en-Y gastric bypass surgery and the other Crohn's ileitis. In the first model, diet-induced obese C57BL/6J male mice underwent either sham or bypass surgery, followed by 3 wk on a high-fat, oxalate-enriched diet. In the second model, SAM

Small intestinal bacterial overgrowth in children: An expert review by the ESPGHAN Gastroenterology Committee.

The aim of this review is to summarize the prevalence, etiology, pathogenesis, diagnosis, and treatments currently available for small intestinal bacterial overgrowth (SIBO) in children. SIBO is a clinical entity characterized by the presence of an excessive number of bacteria in the small bowel leading to several nonspecific gastrointestinal symptoms due to malabsorption and malnutrition, such as bloating, flatulence, belching, diarrhea, abdominal pain, nausea, steatorrhea, fatigue and stunted growth. Initially thought to develop specifically in the context of abnormal or postsurgical gastroi

Comparison of tablet versus liquid ethanol-free Levothyroxine in thyroidectomised patients.

PURPOSE: Since the introduction of an ethanol-free liquid levothyroxine formulation, only few studies assessed its therapeutic efficacy. The present study aimed to compare the efficacy of tablet (T-LT4) versus L-EF-LT4 in achieving the patient-specific TSH target in thyroidectomized patients. The primary objective was to assess whether the required LT4 dose differed between formulations in a real-life setting. METHODS: The study was conducted on 275 patients who underwent total thyroidectomy and were treated with either T-LT4 (n = 152) or L-EF-LT4 (n = 123) between 2020 and 2023. Clinical and

Exocrine Pancreatic Insufficiency: Postoperative Complication Emerging Decades after Surgery?

Exocrine pancreatic insufficiency (EPI) arises from a deficiency in pancreatic enzymes, leading to malabsorption and varied symptoms such as gastrointestinal discomfort, weight loss and steatorrhea. Though EPI commonly follows chronic pancreatitis or pancreatic surgery, its delayed onset is rarely discussed. We report a unique case of EPI manifesting 29 years after pancreaticoduodenectomy. A 71-year-old female, who underwent pancreaticoduodenectomy for carcinoma in 1992, presented with fatigue, weight loss, edema, diarrhea and paresthesias. Physical examination revealed mucocutaneous pallor, a

Long-term antithrombotic therapy practices in adult patients with short bowel syndrome following acute mesenteric ischemia: An international case-based survey.

BACKGROUND & AIMS: Long-term antithrombotic therapy is recommended for short bowel syndrome (SBS) after acute mesenteric ischemia (AMI). However, targeted recommendations on drug selection, dosing, duration and monitoring, are lacking. Current recommendations rely on data from other arterial diseases, often overlooking SBS-related drug absorption issues. To understand current practices, this survey aimed to assess the long-term antithrombotic therapy practices in AMI. METHODS: An e-survey was disseminated to HAN-CIF (Home Artificial Nutrition - Chronic Intestinal Failure) database centers of

Parenteral nutrition: a life-saving intervention for 4 months in short bowel syndrome-a case report and review of the literature.

BACKGROUND: Short bowel syndrome (SBS) in adults is defined as having less than 180 to 200 cm of remaining small bowel. Many literature sources do not provide precise epidemiological data, and challenges in estimating the prevalence of SBS include its multifactorial etiology and varying definitions. The most common pathologies leading to SBS include Crohn disease, mesenteric ischemia, radiation enteritis, post-surgical adhesions, and post-operative complications. CASE PRESENTATION: This article presents a clinical case of a 76-year-old Lithuanian patient who underwent parenteral nutrition for

[SIBO, from myth to reality].

Digestive functional disorders are among the most frequent reasons for medical consultation and a significant source of medical wandering. Therapeutic management of these patients is difficult, particularly due to the absence of specific treatment linked to an incomplete understanding of the pathophysiological mechanisms. In a certain number of these patients, the symptoms are accompanied by a small intestinal bacterial overgrowth (SIBO). This entity, historically identified in specific post-surgical situations, seems finally very common and associated with very diverse pathologies. The diagno

Comparison of One-Year Post-Operative Evolution of Children Born of COVID-19-Positive Mothers vs. COVID-19-Negative Pregnancies Having Congenital Gastrointestinal Malformation and Having Received Proper Parenteral Nutrition during Their Hospital Stay.

Background: The long-term effects on neonates born of COVID-19-positive pregnancies are still unclear. Congenital gastrointestinal malformations (CGIMs) often require urgent surgical intervention and antibiotic coverage. We aim to compare the health status at one-year post-surgical follow-up of cases of CGIM born of COVID-19-positive pregnancies to those of non-COVID-19 pregnancies. Methods: We conducted a comparative, observational study of 41 patients who underwent surgery at our hospital for congenital gastrointestinal malformations in 2022. They were initially treated with antibiotics and

A Comprehensive Review on Bariatric Endoscopy: Where We Are Now and Where We Are Going.

Background: Obesity is a chronic disease that impairs quality of life and leads to several comorbidities. When conservative therapies fail, bariatric surgical options such as Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG) are the most effective therapies to induce persistent weight loss. Over the last two decades, bariatric endoscopy has become a valid alternative to surgery in specific settings. Primary bariatric endoscopic therapies: Restrictive gastric procedures, such as intragastric balloons (IGBs) and endoscopic gastroplasty, have been shown to be effective in inducing weigh

Delta weight loss unlike genetic variation associates with hyperoxaluria after malabsorptive bariatric surgery.

The risk of enteric hyperoxaluria is significantly increased after malabsorptive bariatric surgery (MBS). However, its underlying determinants are only poorly characterized. In this case-control study, we aimed at identifying clinical and genetic factors to dissect their individual contributions to the development of post-surgical hyperoxaluria. We determined the prevalence of hyperoxaluria and nephrolithiasis after MBS by 24-h urine samples and clinical questionnaires at our obesity center. Both hyperoxaluric and non-hyperoxaluric patients were screened for sequence variations in known and ca

Post-surgical exocrine pancreatic insufficiency.

Being an underdiagnosed and under or insufficiently treated condition, surgical pancreatic exocrine insufficiency (PSP) is the condition in which pancreatic enzymes are insufficient for digestion because of gastrointestinal (GI) surgery involving the upper GI tract, biliary ducts, or the pancreas, and and leading to potential malnutrition and deterioration in quality of life. Age, obesity, history of tobacco use, family history of diabetes, surgery due to a malignant tumor, presence of steatorrhea, jaundice, weight loss, and intraoperative findings of hard pancreatic texture have been associat

The effect of fecal microbial transplant on intestinal microbial composition in short-bowel neonatal piglets.

BACKGROUND: Short-bowel syndrome (SBS) in neonates is associated with microbial dysbiosis due to intestinal surgery, prolonged hospitalization, enteral nutrition, and repeated antibiotic exposure. Sepsis and liver disease, leading causes of morbidity and mortality in SBS, may relate to such intestinal dysbiosis. We investigated the safety and feasibility of fecal microbial transplant (FMT) to alter intestinal microbial composition in SBS piglets. METHODS: Following a 75% distal small-intestinal resection, piglets were fed parenteral nutrition with an elemental diet and randomized to saline (S
