PURPOSE: Solitary band intestinal obstruction is receiving renewed interest based on the emerging possibility of laparoscopic intervention restricted to this type of small intestinal obstruction. Therefore, pertinent observations focussing on this patient population undergoing conventional "open" surgery could be of interest.
METHOD: Prospectively collected chart data of all patients operated for solitary band obstruction (SBO) from January 1979 until December 1993 were reviewed and the further course followed for 1-15 (median 8) years.

The authors present 40 cases of Meckel's diverticulum in pediatric patients (average age 6.8 years old). 31 of these (77.5%) were discovered casually in surgery and the other nine (22.5%) presented specific symptoms (three intestinal intussusceptions, three perforations, one hemorrhage, a wet umbilicus and an intestinal obstruction). Newly developed endoscopic instruments and devices, along with advanced endoscopic surgical techniques, have made it possible to perform an increasing variety of endoscopic procedures. We performed 58,938 operations on the abdominal organs. Three hundred and six (37.8%) patients developed early postoperative acute ileus. After its operative elimination, 22 (7.2%) patients required performance of a reoperation. Ninety (29.4%) patients died. Of them in 72, death was caused by peritonitis, in 18--pneumonia, acute cardiovascular failure, impairement in cerebral circulation and pulmonary thromboembolism.

Although postoperative adhesion ileus is the most common cause of small bowel obstruction in adults, its management remains controversial. We retrospectively studied 297 admissions of 227 patients over a period of 14 years to evaluate our conservative approach in managing adhesion ileus. We found that nonoperative therapy of up to 5 days' duration can be used safely for the majority of patients who present with postoperative intestinal obstruction, including those with complete obstruction. For the period of 1977-1987, operations on the abdominal organs were performed in 13,306 patients. Some patients with rectal cancer who undergo exenterative surgery may require radiation therapy as an adjuvant treatment for recurrent or residual disease. Optimal surgical management of neonates with gastroschisis and omphalocele remains controversial. In 12 children colostomy wound closure was undertaken with liberal extraperitoneal mobilisation of the bowel. In all the cases adequate length of the colonic loops could be achieved for resection of the colostomy and end to end anastomosis. There was only minor wound infection in 3 cases. The long-tube decompression was used for treatment of 54 selected patients with diagnosis of early postoperative intestinal obstruction. For this purpose a silicon double-lumen wire-guided tube has been endoscopically introduced into the proximal portion of the small intestine. Forty-three patients were reoperated in the early postoperative period. In 27 of them the cause of early relaparotomy was postoperative intestinal obstruction. Postoperative intussusception in the adult is a rarely reported and poorly understood condition. Presented is the case of a patient developing incomplete small bowel obstruction after an uneventful vaginal vault suspension. Jejunojejunal intussusception, requiring resection, was found at celiotomy. Intussusception must be considered in treating patients developing postoperative intestinal obstruction.

Advantages of the intestine decompression in the prevention and treatment of postoperative obstruction were shown by an analysis of results of the treatment of 348 patients with acute surgical diseases of the abdomen.
