[Progression of pathogenesis, diagnosis and treatment of diversion colitis].

Diversion colitis (DC) is a common non-specific inflammation of the malfunctioning bowel segment after diversion of feces. Although most patients develop DC during stoma, there are no obvious clinical symptoms, and a small number of patients show abdominal pain, mucus discharge and hematochezia. The erythema, diffuse particles and vascular texture blur are the most prominent endoscopic manifestations, and the lymphatic follicular hyperplasia is the most prominent pathological manifestations.Reconstruction of intestinal continuity is the best way to cure recanted colitis. In this review, we sum

The use of diverting colostomies in paediatric peri-anal burns: experience in 45 patients.

BACKGROUND: Peri-anal burns are common in children. Continual exposure to enteric organisms may lead to a contaminated burn wound, invasive sepsis, graft loss, scar contracture, anal and urinary malfunction, and delayed discharge from hospital. Use of a temporary diverting colostomy has been advocated to reduce complications. OBJECTIVES: To review the incidence, indications, methods, bacteriology, therapeutic effects and outcomes of colostomy for perianal burns. METHODS: A prospective study of children with peri-anal burns and stomas over a 17-year period. Prophylactic or therapeutic faecal

Colonic ischemia and perforation associated with enteral feeding through an ileal tube.

Reported complications of enteral feeding through a jejunostomy include diarrhea, intraperitoneal leaks, bowel obstruction, fistula formation, wound infection, tube occlusion, and other mechanical malfunctions. However, the incidence of these complications is very low, and many physicians prefer to feed their patients by means of a jejunal tube instead of parenteral nutrition. A potentially lethal complication is ischemia of the bowel distal to the site of insertion of the feeding catheter. The described cases of bowel ischemia secondary to enteral nutrition invariably occurred at the level of

Stoma complications: a multivariate analysis.

Construction of a gastrointestinal stoma is a frequently performed surgical procedure. We sought to analyze a large cohort to document the frequency and types of ostomy complications and the risk factors associated with them. The charts of patients undergoing a procedure which resulted in ostomy during a 3-year period were reviewed. Demographics, indication, ostomy type/location, perioperative risk factors, and complications were recorded. Case-control methodology was used to determine crude odds ratios and multiple logistic regression was used to calculate adjusted odds ratios. A P value of l

An artificial anal sphincter. Phase 2: Implantable sphincter with a perineal colostomy.

To find an alternative to the stigmata of abdominal colostomy, an artificial anal sphincter (utilizing the currently available artificial urinary sphincter) was implanted around an end perineal colostomy following standard abdominal perineal resection. Of 13 swine, 7 completed full evaluation with working sphincters, 3 animals died prematurely of unrelated causes, and 3 animals were sacrificed due to sphincter slough before inflation was begun. Seven animals had long-term evaluation of the sphincter for 2 to 6 months' duration. In 5 animals (72%) there was complete fecal continence; one animal

Colonic reservoirs.

During the last decades several types of surgical techniques have been utilized in order to provide the patient with a continent colostomy using the colon as a reservoir. None of the methods have gained wide acceptance and continence is usually dependent upon successful irrigation. By constructing a cecal reservoir with an intussuscepted nipple valve, using the same principle as for the continent ileostomy, a colostomy with perfect continence for both gas and feces can be created. Although improving life quality significantly when successful, the method is hampered by a high rate of complicati

Experiences with an artificial sphincter to establish anal continence in dogs.

An implantable device for establishing urinary continence has been developed and has received FDA approval (AS 800, American Medical Systems, Inc.). The authors have applied this device to the control of anal continence in dogs. Fifteen mongrel dogs underwent either anal sphincter disruption (7) or abdominal-perineal resection (8). Such animals have bowel movements almost hourly. In each case, the device was implanted at the time of surgery. In dogs with working cuffs and disabled or absent sphincters, continence was maintained in seven out of 15 animals for periods of 4-8 hours. After cuff ac

Colonic perforation as a complication of peritoneovenous shunt: a case report.

Perforation of the left colon by a peritoneovenous shunt was diagnosed in a patient who had systemic sepsis 6 years after placement of the shunt. The patient was treated successfully by direct tapping of the shunt, which demonstrated enteric flora, and by an exploratory laparotomy, removal of the shunt, bowel resection, and colostomy. Although visceral perforations have been described with ventriculo-peritoneal shunts, we found no published report of an enteric fistula from a peritoneovenous shunt. This complication should be considered in patients with shunt malfunction.

THE SURGICAL MANAGEMENT OF CHRONIC ULCERATIVE COLITIS.

For many years patients with chronic ulcerative colitis were subjected to operation as a last resort, but now, owing to improved surgical techniques and adequate preoperative preparation of the patient, mortality associated with operation has decreased and better-risk patients are being operated upon electively. The experience of the University of Alberta Hospital during the past 10 years in respect of the surgical management of chronic ulcerative colitis is reviewed, on the basis of 105 patients, 36 of whom underwent operation. The indications for surgery include obstruction, suspected carcin
