Retained stones within the cystic duct stump are an uncommon but clinically important cause of postcholecystectomy syndrome. Dilation of the remnant duct secondary to an occluding stone is exceedingly rare, with only isolated reports of massive dilation in the literature. We describe a 67-year-old male (BMI 31.35) who presented two years after laparoscopic cholecystectomy with recurrent right upper quadrant biliary colic. Cross-sectional imaging demonstrated progressive enlargement of the cystic duct stump from mild dilation at 241 days to marked dilation of greater than 40 mm containing a 9 m

INTRODUCTION: Remnant gall bladder and cystic duct stump calculi are uncommon but significant causes of post-cholecystectomy syndrome, often presenting after a prolonged interval and posing diagnostic and technical challenges. Evidence from contemporary Indian cohorts remains limited and heterogeneous.
PATIENTS AND METHODS: This retrospective observational study analysed 22 consecutive patients who underwent completion cholecystectomy for symptomatic remnant gall bladder or cystic duct stump calculi between January 2015 and December 2024 at a tertiary referral centre. Patients with radiologic

BACKGROUND/OBJECTIVES: Post-cholecystectomy syndrome (PCS) remains diagnostically challenging due to overlapping biliary and non-biliary causes. This study aimed to evaluate whether common bile duct (CBD) diameter measured by MRCP can serve as a practical triage parameter in symptomatic PCS patients and to define a data-supported threshold for predicting clinically relevant biliary pathology. Secondary objectives included assessing correlations between MRCP findings and upper endoscopic features.
METHODS: In this retrospective single-center study, symptomatic adults undergoing upper endoscopy

Titanium is widely regarded as a biocompatible and inert material commonly used in surgical implants. Despite this reputation, hypersensitivity reactions to titanium, though rare, can lead to persistent and unexplained postoperative symptoms. We report the case of a 76-year-old woman who developed progressive right upper quadrant abdominal pain, burning sensations, neuropathic symptoms, and fatigue three months after laparoscopic cholecystectomy in which titanium clips were used. Her history included documented systemic intolerance to a titanium dental implant 10 years earlier, which had requi

INTRODUCTION: Obstructive jaundice (OJ) remains a significant clinical and diagnostic challenge due to the high risk of hepatic failure and homeostasis disturbances. The main causes of OJ include biliary calculi (≈50%), malignant tumors (up to 40%), and biliary strictures (≈10%). Postoperative complications occur in 15.6-63.4% of cases, with mortality rates reaching 25-30%, highlighting the need for timely biliary decompression and the use of minimally invasive treatment methods to improve outcomes.
AIM: To analyze the outcomes of the implementation and optimization of surgical treatment meth

AIM: To assess the prevalence of concurrent upper gastrointestinal (UGI) conditions through preoperative UGI endoscopy in patients with symptomatic cholelithiasis.
MATERIALS AND METHODS: This prospective observational study, conducted at a tertiary care facility, involved 103 patients. All participants underwent preoperative endoscopy. Treatment was divided into three categories based on endoscopic findings: cholecystectomy (Group 1), cholecystectomy with medical treatment (Group 2), and postponed surgery (Group 3). Symptom scores were evaluated before surgery and during the postoperative fol

BACKGROUND AND AIMS: Cystic duct remnant (CDR) stones are an under-recognized biliary cause of postcholecystectomy syndrome (PCS). Surgical excision has been the traditional treatment; however, ERCP may offer a less invasive alternative.
METHODS: We conducted a retrospective multicenter study of patients with PCS who underwent ERCP for CDR stones. The primary outcome was technical success (complete stone clearance); secondary outcomes were clinical success and adverse events.
RESULTS: Twenty-one patients were included (mean age, 50 ± 17 years). Technical and clinical success rates were 95% (

Sepsis, a systemic inflammatory response syndrome (SIRS) caused by various bacterial infections, is still a severe health problem worldwide with high morbidity and mortality, whereas early diagnosis and differential diagnosis are still challenge. Human neutrophil CD64 (nCD64), also known as FcγRI, is a transmembrane glycoprotein of the immunoglobulin (Ig) superfamily that bind to the Fc (constant) region of specific portion of antibody with high affinity, which CD64 is a new specific and sensitive biomarker for early diagnosis and prognosis of sepsis. This study is aimed at selecting aptamers

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. A 36-year old obese caucasian woman underwent a laparoscopic cholecystecto

RATIONALE: Triple gallbladder is a rare congenital anatomical abnormality because of the incomplete regression of rudimentary bile ducts and is often not found until it is accidentally detected during imaging research.
PATIENT CONCERNS: We report a rare case of triple gallbladder malformation and review the English literature on biliary tract variation caused by gallbladder malformation. The diagnosis, treatment, and postoperative situation of the patients were summarized and analyzed.
DIAGNOSES: The patient was diagnosed with triple gallbladder malformation.

BACKGROUND: Subtotal cholecystectomy (SC) is considered a safe option for a bailout in the presence of a difficult laparoscopic cholecystectomy with a low incidence of complications.
OBJECTIVE: This report aims to describe the challenges in diagnosing and managing remnant gallbladder.
CASE PRESENTATION: Case 1 is a 31-year-old male who presented with right upper quadrant abdominal pain ten years following SC. Abdominal ultrasound (US) and computed tomography (CT) scan confirmed a ruminant gallbladder. He underwent successful completion of laparoscopic cholecystectomy. Case 2 is a 40-year-old

INTRODUCTION: Post-cholecystectomy syndrome (PCS) is a complex condition characterized by persistent or new symptoms following gallbladder removal, affecting up to 47% of patients. Despite being recognized since 1947, there is still no consensus on its etiology, diagnosis, and treatment.
AREAS COVERED: This narrative review explores the multifactorial etiology of PCS, including biliary and extra-biliary factors, and its varied clinical manifestations. A systematic literature search was conducted using keywords like 'etiology', 'clinical manifestations', 'diagnostic challenges', and 'managemen

AIM: Cystic duct stump remnant causing "postcholecystectomy syndrome" is an uncommon but clinically significant complication. Symptomatic cystic duct stump stones often necessitate redo completion cholecystectomy. Here, we share our experience with the clinical presentation and surgical management of this condition.
PATIENTS AND METHODS: This prospective study included 60 patients with residual cystic duct disease who underwent completion cholecystectomy between August 2016 and September 2024. Investigations included abdominal ultrasound and magnetic resonance cholangiopancreatography. The de

Persistent gastrointestinal symptoms after cholecystectomy are not uncommon and are frequently attributed to functional disorders such as dyspepsia or gastritis. However, rare structural causes (such as retained calculi within the cystic duct stump or a remnant gallbladder) can mimic benign gastrointestinal conditions, particularly in the absence of classical biliary signs. We present the case of a 47-year-old female with a one-year history of postprandial epigastric discomfort, bloating, and dyspepsia. She had undergone an open cholecystectomy five years earlier for gallstone disease. Despite

BACKGROUND AND AIM: Laparoscopic cholecystectomy is the gold standard treatment for gallstones, offering a minimally invasive approach with quicker recovery times. Despite its effectiveness, a significant proportion of patients experience persistent symptoms post-surgery, collectively referred to as post-cholecystectomy syndrome (PCS). The incidence, causes, and management of PCS remain underexplored. This study aimed to evaluate the incidence of PCS among adult patients undergoing elective laparoscopic cholecystectomy for benign gallstone disease at Pilgrim Hospital, Boston, UK, and to identi

Postcholecystectomy syndrome (PCS) may result from either subtotal cholecystectomy or unintentional incomplete resection, leading to remnant gallbladder or residual cystic duct pathology. This scenario often presents years later, complicated by lithiasis that is not amenable to endoscopic treatment and prior surgery performed via laparotomy, typically due to difficult anatomy. Thus, reoperation becomes a technically demanding scenario, revisiting a surgical battlefield shaped by dense adhesions and distorted landmarks. We conducted a Preferred Reporting Items for Systematic Reviews and Meta-An

A 42-year-old male presented with a two-month history of right upper quadrant abdominal pain, worsening over four days. Six months earlier, he had undergone a laparoscopic cholecystectomy. Examination revealed no jaundice or tenderness, and liver function tests were normal. The patient's basic inflammatory markers were within the normal range. Ultrasound revealed a residual gallbladder stump or dilated cystic duct with a calculus, and magnetic resonance cholangiopancreatography (MRCP) demonstrated a 3 × 1 cm stump with an 8 mm hypointense focus. Endoscopic retrograde cholangiopancreatography (

INTRODUCTION: Cholecystectomy is one of the most common operations in surgical departments. Complications after gallbladder removal are mainly bleeding, infection including abscess in the gallbladder bed or in the abdominal wall, wound dehiscence, acute pancreatitis or injury of the bile ducts. In the further course, hernias in the scar may appear after both laparoscopic and open cholecystectomy, strictures of the bile ducts and symptoms of the so-called postcholecystectomy syndrome. The presence of residual gallstones is rare, statistically reported in 0.08-0.3%.
CASE REPORT: The goal of our

Gallstone ileus, a rare complication of cholecystitis, occurs when the distal ileum becomes obstructed by gallstone causing, mechanical bowel obstruction. Gallstone ileus with history of cholecystectomy is exceptionally rare, with few cases documented in literature. This case report details the discovery of a large, impacted gallstone 16 years postcholecystectomy. An 87-year-old male presented with progressively worsening right-sided abdominal pain over 2-3 days. Exploratory laparotomy unveiled a 2.0 cm × 2.5 cm gallstone was obstructing the terminal ileum. Patient improved remarkably postoper
