[Gastric complaints or postcholecystectomy syndrome?].

UNLABELLED: Postcholecystectomy syndrome (PCS) is a complex of symptoms from gastrointestinal tract that could develop and maintain after cholecystectomy. PCS usually consists of: abdominal pain or colic, dyspepsia, constipation or diarrhoea, nausea, bloating, fatty food intolerance. Regarding PSC as disease entity is still a point of many controversies. THE AIM OF THE STUDY: To estimate the prevalence of PCS in patients after cholecystectomy performed in Department of General and Colorectal Surgery of Medical University in Lodz. Material and methods. From the cohort of 243 patients (pts) ope

Somatosensory hypersensitivity in the referred pain area in patients with chronic biliary pain and a sphincter of Oddi dysfunction: new aspects of an almost forgotten pathogenetic mechanism.

BACKGROUND AND AIMS: Somatosensory hyperalgesia in the referred pain area (RPA) in patients with acute or chronic abdominal pain syndromes may result from the convergence of nerve fibers from visceral and somatic tissues at the spinal and supraspinal levels. Chronic biliary pain in patients with the postcholecystectomy syndrome (i.e., biliary hypersensitivity) may be explained by persistent hyperexcitability of neurons in the central nervous system (CNS). The aim of this study was to evaluate the cutaneous neural sensory perception in the RPA in patients with chronic postcholecystectomy biliar

Postcholecystectomy sphincter of oddi dyskinesia--a diagnostic dilemma--role of noninvasive nuclear and invasive manometric and endoscopic aspects.

BACKGROUND: Persistent abdominal pain after cholecystectomy is not uncommon. Sphincter of oddi dysfunction (SOD) is one of the causes for this entity. However, diagnosing SOD is often difficult. Sphincter of oddi manometry (SOM) is the gold standard. Because it is invasive and needs experienced person to perform, simple noninvasive imaging techniques are needed. Other invasive endoscopic methods also play an important role in difficult cases and before therapeutic intervention. METHODS: Retrospective review of the charts of postcholecystectomy patients who presented with persistent abdominal

Metal clip as a nidus for formation of common bile duct stone following laparascopic cholecystectomy.

A 83-year-old woman came to our hospital with fever up to 38.5 degrees C, abdominal pain and jaundice. She had undergone a cholecystectomy for symptomatic gallstones in 1992. The endoscopic retrograde cholangiography showed a dilated common bile duct with a free floating surgical clip with stone formation around this foreign body. After endoscopic sphincterotomy the extraction of the foreign body was successful. We found a two cm cholesterol stone the nidus of which was built by a Filshie-clip. The post endoscopic course was uneventful.

[Spa treatment of postcholecystectomy syndrome].

In 40% cases pain and dyspepsia do not disappear after cholecystectomy made by laparotomic or laparoscopic method. The aim of the study was to introduce a new method of ultrasonic diagnosis of biliary excretion and to formulate principles of postcholecystectomy syndrome treatment depending on the dyskinesia form. The new method diagnosed 6 variants of biliary excretion: normal (7.2%), hypokinetic (42.8%), hypertonic (28.3%), hypertonic-hypokinetic (5.0%), hyperkinetic (14.5%), hypertonic-hyperkinetic (2.2%). The patients received differentiated treatment depending on the type of biliary excret

Diagnosis and management of sphincter of Oddi dysfunction and pancreas divisum.

Sphincter of Oddi dysfunction and pancreas divisum are very distinct anatomic abnormalities, yet are diagnosed in similar clinical situations. While both entities are uncommon, they are most often discovered during the evaluation of postcholecystectomy syndrome, recurrent idiopathic pancreatitis, and biliary or pancreatic pain when first line studies are normal. Treatment consists of surgical sphincteroplasty or endoscopic sphincterotomy for both diagnoses, which result in reliable relief of symptoms for most sphincter of Oddi dysfunction patients but less predictable response in pancreas divi

Postcholecystectomy syndrome in the laparoscopic era.

sWe describe the management of a cystic duct remnant calculus in a 45-year-old male patient who had undergone a laparoscopic cholecystectomy and re-presented with abdominal pain and jaundice. Magnetic resonance cholangiopancreatography was utilized to confirm the diagnosis of an impacted calculus within the remnant cystic duct along with several small retained common bile duct stones. Four sequential endoscopic procedures successfully removed all retained common bile duct calculi to alleviate the biliary obstruction; however, we were unable to treat the cystic duct remnant calculus endoscopica

[Post-cholecystectomy syndrome: the modern view of the problem].

The authors summarize and systematize literature data and their own observations concerning post-cholecystectomy syndrome (PCES), the reasons for and the mechanisms of its development, its clinical variants etc. The authors suggest the following PCES forms should be distinguished: functional ("egenuine") forms, which develop due to gall bladder removal and the loss of its functions, and organic ("conditional") PCES forms, which develop as a consequence ofaflawy surgery and/or preoperative complications of chronic calculous cholecystitis, which dominate in the postoperative clinical picture and

Characterization of functional biliary pain and dyspeptic symptoms in patients with sphincter of Oddi dysfunction: effect of papillotomy.

AIM: To characterize functional biliary pain and other gastrointestinal (GI) symptoms in postcholecystectomy syndrome (PCS) patients with and without sphincter of Oddi dysfunction (SOD) proved by endoscopic sphincter of Oddi manometry (ESOM), and to assess the post-endoscopic sphincterotomy (EST) outcome. METHODS: We prospectively investigated 85 cholecystectomized patients referred for ERCP because of PCS and suspected SOD. On admission, all patients completed our questionnaire. Physical examination, laboratory tests, abdominal ultrasound, quantitative hepatobiliary scintigraphy (QHBS), and

Endoscopic treatment of the "sump syndrome" after choledochoduodenostomy: a new technique using an amplatzer septal occluder.

A 58-year-old male patient had been suffering for 35 years from recurrent cholangitis, biliary sludge and infection-induced stone formation after open cholecystectomy because of empyema of the gallbladder and severe acute and delayed postoperative complications. The pathophysiological origin of this chronic "sump syndrome" was a choledochoduodenostomy which had been performed prophylactically at the time of the initial operation. The patient agreed to an experimental treatment option with use of an Amplatzer atrial-septal defect (ASD) occluder for closure of the symptomatic choledochoduodenal

Pneumobilia: benign or life-threatening.

Pneumobilia, or air within the biliary tree of the liver, suggests an abnormal communication between the biliary tract and the intestines, or infection by gas-forming bacteria. Pneumobilia usually can be distinguished from air in the portal venous system by its appearance on computed tomography (CT) scan. The most common conditions associated with pneumobilia include: 1) a biliary-enteric surgical anastamosis, 2) an incompetent sphincter of Oddi, or 3) a spontaneous biliary-enteric fistula. Three cases of pneumobilia associated with its most common causes are presented and further differential

Imaging patients with "post-cholecystectomy syndrome": an algorithmic approach.

AIM: To assess the role of ultrasound (US), magnetic resonance cholangiopancreatography (MRCP) and liver function tests (LFTs) in the evaluation of selected patients presenting with late post-cholecystectomy syndrome (PCS) who were referred for endoscopic retrograde cholangiopancreatography (ERCP). MATERIALS AND METHODS: In a retrospective study a final group of 42 patients with PCS referred for diagnostic ERCP underwent MRCP and abdominal US. ERCP and MRCP images were assessed for bile duct diameters and the presence of strictures and stones. A common bile duct (CBD) diameter of < 10mm was c

Biliary scintigraphy versus sphincter of Oddi manometry in patients with post-cholecystectomy pain: is it time to disregard the scan?

Sphincter of Oddi (SO) dysfunction is diagnosed using manometry, and patients with an abnormal SO basal pressure respond well to division of the SO. However, manometry is invasive and is associated with a low, yet significant, incidence of complications. Scintigraphy techniques have been developed with the aim of providing a noninvasive means of assessing SO motility. However, when compared with SO manometry these techniques fall short in sensitivity and specificity for diagnosing SO dysfunction. Furthermore, they do not select patients who will respond to treatment. Consequently, the quest fo

Changes of gastrointestinal myoelectric activity and bile acid pool size after cholecystectomy in guinea pigs.

AIM: To investigate the bile acid pool size after cholecystectomy whether or not correlated to the gastrointestinal migrating myoelectric complex (MMC) in guinea pigs. METHODS: Gallbladder motilities were assessed before cholecystectomy. Furthermore, we continuously monitored interdigestive gastrointestinal motilities using bipolar electrodes in conscious guinea pigs before and after surgery at 4 wk in standard diet group and high cholesterol diet (cholesterol gallstone) group. Total bile acid pool sizes were measured by isotope dilution method at meantime. RESULTS: After cholecystectomy, th
