The Liver-Bone Axis in Steatotic Liver Disease.

Steatotic liver disease (SLD) encompasses a spectrum of conditions, including metabolic dysfunction-associated steatotic liver disease (MASLD), metabolic dysfunction and alcohol-related liver disease (MetALD), and alcohol-related liver disease (ALD), all of which may have consequences extending beyond the liver. Growing evidence suggests that SLD is associated with impaired skeletal health through a complex liver-bone axis. This review summarizes current evidence linking SLD with reduced bone mineral density, impaired bone quality, osteoporosis, and fracture risk. In MASLD, skeletal vulnerabil

Cirrhosis Impairs Reconstitution of the Circulating T-Lymphocyte Compartment After Direct-Acting Antiviral Therapy for Hepatitis C Virus Infection.

BACKGROUND: Direct-acting antivirals (DAAs) achieve sustained virological response (SVR) in over 95% of patients with chronic hepatitis C virus (HCV) infection. Although viral eradication allows partial immune restoration, it remains unclear whether established cirrhosis limits recovery of the circulating T lymphocyte compartment after SVR. METHODS: We performed a phenotypic and functional analysis of circulating T-helper (Th) and T-cytotoxic (Tc) lymphocytes in patients with chronic HCV infection without fibrosis (HCV-NoF, n = 21) or with cirrhosis (HCV-cirrhosis, n = 20), as well as in alco

Predictive value of serum HBcrAg in antiviral therapy for chronic hepatitis B.

Chronic hepatitis B (CHB) remains a significant global public health challenge. The goals of its antiviral therapy are gradually shifting from long-term viral suppression toward achieving safer treatment discontinuation, more accurate risk stratification, and deeper functional cure. Traditional serological markers, such as HBV DNA and HBsAg, remain clinically important in CHB. However, their ability to reflect residual viral activity and long-term outcomes is limited after long-term nucleos(t)ide analog (NA) therapy, especially in patients with sustained virological suppression or marked HBsAg

Sex Disparities in Hepatitis C Treatment Initiation: Role of Universal Access to DAA in France (ANRS FANTASIO 2).

BACKGROUND AND AIMS: Ensuring access to direct-acting antivirals (DAA) for all individuals concerned is a priority in hepatitis C elimination efforts. We analysed sex disparities in DAA initiation in France over a 9-year period spanning before and after August 2017, when the country implemented universal DAA access. METHODS: We used data from the French National Health Data System for two periods: 2014-2017 and 2017-2022. Socioeconomic and clinical characteristics of adults identified with chronic hepatitis C and residing in metropolitan France were described at each period entry date. DAA in

Clinical trends in hepatocellular carcinoma from 2005 to 2019 with a focus on metabolic diseases: a German nationwide study.

INTRODUCTION: Hepatocellular carcinoma (HCC) is the leading primary liver cancer and is associated with a high mortality rate. Liver cirrhosis represents the main risk factor for HCC; however, its underlying etiologies have shifted over the last decade. This German population-based study investigated the effects of the changing trends on etiologies of HCC hospital admissions. METHODS: We analyzed 369 369 HCC-admissions in German hospitals from 2005 to 2019 based on the diagnosis-related groups via German International Classification of Diseases and Operation and Procedure key System codes. UFMylation of SHBs increases protein stability by antagonizing ubiquitination and promotes virion production.

UNLABELLED: Post-translational modifications orchestrate virus-host interactions, yet their roles in the hepatitis B virus (HBV) life cycle remain incompletely defined. The small hepatitis B surface antigen (SHBs)-the most abundant HBV protein-drives immune evasion, pathogenesis, and virion/subviral particle (SVP) production. Here, we identify UFL1, the E3 ligase of UFMylation, as a host factor that modifies and stabilizes SHBs by antagonizing its ubiquitination to potentiate HBV output and SHBs-driven pro-oncogenic activities. Using affinity purification-mass spectrometry, we discovered UFL1

Fucosylated α-1-acid glycoprotein as a noninvasive fibrosis marker in chronic viral hepatitis.

We have previously developed a potential non-invasive fibrosis marker in the form of a lectin immunoassay for the detection of fucosylated α1-acid glycoprotein in plasma (P-AGP-FR). We aimed to investigate the diagnostic accuracy of P-AGP-FR for liver cirrhosis and fibrosis in a clinical setting of patients with chronic viral hepatitis. One hundred twenty-four patients, 80 with chronic hepatitis B and 44 with chronic hepatitis C, were recruited at two centers. P-AGP-FR levels were evaluated in relation to fibrosis or cirrhosis according to liver biopsy or transient elastography, and its diagno

Research Progress on Mechanisms of Immune Tolerance Induced by Hepatitis B Surface Antigen in Chronic HBV Infection.

The sustained elevation of hepatitis B surface antigen (HBsAg) facilitates the progression of chronic hepatitis B virus (HBV) pathogenesis, and initiates and sustains host immune tolerance. Immune tolerance is a major factor contributing to viral persistence and viral clearance failure. HBsAg exerts regulatory effects on innate defenses and adaptive responses, limiting the activity of dendritic cells (DCs), natural killer (NK) cells, monocytes/macrophages, T lymphocytes, and B cells. Furthermore, it fosters a tolerogenic environment via interconnected regulatory cell networks, immune checkpoin

SEROLOGICAL CLEARANCE OF HBSAG IN CHRONIC HEPATITIS B: EXPERIENCE IN A UNIVERSITY HOSPITAL.

BACKGROUND: Chronic hepatitis B virus (HBV) infection represents a major public health problem due to its high morbidity and mortality associated with liver cirrhosis and hepatocellular carcinoma. Functional cure, characterized by serological clearance of the HBV surface antigen (HBsAg), is the main therapeutic goal, since sterilizing viral eradication is not yet feasible. HBsAg clearance rates vary widely in the literature, and understanding the factors associated with this event is crucial for optimizing patient management. OBJECTIVE: To investigate the HBsAg clearance rate and identify the

Sequential combination of anti-PD-1 antibody and PEG-IFNα in chronic hepatitis B patients: correlates of HBsAg loss with enhanced T-cell responses.

Pegylated interferon-α (PEG-IFNα) exhibits suboptimal efficacy in achieving a functional cure in patients with chronic hepatitis B (CHB), partially due to impaired hepatitis B virus (HBV)-specific T-cell function. We conducted a pilot study (ClinicalTrials.gov: NCT06457477) to explore the efficacy, safety, and immunological changes associated with a response-guided sequential strategy of anti-PD-1 antibody (αPD-1) and PEG-IFNα. Ten virologically suppressed CHB patients receiving nucleos(t)ide analogues (NAs), all of whom had previously received PEG-IFNα and had HBsAg levels ≤200 IU/mL at enrol

Hepatitis B Virus: Epidemiology, Prophylaxis, Therapy, Clinical Outcomes, and Novel Therapeutic Directions.

Hepatitis B virus (HBV) infection is a worldwide health concern that infects nearly 254 million people globally and causes more than 1 million deaths annually. The highest prevalence is seen in sub-Saharan Africa and the Western Pacific region. Cirrhosis, liver failure, and hepatocellular carcinoma (HCC) are reported as leading complications of chronic HBV. The route of transmission of this infection is mainly by exposure to infected blood and bodily fluids. Transmission from mother-to-child remains the predominant route in highly endemic areas. Vaccination has significantly reduced HBV seropr

Targeting cccDNA for Degradation: Endogenous- and Exogenous-Based Methods of Nuclear-Localized DNA Destruction in the Context of Chronic Hepatitis B Infection.

The ability to facilitate cellular degradation or achieve therapeutic destruction of targeted noxious DNA is recognized as a major goal for the treatment of various viral infections. In the case of chronic hepatitis B, targeted DNA degradation of the episomal viral genome found in the nucleus of infected hepatocytes is key to achieving a true cure. However, mechanisms of targeted DNA degradation without excessive broadscale cytotoxicity are still under investigation by researchers. This review summarizes several current systems known to have the ability to target nuclear-localized DNA, includi

Immune Control, HBsAg Loss or Flare Requiring Liver Transplant-Diverse Outcomes After Stopping NUC Therapy.

Hepatitis B surface antigen (HBsAg) loss is rarely achieved during long-term nucleos(t)ide analogue (NUC) therapy of chronic hepatitis B. Treatment discontinuation may lead to immune activation and HBsAg loss. In a prospective study, HBeAg-negative patients without liver cirrhosis stopped NUC therapy after a duration of at least 3 years. Follow-up for 2 years comprised monthly monitoring for the first six months. Twenty-five patients who discontinued therapy and six randomized controls who continued were included. After stopping NUC therapy, five patients (20%) developed an HBsAg seroclearance

Herpes Simplex Virus Hepatitis: An Overlooked Cause of Acute Liver Failure Across a Broad Spectrum of Immunocompromise.

BACKGROUND AND AIMS: Herpes simplex virus (HSV) infection is common worldwide, but hepatic involvement is rare. HSV hepatitis is an uncommon yet frequently catastrophic cause of acute hepatitis and acute liver failure (ALF), accounting for < 1% of ALF cases while carrying very high mortality when diagnosis and antiviral therapy are delayed. Although classically associated with profound immunosuppression and late pregnancy, HSV hepatitis is increasingly recognized in patients with less obvious or "non-traditional" forms of immune dysfunction. METHODS: We describe three cases of HSV hepatitis p

Matrix Metalloproteinases and Viral Hepatitis: A Complex Interplay Driving Liver Fibrosis, Immune Dysregulation, and Hepatocarcinogenesis.

Chronic viral hepatitis caused by hepatitis B virus (HBV) and hepatitis C virus (HCV) remains a leading global public health burden, driving progressive liver fibrosis, cirrhosis, and hepatocellular carcinoma (HCC) through complex interactions between viral replication, host immune responses, and extracellular matrix (ECM) remodelling. Matrix metalloproteinases (MMPs) are a family of zinc-dependent endopeptidases that serve as central regulators of ECM homoeostasis and immune modulation in the liver. While physiological MMP activity is essential for tissue repair and immune surveillance, dysre

Elbasvir Inhibits Hepatitis E Virus Internalization and, in Combination with Ribavirin, Achieves Sustained Viral Suppression In Vitro.

Hepatitis E virus (HEV) infection is generally self-limiting in immunocompetent individuals but may progress to chronic infection in immunocompromised patients, underscoring the need for effective antiviral therapies. Although ribavirin is currently used off-label for HEV treatment, its associated adverse effects highlight the need for safer alternatives. In this study, we screened an anti-viral compound library comprising 800 compounds using three HEV reporter systems designed to target distinct stages of the viral life cycle. Candidate compounds were further evaluated in PLC/PRF/5 cells usin

Role of viral and host factors in determining the outcome of HBV-associated acute liver failure.

BACKGROUND: Acute hepatitis B virus (HBV) infection encompasses a broad clinical spectrum, from self-limited hepatitis to acute liver injury (ALI), acute liver failure (ALF), subacute liver failure, and ALF on chronic HBV reactivation. This study investigates how host and viral factors interact to shape the clinical spectrum of HBV-associated ALF. METHODS: We analyzed 231 serial serum samples from 49 well-characterized patients including 36 with HBV-associated ALF and 13 with acute hepatitis B. Among ALF patients, 7 had ALI, 7 acute and 11 subacute liver failure, and 11 ALF on chronic HBV. Vi

Oropouche virus causes acute hepatitis in mice controlled by type I interferons.

Oropouche virus (OROV), a member of the Peribunyaviridae family endemic to South America, is a current public health threat. The recent OROV outbreak driven by a novel reassortant strain has caused a dramatic increase in cases in 2024 (13,785 in Brazil, versus only 261 from 2015 to 2022) with sustained levels of transmission in 2025. Previously underreported outcomes have been recognized, including miscarriage, microcephaly, encephalitis, and death. OROV lethality in humans has been attributed to severe coagulopathy with liver involvement, and epidemiological data suggest that acute hepatitis

Hepatitis A resurgence: time to reconsider routine pediatric immunization.

The shifting epidemiological landscape of hepatitis A virus (HAV) infection represents a growing concern across Europe, driven by a significant 2025 resurgence and widespread pediatric outbreaks. Although perceived as a mild childhood infection, HAV can cause severe clinical complications, including acute liver failure requiring emergency liver transplantation, and death. Furthermore, the clinical and financial burden is heavily compounded by atypical biphasic or relapsing courses occurring in up to 20% of cases, which inflate healthcare costs through prolonged hospitalizations.
