Resolving human α versus β cell fate allocation for the generation of stem cell-derived islets.

Stem cell-derived glucagon-(α) and insulin-producing (β) cells allow to engineer in vitro biomimetics of islet of Langerhans, the micro-organ controlling glycemia; however, a knowledge gap in the mechanism by which human stem cell-derived α and β cells are specified persists. Mouse studies postulated that Aristaless Related homeobox (Arx) and Paired box 4 (Pax4) transcription factors cross-inhibit each other in endocrine progenitors to promote α/β fate allocation, respectively. To test this model in human, we combine lineage labelling with single-cell multiomic analysis in our newly generated

An lncRNA-aware single-cell framework with donor-level validation identifies reproducible MEG3 enrichment in human liver sinusoidal endothelial cells.

Long non-coding RNAs (lncRNAs) associated with metabolic liver disease are usually identified from bulk tissue, which cannot resolve the hepatic cell types that express them, and default single-cell pipelines discard most lncRNAs at feature selection. We present an lncRNA-aware single-cell analysis framework - retaining all detectable GENCODE v45 lncRNAs during highly variable gene selection - combined with donor-level validation that guards against pseudoreplication. The framework applies to already published data and needs no lncRNA-specific protocol. Applying it to the human Liver Cell Atla

Evidence on the Relationships Between Total Fat Intake and Health Outcomes Other Than Unhealthy Weight Gain in Adults and Children: A Scoping Review.

INTRODUCTION: Global guidance provides evidence-informed recommendations on total fat intake to prevent unhealthy weight gain in adults and children, however, evidence regarding total fat intake and other health outcomes, particularly non-communicable diseases (NCDs), also exists. This scoping review aimed to map and identify gaps in this body of evidence. METHODS: Four databases were searched from 2014 to 2024 for published systematic reviews (SRs), in any population, assessing the effects or associations of reduced total fat intake (e.g., ≤ 30% total energy [TE]), compared with usual fat in

Burn MASCAL - a comprehensive case report.

In this case report, we present the management of an uncommon case of genital burns in a patient with pre-existing phimosis. Generally, a burn wound on the prepuce is -treated with a standard burn wound care strategy; however, in this case, circumcision was performed to address both the phimosis and the genital burns simultaneously. To the best of our knowledge, no similar cases have been reported in the literature. Genital burns are not often encountered due to their protected anatomic location and the additional coverage provided by clothing. Thus, genital burns are usually associated with e

Alcohol use: less is better. An umbrella systematic review of clinical interventions, policies, and dose-response health risks in adults.

BACKGROUND: Alcohol is a major modifiable cause of morbidity, premature mortality and health inequalities, yet evidence informing "low-risk" thresholds and prevention strategies is fragmented. METHODS: Umbrella systematic review conducted according to PRISMA 2020 (protocol on OSF). PubMed/MEDLINE and Scopus were searched (Jan 2015-Mar 2026). An overlap-management approach selected an anchor synthesis per research question (Q1-Q37); supporting records were retained for triangulation. Quality appraisal used design-appropriate tools. Synthesis was narrative. RESULTS: Of 14,991 records, 49 were

Inhibitory probes for spatiotemporal analysis of Gαs protein signaling.

Gαs serves as the prototypical signal transducer for G-protein-coupled receptors (GPCRs) and is the heterotrimeric G protein most frequently mutated in cancer. The classical view of the plasma membrane as the only cellular location where GPCR signal transduction occurs has been challenged by evidence suggesting that Gs also signals from intracellular compartments. However, progress on this topic has stalled because of insufficient approaches with adequate spatiotemporal resolution. Here we describe genetically encoded probes and cell-penetrating compounds that block the effector-binding site o

Rationale, Design, and Participant Baseline Characteristics of a Parallel Randomized Trial of the Effect of Replacing SSBs with Cow's Milk Versus Soymilk on Intrahepatocellular Lipid and Other Cardiometabolic Risk Factors in Adults with Obesity Who Consume Sugar-Sweetened Beverages: The Soy Treatment Evaluation for Metabolic health (STEM) Trial.

Background/Objectives: Liver fat represents an early metabolic lesion in the development of diabetes and its cardiometabolic complications. Diets high in free sugars, particularly from sugar-sweetened beverages (SSBs), are associated with abdominal obesity and increased cardiometabolic risk, prompting global guidelines to limit SSBs as a major public health strategy. Low-fat cow's milk is promoted as the preferred caloric replacement strategy for SSBs due to its high nutritional value and cardiometabolic advantages. Fortified soymilk is a plant-based alternative with approved health claims for

TNF-α alters dedifferentiation of enterochromaffin cells, redirecting toward neuroendocrine tumors.

Small intestinal neuroendocrine tumors (SI-NETs) are serotonin-secreting, well-differentiated neuroendocrine tumors of enterochromaffin (EC) cell origin. However, EC cell-derived tumorigenesis remains poorly understood. Prior studies using TPH1 Cre-ERT2-driven RPM mice [EC cell-targeted RB1 (R) and Trp53 (P) loss and Myc (M) gain] showed nonendocrine adenocarcinomas in the small intestine through dedifferentiation of EC cells to intestinal stem cells, which are prone to transformation. However, these studies were limited by early death from tumors at other sites, leaving the potential for SI-N

Human cathelicidin LL-37 rapidly disrupted colonic epithelial integrity.

The intestinal barrier, held together by epithelial cells and intercellular tight junction (TJ) proteins, prevents the penetration of microbial pathogens. Concurrently, intestinal epithelial cells secrete antimicrobial peptides, including cathelicidin. Cathelicidin has direct antibacterial and immunomodulatory functions, although its role in intestinal integrity remains elusive. In this study, we demonstrate that direct stimulation of human colonic epithelial (T84) cells with human cathelicidin, LL-37, resulted in a rapid and transient increase in epithelial cell permeability. This increased p

Shared Genetics in Celiac Disease and Inflammatory Bowel Disease Specify a Greater Role for Intestinal Epithelial Cells.

The contribution of genetics to the development of gut-related autoimmune diseases such as celiac disease (CeD) and inflammatory bowel diseases (IBDs) is well-established, especially in immune cells, but pinpointing the significance of genetic variants to other cell types is more elusive. Increasing evidence indicates that intestinal epithelial cells are active players in modulating the immune response, suggesting that genetic variants affecting these cells could change cell behavior during disease. Moreover, fine-mapping genetic variants and causal genes to relevant cell types can help to ide

Dolichocolon is common in pediatric gastroenterology patients with constipation and associated complaints.

OBJECTIVES: Dolichocolon (DC), classified under International Classification of Diseases, Tenth Revision (ICD-10) code Q43.8 ("Other specified congenital malformations of intestine"), refers to an elongated or redundant large intestine. Recent studies in adults indicate a role for dolichocolon in constipation and disorders of gut-brain interaction. Contrast enema (CE) imaging is commonly used to identify DC, typically defined as a sigmoid colon extending above, or a transverse colon descending below, the iliac crest line. Despite its potential relevance, the prevalence and clinical significanc

The impact of gut-liver-derived mediators on the organ crosstalk with brain, heart, and kidney: A systematic review.

INTRODUCTION: The current understanding of interactions and crosstalk among essential organs remains incomplete, mainly due to the limitations of studies on the systemic mechanisms at play. The gut and the liver are essential for the functioning of the entire body, and their derived mediators circulate through blood or lymph, impacting other organs like the brain, heart, and kidneys. AIM: This publication reviews gut-liver-derived mediators, which were tested and validated in vivo in humans and rodents, together with the current knowledge of their systemic effects on key vital organs. Postoperative nutritional support after pancreaticoduodenectomy in adults.

BACKGROUND: Resection of the head of the pancreas is most commonly done by a pancreaticoduodenectomy, known as a Whipple procedure. The most common indication for pancreaticoduodenectomy is malignancy, but can include benign tumours and chronic pancreatitis. Complete surgical resection, with negative margins, provides the best prospect of long-term survival. Pancreaticoduodenectomy involves specific and unique alterations to the digestive system and maintaining nutritional status (optimising outcomes and achieving resumption of a normal diet) in patients with cancer after major surgery is a ch
