Early-admission serum albumin and hospitalization burden in a selected serial-culture cohort of patients with diabetic foot ulcers and subsequent Pseudomonas aeruginosa detection: a retrospective cohort study.

BACKGROUND: Patients with diabetic foot ulcers (DFUs) who undergo repeat wound culture and subsequent Pseudomonas aeruginosa (PA) detection form a selected clinical trajectory. Early-admission serum albumin reflects inflammation, capillary permeability, hepatic synthesis, protein loss, hydration, and nutritional reserve; it is not a stand-alone measure of nutritional status. We examined its association with hospitalization burden conditional on this serial-culture trajectory. METHODS: This single-center retrospective cohort included 408 hospitalized patients with DFU who had a PA-positive wou

Integrated Genomic Phenotypic and Host-response Analysis of Multidrug-resistant Diabetic Foot Infections: A Tertiary Center Study from Kerala India.

BACKGROUND: Diabetic foot infections (DFIs) are a primary driver of amputation, worsened by multidrug-resistant (MDR) organisms. The link between pathogen genomics, virulence (e.g., biofilm), and host immunity is poorly quantified. This study aimed to integrate these multi-omic data streams to identify predictors of treatment failure. MATERIALS AND METHODS: This prospective, single-center cohort study enrolled 150 DFI patients. Deep-tissue and autologous healthy-skin biopsies were collected. Pathogens were identified (culture/16S rRNA), with susceptibility and biofilm capacity tested. MDR iso

A Retrospective Study of Clinical, Microbiological, and Ulcer Severity Patterns of Diabetic Foot Presenting to a Tertiary Care Hospital.

BACKGROUND: Diabetic foot ulcers (DFUs) are a major complication of diabetes and are frequently associated with infection, antimicrobial resistance, and limb amputation. Regional Indian data on microbiological patterns and antimicrobial resistance remain limited. METHODS: This retrospective observational study evaluated 40 patients with DFUs presenting to a tertiary care hospital over a 12-month period. Clinical and laboratory parameters, including glycated hemoglobin (HbA1c), C-reactive protein (CRP), total leukocyte count (TLC), and serum albumin, were analyzed. Ulcer severity was graded us

Clinical Outcomes of Topical Recombinant Human Epidermal Growth Factor in Diabetic Foot Ulcers: A Retrospective Single-Centre Cohort Study.

To describe recorded outcomes of diabetic foot ulcers treated with topical recombinant human epidermal growth factor as part of routine care. This retrospective, single-centre cohort study included 150 consecutive patients receiving Regen-D 150 without a control group. We added treatment when standard care did not produce adequate granulation. We defined the primary outcome as complete healing of the index ulcer without amputation. The primary outcome occurred in 101/150 patients (67.3%; 95% confidence interval 59.5-74.3). Sixteen underwent amputation (11 minor, 5 major), 27 had ongoing status

Severe Osteoarthritis and Avascular Necrosis of the Hips in a 37-Year-Old Man: Case Report.

BACKGROUND: Hip osteoarthritis has been considered a condition commonly encountered in the elderly, however, it is predicted to be increasing in prevalence in the population below age 50. Avascular necrosis (AVN) is usually caused by a compromise in blood flow, usually occurring in patients with risk factors. CASE REPORT: A 37-year-old man presented with left-sided hip pain. Magnetic resonance imaging revealed severe left hip osteoarthritis, as well as bilateral hip AVN, although the patient had no known risk factors. CONCLUSIONS: Historically, differential diagnoses for hip pain in younger

Persistent Shoulder Instability and Retained Primitive Reflexes. Addressing a Potential Gap in Adult Musculoskeletal Rehabilitation Care.

SYNOPSIS: Using a case of recalcitrant type III shoulder instability, this editorial explores the potentially overlooked link between retained primitive reflexes and persistent musculoskeletal conditions. We explore how incomplete primitive reflex integration may disrupt motor control, compromise dynamic stability, and interact with neurodevelopmental traits like generalized joint hypermobility. Addressing a potential gap in adult musculoskeletal care, we establish a theoretical framework and foundational justification for future research. To investigate this potential paradigm shift, we outli

Systematic review and meta-analysis on comparing treatment approaches of cervical fractures in ankylosing spondylitis to determine improvement in outcomes.

BACKGROUND & OBJECTIVE: Anterior, posterior and combine surgical approach for treatment of cervical fractures in ankylosing spondylitis is debatable in current existing literature. There are recognized advantages and disadvantages associated with anterior, posterior, and combined surgical approaches to the cervical spine when treating fractures in patients with ankylosing spondylitis (AS). Despite growing surgical experience, the optimal approach with respect to neurological recovery, complication rates, and mortality remains unclear, and the choice is largely guided by surgeon expertise and p

Asymmetric cement distribution in the coronal plane increases the risk of prosthetic loosening following total knee arthroplasty: an in-silico study.

BACKGROUND: Total knee arthroplasty (TKA) is the standard treatment for end-stage knee osteoarthritis (KOA). Aseptic loosening of the tibial component is a common complication following TKA. Previous studies have indicated that reduced cement volume and thinner cement mantles can lead to stress concentration in the cancellous bone beneath the tibial plateau, which serves as an etiological basis for prosthetic loosening. Our preliminary study has shown that variations in cement distribution can alter the local biomechanical environment. Asymmetric cement distribution is a frequently observed cl

CYBB regulates synovial fibroblast inflammatory activation and sEV-mediated synovial-cartilage communication in osteoarthritis.

OBJECTIVE: Although synovial inflammation is recognized as a factor in the progression of osteoarthritis (OA), the specific role of fibroblast-like synoviocytes (FLS) in cartilage degradation has yet to be fully elucidated. This study sought to identify key regulators that connect inflammatory FLS to chondrocyte dysfunction. METHODS: An integrated re-analysis of bulk and single-cell RNA sequencing data from OA synovial tissues was conducted to identify consistently upregulated genes. Functional assays, including migration and invasion tests, were performed on inflammatory FLS. FLS with silenc

Beyond the jaw: vocal, oropharyngeal and cognitive-linguistic biomarkers for precision pain phenotyping in TMD.

Temporomandibular disorders (TMDs) represent a group of conditions affecting the temporomandibular joint and its associated musculature. TMD is the leading cause of chronic orofacial pain. Current assessment relies heavily on patient-reported outcomes, which capture pain intensity and its impact but provide limited mechanistic or functional insight. This narrative conceptual review proposes a framework for investigating vocal, oropharyngeal, and cognitive-linguistic (VOCL) features as candidate behavioral biomarkers for precision pain phenotyping in TMD. We synthesize the literature from TMD a

Altered brain function and its spatial correlations with neurotransmitter maps in anterior shoulder pain patients.

Neuroimaging studies have reported that chronic pain may be associated with brain functional changes and related molecular mechanisms. However, there are few studies in anterior shoulder pain (ASP). Thus, we aimed to assess the possible brain functional changes and their associations with neurotransmitter receptor maps in ASP patients. Resting-state functional magnetic resonance imaging data were obtained from 27 ASP patients and 20 healthy controls (HCs). Regional homogeneity (ReHo) and fractional amplitude of low-frequency fluctuation (fALFF) methods were used to examine altered brain functi
